    Risk factors associated with wheezing in infants

    OBJECTIVE: to identify possible risk factors associated with wheezing in infants (12-15 months-old) in the state of Mato Grosso, Brazil. METHODS: this was a cross-sectional study performed by applying a standardized written questionnaire from the international study on wheezing in infants (Estudio Internacional de Sibilancia en Lactantes - EISL), phase 3. Parents and/or guardians of infants were interviewed at primary health care clinics or at home from August of 2009 to November of 2010. Factors associated to wheezing were studied using bivariate and multivariate analysis (using the Statistical Package for Social Sciences [SPSS] v.18.0), and expressed as odds ratios (OR) and 95% confidence intervals (95% CI). RESULTS: the written questionnaire was answered by 1,060 parents and/or guardians. The risk factors for wheezing were: history of asthma in the family [mother (OR = 1.62; 95% CI = 1.07-2.43); father (OR = 1.98; 95% CI = 1.22-3.23); siblings (OR = 2.13; 95% CI = 1.18-3.87)]; history of previous pneumonia (OR = 10.80; 95% CI = 4.52-25.77); having had more than six upper respiratory tract infections (URTIs) (OR = 2.95; 95% CI = 2.11-4.14); having had first URTI before the third month of life (OR = 1.50; 95% CI = 1.04-2.17); living in a moderately polluted area (OR = 1.59; 95% CI = 1.08-2.33); paracetamol use for URTI (OR = 2.13; 95% CI = 1.54-2.95); and antibiotic use for skin infection (OR = 2.29; 95% CI = 1.18-4.46). CONCLUSIONS: the study of risk factors for wheezing in the first year of life is important to help physicians identify young children at high risk of developing asthma and to improve public health prevention strategies in order to reduce the morbidity of wheezing in childhood.OBJETIVO: identificar possíveis fatores de risco associados à sibilância em lactentes (12 a 15 meses) no estado de Mato Grosso, Brasil. MÉTODOS: estudo transversal, utilizando o questionário escrito padronizado do Estudio Internacional de Sibilancia en Lactantes (EISL) - fase 3. Pais e/ou responsáveis pela criança foram entrevistados em Unidades Básicas de Saúde quando as procuravam para imunização de rotina ou durante visitas nos domicílios de crianças matriculadas nos programas de saúde da família, no período de agosto de 2009 a novembro de 2010. Fatores associados à sibilância foram avaliados utilizando análise bivariada e multivariada e expressos como odds ratio (OR) e intervalo de confiança 95% (IC95%). RESULTADOS: ao todo, um mil e sessenta (n = 1.060) pais e/ou responsáveis responderam o questionário escrito. Os fatores de risco para sibilância foram: história familiar de asma [mãe (OR = 1,62; IC95%= 1,07-2,43); pai (OR = 1,98; IC95% = 1,22-3,23); irmãos (OR = 2,13; IC95% = 1,18-3,87)]; história prévia de pneumonia (OR = 10,8; IC 95% = 4,52-25,77); ter mais de seis infecções de vias aéreas superiores (IVAS) (OR = 2,95; IC95% = 2,11-4,14) e a primeira IVAS antes dos três meses de idade (OR = 1,50; IC95% = 1,04-2,17); residir em local com moderada poluição (OR = 1,59; IC95% = 1,08-2,33); uso de paracetamol por IVAS (OR = 2,13; IC95% = 1,54-2,95); e uso de antibiótico por infecção na pele (OR = 2,29; IC 95% = 1,18-4,46).     Optimal Microgrid Topology Design and Siting of Distributed Generation Sources Using a Multi-Objective Substrate Layer Coral Reefs Optimization Algorithm

    n this work, a problem of optimal placement of renewable generation and topology design for a Microgrid (MG) is tackled. The problem consists of determining the MG nodes where renewable energy generators must be optimally located and also the optimization of the MG topology design, i.e., deciding which nodes should be connected and deciding the lines’ optimal cross-sectional areas (CSA). For this purpose, a multi-objective optimization with two conflicting objectives has been used, utilizing the cost of the lines, C, higher as the lines’ CSA increases, and the MG energy losses, E, lower as the lines’ CSA increases. To characterize generators and loads connected to the nodes, on-site monitored annual energy generation and consumption profiles have been considered. Optimization has been carried out by using a novel multi-objective algorithm, the Multi-objective Substrate Layers Coral Reefs Optimization algorithm (Mo-SL-CRO). The performance of the proposed approach has been tested in a realistic simulation of a MG with 12 nodes, considering photovoltaic generators and micro-wind turbines as renewable energy generators, as well as the consumption loads from different commercial and industrial sites.     Espirometría, pletismografía y reactividad bronquial a metacolina en niños con síntomas de asma autoinformados versus niños sin asma

