    Starving the messenger: A study of journalists’ conditions of service in Malawi

    This study used in-depth interviews to investigate the conditions of service and welfare of journalists in Malawi. It found that while the Media Council of Malawi code of professional ethics and standards and in-house policies expected the best out the journalists, the majority of the journalists, mostly junior reporters, were grossly underpaid; far below the monthly living wage. It also found that dejection, fatalism, and individualism pervade journalists so much so that some of them seemed resigned to their status quo. Media owners seemed unwilling to engage in any debate about their journalist employees’ welfare. The study concludes that under such circumstances, many Malawian journalists were likely to be tempted to take bribes and engage in other forms of corruption as coping mechanisms.Key words: Afriethics, umunthu, ubuntu, journalism, welfare, living wag

    The patient-provider relationship and antenatal care uptake at two referral hospitals in Malawi: A qualitative study

    Background In post-stroke patients, impairment of quality of life (QOL) has been associated with functional impairment, age, anxiety, depression, and fatigue. Good social support, higher education, and better socioeconomic status are associated with better QOL among stroke survivors. In Africa, studies from Nigeria and Tanzania have reported on post-stroke QOL.Background Approximately 90% of Malawian women attend antenatal care at least once during their pregnancies; however, most mothers first present during months five and six and do not adhere to the World Health Organization’s recommended four visits. The objective of this study was to explore the role the patient-provider relationship has on antenatal care uptake.Methods A qualitative study, consisting of interviews with 20 urban pregnant mothers and eight health workers, was conducted from September to December 2014. Two large tertiary care hospitals in the Central and Southern regions of Malawi were selected as study sites.Results Several factors influenced antenatal care attendance. Significant barriers reported included the patient-provider relationship, clinic wait times, family and friend support, distance from home to the clinic, transportation, cost, and number of visits. The patient-provider relationship appears to have a large impact on antenatal clinic participation. Mothers indicated that health workers often mistreat or demean them during visits. Additionally, health workers revealed that, due to staff shortages, patients often do notreceive the care they deserve.Conclusions The results of this study suggest that, in addition to other factors, healthcare provider attitudes influence antenatal clinic attendance. Improving the patient-provider relationship may increase antenatal clinic attendance and decrease pregnancy complications during pregnancy. Professional development opportunities and quality improvement programmes are would help improve patient care and health outcomes while the continued staff shortages in the country are addressed

    Simulations of the OzDES AGN Reverberation Mapping Project

    As part of the OzDES spectroscopic survey we are carrying out a large scale reverberation mapping study of ∼\sim500 quasars over five years in the 30 deg2^2 area of the Dark Energy Survey (DES) supernova fields. These quasars have redshifts ranging up to 4 and have apparent AB magnitudes between 16.8<r<22.516.8<r<22.5 mag. The aim of the survey is to measure time lags between fluctuations in the quasar continuum and broad emission line fluxes of individual objects in order to measure black hole masses for a broad range of AGN and constrain the radius-luminosity (R−LR-L) relationship. Here we investigate the expected efficiency of the OzDES reverberation mapping campaign and its possible extensions. We expect to recover lags for ∼\sim35-45\% of the quasars. AGN with shorter lags and greater variability are more likely to yield a lag, and objects with lags ≲\lesssim6 months or ∼\sim1 year are expected be recovered the most accurately. The baseline OzDES reverberation mapping campaign is predicted to produce an unbiased measurement of the R−LR-L relationship parameters for Hβ\beta, Mg II λ\lambda2798, and C IV λ\lambda1549. However, extending the baseline survey by either increasing the spectroscopic cadence, extending the survey season, or improving the emission line flux measurement accuracy will significantly improve the R−LR-L parameter constraints for all broad emission lines.Comment: Published online in MNRAS. 28 page

    Skeletal Muscle Fatigability in Heart Failure

    Evidence suggests that heart failure (HF) patients experience skeletal muscle fatigability in the lower extremity during single-limb tasks. The contribution of skeletal muscle fatigability to symptoms of exercise intolerance (perceived fatigue and dyspnea) is relatively unclear. Symptomatic or ‘perceived’ fatigue is defined by the sensations of exhaustion or tiredness that patients experience either at rest or while performing a motor task. Although factors that contribute to symptoms of fatigue in patients with HF are multifactorial; the skeletal muscle likely plays a major role. Skeletal muscle fatigability, as opposed to symptomatic fatigue, is an objective measure of a reduction in muscle force or power or reduced ability of the muscles to perform over time. Indeed, evidence suggests that patients with HF experience greater skeletal muscle fatigability which may contribute to a diminution in motor performance and the overall symptomatology that is hallmark of exercise intolerance in HF. This review will discuss (1) skeletal muscle fatigability in patients with HF, (2) the mechanisms contributing to locomotor skeletal muscle fatigability in HF and (3) the relationship of fatigability to symptoms of perceived fatigue and exercise intolerance in HF patients. Evidence suggests that cardiac dysfunction alone does not contribute to exercise intolerance. Therefore, mechanisms of skeletal muscle fatigability and their contribution to symptoms of fatigue and exercise intolerance, is an increasingly important consideration as we develop rehabilitative strategies for improving motor performance and functional capacity in patients with HF

