Patient Satisfaction in the Spanish National Health Service: Partial Least Squares Structural Equation Modeling

The aim of this article was to determine which key indicators influence patient satisfaction with the Spanish NHS to provide useful information for policy decision-making. A total of 33 variables for each of the 17 Spanish autonomous communities were collected from the statistical portal of the Spanish Ministry of Health, Social Services, and Equality between 2005 and 2016. A cross-sectional study was applied using Partial Least Squares to a Structural Equation Model (PLS-SEM). The influence of expenditures, resource allocation, and safety were hypothesized about patient satisfaction. Gross Domestic Product (GDP) and life expectancy were used as control variables. Moreover, the influence of resource allocation on use was tested. The model explained 57.1% of patient satisfaction with the Spanish NHS. It was positively influenced mainly by resource allocation and expenditures, followed by safety and life expectancy. Additionally, resources directly influenced the level of use. The number of hospital beds, hemodialysis equipment, rate of adverse drug reactions, and expenditure positively influenced patient satisfaction. In contrast, the number of posts in day hospitals, the hospital infection rate, and the percentage of pharmacy spending negatively influenced patient satisfaction

La carpología, el estudio de los frutos, semillas, tubérculos, rizomas y bulbos recuperados en yacimientos arqueológicos, ha sido aplicada mayoritariamente a contextos correspondientes a sociedades productoras, mientras que los estudios carpológicos de yacimientos de cazadores-recolectores son escasos. En los últimos años, estos han experimentado un incremento, desvelándose el valor de la información que proporcionan: económica, cultural, botánica y ecológica. Presentamos en este artículo la metodología aplicada para el análisis carpológico de un conjunto de muestras procedentes de un nivel fechado en el Magdaleniense medio de la Cova de les Cendres (Teulada-Moraira, Alicante)

Evolution of the Field of Social Media Research through Science Maps (2008-2017)

The objectives of this work were to discover the main points of interest in the field of research on Social Media, within the scientific area of Communication, and to analyse how it has evolved. A methodology based on the analysis of co-words and visualisation techniques was applied. The data was obtained from scientific publications indexed in the Web of Science (WoS) database, during the periods 2008-2012 and 2013-2017. The resulting maps showed that, during the period 2008-2012, the main areas of interest were web 2.0 and the internet in terms of social networking sites. However, during the period 2013-2017, there was a strong upward trend in the impact of social networks and platforms, especially Twitter and Facebook, in many areas (such as social movements, public relations and publicity, distribution of content, crisis communication, participatory journalism, political communication, or the configuration of public identities through social platforms, with special emphasis on youth). Finally, new scientific challenges were found in automatic analysis of content and management of big data. In conclusion, it was possible to transform a complex, underlying, dynamic and multidimensional reality into visible representations that could help experts in the field to better understand the evolution of research on Social Media

¿Hacia una bioética global? A propósito de la propuesta de V. R. POTTER

The oncologist V. R. Potter set out the need to connect experimental and spiritual sciences by building a bridge between them (Bioethics) in order to favour the creation of a new culture for future survival. This proposal aroused great interest within the field of Medicine, in which medical Bioethics has widely developed. Some years later, Potter stressed again the need to widen the scope of Bioethics until achieving a Global Bioethics, according to his initial proposal. En alemán:Der Onkologe V. R. Potter schlug vor, die empirischen Wissenschaften und die Geisteswissenschaften unter einem Dach (Bioethik) zusammenzuführen. Sein Ziel war es, die Entstehung einer neue Kultur des Überlebens zu fördern. Dieser Vorschag stieß auf breite Anerkennung in der Medizin, in welcher sich die medizinische Bioethik umfassend entwickelt hat. Jahre später drang Potter erneut darauf, den Einflussbereich der Bioethik zu erweitern. Dabei folgte er seinem ursprünglichem Vorschlag, bis er es erreichte, ihn mit den Inhalten einer globalen Bioethik zu füllen

Personas con discapacidad intelectual: igualdad jurídica, protección asistencial y asistencia sanitaria

