    Effectiveness and safety of obeticholic acid in a Southern European multicenter cohort of patients with primary biliary cholangitis and suboptimal response to ursodeoxycholic acid

    Background Obeticholic acid (OCA) was recently approved as the only on-label alternative for patients with primary biliary cholangitis (PBC) with intolerance or suboptimal response to ursodeoxycholic acid (UDCA). However, few data are available outside clinical trials. Aim To assess the effectiveness and safety of OCA in a real-world cohort of patients with non-effective UDCA therapy. Methods Open-label, prospective, real-world, multicentre study, enrolling consecutive patients who did not meet Paris II criteria, from 18 institutions in Spain and Portugal. Effectiveness was assessed by the changes in GLOBE and UK-PBC scores from baseline. POISE and Paris II criteria were evaluated after 12 months of OCA . Liver fibrosis was evaluated by FIB-4 and AST to platelet ratio index (APRI). Results One hundred and twenty patients were eligible, median time since PBC diagnosis 9.3 (4.0-13.8) years, 21.7% had cirrhosis, and 26.7% received had previous or concomitant treatment with fibrates. Seventy-eight patients completed at least 1 year of OCA. The Globe-PBC score decreased to 0.17 (95% CI 0.05 to 0.28; P = 0.005) and the UK-PBC score decreased to 0.81 (95% CI -0.19 to 1.80; P = 0.11). There was a significant decrease in alkaline phosphatase of 81.3 U/L (95% CI 42.5 to 120; P < 0.001), ALT 22.1 U/L (95% CI 10.4 to 33.8; P < 0.001) and bilirubin 0.12 mg/dL (95% CI 0 to 0.24; P = 0.044). FIB-4 and APRI remained stable. According to the POISE criteria, 29.5% (23 out of 78) achieved response. The adverse events rate was 35%; 11.67% discontinued (8.3% due to pruritus). Conclusions This study supports data from phase III trials with significant improvement of PBC-Globe continuous prognostic marker score among OCA-treated patients with good tolerability

    Effectiveness and safety of first-generation protease inhibitors in clinical practice: Hepatitis C virus patients with advanced fibrosis

    AIM: To evaluates the effectiveness and safety of the first generation, NS3/4A protease inhibitors (PIs) in clinical practice against chronic C virus, especially in patients with advanced fibrosis. METHODS: Prospective study and non-experimental analysis of a multicentre cohort of 38 Spanish hospitals that includes patients with chronic hepatitis C genotype 1, treatment-nai¨ve (TN) or treatment-experienced (TE), who underwent triple therapy with the first generation NS3/4A protease inhibitors, boceprevir (BOC) and telaprevir (TVR), in combination with pegylated interferon and ribavirin. The patients were treatment in routine practice settings. Data on the study population and on adverse clinical and virologic effects were compiled during the treatment period and during follow up. RESULTS: One thousand and fifty seven patients were included, 405 (38%) were treated with BOC and 652 (62%) with TVR. Of this total, 30% (n = 319) were TN and the remaining were TE: 28% (n = 298) relapsers, 12% (n = 123) partial responders (PR), 25% (n = 260) null-responders (NR) and for 5% (n = 57) with prior response unknown. The rate of sustained virologic response (SVR) by intention-to-treatment (ITT) was greater in those treated with TVR (65%) than in those treated with BOC (52%) (P < 0.0001), whereas by modified intention-to-treatment (mITT) no were found significant differences. By degree of fibrosis, 56% of patients were F4 and the highest SVR rates were recorded in the non-F4 patients, both TN and TE. In the analysis by groups, the TN patients treated with TVR by ITT showed a higher SVR (P = 0.005). However, by mITT there were no significant differences between BOC and TVR. In the multivariate analysis by mITT, the significant SVR factors were relapsers, IL28B CC and non-F4; the type of treatment (BOC or TVR) was not significant. The lowest SVR values were presented by the F4-NR patients, treated with BOC (46%) or with TVR (45%). 28% of the patients interrupted the treatment, mainly by non-viral response (51%): this outcome was more frequent in the TE than in the TN patients (57% vs 40%, P = 0.01). With respect to severe haematological disorders, neutropaenia was more likely to affect the patients treated with BOC (33% vs 20%, P = 0.0001), and thrombocytopaenia and anaemia, the F4 patients (P = 0.000, P = 0.025, respectively). CONCLUSION: In a real clinical practice setting with a high proportion of patients with advanced fibrosis, effectiveness of first-generation PIs was high except for NR patients, with similar SVR rates being achieved by BOC and TVR

    Después de graduarme, mi experiencia profesional consiste en liderar, supervisar y gestionar proyectos en la disciplina de mantenimiento, proyectos electromecánicos, paradas de planta y obras civiles. En el año 2019 se logró constituir la empresa de consultoría organizacional Grupo Cimavik S.A.C. (Anexo 1) en alianza con mi socia la Ing. Mayra Inchicaqui Rosales. Esto debido a la demanda y la necesidad del mercado, la empresa en el desarrollo de sus proyectos tiene diferentes problemas en cuanto a su dirección, su metodología de trabajo y los resultados que genera, debido al enfoque metodológico. trabajo empírico, de igual manera, no se establecen procedimientos y existen deficiencias en la comunicación, ya que los jefes de proyecto de las obras, a pesar de la gran experiencia, carecen de algunas herramientas de gestión de proyectos. En este sentido, se ha definido e implementado un conjunto de actividades recomendadas por la dirección del proyecto en base al PMBOK. También se ha puesto en marcha un conjunto de actividades con el objetivo de homologar los procesos de gestión implantados en CIMAVICK S.A.C

    Digestion and energy value of macerated sudangrass hay used in growing-finishing diets for feedlot cattle

    Mechanical maceration enhances site and extent of digestion of low-moisture, low-quality forages. Four Holstein steers (172 ± 8 kg) with cannulas in rumen and proximal duodenum were used in 4x4 Latin square design to evaluate the process of mechanical maceration of sudangrass hay on the characteristics of ruminal and total tract digestion. Treatments consisted of a steam-flaked corn-based growing diet supplemented with 21 % forage (DM basis) as: i) wheat straw (Triticum aestivum, STRW), ii) sudangrass hay (Sorghum sudanense, SG), iii) macerated SG at intensity of 4,134 kPa (MAC600) and 4) macerated SG at intensity of 6,200 kPa (MAC900). All forages were ground to pass through a 3.8 cm screen before incorporation into complete mixed diets (21:79 forage to concentrate ratio). Maceration did not affect (p &#8805; 0.17) on site and extent of OM, N and ADF, and DE of sudangrass supplemented diets. Characteristics of ruminal digestion of OM, ADF, starch, as well as, microbial efficiency (microbial N, g kg-1 of OM fermentad) and protein efficiency (nonammonia N, g g-1 of N intake) were not different (p &#8805; 0.11) for wheat straw versus sudangrass supplemented diets. However, total tract digestion of OM, ADF, N, and DE diet were greater (p &#8804; 0.05) for sudangrass than for wheat straw supplemented diets. Using the replacement technique, DE value of SG averaged 9.59 MJ kg-1, very close to the expected value given its chemical composition. Mechanical maceration did not enhance the feeding value of sudangrass hay.
