A Sexually and Physically Abused Child: His Inner World
This paper describes how a sexually abused 8 year old boy\u27s internal dynamics correlated with and influenced his behavior while he was on an inpatient unit. Through play therapy and unit staff\u27s observation, the psychopathological sequelae to the abuse unfolded in a very interesting way. This process of correlating play material with observations of behavior on the unit, helped the treatment team in understanding the child\u27s inner world
Amino acid stable N isotope estimations reveal uniform diazotrophic contributions across zooplankton size fractions in the subtropical N Atlantic
presentacionNatural abundance of stable N isotopes (d15N) in both individual amino acids and bulk organic matter of size-fractionated plankton samples were compared to analyze the differential impact of nitrogen fixation through the food web, in a transect across the subtropical North Atlantic. Low d15N values in the central and western regions were consistent with the prevalence of nitrogen fixation, while maximum d15N values tracked the influence of West African upwelling in the eastern zone. Compound-specific amino acid isotope data (CSI-AA) revealed relatively low variability in the impact of diazotrophic nitrogen within the different plankton size fractions, while d15N of bulk organic matter showed higher variability with size. Moreover, CSI-AA results also indicated a greater importance of diazotrophy than suggested by bulk d15N values. Trophic position estimates using CSI-AA showed the expected general increase with mean plankton size class and varied in a relatively narrow range (1.8 to 2.5), with the lowest values in the central zone. Using isotopic δ15N values of individual amino acids (in particular Phe and Thr), as well as reconstructed total protein d15N values, a set of new relationships with bulk plankton d15N was determined, aimed to improve the use CSI-AA data in tracing direct plankton contributions to organic nitrogen pools in the ocean. These new results represent the most detailed investigation of CSI-AA data in zooplankton size classes to date, and point to a key role of large zooplankton in the transmission of the diazotrophic nitrogen up oceanic food webs.Malaspina-2010(CSD2008-00077), NSF Grant OCE-113181
Regulation and role of the PP2A-B56 holoenzyme family in cancer
Protein phosphatase 2A (PP2A) inactivation is common in cancer, leading to sustained activation of pro-survival
and growth-promoting pathways. PP2A consists of a scaffolding A-subunit, a catalytic C-subunit, and a regulatory B-subunit. The functional complexity of PP2A holoenzymes arises mainly through the vast repertoire of
regulatory B-subunits, which determine both their substrate specificity and their subcellular localization.
Therefore, a major challenge for developing more effective therapeutic strategies for cancer is to identify the
specific PP2A complexes to be targeted. Of note, the development of small molecules specifically directed at
PP2A-B56α has opened new therapeutic avenues in both solid and hematological tumors. Here, we focus on the
B56/PR61 family of PP2A regulatory subunits, which have a central role in directing PP2A tumor suppressor
activity. We provide an overview of the mechanisms controlling the formation and regulation of these complexes,
the pathways they control, and the mechanisms underlying their deregulation in cancer
Equine nutrition: a survey of perceptions and practices of horse owners undertaking a massive open online course in equine nutrition
An online survey was designed to ascertain the following information: demographics, current feeding practices, and perceptions and knowledge of equine nutrition, including nutrition-related disorders. Response rate was 34% (6,538 respondents). More than 80% of respondents were horse owners or caretakers, with the majority owning between one and five horses (75%) aged 5 years and older (74%). Most kept their horses for pleasure (54%), with 33% using them mostly for competition and 13% using them for an equal mix of both pleasure and competition. Concentrates were fed by the majority (87%), and more than 70% stated that their horses had some access to pasture. Over half of respondents (60%) regularly monitored their horses' weight, with most doing this monthly. Weight tapes were most commonly used (62%), although many reported to guess the weight of their horse(s) with very few (5%) using weight scales. Under half (46%) stated that they regularly used body condition scoring (BCS), many did not use BCS at all (24%), and some did not know what BCS was (10%). Of those that did use BCS, most (36%) did this monthly, with others weekly (25%), daily (14%), and when they remembered (15%). Overall knowledge of nutrition was reported by most as average (median, 3 on Likert scale—average); however, respondents were less knowledgeable on the management of nutrition-related disorders