    Aunque los estudios globales han proporcionado información valiosa sobre la prevalencia del asma en niños de América Latina, hay muy poca información en la región acerca de si el asma actual reportada por los niños se asocia con características funcionales de esta como disminución de la función pulmonar e hiperreactividad bronquial (BHR). Este estudio examinó la relación entre las sibilancias auto informadas en los últimos 12 meses, la espirometría, la pletismografía, la capacidad de respuesta de las vías respiratorias y la atopia en dos muestras aleatorias (60 cada una) de niños de 13 a 14 años que habían tenido o no sibilancias en los últimos 12 meses. En comparación con los niños sin sibilancias, aquellos que informaron sibilancias actuales tuvieron una hiperreactividad bronquial (HRB) significativamente mayor a la metacolina (55% frente a 18,2%, p=0.007), función pulmonar significativamente más baja en la espirometría (p=0.001) y una resistencia significativamente mayor en pletismografía (p=0.001). La atopia fue significativamente mayor en los que informaron síntomas de asma actuales (p=0.007). Nuestros resultados muestran que los adolescentes que reportan síntomas actuales del asma mediante un cuestionario tienen una HRB significativamente más alta y una función pulmonar más baja que los niños sin antecedentes de asma

    OBJECTIVE: To evaluate the prevalence and the clinical characteristics of wheezing in infants aged 12 to 15 months in the city of Cuiabá, Mato Grosso State, Midwest Brazil. METHODS: Parents and/or guardians of infants were interviewed and completed a written standardized questionnaire of the Estudio Internacional de Sibilancia en Lactante (EISL) - phase 3 at primary healthcare clinics at the same day of children vaccination or at home, from August of 2009 to November of 2010. RESULTS: 1,060 parents and/or guardians completed the questionnaire, and 514 (48.5%) infants were male. Among the studied infants, 294 (27.7%) had at least one episode of wheezing during the first year of life, beggining at 5.8±3.0 months of age, with a predominance of male patients. The prevalence of occasional wheezing (<3 episodes of wheezing) was 15.0% and recurrent wheezing (≥3 episodes) was 12.7%. Among the infants with recurrent wheezing, the use of inhaled β2-agonist, oral corticosteroid, leukotriene receptor antagonist, as well as night symptoms, respiratory distress, and hospitalization due to severe episodes were significantly more frequent. Physician-diagnosed asthma was observed in 28 (9.5%) of the wheezing infants. Among the wheezing infants, 80 (27.7%) were diagnosed with pneumonia, of whom 33 (11.2%) required hospitalization; neverthless, no differences between occasional and recurrent wheezing infants were found. CONCLUSIONS: The prevalence of recurrent wheezing and physician-diagnosed asthma in infant were lower compared with those observed in other Brazilian studies. Recurrent wheezing had early onset and high morbity.OBJETIVO: Determinar a prevalência e as características clínicas da sibilância em lactente (12-15 meses) residentes em Cuiabá (MT). MÉTODOS: Pais e/ou responsáveis pela criança foram entrevistados e responderam ao questionário escrito padronizado do Estudio Internacional de Sibilanciaen Lactantes (EISL) - fase 3, em unidades básicas de saúde por ocasião da vacinação de rotina ou durante visitas nos domicílios de criança matriculadas nos programas de saúde da família no período de agosto de 2009 a novembro de 2010. RESULTADOS: 1060 pais e/ou responsáveis responderam ao questionário escrito, sendo 514 (48,5%) lactentes do sexo masculino. Dos lactentes, 294 (27,7%) tiveram pelo meno um episódio de sibilância no primeiro ano de vida, com início aos 5,8±3,0 meses e predomínio em meninos. A prevalência de sibilância ocasional (<3 episódios de sibilância) foi 15% e a recorrente (≥3 episódios) foi 12,7%. Entre estes, o uso de broncodilatador inalado, corticosteroide oral, antileucotrieno, presença de sintomas noturnos, dificuldade para respirar e internação por sibilância foram significantemente mais frequentes. Diagnóstico médico de asma foi evidenciado em 28 (9,5%) dos lactentes sibilantes. Dos lactentes sibilantes, 80 (27,7%) relataram ocorrência prévia de pneumonia, sendo que 33 (11,2%) necessitaram internação para tratamento, porém não houve diferença entre os grupos de sibilantes. CONCLUSÃO: A prevalência de sibilância recorrente em lactentes foi mais baixa em comparação a outros estudos brasileiros, assim como o diagnóstico médico de asma.     Prevalence of recurrent wheezing in infants

