    Socially assistive robots in health and social care: Acceptance and cultural factors. Results from an exploratory international online survey

    Aim: This study explored the views of an international sample of registered nurses and midwives working in health and social care concerning socially assistive robots (SARs), and the relationship between dimensions of culture and rejection of the idea that SARs had benefits in these settings. Methods: An online survey was used to obtain rankings of (among other topics) the extent to which SARs have benefits for health and social care. It also asked for free text responses regarding any concerns about SARs. Results: Most respondents were overwhelmingly positive about SARs' benefits. A small minority strongly rejected this idea, and qualitative analysis of the objections raised by them revealed three major themes: things might go wrong, depersonalization, and patient‐related concerns. However, many participants who were highly accepting of the benefits of SARs expressed similar objections. Cultural dimensions of long‐term orientation and uncertainty avoidance feature prominently in technology acceptance research. Therefore, the relationship between the proportion of respondents from each country who felt that SARs had no benefits and each country's ratings on long‐term orientation and uncertainty avoidance were also examined. A significant positive correlation was found for long‐term orientation, but not for uncertainty avoidance. Conclusion: Most respondents were positive about the benefits of SARs, and similar concerns about their use were expressed both by those who strongly accepted the idea that they had benefits and those who did not. Some evidence was found to suggest that cultural factors were related to rejecting the idea that SARs had benefits

    Cannabis use in male and female first episode of non-affective psychosis patients: Long-term clinical, neuropsychological and functional differences

    BACKGROUND: Numerous studies show the existence of a high prevalence of cannabis use among patients with psychosis. However, the differences between men and women who debut with a first episode of psychosis (FEP) regarding cannabis use have not been largely explored. The aim of this study was to identify the specific sex factors and differences in clinical evolution associated with cannabis use. METHOD: Sociodemographic characteristics at baseline were considered in our sample of FEP patients to find differences depending on sex and the use of cannabis. Clinical, functional and neurocognitive variables at baseline, 1-year, and 3-years follow-up were also explored. RESULTS: A total of 549 patients, of whom 43% (N = 236) were cannabis users, 79% (N = 186) male and 21% (N = 50) female, were included in the study. There was a clear relationship between being male and being a user of cannabis (OR = 5.6). Cannabis users were younger at illness onset. Longitudinal analysis showed that women significantly improved in all three dimensions of psychotic symptoms, both in the subgroup of cannabis users and in the non-users subgroup. Conversely, subgroups of men did not show improvement in the negative dimension. In cognitive function, only men presented a significant time by group interaction in processing speed, showing a greater improvement in the subgroup of cannabis users. CONCLUSION: Despite knowing that there is a relationship between cannabis use and psychosis, due to the high prevalence of cannabis use among male FEP patients, the results showed that there were very few differences in clinical and neurocognitive outcomes between men and women who used cannabis at the start of treatment compared to those who did not

    WASH and Health in Sindhupalchowk District of Nepal after the Gorkha Earthquake

    An earthquake of magnitude 7.8 MW and 6.8 MW struck Nepal on 25 April and 12 May, 2015, respectively, which caused massive damage. In such crises, understanding the water, sanitation, and hygiene (WASH) situation is of paramount importance. Therefore, we aimed to assess the WASH situation and its impact on health, particularly in the Sindhupalchowk district. A questionnaire survey and microbial analysis of water samples were conducted. Descriptive statistics and parametric and non-parametric statistical tests were employed. The results revealed that 97.1% of water samples from the source during the pre-monsoon season and 98.5% during the monsoon season had fecal contamination. Similarly, 92.8% of water samples during the pre-monsoon season and 96.7% during the monsoon season at point of use (PoU) had fecal contamination. Furthermore, water consumption was comparatively less during the pre-monsoon season. The increase in water consumption improved hygiene behavior and lowered the prevalence of waterborne diseases. Similarly, less water consumption affected water handling behavior; for example, the cleaning interval of storage vessels was less frequent. An increase in cleaning interval resulted in fecal contamination of water at PoU. The findings of this study can be useful in the review of existing WASH policy and plans and integration with the disaster management plan for disaster risk reduction

    Use of Analgesia in an Emergency Department

    Introduction: Pain is a common presentation to the emergency department but often overlooked with little research done on the topic in Nepal. We did an observational retrospective study on 301 patients in the emergency ward of BP Koirala Institute of Health Sciences with the objective of finding the practice of analgesia. The specific focus was on the time to analgesia, drugs for analgesia and method of pain assessment. Methods: Case file analysis of patients discharged home after presenting with pain was performed. Time to analgesia and other factors were analyzed with descriptive statistics. Results: Diclofenac injection intramuscular (80%) was the commonest analgesic used. Assessment methods and record keeping were poor. Pain in the abdomen was the commonest. The median time to analgesia from triage was 45 minutes (IQR 30 to 80) and the median time to analgesia from doctor evaluation was 40 minutes (IQR 20 to 70). Conclusions: Time to analgesia from triage and doctors assessment in our set up is comparable to others. The quality of documentation is poor. Problems with pain identification and assessment may lead to inadequate analgesia so reinforcing the use of pain descriptor at triage itself with pain score would be helpful in adopting a protocol based management of pain.     Lake Watershed Dynamics and Bathymetry Modeling of Rara and Begnas Lakes in Nepal

    Lake evolution and its changes over time are an evident and easily measurable signal of human activities and climate change impacts in mountain regions. This study presents bathymetric modeling of permanent lakes (Begnas and Rara Lakes) located in two different geographic settings of Nepal. Moreover, temporal changes in land cover and soil erosion of the lake watersheds, as well as climatic trends around these lakes, are assessed. This study supports establishing reference sites for exploring scientific evidence on the impacts of anthropogenic and climate change on lake hydrological systems. Second-order polynomial models best represent the relationship between lake depth and volume. Rara Lake had a maximum depth of 169 m with an area of 10.52 km2 and a volume of 1013.305 million cubic meters (Mm3), whereas Begnas Lake had a maximum depth of 12.5 m with an area of 2.98 ± 0.10 km2 and a water volume of 13.539 Mm3 in the year 2019. Both lake regions are experiencing changes in temperature and rainfall. The area and volume of Rara Lake and its watershed have been relatively stable even with minimal land-cover change during the recent decades. Begnas Lake and its watershed have experienced significant changes in the last few decades. This study concludes that human activities in the Begnas Lake watersheds are the primary source of lake area variation rather than climate change

    Introduction: Trauma is a major and increasing global health concern in the recent world is now the leading cause of death among people less than 18 years old. Methods: This is a retrospective cross sectional study done in Emergency Department of B.P.Koirala Institute of Health Sciences. Patients aged less than 16 years were included with history of Trauma from January 1st 2013 to 31st December 2013.The details of patient were taken from computerized medical records of the hospital. The data Demographic data, Pattern of injury, Mode of injury, Diagnosis and outcomes were tabulated in Microsoft excel. Results: Total 3958 pediatrics patient were enrolled in this study. Male: Female=3.6:1,Mean age 9.6±3.2 years, fall injury & RTAs common modes and fractures of limbs, soft tissue injuries and cut injuries were common patterns. Conclusions: Injury is a serious problem for children in Low Income countries like Nepal
