    Reforming education in Pakistan – tracing global links

    This paper is about tracing global links in national education reforms in Pakistan. The paper tries to describe globalization and its general effects on national policies. It particularly highlights the effects of globalization on education reforms, which are: competitiveness-driven, finance-driven and equity-driven. In light of these global education reform patterns the paper explores the major educational reforms being pursued in Pakistan since 1990s. The paper argues that globalization has seriously challenged the national reform policies, which are now becoming more and more globally driven. Rather than out rightly rejecting all global policies, engaging critically with them is the stance of the authors

    Transforming the Work of Geographical Indications to Decolonize Racialized Labor and Support Agroecology

    Critical scholarship on geographical indications (GIs) has increasingly focused upon their role in fostering development in the Global South. Recent work has drawn welcome attention to issues of governance and sparked new debates about the role of the state in GI regulation. We argue that this new emphasis needs to be coupled with a greater focus upon local social relations of power and interlinked issues of social justice. Rather than see GI regimes as apolitical technical administrative frameworks, we argue that they govern emerging public goods that should be forged to redress extant forms of social inequality and foster the inclusion of marginalized actors in commodity value chains. In many areas of the world, this will entail close attention to the historical specificities of colonial labor relations and their neocolonial legacies, which have entrenched conditions of racialized and gendered dispossession, particularly in plantation economies. Using examples from South Africa and South Asia, we illustrate how GIs conventionally reify territories in a fashion that obscures and/or naturalizes exploitative conditions of labor and unequal access to land based resources, which are legacies of historical disenfranchisement. Like other forms of neoliberal governmentality that support private governance for public ends, however, GIs might be shaped to support new forms of social justice. We show how issues of labor and place-based livelihoods increasingly influence new policy directions within Fair Trade agendas while concerns with “decolonizing” agricultural governance now animate certification initiatives emerging from new social movements. Both initiatives provide models for shaping the governance and regulation of GIs in projects of rural territorial development that encompass principles of rights-based development to further social movements for rural social justice

    Women’s perception and attitude towards male dominancy and controlling behaviors

    Introduction/Background: The study was conducted in urban Karachi, Pakistan to investigate women’s perceptions and attitudes towards male dominancy, female autonomy, and controlling behavior of husbands. Method: This was investigated in a population based study with a cross-sectional design, involving married women aged 25 to 60 years. A Structured questionnaire developed by World Health Organisation (WHO) on violence was used. Community midwives interviewed these married women living in pre-selected low, middle and upper socio-economic areas of urban Karachi, Pakistan.Findings: This study revealed women’s overall perception regarding male dominancy and controlling behavior and highlighted this attitude, as being acceptable to women. It was found that 96.7% (n=734) of females perceived their husband as authority figure in the family and 99.2% (n=753) consider themselves as good wives by being obedient to their husbands. Not only this, 88.9% (n=675) females perceived performing sex unwillingly with their husbands as an obligation. Conclusion: Perceptions of women regarding gender discrimination and male dominancy play a vital role in accepting gender discrimination. This study identified the declining status of women as compared to men in marital life due to various socio-cultural and religious beliefs, which promote controlling behaviour by the male partner.     Comparison of superficial surgical site infection following use of diathermyand scalpel for making skin incision in inguinal hernioplasty

    The method of making surgical incision remains a complex problem. Although controversial, the use of diathermy instead of scalpel for skin incision and underlying tissue dissection is gradually gaining wide acceptance. This is due to the observation that no change in wound complication rate or postoperative pain is reportedwith the use ofDiathermy. However, the fear of excessive scarring and poorwound healing has curtailed itswidespread use for skin incision. The objective of the study is to compare superficial surgical site infection (SSSI) in diathermy and scalpel skin incision in inguinal hernioplasty. Quasi experimental study. Study was conducted at Surgical Unit II, Holy Family Hospital. Rawalpindi from1 Jan. 2008 to 30 September. 2008. Atotal of 80 patients who presented with inguinal hernias were included in the study. Patientswere divided in two groups. Group1: In 40 patients skin incisionwasmadewithDiathermy,Group 2: The other 40 had skin incisionwith scalpel. Themean age of patients in the intervention group (Group 1)was 50 years while in the control group (Group 2) itwas 46 years. 48% patients in Group 1 and 55% in the Group 2 had indirect inguinal hernias. SSSI was noted in 12.5% cases in Group 1 whereas in Group 2 it was 17.5% but this difference was not found to be statistically significant (p value=0.378).     Management and Complications of Arnold Chiari Hydrocephalus at Tertiary Health Care Center

