Equipment review: Mechanical effects of heat-moisture exchangers in ventilated patients
Although they represent a valuable alternative to heated humidifiers, artificial noses have unfavourable mechanical effects. Most important of these is the increase in dead space, with consequent increase in the ventilation requirement. Also, artificial noses increase the inspiratory and expiratory resistance of the apparatus, and may mildly increase intrinsic positive end-expiratory pressure. The significance of these effects depends on the design and function of the artificial nose. The pure humidifying function results in just a moderate increase in dead space and resistance of the apparatus, whereas the combination of a filtering function with the humidifying function may critically increase the volume and the resistance of the artificial nose, especially when a mechanical filter is used. The increase in the inspiratory load of ventilation that is imposed by artificial noses, which is particularly significant for the combined heat-moisture exchanger filters, should be compensated for by an increase either in ventilator output or in patient's work of breathing. Although both approaches can be tolerated by most patients, some exceptions should be considered. The increased pressure and volume that are required to compensate for the artificial nose application increase the risk of barotrauma and volutrauma in those patients who have the most severe alterations in respiratory mechanics. Moreover, those patients who have very limited respiratory reserve may not be able to compensate for the inspiratory work imposed by an artificial nose. When we choose an artificial nose, we should take into account the volume and resistance of the available devices. We should also consider the mechanical effects of the artificial noses when setting mechanical ventilation and when assessing a patient's ability to breathe spontaneously
Durable graphite oxide nanocoating for high performing flame retarded foams
Recent developments in the design of water-based coatings encompassing platelet-like nanoparticles have clearly demonstrated the flame retardant potential of this approach for open cell flexible foams. However, the relatively high number of deposition steps required and the limited reports on the durability of the deposited coatings to multiple compression cycles currently represent the main constraints to this approach. This paper addresses these limitations by exploiting a few steps deposition procedure to produce coatings with durable flame retardant properties. Graphite oxide, sodium alginate and sodium hexametaphosphate were combined in a continuous protective coating that extends to the complex three-dimensional structure of the foam. The flame retardant properties of the coatings were evaluated before and after 1000 compression cycles. Even after such multiple deformations, the coated foams showed no melt dripping and self-extinguishment during flammability tests, as well as a highly reduced heat release rates (-70%) and total smoke release (-70%) during cone calorimetry tests. Furthermore, the ability to withstand the penetration of an impinging flame focused on one side of the coated foam for more than 5 min was also maintained. These results clearly demonstrate the durability of the coated foams, opening to real life application fields such as transports seats where high levels of flame retardancy must be maintained for long time under frequent mechanical stress
Background modeling for video sequences by stacked denoising autoencoders
Nowadays, the analysis and extraction of relevant information in visual data flows is of paramount importance. These images sequences can last for hours, which implies that the model must adapt to all kinds of circumstances so that the performance of the system does not decay over time. In this paper we propose a methodology for background modeling and foreground detection, whose main characteristic is its robustness against stationary noise. Thus, stacked denoising autoencoders are applied to generate a set of robust characteristics for each region or patch of the image, which will be the input of a probabilistic model to determine if that region is background or foreground. The evaluation of a set of heterogeneous sequences results in that, although our proposal is similar to the classical methods existing in the literature, the inclusion of noise in these sequences causes drastic performance drops in the competing methods, while in our case the performance stays or falls slightly.Universidad de Málaga. Polyelectrolyte-coated mesoporous bioactive glasses via layer-by-layer deposition for sustained co-delivery of therapeutic ions and drugs