    COVID deaths in South Africa: 99 days since South Africa’s first death

    Background. Understanding the pattern of deaths from COVID-19 in South Africa (SA) is critical to identifying individuals at high risk of dying from the disease. The Minister of Health set up a daily reporting mechanism to obtain timeous details of COVID-19 deaths from the provinces to track mortality patterns.Objectives. To provide an epidemiological analysis of the first COVID-19 deaths in SA.Methods. Provincial deaths data from 28 March to 3 July 2020 were cleaned, information on comorbidities was standardised, and data were aggregated into a single data set. Analysis was performed by age, sex, province, date of death and comorbidities.Results. SA reported 3 088 deaths from COVID-19, i.e. an age-standardised death rate of 64.5 (95% confidence interval (CI) 62.3 - 66.8) deaths per million population. Most deaths occurred in Western Cape (65.5%) followed by Eastern Cape (16.8%) and Gauteng (11.3%). The median age of death was 61 years (interquartile range 52 - 71). Males had a 1.5 times higher death rate compared with females. Individuals with two or more comorbidities accounted for 58.6% (95% CI 56.6 - 60.5) of deaths. Hypertension and diabetes were the most common comorbidities reported, and HIV and tuberculosis were more common in individuals aged &lt;50 years.Conclusions. Data collection for COVID-19 deaths in provinces must be standardised. Even though the data had limitations, these findings can be used by the SA government to manage the pandemic and identify individuals who are at high risk of dying from COVID-19

In Italia le aree individuate sono Monte Arci in Sardegna, Lipari e Pantelleria in Sicilia e Palmarola nel Lazio.&lt;/p&gt;&lt;p&gt; &lt;/p&gt;&lt;p&gt;&lt;strong&gt;Obsidian  from  Palmarola:  source  markers &lt;/strong&gt;&lt;/p&gt;&lt;p&gt;A  chemical  characterization  of  the  main  components  and  traces  of  obsidian  allows identi cation  of  commercial  routes  in  antiquity.  Obsidian  sources in Prehistoric Italy were mainly four: Monte Arci in Sardinia, the islands of Lipari and Pantelleria in Sicily and the island of Palmarola in Lazio. A survey methodology on samples of obsidian from several Neolithic sites in Lazio has been developed, comparing three different analytical techniques: SEM-EDS, XRF and LA-ICP-MS, in order to obtain a wider range of data to be compared with literature records and processed by Principal Component Analysis (PCA), to de ne relationships between samples and to assign each nding its origin.&lt;/p&gt

    High-redshift Extremely Red Quasars in X-Rays

    Quasars may have played a key role in limiting the stellar mass of massive galaxies. Identifying those quasars in the process of removing star formation fuel from their hosts is an exciting ongoing challenge in extragalactic astronomy. In this paper, we present X-ray observations of 11 extremely red quasars (ERQs) with L bol ~ 1047 erg s−1 at z = 1.5–3.2 with evidence for high-velocity (v ⩾\geqslant 1000 km s−1) [O iii] λ5007 outflows. X-rays allow us to directly probe circumnuclear obscuration and to measure the instantaneous accretion luminosity. We detect 10 out of 11 ERQs available in targeted and archival data. Using a combination of X-ray spectral fitting and hardness ratios, we find that all of the ERQs show signs of absorption in the X-rays with inferred column densities of N H ≈ 1023 cm−2, including four Compton-thick candidates (N H ⩾\geqslant 1024 cm−2). We stack the X-ray emission of the seven weakly detected sources, measuring an average column density of N H ~ 8 × 1023 cm−2. The absorption-corrected (intrinsic) 2–10 keV X-ray luminosity of the stack is 2.7 × 1045 erg s−1, consistent with X-ray luminosities of type 1 quasars of the same infrared luminosity. Thus, we find that ERQs are a highly obscured, borderline Compton-thick population, and based on optical and infrared data we suggest that these objects are partially hidden by their own equatorial outflows. However, unlike some quasars with known outflows, ERQs do not appear to be intrinsically underluminous in X-rays for their bolometric luminosity. Our observations indicate that low X-rays are not necessary to enable some types of radiatively driven winds

    Spatial variation in prostate cancer survival in the Northern and Yorkshire region of England using Bayesian relative survival smoothing

    Primary Care Trust (PCT) estimates of survival lack robustness as there are small numbers of deaths per year in each area, even when incidence is high. We assess PCT-level spatial variation in prostate cancer survival using Bayesian spatial models of excess mortality. We extracted data on men diagnosed with prostate cancer between 1990 and 1999 from the Northern and Yorkshire Cancer Registry and Information Service database. Models were adjusted for age at diagnosis, period of diagnosis and deprivation. All covariates had a significant association with excess mortality; men from more deprived areas, older age at diagnosis and diagnosed in 1990–1994 had higher excess mortality. The unadjusted relative excess risks (RER) of death by PCT ranged from 0.75 to 1.66. After adjustment, areas of high and low excess mortality were smoothed towards the mean, and the RERs ranged from 0.74 to 1.49. Using Bayesian smoothing techniques to model cancer survival by geographic area offers many advantages over traditional methods; estimates in areas with small populations or low incidence rates are stabilised and shrunk towards local and global risk estimates improving reliability and precision, complex models are easily handled and adjustment for covariates can be made