    OBJECTIVE: To identify the prevalence of recurrent wheezing in infants in the city of Curitiba, PR, Brazil. METHODS: A cross-sectional study carried out by means of administering questionnaires to the parents of infants aged 12 to 15 months attending health centers for immunization during the period between August 2005 and December 2006. This is a standardized and validated instrument consisting of questions on demographic characteristics, wheezing, respiratory infections and risk factors. At the time of the study the City Health Department had 107 health centers, 35 of which were selected by lots and distributed homogeneously across the municipal territory. RESULTS: A total of 1,364 infants (45.4%) had episodes of wheezing during their first 12 months of life, with onset at 5.5±3.1 months (mean ± standard deviation), and 678 (22.6%) had had three or more episodes. In 84.6% of the wheezing children treatment was with β2-agonists, with inhaled corticosteroids in 18.5%, oral corticosteroids in 24.3% and leukotriene receptor antagonists were used with 5.4%. The wheezing children exhibited nocturnal symptoms, intense difficulty breathing and visits to emergency services in the proportions of 58.9, 46.2 and 57.6%, respectively; 12.7% were admitted to hospital for asthma and 10.9% had had a medical diagnosis of asthma. Nocturnal symptoms, visits to emergency, severity of symptoms, hospital admissions for asthma and medical diagnoses of asthma were all more common among those who had suffered three or more crises (p < 0.001). CONCLUSIONS: There is an elevated prevalence of wheezing among the infants of Curitiba, with early onset and elevated morbidity. It is possible that these infants represent a large contingent of asthmatics.OBJETIVO: Verificar a prevalência da sibilância recorrente em lactentes na cidade de Curitiba (PR). MÉTODOS: Estudo transversal pela aplicação de questionários aos pais de lactentes, idade entre 12 e 15 meses, que procuraram unidades de saúde para imunização no período entre agosto de 2005 e dezembro de 2006. Este instrumento padronizado e validado consiste de perguntas sobre características demográficas, sibilância, infecções respiratórias e fatores de risco. Durante o período de estudo, a Secretaria Municipal de Saúde possuía 107 unidades de saúde, das quais 35 foram selecionadas por sorteio e distribuídas homogeneamente no território municipal. RESULTADOS: Um total de 1.364 lactentes (45,4%) apresentou um ou mais episódios de sibilância nos primeiros 12 meses de vida, com início aos 5,5±3,1 meses (média ± desvio padrão), e 678 (22,6%) tiveram três ou mais episódios. Utilizaram β2-agonistas, corticóides inalatórios, corticóides orais e antagonistas do receptor de leucotrienos 84,6, 18,5, 24,3 e 5,4% dos sibilantes, respectivamente. Sintomas noturnos, dificuldade intensa para respirar e visitas a emergência estiveram presente em 58,9, 46,2 e 57,6% dos lactentes que sibilaram; destes, 12,7% foram hospitalizados por asma e 10,9% tiveram diagnóstico médico de asma. Sintomas noturnos, visitas à emergência, gravidade dos sintomas, hospitalização por asma e diagnóstico médico de asma foram mais freqüentes entre os que apresentaram três ou mais crises (p < 0,001). CONCLUSÃO: A prevalência de lactentes sibilantes em Curitiba é elevada, com início precoce e alta morbidade.     Prevalencia y perfil de sensibilización a aeroalérgenos en 1.199 niños asmáticos: serie consecutiva de casos