Die spanische Gesetzgebung erstreckt sich auf den Schutz von Behinderten, insbesondere auch geistig Behinderten, so, wie es der Artikel 49 der Spanischen Verfassung ausführt. Die Gesetzgebung erfolgte in drei Schritten. Der erste Schritt erfolgte mit dem Gesetz 13/1982 vom 7. April zur «Sozialen Integration der Behinderten»; danach mit dem Gesetz 51/2003, vom 2. Dezember zur «Chancengleichheit, Nichtdiskriminierung und allgemeinem schrankenlosem Zugang von Behinderten». Und schließlich im dritten Schritt durch Einbeziehung der Behinderten in das Gesetz 39/2006, vom 14. Dezember zur «Förderung der personalen Eigenständigkeit und Betreuung von Pflegebedürftigen». Der rote Faden, der alle drei sich ergänzenden und nicht widersprechenden Gesetzesinitiativen durchzieht, findet sich in der Anwendung der Gleichheit, wie sie im Artikel 9,2 der Spanischen Verfassung festgehalten wurde: D.h. in der Mandatserteilung gegenüber allen öffentlichen Entscheidungsträgern, daran mitzuwirken, die realen und effektiven Bedingungen der Freiheit und Gleichheit der Einzelpersonen und deren Gruppen zu fördern, die Hindernisse zu beseitigen, die ihre Erfüllung behindern oder erschweren, sowie die Beteiligung aller Bürger am politischen, wirtschaftlichen, kulturellen und sozialen Leben zu erreichen. Aus dieser Perspektive heraus untersucht diese Studie die geistige Behinderung, durch alle Gesetzesreformen hindurch, unter dem Gesichtspunkt der Rechtsgleichheit, der pflegerischen Betreuung und dem Krankenversicherungsschutz.Spanish legislation has addressed the protection of persons with disabilities and, specifically, those with intellectual disabilities, on an ongoing basis, at three specific times, as a development of Article 49 of the Spanish Constitution. Initially, with the Law 13/1982, of 7 April, on Social Integration of the Disabled; subsequently, with the Law 51/2003, of 2 December, on Equal Opportunities, non-discrimination and the universal accessibility of disabled persons. And, finally, including people with this disability in the Law 39/2006, of 14 December, on the Promotion of Personal Autonomy and Care for Dependent Persons. The main theme of the three legislative initiatives, that are not exclusive, but tend to complement one another, is found in the application of the principle of equality in accordance with that stipulated in Article 9.2 of the Spanish Constitution, that is to say, in the mandate to public powers to promote the conditions so that freedom and equality of the individual and of the groups to which they belong are real and effective; to remove the obstacles that impede or hinder their full enjoyment and to facilitate the participation of all citizens in political, economic, cultural and social life. From this perspective, this study examines intellectual disability, through legislative reforms, from the perspective of legal equality, welfare and health care.La legislación española ha abordado la protección de las personas con discapacidad, y, en concreto, de discapacidad intelectual, de manera continuada en tres momentos concretos, como desarrollo del art. 49 de la Constitución. En un primer momento con la Ley 13/1982, de 7 de abril, de integración social de los minusválidos; posteriormente, con la Ley 51/2003, de 2 de diciembre, de igualdad de oportunidades, no discriminación y accesibilidad universal de las personas con discapacidad, y finalmente, incluyendo a las personas con esta discapacidad en la Ley 39/2006, de 14 de diciembre, de promoción de la autonomía personal y atención a las personas en situación de dependencia. El hilo conductor de las tres iniciativas legislativas, que no se excluyen, sino que tienden a complementarse, se encuentra en la aplicación del principio de igualdad de acuerdo con lo previsto en el art. 9.2 de la Constitución, es decir, en el mandato a los poderes públicos de promover las condiciones para que la libertad y la igualdad del individuo y de los grupos en que se integra sean reales y efectivas; remover los obstáculos que impidan o dificulten su plenitud, y facilitar la participación de todos los ciudadanos en la vida política, económica, cultural y social. Desde esta perspectiva en este estudio se examina la discapacidad intelectual a través de las reformas legislativas, desde la perspectiva de la igualdad jurídica, la protección asistencial y la asistencia sanitaria