t(10;16)(q22;p13) and MORF-CREBBP fusion is a recurrent event in acute myeloid leukemia
Recently, it was shown that t(10;16)(q22;p13) fuses the MORF and CREBBP genes in a case of childhood acute myeloid leukemia (AML) M5a, with a complex karyotype containing other rearrangements. Here, we report a new case with the MORF-CREBBP fusion in an 84-year-old patient diagnosed with AML M5b, in which the t(10;16)(q22;p13) was the only cytogenetic aberration. This supports that this is a recurrent pathogenic translocation in AML
Molecular features in a biphenotypic small cell sarcoma with neuroectodermal and muscle differentiation
We report a case of a 13-year-old girl with soft tissue sarcoma of the hand,
which showed muscle and neuroectodermal immunophenotypes. Molecular studies were
performed on RNA collected from fine-needle aspiration (FNA) cytology and
peripheral blood samples by nested reverse transcriptase-polymerase chain
reaction (RT-PCR) and Southern blot analysis. This biphenotypic tumor showed
simultaneous expression of EWS-FLI1 and PAX3-FKHR transcripts, specific of Ewing
family tumors and alveolar rhabdomyosarcoma, respectively. Although childhood
sarcomas with simultaneous muscle and neural differentiation have been described
to have EWS-FLI1 transcripts, there are no reports of tumors with both

Prolonged survival of patients with angioimmunoblastic T-cell lymphoma after high-dose chemotherapy and autologous stem cell transplantation: the GELTAMO experience
Abstract
OBJECTIVES:
Angioimmunoblastic T-cell lymphoma (AIL) is a rare lymphoma with a poor prognosis and no standard treatment. Here, we report our experiences with 19 patients treated with high-dose chemotherapy and autologous stem cell transplantation (HDC/ASCT) within the GELTAMO co-operative group between 1992 and 2004.
METHODS:
The median age at transplantation was 46 yr. Fifteen patients underwent the procedure as front-line therapy and four patients as salvage therapy. Most patients received peripheral stem cells (90%) coupled with BEAM or BEAC as conditioning regimen (79%).
RESULTS:
A 79% of patients achieved complete response, 5% partial response and 16% failed the procedure. After a median follow-up of 25 months, eight patients died (seven of progressive disease and secondary neoplasia), while actuarial overall survival and progression-free survival at 3 yr was 60% and 55%. Prognostic factors associated with a poor outcome included bone marrow involvement, transplantation in refractory disease state, attributing more than one factor of the age-adjusted-International Prognostic Index, Pretransplant peripheral T-cell lymphoma (PTCL) Score or Prognostic Index for PTCL.
CONCLUSIONS:
More than half of the patients with AIL that display unfavourable prognostic factors at diagnosis or relapse would be expected to be alive and disease-free after 3 yr when treated with HDC/ASCT. Patients who are transplanted in a refractory disease state do not benefit from this procedure

La atención primaria de salud (APS) es un ámbito idóneo para el abordaje preventivo de los conflictos psicosociales y los trastornos mentales por 3 razones. La primera, porque probablemente sea, junto con la escuela, el dispositivo social con el que contactan mayor número de ciudadanos a lo largo del año y de la vida de cada uno de ellos. En segundo lugar, porque múltiples investigaciones apuntan que los pacientes con trastornos mentales o conflictos psicosociales crónicos suelen consultar de forma repetida con los diversos dispositivos de APS. Y en tercer lugar, porque, a niveles pragmático y sanitario, los trastornos emocionales que no se detectan precozmente por los profesionales de APS tienen una peor evolución. De ahí la importancia de una perspectiva biopsicosocial real, desde una aproximación de la atención sanitaria centrada en el consultante en tanto que miembro de la comunidad; es decir, desde una perspectiva en la cual lo “psicológico” y lo “emocional” no resulten “apeados” del modelo teórico o práctico, sino integrado en una visión holística del ser humano-consultante y comprendiendo que esa interpretación no es un adorno “humanístico” y sensible, sino un componente tecnológico, además de humano, indispensable para una aproximación asistencial de calidad..
