THE RELATIONSHIP BETWEEN THE SPINAL CANAL DIAMETER AND THE NEUROLOGICAL DEFICIT SEVERITY IN SPINAL INJURIES
Amaç: Torakolomber vertebra kırıklı hastalarda travmadan sonra (3 ay içinde) ve geç postoperatif (6 ay içinde) dönemde Bilgisayarlı Tomografi (BT) tetkiki ile spinal kanal daralma oranını ve aynı dönemlerde nörolojik bakı ile nörolojik yaralanma düzeylerini belirleyerek; aralarındaki korelasyonu ve BT'nin bu değerlendirmedeki yerini belirlemeyi amaçladık. Gereç ve yöntem: Torakolomber vertebra kırığı tanısı almış ve cerrahi tedavi görmüş 28 hasta preoperatif ve geç postoperatif dönemde BT ile incelendi. Ayrıca her hastanın Frankel Sınıflamasına göre nörolojik değerlendirmeleri yapıldı. Her hastada kanal işgal yüzdeleri tek tek belirlenip istatistiksel hesaplaması yapıldı. Bulgular: Olguların %61'i yüksekten düşme, %39'u trafik kazasıydı. Ortalama yaş 34,6 olup hastaların %57'si erkek, %43'ü kadındı. 28 hastanın 18'inde (%64,3) nörolojik yaralanma mevcuttu. Frankel A'dan Frankel E'ye kadar beş ayrı nörolojik derecede ortalama kanal işgal oranı sırasıyla %66, %51, %37, %21 ve %27 idi. Kanal darlığının artması ile nörolojik yaralanmanın ağırlaşması arasında doğru orantılı ve anlamlı bir ilişki bulundu. Geç dönemde kanal işgal oranı ile nörolojik durum arasında ise anlamlı bir ilişki bulunmadı. Yaralanmanın seviyesinin de nörolojik yaralanma derecesini etkilediği görüldü. Sonuç: Torakolomber vertebra kırıklarında nörolojik yaralanma primer olarak ilk travmanın ağırlığı ile belirlenmektedir. Yaralanmanın seviyesiyle birlikte ilk travma anında oluşan kanaldaki darlığın miktarı nörolojik yaralanmanın ağırlığını belirleyici unsurlardandır. Spinal kanalın görüntülenmesinde, kanal işgal oranının tesbitinde, tedavi planlanması ve postoperatif komplikasyonların değerlendirilmesinde BT üstün ve değerli bir görüntüleme yöntemidir. Objective: Patients with thoracolomber fractures were evaluated with c.t scan before and after operation in order to detect the degree of obliteration in the spinal canal. Also neurological evaluation was done. The relationship between spinal canal obliteration and the neurolojical deficit and the role of CT in this evaluation was investigated. Material and method: Pre and postoperative CT scans were performed in 28 cases with vertebra fractures. Also they were evaluated neurologically according to the Frankel Classification. Results: 61% of the patients fell from a high place, 39% had a traffic accident. Avarage age was 34.6. 57% was male, 43% was female. Spinal canal obliteration per-cent was measured for each patient. 64.3% of the patients had neurological deficit. Spinal canal obliteration was classified neurologically as Frankel A, B, C, D, E and the ratios were as follows in the same order 66%, 51%, 37%, 21%and 27%. The spinal canal narrowing and neurological deficit severity was correlated well. But there wasn't such a correlation at the late stage. Also the level of trauma affected the neurological deficit severity. Conclusion: In thoracolomber vertebra fractures neurological deficit is primerally correlated with the severity of trauma at first. The spinal narrowing and the level of trauma verrifies the severity of neurological deficit. CT examination is a good guide for planning surgery, finding out the spinal narrowing ratio and detecting the postoperative complications
A giant deep palmar lipoma presented with compressive neuropathy
Lipomlar, en yaygın benign yumusak doku tümörlerinden biridir. Üst ekstremite sıklıkla yerlesim bölgesi
olmakla birlikte, derin palmar yerlesim nadirdir ve saptandıklarında bu düzeyde genellikle çevre dokulara basıya
neden olmazlar. 42 yasında kadın olgu, sag el palmar yüzde, karpo-metakarpal bölgede, büyük boyutlarda agrısız
yumusak doku sisligi, 1-4. parmaklarda agrı, parestezi, hareketlerde kısıtlılık ve basparmakta güç kaybı
sikayetleri ile basvurdu. Ultrasonografide yag içerikli kitle lezyonu saptandı. Manyetik rezonans görüntülemede,
derin palmar bölgede, iyi sınırlı, proksimalinde karpal tünele dogru uzanımgösteren, median sinire basıya neden
olan 6x5x4 cm boyutlarda lipom saptandı. Klinik bulgu vermesi nedeniyle, operasyon kararı verilen olguda,
operasyon ardından klinik sikayetleri geriledi.Lipomas are one of the commonest benign soft tissue tumors. Lipomas are common in upper limbs, but they
rarely occur in the deep palm and usually do not compress the surrounding structures when diagnosed. A42 years
old female patient presented with a large painless swelling at right palmar metacarpophalengial area, paresthesia
and pain of the 1-4. fingers, movement limitation and thumb weakness. Ultrasound examination suggested the
swelling to be of fatty nature. MRI showed a well circumscribed soft tissue swelling (6x5x4cm) in the deep
