    The dental profession is well placed to contribute important information in child protection cases but no previous research has been reported that assesses the volume or impact of this information. Comprehensive oral assessment clinics were introduced and established as an integral part of comprehensive medical assessments for children with welfare concerns in Greater Glasgow and Clyde. An assessment protocol and standardised paperwork for comprehensive oral assessments were developed to enhance information sharing and patient access to appropriate care.     The competing chemical and physical effects of transient fuel enrichment on heavy knock in an optical spark ignition engine

    The work was concerned with improving understanding of the chemical and physical trade-offs when employing transient over-fuelling to control auto-ignition in gasoline spark ignition engines under knock intensities not usually tolerated in optical engines. The single cylinder engine used included full bore overhead optical access capable of withstanding unusually high in-cylinder pressures. Heavy knock was deliberately induced by adopting inlet air heating and a primary reference fuel blend of reduced octane rating. High-speed chemiluminescence imaging and simultaneous in-cylinder pressure data measurement were used to evaluate the combustion events. Under normal operation the engine was operated under port fuel injection with a stoichiometric air-fuel mixture. Multiple centred auto-ignition events were regularly observed, with knock intensities of up to ~30bar. Additional excess fuel was then introduced directly into the end-gas in short transient bursts. As the mass of excess fuel was progressively increased a trade-off was apparent, with knock intensity first increasing by up to 65% before lower unburned gas temperatures suppressed knock under extremely rich conditions. This trade-off is not usually observed during conventional low intensity knock suppression via over-fuelling and has been associated with the competing effects of reducing auto-ignition delay time and charge cooling/ratio of specific heats. Overall, the results demonstrate the risks in employing excess fuel to suppress knock deep within a heavy knocking combustion regime (potentially including a Super-Knock regime)

    Evolutionary Games with Affine Fitness Functions: Applications to Cancer

    We analyze the dynamics of evolutionary games in which fitness is defined as an affine function of the expected payoff and a constant contribution. The resulting inhomogeneous replicator equation has an homogeneous equivalent with modified payoffs. The affine terms also influence the stochastic dynamics of a two-strategy Moran model of a finite population. We then apply the affine fitness function in a model for tumor-normal cell interactions to determine which are the most successful tumor strategies. In order to analyze the dynamics of concurrent strategies within a tumor population, we extend the model to a three-strategy game involving distinct tumor cell types as well as normal cells. In this model, interaction with normal cells, in combination with an increased constant fitness, is the most effective way of establishing a population of tumor cells in normal tissue.Comment: The final publication is available at http://www.springerlink.com, http://dx.doi.org/10.1007/s13235-011-0029-

    Evaluating maize genotype performance under low nitrogen conditions using RGB UAV phenotyping techniques

    Maize is the most cultivated cereal in Africa in terms of land area and production, but low soil nitrogen availability often constrains yields. Developing new maize varieties with high and reliable yields using traditional crop breeding techniques in field conditions can be slow and costly. Remote sensing has become an important tool in the modernization of field-based high-throughput plant phenotyping (HTPP), providing faster gains towards the improvement of yield potential and adaptation to abiotic and biotic limiting conditions. We evaluated the performance of a set of remote sensing indices derived from red–green–blue (RGB) images along with field-based multispectral normalized difference vegetation index (NDVI) and leaf chlorophyll content (SPAD values) as phenotypic traits for assessing maize performance under managed low-nitrogen conditions. HTPP measurements were conducted from the ground and from an unmanned aerial vehicle (UAV). For the ground-level RGB indices, the strongest correlations to yield were observed with hue, greener green area (GGA), and a newly developed RGB HTPP index, NDLab (normalized difference Commission Internationale de I´Edairage (CIE)Lab index), while GGA and crop senescence index (CSI) correlated better with grain yield from the UAV. Regarding ground sensors, SPAD exhibited the closest correlation with grain yield, notably increasing in its correlation when measured in the vegetative stage. Additionally, we evaluated how different HTPP indices contributed to the explanation of yield in combination with agronomic data, such as anthesis silking interval (ASI), anthesis date (AD), and plant height (PH). Multivariate regression models, including RGB indices (R2 > 0.60), outperformed other models using only agronomic parameters or field sensors (R2 > 0.50), reinforcing RGB HTPP’s potential to improve yield assessments. Finally, we compared the low-N results to the same panel of 64 maize genotypes grown under optimal conditions, noting that only 11% of the total genotypes appeared in the highest yield producing quartile for both trials. Furthermore, we calculated the grain yield loss index (GYLI) for each genotype, which showed a large range of variability, suggesting that low-N performance is not necessarily exclusive of high productivity in optimal conditions.This research and APC was funded by Bill & Melinda Gates Foundation and USAID Stress Tolerant Maize for Africa program, grant number [OPP1134248], and the MAIZE CGIAR research program.     Spatially resolved observations of a split-band coronal type-II radio burst