In the field of bone regeneration, considerable attention has been addressed towards the use of mesoporous bioactive glasses (MBGs), as multifunctional therapeutic platforms for advanced medical devices. In fact, their extremely high exposed surface area and pore volume allow to load and the release of several drugs, while their framework can be enriched with specific therapeutic ions allowing to boost the tissue regeneration. However, due to the open and easily accessible mesopore structure of MBG, the release of the incorporated therapeutic molecules shows an initial burst effect leading to unsuitable release kinetics. Hence, a still open challenge in the design of drug delivery systems based on MBGs is the control of their release behavior. In this work, Layer-by-layer (LbL) deposition of polyelectrolyte multi-layers was exploited as a powerful and versatile technique for coating the surface of Cu-substituted MBG nanoparticles with innovative multifunctional drug delivery systems for co-releasing of therapeutic copper ions (exerting pro-angiogenic and anti-bacterial effects) and an anti-inflammatory drug (ibuprofen). Two different routes were investigated: in the first strategy, chitosan and alginate were assembled by forming the multi-layered surface, and, successively, ibuprofen was loaded by incipient wetness impregnation, while in the second approach, alginate was replaced by ibuprofen, introduced as polyelectrolyte layer. Zeta-potential, TGA and FT-IR spectroscopy were measured after the addition of each polyelectrolyte layer, confirming the occurrence of the stepwise deposition. In addition, the in vitro bioactivity and the ability to modulate the release of the cargo were evaluated. The polyelectrolyte coated-MBGs were proved to retain the peculiar ability to induce hydroxyapatite formation after 7 days of soaking in Simulated Body Fluid. Both copper ions and ibuprofen were co-released over time, showing a sustained release profile up to 14 days and 24 h, respectively, with a significantly lower burst release compared to the bare MBG particles
Urinary estrogen metabolites and prostate cancer : a case-control study and meta-analysis
Objective: To investigate prostate cancer (Pca) risk in relation to estrogen metabolism, expressed as urinary 2-hydroxyestrone (2-OHE1), 16α-hydroxyestrone (16α-OHE1) and 2-OHE1 to 16α-OHE1 ratio.
Methods: We conducted a case-control study within the Western New York Health Cohort Study (WNYHCS) from 1996 to 2001. From January 2003 through September 2004, we completed the re-call and follow-up of 1092 cohort participants. Cases (n = 26) and controls (n = 110) were matched on age, race and recruitment period according to a 1:4 ratio. We used the unconditional logistic regression to compute crude and adjusted odds ratios (OR) and 95% confident interval (CI) of Pca in relation to 2-OHE1, 16αOHE1 and 2-OHE1 to 16α-OHE1 by tertiles of urine concentrations (stored in a biorepository for an average of 4 years). We identified age, race, education and body mass index as covariates. We also conducted a systematic review of the literature which revealed no additional studies, but we pooled the results from this study with those from a previously conducted case-control study using the DerSimonian-Laird random effects method.
Results: We observed a non-significant risk reduction in the highest tertile of 2-OHE1 (OR 0.72, 95% CI 0.25-2.10). Conversely, the odds in the highest tertile of 16α-OHE1 showed a non-significant risk increase (OR 1.76 95% CI 0.62-4.98). There was a suggestion of reduced Pca risk for men in the highest tertile of 2-OHE1 to 16α-OHE1 ratio (OR 0.56, 95% CI 0.19-1.68). The pooled estimates confirmed the association between an increased Pca risk and higher urinary levels of 16α-OHE1 (third vs. first tertile: OR 1.82, 95% CI 1.09-3.05) and the protective effect of a higher 2-OHE 1 to 16α-OHE1 ratio (third vs. first tertile: OR 0.53, 95% CI 0.31-0.90).
Conclusion: Our study and the pooled results provide evidence for a differential role of the estrogen hydroxylation pathway in Pca development and encourage further study
Background modeling by shifted tilings of stacked denoising autoencoders
The effective processing of visual data without interruption is currently of supreme importance. For that purpose, the analysis system must adapt to events that may affect the data quality and maintain its performance level over time. A methodology for background modeling and foreground detection, whose main characteristic is its robustness against stationary noise, is presented in the paper. The system is based on a stacked denoising autoencoder which extracts a set of significant features for each patch of several shifted tilings of the video frame. A probabilistic model for each patch is learned. The distinct patches which include a particular pixel are considered for that pixel classification. The experiments show that classical methods existing in the literature experience drastic performance drops when noise is present in the video sequences, whereas the proposed one seems to be slightly affected. This fact corroborates the idea of robustness of our proposal, in addition to its usefulness for the processing and analysis of continuous data during uninterrupted periods of time.Universidad de Málaga.