palmar, carpal-metacarpal space, pressuring the median nerve. The patient was operated due to the presence of the
clinical symptoms and become symptom free after surgery
COMPARISON OF COMPUTERIZED TOMOGRAPHIC AND HISTOPATHOLOGICAL ,TUMOR (T) STAGING OF LARYNGEAL CANCERS
Amaç: Larenks kanserlerinde bilgisayarlı tomografi (BT) bulgularının ameliyat sonrası histopatolojik sonuçlarla karşılaştırılması. Gereç ve yöntem: Biopsi ile larenks kanseri tanısı almış yaş ortalaması 54,5 olan, 49'u erkek 52 hasta ameliyat öncesi BT tetkiki ile tümör lokalizasyonu (T) ve derin invazyon (preepiglottik ve paraglottik boşluk, subglottik alan ve kıkırdaklar) yönünden değerlendirildi. Sonuçlar patolojik (p) bakı sonuçları ile karşılaştırıldı. Bulgular: Doğruluk oranı glottik yerleşimli olanlarda %80, supraglottik yerleşimli olanlarda %64, transglottik olanlarda %76 olup, genel doğruluk oranı %73 olarak bulundu. Sonuç: T3 ve T4 evredeki tümörlerde BT evre doğruluk oranı, T1 ve T2 evre tümörlere göre yüksek bulundu. Bu doğruluk oranları literatürdekine paraleldir. Klinik muayeneyle birlikte BT değerlendirilmesi histopatolojik evrelemenin doğruluğunu arttırmaktadır. Objective: We compare the findings of computerized tomographic (CT) imaging with histopathologic results in larengeal cancer. Patients and method: Fifty-two patients (49 males, mean age 54,5) were evaluated preoperatively by CT with regard to localization (T) and deep involvement (preepiglottic space, laryngeal cartilages, paraglottic and subglottic areas). The results were correlated with histopathologic (p) findings. Results: The accuracy of CT were 80% for glottic; 64% for supraglottic, 76% for transglottic region tumors. Overall accuracy was 73%. Conclusion: Histopathologic and CT staging results are highly correlated in T3 and T4 stage tumors than T1 and T2 stage tumors, similar to those in the literature. The accuracy of histopathologic staging enhances by concomitant use of clinical and radiologic findings
Yüksek risk grubu olgularda meme MR görüntülemenin mamografik bulgulara katkısı
Objective: We aimed to examine the contribution of breast
magnetic resonance imaging (MRI) to mammography in
high-risk patients with partial mastectomy, and/or with a
family history.
Material and Methods: 80 patients were scanned with
mammography and MRI at our hospital. 30 of the patients
had partial mastectomy due to the diagnosis of breast
cancer. 52 patients had family history of breast cancer.
Mammography scannings were performed under normal
circumstances in standard craniocaudal (CC) and
mediolateral (MLO) positions or in some special circumstances,
scanings were performed using spot and magnification
graphies. MRI scannings were performed using 1.5
Tesla MRI scanner with dual breast coil.
Results: Fifty two of the patients enrolled in this study had
a relative with breast cancer. 30 (37,5 %) out of 80 patients
had history of breast cancer before. In 12 (15 %) out of 80
patients, the advantage of breast MRI to mammography
could not be proven. In 54 (67,5 %) of the patients breast
MRI had an advantage over mammography. In 5 (6,5 %) of
the patients MRI had false negative results when compared
with the pathology results. When compared with the pathology
results in 9 (19 %) patients MRI scanning results
were false positive.
Conclusion: In this study, it was concluded that breast MRI
scanning has reasonable advantage when compared to
mammography for the patients who had breast cancer
diagnosis or who has a first degree relative with breast
cancer.Amaç: Meme kanseri nedeniyle parsiyel mastektomi veya
aile öyküsü gibi, meme kanseri için yüksek risk grubu olgularda,
meme manyetik rezonans görüntüleme (MRG)’nin
mamografik görüntülemeye katkısını değerlendirmeyi
amaçladık.
Gereç ve Yöntem: Seksen olgu, bölümümüzde mamografi
ve MRG ile değerlendirildi. 30 olgu, meme kanseri tanısı ile
parsiyel mastektomi geçirmişti. 52 olgu, birinci derecede
akrabada meme kanseri öyküsüne sahipti. Mamografik
görüntüleme, standart kraniokaudal ve mediolateral pozisyonlarda
elde olundu. Gerekli durumlarda spot ve magnifiye
grafiler eklendi. MR görüntüleri, 1,5 Tesla MRG cihazında,
dual meme koili ile elde edildi.
Bulgular: Elli iki olguda aile öyküsü mevcuttu. 30(% 37,5)
olguda daha önce geçirilmiş meme kanseri ve tedavi öyküsü
vardı. 80 olgudan, 12'sinde (% 15), meme MRG görüntülemenin
mamografi bulgularına herhangi katkısı veya ek

Idiopathic Granulomatous Mastitis Associated with Erythema Nodosum
Background: Idiopathic granulomatous mastitis (IGM) is an uncommon benign chronic inflammatory breast disease, and erythema nodosum (EN) is an extremely rare systemic manifestation of IGM. Here, we report a rare case of IGM accompanied by EN.