    Context. The origin of coronal type-II radio bursts and of their band-splitting are still not fully understood. Aims. To make progress in solving this problem on the basis of one extremely well observed solar eruptive event. Methods. The relative dynamics of multi-thermal eruptive plasmas, observed in detail by the SDO/AIA and of the harmonic type-II burst sources, observed by the NRH at ten frequencies from 445 to 151 MHz, is studied for the partially behind the limb event on 3 November 2010. Special attention is given to the band-splitting of the burst. Analysis is supplemented by investigation of coronal hard X-ray (HXR) sources observed by the RHESSI. Results. It is found that the flare impulsive phase was accompanied by the formation of a double coronal HXR source, whose upper part coincided with the hot (T~10 MK) eruptive plasma blob. The leading edge (LE) of the eruptive plasmas (T~1-2 MK) moved upward from the flare region with the speed of v=900-1400 km/s. The type II burst source initially appeared just above the LE apex and moved with the same speed and in the same direction. After about 20 s it started to move about twice faster, but still in the same direction. At any given moment the low frequency component (LFC) source of the splitted type-II burst was situated above the high frequency component (HFC) source, which in turn was situated above the LE. It is also found that at a given frequency the HFC source was located slightly closer to the photosphere than the LFC source. Conclusions. The shock wave, which could be responsible for the observed type-II radio burst, was initially driven by the multi-temperature eruptive plasmas, but later transformed to a freely propagating blast shock wave. The most preferable interpretation of the type-II burst splitting is that its LFC was emitted from the upstream region of the shock, whereas the HFC - from the downstream region.Comment: 14 pages, 10 figure

    Are Tanzanian patients attending public facilities or private retailers more likely to adhere to artemisinin-based combination therapy?

    BACKGROUND: Artemisinin combination therapy (ACT) is first-line treatment for malaria in most endemic countries and is increasingly available in the private sector. Most studies on ACT adherence have been conducted in the public sector, with minimal data from private retailers. METHODS: Parallel studies were conducted in Tanzania, in which patients obtaining artemether-lumefantrine (AL) at 40 randomly selected public health facilities and 37 accredited drug dispensing outlets (ADDOs) were visited at home and questioned about doses taken. The effect of sector on adherence, controlling for potential confounders was assessed using logistic regression with a random effect for outlet. RESULTS: Of 572 health facility patients and 450 ADDO patients, 74.5% (95% CI: 69.8, 78.8) and 69.8% (95% CI: 64.6, 74.5), respectively, completed treatment and 46.0% (95% CI: 40.9, 51.2) and 34.8% (95% CI: 30.1, 39.8) took each dose at the correct time ('timely completion'). ADDO patients were wealthier, more educated, older, sought care later in the day, and were less likely to test positive for malaria than health facility patients. Controlling for patient characteristics, the adjusted odds of completed treatment and of timely completion for ADDO patients were 0.65 (95% CI: 0.43, 1.00) and 0.69 (95% CI: 0.47, 1.01) times that of health facility patients. Higher socio-economic status was associated with both adherence measures. Higher education was associated with completed treatment (adjusted OR = 1.68, 95% CI: 1.20, 2.36); obtaining AL in the evening was associated with timely completion (adjusted OR = 0.35, 95% CI: 0.19, 0.64). Factors associated with adherence in each sector were examined separately. In both sectors, recalling correct instructions was positively associated with both adherence measures. In health facility patients, but not ADDO patients, taking the first dose of AL at the outlet was associated with timely completion (adjusted OR = 2.11, 95% CI: 1.46, 3.04). CONCLUSION: When controlling for patient characteristics, there was some evidence that the adjusted odds of adherence for ADDO patients was lower than that for public health facility patients. Better understanding is needed of which patient care aspects are most important for adherence, including the role of effective provision of advice

