    Effectiveness and safety of anti-CGRP monoclonal antibodies in patients over 65 years: a real-life multicentre analysis of 162 patients

    Background Anti-CGRP monoclonal antibodies have shown notable effectiveness and tolerability in migraine patients; however, data on their use in elderly patients is still lacking, as clinical trials have implicit age restrictions and real-world evidence is scarce. In this study, we aimed to describe the safety and effectiveness of erenumab, galcanezumab and fremanezumab in migraine patients over 65 years old in real-life. Methods In this observational real-life study, a retrospective analysis of prospectively collected data from 18 different headache units in Spain was performed. Migraine patients who started treatment with any anti-CGRP monoclonal antibody after the age of 65 years were included. Primary endpoints were reduction in monthly migraine days after 6 months of treatment and the presence of adverse effects. Secondary endpoints were reductions in headache and medication intake frequencies by months 3 and 6, response rates, changes in patient-reported outcomes and reasons for discontinuation. As a subanalysis, reduction in monthly migraine days and proportion of adverse effects were also compared among the three monoclonal antibodies. Results A total of 162 patients were included, median age 68 years (range 65-87), 74.1% women. 42% had dyslipidaemia, 40.3% hypertension, 8% diabetes, and 6.2% previous cardiovascular ischaemic disease. The reduction in monthly migraine days at month 6 was 10.17.3 days. A total of 25.3% of patients presented adverse effects, all of them mild, with only two cases of blood pressure increase. Headache and medication intake frequencies were significantly reduced, and patient-reported outcomes were improved. The proportions of responders were 68%, 57%, 33% and 9% for reductions in monthly migraine days >= 30%,>= 50%,>= 75% and 100%, respectively. A total of 72.8% of patients continued with the treatment after 6 months. The reduction in migraine days was similar for the different anti-CGRP treatments, but fewer adverse effects were detected with fremanezumab (7.7%). Conclusions Anti-CGRP mAbs are safe and effective treatments in migraine patients over 65 years old in real-life clinical practice

    First record of Glyphonycteris Sylvestris Thomas, 1896 (Chiroptera: phyllostomidae: Glyphonycterinae) for Argentina, with comments on its Karyotype

    Glyphonycteris is a poorly known genus of bats that inhabit lowlands and forested mountains of Central and South America. This genus comprises three nominal species, with G. sylvestris being the most widely distributed species in South America. The karyological data previously available for this genus are restricted to those resulting from conventional staining techniques applied to a few specimens from Surinam. Here, we report the first record of G. sylvestris in Argentina extending its southern distribution. Moreover, our new locality represents the seventh record from the Atlantic Forests Biome and the first from Araucaria Moist Forest ecoregions. Our specimen overall exhibits the diagnostic characteristics described for the species except for some differences in the distribution of hairs on the wings and limbs. The fecal sample obtained from our specimen contained only seeds of Piper aduncum. The karyotype was 2n = 22 with a FN = 42; the analysis of fluorochrome banding patterns in the chromosomal pericentromeric regions shows DAPI (4,6-diamidino-2-phenylindole) positive blocks in pairs 5 and 8, whereas pericentromeric regions stained with CMA3 (Chromomicine A3) are positive in pairs 1, 2, 3, 4 and 11. Moreover, the interstitial region of chromosomes shows DAPI and CMA3 positive blocks in pairs 1, 2, 3, 4 and 5. These karyological data are novel to G. sylvestris, contributing to the scarce knowledge of the genus. Thus, filling blanks of information and enabling further comparative chromosome analysis.Glyphonycteris es un género poco conocido de murciélago que habita los bosques de tierras bajas y de montañas de Centroamérica y Sudamérica. Este género incluye tres especies nominales; donde G. sylvestris es la más ampliamente distribuida al sur de Sudamérica. Los datos de cariotipo para este género están restringidos a técnicas de tinción convencional realizadas sobre unos pocos especímenes de Surinam. En este trabajo se presenta el primer registro de G. sylvestris en Argentina extendiendo su distribución sur. Además, esta nueva localidad representa el séptimo registro para el Bioma del Bosque Atlántico y el primero para la ecorregión de los Bosques Húmedos de Araucaria. Nuestro espécimen exhibe los caracteres diagnósticos para la especie excepto por algunas diferencias en la distribución de los pelos en alas y miembros. La muestra fecal obtenida de nuestro espécimen solo contenía semillas de Piper aduncum. El cariotipo tuvo un 2n = 22 y FN = 42; los patrones de bandeo con _uorocromos en la región pericentromérica muestra bloques de DAPI (4,6-diamino-2-fenilindol) positivo en los pares 5 y 8, mientras que las regiones pericentroméricas teñidas con CMA3 (Chromomicina A3) son positivas en los pares 1, 2, 3, 4 y 11. Además, en las regiones intersticiales de los cromosomas se observaron bloques DAPI y CMA3 positivos en los pares 1, 2, 3, 4 y 5. Estos datos cariológicos son novedosos para G. sylvestris contribuyendo al escaso conocimiento del género.     Detection of specific periodontal microorganisms from bacteraemia samples after periodontal therapy using molecular-based diagnostics