    Indexación: Web of Science; Scielo.Background: The prevalence of atopy in asthmatic children is widely variable around the world as demonstrated by large multicentric international studies. Aim: To determine the prevalence of atopy, defined as a positive reaction to one or more allergens in the skin prick test (SPT), in children with persistent asthma. Material and Methods: We studied 1,199 children (54% male), aged between 4 and 16 years with confirmed diagnosis of asthma and followed at a Department of Pediatric Respiratory Medicine, between 2006 and 2011. SPT was performed according to international recommendations using standardized aeroallergens, in the forearm. A positive reaction was defined as a wheal ≥ 3 mm to one or more allergens. Results: The overall prevalence of atopy (positive SPT) was 49.4% (95% confidence interval (CI) 46.5-52.2) and there was a significant trend towards a higher prevalence with increasing age (p < 0.01). The main allergens with positive reactions were Dermatophagoides with 24.9% (95% CI 26.7-31.9), grass 24.0% (95% CI 21.6-26.5), weeds 19.0% (95% CI 16.9-21.4), cat 17.7% (95% CI 15.4-20.2), and Alternaria with 11.0% (95% CI 9.1-13.1). Sixty five percent of positive children reacted to one or more allergens. There were no adverse reactions. Conclusions: In the southern metropolitan area of Santiago de Chile, half of children with asthma are sensitized to common aeroallergens.     Prevalence of wheezing and associated risk factors among infants in Recife, Pernambuco State, Brazil

    The aim of this study was to verify the prevalence of wheezing in infants (< 1 year of age) in Recife, Pernambuco State, Brazil, and to identify associated risk factors. Sample and methods: the study was performed according to the protocol of the International Study of Wheezing in Infants (EISL) in children ranging from 12 to 15 months of age. The sample was analyzed for presence or absence of wheezing. A total of 1,071 parents of children ranging from 12 to 15 months of age were interviewed. Prevalence of wheezing in the first year of life was 43%, with no difference between the sexes. Wheezing in the first year of life was associated with pneumonia, family history of asthma, more than nine episodes of upper airway infection, and the first cold before six months of age (p < 0.001). Prevalence of wheezing in the first year of life was high in Recife. Early onset (and high number) of colds, family history of asthma, and pneumonia were associated with wheezing in these children.Verificar a prevalência de sibilância no primeiro ano de vida em lactentes da cidade do Recife, Pernambuco, Brasil e identificar fatores de risco a ela associados. Estudo realizado segundo o protocolo do Estudio Internacional de Sibilancia en Lactantes (EISL) em crianças com idades entre 12 e 15 meses. A amostra foi analisada segundo a presença ou não de sibilância. Foram entrevistados 1.071 pais por crianças com idades entre 12 e 15 meses. A prevalência de sibilância no primeiro ano de vida foi de 43%, sem diferenças quanto aos gêneros. O relato de sibilância no primeiro ano de vida foi associado a ter tido pneumonia, ter familiares com asma, mais de nove episódios de infecções de vias aéreas superiores e primeiro episódio de resfriado antes de seis meses de idade (p < 0,001). A prevalência de sibilância no primeiro ano de vida entre lactentes da cidade do Recife é alta.     Compound-specific carbon isotope analysis of short-chain fatty acids from pine tissues: characterizing paleo-fire residues and plant exudates

    Different types of plant tissues and resin can account for the wax lipids found in sedimentary contexts and archaeological samples. Consequently, there is increasing research to characterize the fatty acid carbon isotope ratios of different plant anatomical parts and their plant exudates (resin). With the aim to explore isotopic differences between plant tissues, state of the fine organic matter, effect of thermal degradation, and to identify plant residues we measured the δ13C values of short-chain fatty acids (δ13C16:0 and δ13C18:0) in: i) dead and fresh (collected and immediately dried) pine needles and branches (Pinus canariensis) and pine resin from laboratory-controlled heating experiments and ii) sediment and charred pine tissue samples from a wild pine forest fire. Our results are compared to previously published experimental open-air fire experiments and pine-fuelled archaeological combustion features. We found that for both fatty acid types, there are differences in δ13C signatures among anatomical parts and initial moisture content. These data allow us to characterize the isotopic signature of pine tissue and the effect of degradation on isotopic biomarkers, as well as to estimate combustion temperatures in pine-fuelled anthropogenic fires.Grant project PALEOCHAR – 648871info:eu-repo/semantics/publishedVersio