Case Report: A 32-year-old patient was admitted to our clinic with a history of a tender mass in the right breast. On physical examination, the right breast contained a hard, tender mass in the lower half with in-drawing of the nipple. She had florid EN affecting both legs. She was evaluated with mammography, ultrasound, power Doppler ultrasound, non-enhancing magnetic resonance imaging (MRI), dynamic contrast-enhanced MRI, fine needle aspiration biopsy (FNAB) and excisional biopsy. Time-intensity curves showed a type II pattern on dynamic contrast-enhanced MRI, which has an intermediate probability for malignancy. The FNAB reported a benign cytology suggestive of a granulomatous inflammation, which was also supported by the histopathological findings. A partial mastectomy was performed following medical treatment. There was no recurrence at 1-year follow-up.
Conclusion: IGM should be considered in the differential diagnosis of EN. Although histopathological examination remains the only method for the definite diagnosis of IGM, MRI can be helpful in the diagnosis or differentiation of benign lesions from malignant ones
Postmenopausal hormone replacement therapy; changes of the mammographic findings
Amaç: HRT’ nin mammografik değişikliklere, özellikle dansite artışına yol açtığı bilinmektedir. Mammografik dansite artışı da mammografik duyarlılığı azaltmaktadır. Çalışmamızda, tedavi protokolleri ve demografik bilgileri de göz önüne alarak HRT’ nin mammografik bulgulara etkisini göstermeyi amaçladık. Gereç ve Yöntem: Menapoz kliniğine başvuran 3550 olgudan en az 1 yıl HRT alan, en az 1 yıldır menapozda olduğu bilinen 1025 olguyu değerlendirmeye aldık. 111 HRT almayan kontrol olgu grubu seçtik. En az 1 yıl aralıklarla alınan 2 mammogramını geriye dönük değerlendirdik. Dansitometrik değerlendirmede (nicel değerlendirme), Wolfe klasifikasyonunda HRT öncesi ve sonrası bulgularını karşılaştırdık. Yeni gelişen lezyonları, varolan lezyonlarda izlenen değişiklikleri değerlendirdik. Değerlendirmeyi, devamlı kombine, siklik kombine tedavi alan, östrojen-konjuge östrojen tedavisi alan, tibolone-sentetik preparat alan ve kontrol grubu olmak üzere HRT protokollerine göre yaptık. Bulgular: En belirgin dansite artışı devamlı kombine tedavi alan grupta izlendi. (%49.8 ve p=0,001). Östrojen alan grupta izlenen dansite artışı devamlı kombine tedavi alan gruptan sonra en belirgin olarak izlenmekteydi (%42 ve p=0,026). Tibolone alan grupta dansite artışı tüm değerlendirmelerde istatistiksel olarak anlamlı değildi (%28 ve p>0,05). Kontrol grubunda dansite değişikliği azalma yönünde ve belirgindi (%44 ve p<0,0000). Östrojen alan grupta, HRT sonrası yeni lezyon gelişimi istatistiksel olarak anlamlıydı(%5, ve p<0,02). (2 olgu DCIS, 2 olgu invaziv duktal Ca) Tibolone alan grupta lezyonlarda gerileme yönünde anlamlı sonuç saptandı. (%3,5 ve p<0,05). Sonuç: HRT protokollerinden, devamlı kombine tedavinin meme dansitesini belirgin derecede arttırdığını ve tibolone’ un meme dansitesine en az etkili preparat olduğunu saptadık. Malign yeni lezyon gelişimi östrojen alan grupta anlamlıydı.Objective: HRT is known to result mammographic alterations, especially elevating the density that decreases the sensitivity of the mammography. In this study we have aimed to display the effect of HRT on the mammographic fi ndings in consideration with the therapy protocols. Material and Methods: 1025 cases who were in menopause for at least 1 year and administered HRT for at least 1 year were chosen as a study group that was compared to the control group which includes 111 cases who did not received HRT. Two consecutive mammographies were retrospectively evaluated. Densitometric measurements (quantitative method), Wolfe classifi cation fi ndings were compared between before and after HRT. The new developing lesions, alterations in the present lesions were also evaluated by dividing the cases according to protocols; continues combined, cyclic combined, estrogenconjugated estrogen, tibolone, the control group. Results: The most prominent increase in the density was in the continues combined group(%49,p=0.001). Estrogen therapy group was observed. Tibolone had the minimal effect(%28,p>0.05). The increase in the density of the tibolone group was statistically insignifi cant in all evaluations. In the control group, the alteration was towards decrease in the density (%44, p<0.0000).. In the estrogen therapy group, the development of malignant lesions was statistically signifi cant (%5, p<0.02). (2 case DCIS, 2 case IDC). In the tibolone group the regression in the lesions was signifi cant. Conclusion: Continues combined therapy increases the density of breast significantly, tibolone is the minimal effective drug.
