Home parenteral nutrition with an omega-3-fatty-acid-enriched MCT/LCT lipid emulsion in patients with chronic intestinal failure (the HOME study):study protocol for a randomized, controlled, multicenter, international clinical trial
BACKGROUND: Home parenteral nutrition (HPN) is a life-preserving therapy for patients with chronic intestinal failure (CIF) indicated for patients who cannot achieve their nutritional requirements by enteral intake. Intravenously administered lipid emulsions (ILEs) are an essential component of HPN, providing energy and essential fatty acids, but can become a risk factor for intestinal-failure-associated liver disease (IFALD). In HPN patients, major effort is taken in the prevention of IFALD. Novel ILEs containing a proportion of omega-3 polyunsaturated fatty acids (n-3 PUFA) could be of benefit, but the data on the use of n-3 PUFA in HPN patients are still limited. METHODS/DESIGN: The HOME study is a prospective, randomized, controlled, double-blind, multicenter, international clinical trial conducted in European hospitals that treat HPN patients. A total of 160 patients (80 per group) will be randomly assigned to receive the n-3 PUFA-enriched medium/long-chain triglyceride (MCT/LCT) ILE (Lipidem/Lipoplus® 200 mg/ml, B. Braun Melsungen AG) or the MCT/LCT ILE (Lipofundin® MCT/LCT/Medialipide® 20%, B. Braun Melsungen AG) for a projected period of 8 weeks. The primary endpoint is the combined change of liver function parameters (total bilirubin, aspartate transaminase and alanine transaminase) from baseline to final visit. Secondary objectives are the further evaluation of the safety and tolerability as well as the efficacy of the ILEs. DISCUSSION: Currently, there are only very few randomized controlled trials (RCTs) investigating the use of ILEs in HPN, and there are very few data at all on the use of n-3 PUFAs. The working hypothesis is that n-3 PUFA-enriched ILE is safe and well-tolerated especially with regard to liver function in patients requiring HPN. The expected outcome is to provide reliable data to support this thesis thanks to a considerable number of CIF patients, consequently to broaden the present evidence on the use of ILEs in HPN. miRNA contributions to pediatric-onset multiple sclerosis inferred from GWAS.
ObjectiveOnset of multiple sclerosis (MS) occurs in childhood for approximately 5% of cases (pediatric MS, or ped-MS). Epigenetic influences are strongly implicated in MS pathogenesis in adults, including the contribution from microRNAs (miRNAs), small noncoding RNAs that affect gene expression by binding target gene mRNAs. Few studies have specifically examined miRNAs in ped-MS, but individuals developing MS at an early age may carry a relatively high burden of genetic risk factors, and miRNA dysregulation may therefore play a larger role in the development of ped-MS than in adult-onset MS. This study aimed to look for evidence of miRNA involvement in ped-MS pathogenesis.MethodsGWAS results from 486 ped-MS cases and 1362 controls from the U.S. Pediatric MS Network and Kaiser Permanente Northern California membership were investigated for miRNA-specific signals. First, enrichment of miRNA-target gene network signals was evaluated using MIGWAS software. Second, SNPs in miRNA genes and in target gene binding sites (miR-SNPs) were tested for association with ped-MS, and pathway analysis was performed on associated target genes.ResultsMIGWAS analysis showed that miRNA-target gene signals were enriched in GWAS (P = 0.038) and identified 39 candidate biomarker miRNA-target gene pairs, including immune and neuronal signaling genes. The miR-SNP analysis implicated dysregulation of miRNA binding to target genes in five pathways, mainly involved in immune signaling.InterpretationEvidence from GWAS suggests that miRNAs play a role in ped-MS pathogenesis by affecting immune signaling and other pathways. Candidate biomarker miRNA-target gene pairs should be further studied for diagnostic, prognostic, and/or therapeutic utility
Universality in Systems with Power-Law Memory and Fractional Dynamics
There are a few different ways to extend regular nonlinear dynamical systems
by introducing power-law memory or considering fractional
differential/difference equations instead of integer ones. This extension
allows the introduction of families of nonlinear dynamical systems converging
to regular systems in the case of an integer power-law memory or an integer
order of derivatives/differences. The examples considered in this review
include the logistic family of maps (converging in the case of the first order
difference to the regular logistic map), the universal family of maps, and the
standard family of maps (the latter two converging, in the case of the second
difference, to the regular universal and standard maps). Correspondingly, the
phenomenon of transition to chaos through a period doubling cascade of
bifurcations in regular nonlinear systems, known as "universality", can be
extended to fractional maps, which are maps with power-/asymptotically
power-law memory. The new features of universality, including cascades of
bifurcations on single trajectories, which appear in fractional (with memory)

Dark Force Detection in Low Energy e-p Collisions
We study the prospects for detecting a light boson X with mass m_X < 100 MeV
at a low energy electron-proton collider. We focus on the case where X
dominantly decays to e+ e- as motivated by recent "dark force" models. In order
to evade direct and indirect constraints, X must have small couplings to the
standard model (alpha_X 10 MeV).