    Objective: to investigate the role of CT and MRI in diagnosis of Arnold chiari malformation of hydrocephalus and its treatment with VP shunt along with complications. Study Design: Prospective study Place and duration: Department of Neurosurgery, DG Khan Medical College from May 4, 2018 to May 4, 2019. Methodology: Fifty patients of congenital hydrocephalus who were not treated previously were selected. Detailed history about disease and clinical examination of patients was performed. Follow ups were done at neurosurgery OPD. SPSS software for data analysis was used and mean ± SD, frequency and percentages were calculated for variables. P value ≤0.05 was considered as significant. Results: Treatment in case of congenital hydrocephalus as VP shunt, intra-aneurysmal coiling, excision of meningomyelocele with VP shunt and suboccipital craniectomy + upper cervical laminectomy (scucl) were observed as 10%, 6%, 8% and 8% respectively.  Twelve percent of patients were not treated. Conclusion: CT and MRI are the main diagnostic tools for diagnosis of Arnold chiari malformation and VP shunt is the treatment of choice.     {HandVoxNet}: {D}eep Voxel-Based Network for {3D} Hand Shape and Pose Estimation from a Single Depth Map

    3D hand shape and pose estimation from a single depth map is a new and challenging computer vision problem with many applications. The state-of-the-art methods directly regress 3D hand meshes from 2D depth images via 2D convolutional neural networks, which leads to artefacts in the estimations due to perspective distortions in the images. In contrast, we propose a novel architecture with 3D convolutions trained in a weakly-supervised manner. The input to our method is a 3D voxelized depth map, and we rely on two hand shape representations. The first one is the 3D voxelized grid of the shape which is accurate but does not preserve the mesh topology and the number of mesh vertices. The second representation is the 3D hand surface which is less accurate but does not suffer from the limitations of the first representation. We combine the advantages of these two representations by registering the hand surface to the voxelized hand shape. In the extensive experiments, the proposed approach improves over the state of the art by 47.8% on the SynHand5M dataset. Moreover, our augmentation policy for voxelized depth maps further enhances the accuracy of 3D hand pose estimation on real data. Our method produces visually more reasonable and realistic hand shapes on NYU and BigHand2.2M datasets compared to the existing approaches

    Backtracking Search Optimization for Collaborative Beamforming in Wireless Sensor Networks

    Due to energy limitation and constraint in communication capabilities, the undesirable high battery power consumption has become one of the major issues in wireless sensor network (WSN). Therefore, a collaborative beamforming (CB) method was introduced with the aim to improve the radiation beampattern in order to compensate the power consumption. A CB is a technique which can increase the sensor node gain and performance by aiming at the desired objectives through intelligent capabilities. The sensor nodes were located randomly in WSN environment. The nodes were designed to cooperate among each other and act as a collaborative antenna array. The configuration of the collaborative nodes was modeled in circular array formation. The position of array nodes was determined by obtaining the optimum parameters pertaining to the antenna array which implemented by using Backtracking Search Optimization Algorithm (BSA). The parameter considered in the project was the side-lobe level minimization. It was observed that, the suppression of side-lobe level for BSA was better compared to the radiation beampattern obtained for conventional uniform circular array

    An evaluation of topical and local anesthesia in phacoemulsification

    Objective: To assess the comparative efficacy of topical and local anesthesia in phacoemulsification.MATERIAL AND Methods: The medical records of 186 men and women between the ages of 45-85 years, who underwent elective cataract surgery by phacoemulsification technique, under the care of one surgeon, over a period of one year, from March 1999-March 2000 were reviewed.Results: Of 186 patients who underwent small incision, self-healing phacoemulsification cataract surgery, 124 received topical and 62 local anesthesia. The most common cataract types were nuclear sclerosis. The duration of surgery with topical anesthesia was shorter. Sutures and eye padding were more frequently applied for procedures done under local anesthesia. Uncorrected visual acuity in the first post-op week was between 20/20-20/50 for 53.6% of the cases done under topical compared to 30.9% in local anesthesia. A similar trend was noted in the visual acuity one month post operatively.CONCLUSION: The uncorrected visual acuity improves faster and the duration of surgery is shorter when topical anesthesia is used

    Cannabinoids for the Treatment of Dermatologic Conditions

    In recent years, cannabinoid (CB) products have gained popularity among the public. The anti-inflammatory properties of CBs have piqued the interest of researchers and clinicians because they represent promising avenues for the treatment of autoimmune and inflammatory skin disorders that may be refractory to conventional therapy. The objective of this study was to review the existing literature regarding CBs for dermatologic conditions. A primary literature search was conducted in October 2020, using the PubMed and Embase databases, for all articles published from 1965 to October 2020. Review articles, studies using animal models, and nondermatologic and pharmacologic studies were excluded. From 248 nonduplicated studies, 26 articles were included. There were 13 articles on systemic CBs and 14 reports on topical CBs. Selective CB receptor type 2 agonists were found to be effective in treating diffuse cutaneous systemic sclerosis and dermatomyositis. Dronabinol showed efficacy for trichotillomania. Sublingual cannabidiol and Δ-9-tetrahydrocannabinol were successful in treating the pain associated with epidermolysis bullosa. Available evidence suggests that CBs may be effective for the treatment of various inflammatory skin disorders. Although promising, additional research is necessary to evaluate efficacy and to determine dosing, safety, and long-term treatment guidelines
