Interrelations between Stochastic Equations for Systems with Pair Interactions
Several types of stochastic equations are important in thermodynamics,
chemistry, evolutionary biology, population dynamics and quantitative social
science. For systems with pair interactions four different types of equations
are derived, starting from a master equation for the state space: First,
general mean value and (co)variance equations. Second, Boltzmann-like
equations. Third, a master equation for the configuration space allowing
transition rates which depend on the occupation numbers of the states. Fourth,
a Fokker-Planck equation and a ``Boltzmann-Fokker-Planck equation''. The
interrelations of these equations and the conditions for their validity are
worked out clearly. A procedure for a selfconsistent solution of the nonlinear
equations is proposed. Generalizations to interactions between an arbitrary
number of systems are discussed.Comment: For related work see

The Origins of Phase Transitions in Small Systems
The identification and classification of phases in small systems, e.g.
nuclei, social and financial networks, clusters, and biological systems, where
the traditional definitions of phase transitions are not applicable, is
important to obtain a deeper understanding of the phenomena observed in such
systems. Within a simple statistical model we investigate the validity and
applicability of different classification schemes for phase transtions in small
systems. We show that the whole complex temperature plane contains necessary

Utilisation of Postnatal Care among Rural Women in Nepal
Background: Postnatal care is uncommon in Nepal, and where it is available the quality is often poor. Adequate utilisation of postnatal care can help reduce mortality and morbidity among mothers and their babies. Therefore, our study assessed the utilisation of postnatal care at a rural community level.
Methods: A descriptive, cross-sectional study was carried out in two neighbouring villages in early 2006. A total of 150 women who had delivered in the previous 24 months were asked to participate in the study using a semi-structured questionnaire.
Results: The proportion of women who had received postnatal care after delivery was low (34%). Less than one in five women (19%) received care within 48 hours of giving birth. Women in one village had less access to postnatal care than women in the neighbouring one. Lack of awareness was the main barrier to the utilisation of postnatal care.
The woman's own occupation and ethnicity, the number of pregnancies and children and the husband's socio-economic status, occupation and education were significantly associated with the utilisation of postnatal care.
Multivariate analysis showed that wealth as reflected in occupation and having attended antenatal are important factors associated with the uptake of postnatal care. In addition, women experiencing health problems appear strongly motivated to seek postnatal care.
Conclusion: The postnatal care has a low uptake and is often regarded as inadequate in Nepal. This is an important message to both service providers and health-policy makers. Therefore, there is an urgent need to assess the actual quality of postnatal care provided. Also there appears to be a need for awareness-raising programmes highlighting the availability of current postnatal care where this is of sufficient quality

Complete Experimental Structure Determination of the p(3x2)pg Phase of Glycine on Cu{110}
We present a quantitative low energy electron diffraction (LEED) surface-crystallograpic
study of the complete adsorption geometry of glycine adsorbed on Cu{110} in the ordered
p(3×2) phase. The glycine molecules form bonds to the surface through the N atoms of the
amino group and the two O atoms of the de-protonated carboxylate group, each with separate
Cu atoms such that every Cu atom in the first layer is involved in a bond. Laterally, N atoms are
nearest to the atop site (displacement 0.41 Å). The O atoms are asymmetrically displaced from
the atop site by 0.54 Å and 1.18 Å with two very different O-Cu bond lengths of 1.93 Å and
2.18 Å. The atom positions of the upper-most Cu layers show small relaxations within 0.07 Å
of the bulk-truncated surface geometry. The unit cell of the adsorbate layer consists of two
glycine molecules, which are related by a glide-line symmetry operation. This study clearly
shows that a significant coverage of adsorbate structures without this glide-line symmetry must
be rejected, both on the grounds of the energy dependence of the spot intensities (LEED-IV
curves) and of systematic absences in the LEED pattern
Neutron Scattering and Its Application to Strongly Correlated Systems
Neutron scattering is a powerful probe of strongly correlated systems. It can
directly detect common phenomena such as magnetic order, and can be used to
determine the coupling between magnetic moments through measurements of the
spin-wave dispersions. In the absence of magnetic order, one can detect diffuse
scattering and dynamic correlations. Neutrons are also sensitive to the
arrangement of atoms in a solid (crystal structure) and lattice dynamics
(phonons). In this chapter, we provide an introduction to neutrons and neutron
sources. The neutron scattering cross section is described and formulas are
given for nuclear diffraction, phonon scattering, magnetic diffraction, and
magnon scattering. As an experimental example, we describe measurements of
antiferromagnetic order, spin dynamics, and their evolution in the