    Co-opetition models for governing professional football

    In recent years, models for co-creating value in a business-to-business context have often been examined with the aim of studying the strategies implemented by and among organisations for competitive and co-operative purposes. The traditional concepts of competition and co-operation between businesses have now evolved, both in terms of the sector in which the businesses operate and in terms of the type of goods they produce. Many researchers have, in recent times, investigated the determinants that can influence the way in which the model of co-opetition can be applied to the football world. Research interest lies in the particular features of what makes a good football. In this paper, the aim is to conduct an analysis of the rules governing the “football system”, while also looking at the determinants of the demand function within football entertainment. This entails applying to football match management the co-opetition model, a recognised model that combines competition and co-operation with the view of creating and distributing value. It can, therefore, be said that, for a spectator, watching sport is an experience of high suspense, and this suspense, in turn, depends upon the degree of uncertainty in the outcome. It follows that the rules ensuring that both these elements can be satisfied are a fertile ground for co-operation between clubs, as it is in the interest of all stakeholders to offer increasingly more attractive football, in comparison with other competing products.     Variation of health-related quality of life assessed by caregivers and patients affected by severe childhood infections.

    BACKGROUND: The agreement between self-reported and proxy measures of health status in ill children is not well established. This study aimed to quantify the variation in health-related quality of life (HRQOL) derived from young patients and their carers using different instruments. METHODS: A hospital-based cross-sectional survey was conducted between August 2010 and March 2011. Children with meningitis, bacteremia, pneumonia, acute otitis media, hearing loss, chronic lung disease, epilepsy, mild mental retardation, severe mental retardation, and mental retardation combined with epilepsy, aged between five to 14 years in seven tertiary hospitals were selected for participation in this study. The Health Utilities Index Mark 2 (HUI2), and Mark 3 (HUI3), and the EuroQoL Descriptive System (EQ-5D) and Visual Analogue Scale (EQ-VAS) were applied to both paediatric patients (self-assessment) and caregivers (proxy-assessment). RESULTS: The EQ-5D scores were lowest for acute conditions such as meningitis, bacteremia, and pneumonia, whereas the HUI3 scores were lowest for most chronic conditions such as hearing loss and severe mental retardation. Comparing patient and proxy scores (n = 74), the EQ-5D exhibited high correlation (r = 0.77) while in the HUI2 and HUI3 patient and caregiver scores were moderately correlated (r = 0.58 and 0.67 respectively). The mean difference between self and proxy-assessment using the HUI2, HUI3, EQ-5D and EQ-VAS scores were 0.03, 0.05, -0.03 and -0.02, respectively. In hearing-impaired and chronic lung patients the self-rated HRQOL differed significantly from their caregivers. CONCLUSIONS: The use of caregivers as proxies for measuring HRQOL in young patients affected by pneumococcal infection and its sequelae should be employed with caution. Given the high correlation between instruments, each of the HRQOL instruments appears acceptable apart from the EQ-VAS which exhibited low correlation with the others