    The aim of this study was to assess the presence of subgingival pathogens in peripheral blood samples from periodontitis patients before and after scaling and root planing (Sc/RP) using nested polymerase chain reaction (nested PCR). Peripheral blood samples were obtained from 42 patients with severe generalized chronic or aggressive periodontitis. In each patient, four samples of peripheral blood were drawn at different times: immediately before the Sc/RP procedure; immediately after Sc/RP; 15 and 30 min. post‐Sc/RP. Blood samples were analysed for bacteraemia with anaerobic culturing and nested PCR, using universal bacterial primers that target the 16S‐rRNA gene of most bacteria, subsequently re‐amplified with specific primers to Porphyromonas gingivalis , Aggregatibacter actinomycetemcomitans , Tannerella forsythia , Eikenella corrodens , Campylobacter rectus and Prevotella intermedia , using a modified phenol–chloroform method for DNA extraction. Presence of specific periodontal pathogens in peripheral blood after treatment was detected in 54.8% of the patients, in 47.6% with anaerobic culturing and in 19% with nested PCR. In 16.6%, the periodontal pathogens were detected before Sc/RP. P. gingivalis and A. actynomicetemcomitans were the pathogens most frequently detected in the bloodstream before and after Sc/RP. Nested PCR demonstrated the presence of DNA from periodontal pathogens in blood samples in severe periodontitis patients before, during and after periodontal therapy. The use of these molecular‐based techniques may improve the accuracy from the results obtained by haemoculture

    Structure, Absolute Configuration, and Antiproliferative Activity of Abietane and Icetexane Diterpenoids from Salvia ballotiflora

    From the aerial parts of Salvia ballotiflora, eleven diterpenoids were isolated; among them, four icetexanes and one abietane (1–5) are reported for the first time. Their structures were established by spectroscopic means, mainly 1H- and 13C-NMR, including 1D and 2D homo- and hetero-nuclear experiments. Most of the isolated diterpenoids were tested for their antiproliferative, anti-inflammatory, and radical scavenging activities using the sulforhodamine B assay on six cancer cell lines, the TPA-induced ear edema test in mice, and the reduction of the DPPH assay, respectively. Some diterpenoids showed anti-proliferative activity, these being icetexanes 6 and 3, which were the most active with IC50 (μM) = 0.27 ± 0.08 and 1.40 ± 0.03, respectively, for U251 (human glioblastoma) and IC50 (μM) = 0.0.46 ± 0.05 and 0.82 ± 0.06 for SKLU-1 (human lung adenocarcinoma), when compared with adriamycin (IC50 (μM) = 0.08 ± 0.003 and 0.05 ± 0.003, as the positive control), respectively. Compounds 3 and 10 showed significant reduction of the induced ear edema of 37.4 ± 2.8 and 25.4 ± 3.0% (at 1.0 μmol/ear), respectively. Compound 4 was the sole active diterpenoid in the antioxidant assay (IC50 = 98. 4 ± 3.3), using α-tocopherol as the positive control (IC50 (μM) = 31.7 ± 1.04). The diterpenoid profile found is of chemotaxonomic relevance and reinforces the evolutionary link of S. ballotiflora with other members of the section Tomentellae

    Development of chromanes as novel inhibitors of the uncoupling proteins

    11 páginas, 6 figuras, 1 tabla, 4 figuras suplementarias -- PAGS nros. 264-274The uncoupling proteins (UCPs) are mitochondrial carriers that modulate the energetic efficiency and, as a result, can lower superoxide levels. Here, we describe the discovery of a small-molecule inhibitor of the UCPs. Screening of potential UCP1 regulators led to the identification of chromane derivatives that inhibit its proton conductance. Members of the UCP family can act as a defense against oxidative stress and, thus, UCP2 plays a protective role in tumor cells. High UCP2 levels have been associated with chemoresistance. We demonstrate that chromanes also inhibit UCP2 and, in HT-29 human carcinoma cells, cause oxidative stress. The chromane derivatives can act synergistically with chemotherapeutic agents; for instance, they increase the toxicity of arsenic trioxide in HT-29 cells.