Nonequilibrium relaxation of the two-dimensional Ising model: Series-expansion and Monte Carlo studies
We study the critical relaxation of the two-dimensional Ising model from a
fully ordered configuration by series expansion in time t and by Monte Carlo
simulation. Both the magnetization (m) and energy series are obtained up to
12-th order. An accurate estimate from series analysis for the dynamical
critical exponent z is difficult but compatible with 2.2. We also use Monte
Carlo simulation to determine an effective exponent, z_eff(t) = - {1/8} d ln t
/d ln m, directly from a ratio of three-spin correlation to m. Extrapolation to
t = infinity leads to an estimate z = 2.169 +/- 0.003.Comment: 9 pages including 2 figure
Vaginal noise: prevalence, bother and risk factors in a general female population aged 45–85 years
Item does not contain fulltextINTRODUCTION AND HYPOTHESIS: Vaginal noise (VN) is a symptom of pelvic floor (PF) dysfunction and has been described in a few studies. No other risk factors have been described besides parity and pelvic organ prolapse (POP). Underlying mechanisms of VN are unclear. Aims of this study were to describe prevalence, bother and relation between VN and PF (muscle)(dys)function. METHODS: A cross-sectional study was performed on a general population of 2,921 women (aged 45-85 years). Questionnaires were filled in by 1,397 women, and 800 were selected at random to undergo vaginal examination for POP Quantification and PF muscle function assessment. Chi-square tests, Student's t test and multivariate logistic regression were performed (P < 0.05). RESULTS: Response rate was 62.7%. Prevalence of VN was 12.8%; 72.1% reported only a little bother. Odds ratios for parity and solid stool were high. CONCLUSIONS: VN was strongly related to many symptoms of pelvic floor dysfunction, but it was only causing a little bother
Treatment of uterine prolapse stage 2 or higher: a randomized multicenter trial comparing sacrospinous fixation with vaginal hysterectomy (SAVE U trial)

97471.pdf (publisher's version ) (Open Access)BACKGROUND: Pelvic organ prolapse is a common health problem, affecting up to 40% of parous women over 50 years old, with significant negative influence on quality of life. Vaginal hysterectomy is currently the leading treatment method for patients with symptomatic uterine prolapse. Several studies have shown that sacrospinous fixation in case of uterine prolapse is a safe and effective alternative to vaginal hysterectomy. However, no large randomized trials with long-term follow-up have been performed to compare efficacy and quality of life between both techniques.The SAVE U trial is designed to compare sacrospinous fixation with vaginal hysterectomy in the treatment of uterine prolapse stage 2 or higher in terms of prolapse recurrence, quality of life, complications, hospital stay, post-operative recovery and sexual functioning. METHODS/DESIGN: The SAVE U trial is a randomized controlled multi-center non-inferiority trial. The study compares sacrospinous fixation with vaginal hysterectomy in women with uterine prolapse stage 2 or higher. The primary outcome measure is recurrence of uterine prolapse defined as: uterine descent stage 2 or more assessed by pelvic organ prolapse quantification examination and prolapse complaints and/or redo surgery at 12 months follow-up. Secondary outcomes are subjective improvement in quality of life measured by generic (Short Form 36 and Euroqol 5D) and disease-specific (Urogenital Distress Inventory, Defecatory Distress Inventory and Incontinence Impact Questionnaire) quality of life instruments, complications following surgery, hospital stay, post-operative recovery and sexual functioning (Pelvic Organ Prolapse/Urinary Incontinence Sexual Questionnaire). Analysis will be performed according to the intention to treat principle. Based on comparable recurrence rates of 3% and considering an upper-limit of 7% to be non-inferior (beta 0.2 and one sided alpha 0.025), 104 patients are needed per group. DISCUSSION: The SAVE U trial is a randomized multicenter trial that will provide evidence whether the efficacy of sacrospinous fixation is similar to vaginal hysterectomy in women with uterine prolapse stage 2 or higher. Dynamic simulations on the mitochondrial fatty acid Beta-oxidation network
<p>Abstract</p> <p>Background</p> <p>The oxidation of fatty acids in mitochondria plays an important role in energy metabolism and genetic disorders of this pathway may cause metabolic diseases. Enzyme deficiencies can block the metabolism at defined reactions in the mitochondrion and lead to accumulation of specific substrates causing severe clinical manifestations. Ten of the disorders directly affecting mitochondrial fatty acid oxidation have been well-defined, implicating episodic hypoketotic hypoglycemia provoked by catabolic stress, multiple organ failure, muscle weakness, or hypertrophic cardiomyopathy. Additionally, syndromes of severe maternal illness (HELLP syndrome and AFLP) have been associated with pregnancies carrying a fetus affected by fatty acid oxidation deficiencies. However, little is known about fatty acids kinetics, especially during fasting or exercise when the demand for fatty acid oxidation is increased (catabolic stress).</p> <p>Results</p> <p>A computational kinetic network of 64 reactions with 91 compounds and 301 parameters was constructed to study dynamic properties of mitochondrial fatty acid β-oxidation. Various deficiencies of acyl-CoA dehydrogenase were simulated and verified with measured concentrations of indicative metabolites of screened newborns in Middle Europe and South Australia. The simulated accumulation of specific acyl-CoAs according to the investigated enzyme deficiencies are in agreement with experimental data and findings in literature. Investigation of the dynamic properties of the fatty acid β-oxidation reveals that the formation of acetyl-CoA – substrate for energy production – is highly impaired within the first hours of fasting corresponding to the rapid progress to coma within 1–2 hours. LCAD deficiency exhibits the highest accumulation of fatty acids along with marked increase of these substrates during catabolic stress and the lowest production rate of acetyl-CoA. These findings might confirm gestational loss to be the explanation that no human cases of LCAD deficiency have been described.</p> <p>Conclusion</p> <p>In summary, this work provides a detailed kinetic model of mitochondrial metabolism with specific focus on fatty acid β-oxidation to simulate and predict the dynamic response of that metabolic network in the context of human disease. Our findings offer insight into the disease process (e.g. rapid progress to coma) and might confirm new explanations (no human cases of LCAD deficiency), which can hardly be obtained from experimental data alone.</p