By comparing the signal and background cross sections for the e- p e+ e- final
state, we conclude that dark force detection requires an integrated luminosity
of around 1 inverse attobarn, achievable with a forthcoming JLab proposal.Comment: 38 pages, 19 figures; v2, references adde
Investigating the missing data mechanism in quality of life outcomes: a comparison of approaches
Background: Missing data is classified as missing completely at random (MCAR), missing at
random (MAR) or missing not at random (MNAR). Knowing the mechanism is useful in identifying
the most appropriate analysis. The first aim was to compare different methods for identifying this
missing data mechanism to determine if they gave consistent conclusions. Secondly, to investigate
whether the reminder-response data can be utilised to help identify the missing data mechanism.
Methods: Five clinical trial datasets that employed a reminder system at follow-up were used.
Some quality of life questionnaires were initially missing, but later recovered through reminders.
Four methods of determining the missing data mechanism were applied. Two response data
scenarios were considered. Firstly, immediate data only; secondly, all observed responses
(including reminder-response).
Results: In three of five trials the hypothesis tests found evidence against the MCAR assumption.
Logistic regression suggested MAR, but was able to use the reminder-collected data to highlight
potential MNAR data in two trials.
Conclusion: The four methods were consistent in determining the missingness mechanism. One
hypothesis test was preferred as it is applicable with intermittent missingness. Some inconsistencies between the two data scenarios were found. Ignoring the reminder data could potentially give a distorted view of the missingness mechanism. Protecting eyewitness evidence: Examining the efficacy of a self-administered interview tool
Given the crucial role of eyewitness evidence, statements should be obtained as soon as possible after an incident. This is not always achieved due to demands on police resources. Two studies trace the development of a new tool, the Self-Administered Interview (SAI), designed to elicit a comprehensive initial statement. In Study 1, SAI participants reported more correct details than participants who provided a free recall account, and performed at the same level as participants given a Cognitive Interview. In Study 2, participants viewed a simulated crime and half recorded their statement using the SAI. After a delay of 1 week, all participants completed a free recall test. SAI participants recalled more correct details in the delayed recall task than control participants
Rotation Curves of Spiral Galaxies
Rotation curves of spiral galaxies are the major tool for determining the
distribution of mass in spiral galaxies. They provide fundamental information
for understanding the dynamics, evolution and formation of spiral galaxies. We
describe various methods to derive rotation curves, and review the results
obtained. We discuss the basic characteristics of observed rotation curves in
relation to various galaxy properties, such as Hubble type, structure,

Two-Nucleon Momentum Distributions Measured in 3He(e,e'pp)n
We have measured the 3He(e,e'pp)n reaction at 2.2 GeV over a wide kinematic
range. The kinetic energy distribution for `fast' nucleons (p > 250 MeV/c)
peaks where two nucleons each have 20% or less, and the third nucleon has most
of the transferred energy. These fast pp and pn pairs are back-to-back with
little momentum along the three-momentum transfer, indicating that they are
spectators. Experimental and theoretical evidence indicates that we have
measured distorted two-nucleon momentum distributions by striking the third
nucleon and detecting the spectator correlated pair.Comment: 6 pages, 5 figures, submitted to PR
Exclusive electroproduction on the proton at CLAS
The reaction has been measured, using the 5.754
GeV electron beam of Jefferson Lab and the CLAS detector. This represents the
largest ever set of data for this reaction in the valence region. Integrated
and differential cross sections are presented. The , and
dependences of the cross section are compared to theoretical calculations based
on -channel meson-exchange Regge theory on the one hand and on quark handbag
diagrams related to Generalized Parton Distributions (GPDs) on the other hand.
The Regge approach can describe at the 30% level most of the features
of the present data while the two GPD calculations that are presented in this
article which succesfully reproduce the high energy data strongly underestimate
the present data. The question is then raised whether this discrepancy
originates from an incomplete or inexact way of modelling the GPDs or the
associated hard scattering amplitude or whether the GPD formalism is simply
inapplicable in this region due to higher-twists contributions, incalculable at
