    Trends in blindness due to diabetic retinopathy among adults aged 18-69 years over a decade in Ireland

    Aims: To describe trends in the incidence of visual impairment and blindness due to diabetic retinopathy among adults aged 18–69 years in Ireland between 2004 and 2013. Methods: Data on visual impairment due to diabetic retinopathy in adults aged 18–69 years or over who are registered with the National Council for the Blind of Ireland, (2004–2013) were analysed. Annual incidence rates were calculated for the adult population and the population with diagnosed diabetes. Poisson regression was used to test for changes in rates over time. The relative, attributable and population risk of blindness and visual impairment due to diabetic retinopathy were calculated for 2013. Results: Over the decade, the prevalence of diagnosed diabetes increased from 2.1% to 3.6%. Among people with diagnosed diabetes, the incidence of visual impairment due to diabetic retinopathy increased from 6.4 (95% CI 2.4–13.9) per 100,000 in 2004 to 11.7 (95% CI 5.9–21.0) per 100,000 in 2013. The incidence of blindness due to diabetic retinopathy varied from 31.9 per 100,000 (95% CI 21.6–45.7) in 2004 to 14.9 per 100,000 (95% CI 8.2–25.1) in 2013. Conclusions: Our findings indicate the need for increased attention to preventive measures for microvascular complications among adults with diabetes in Ireland. Retinopathy screening has been standardised in Ireland, these findings provide useful baseline statistics to monitor the impact of this population-based screening programme

    Risk factors for macro- and microvascular complications among older adults with diagnosed type 2 diabetes: findings from The Irish Longitudinal Study on Ageing

    Objective. To explore risk factors for macro- and microvascular complications in a nationally representative sample of adults aged 50 years and over with type 2 diabetes in Ireland. Methods. Data from the first wave of The Irish Longitudinal Study on Ageing (TILDA) (2009–2011) was used in cross-sectional analysis. The presence of doctor diagnosis of diabetes, risk factors, and macro and microvascular complications were determined by self-report. Gender-specific differences in risk factor prevalence were assessed with the chi-squared test. Binomial regression analysis was conducted to explore independent associations between established risk factors and diabetes-related complications. Results. Among 8175 respondents, 655 were classified as having type 2 diabetes. Older age, being male, a history of smoking, a lower level of physical activity, and a diagnosis of high cholesterol were independent predictors of macrovascular complications. Diabetes diagnosis of 10 or more years, a history of smoking, and a diagnosis of hypertension were associated with an increased risk of microvascular complications. Older age, third-level education, and a high level of physical activity were protective factors (\u1d45d < 0.05). Conclusions. Early intervention to target modifiable risk factors is urgently needed to reduce diabetes-related morbidity in the older population in Ireland

    Minimal versus specialist equipment for the delivery of pulmonary rehabilitation in COPD

    Background: Evidence for pulmonary rehabilitation(PR)largely comes from trials where the intervention used specialist aerobic and/or resistance equipment.Limited data exist to demonstrate the efficacy of PR in community settings with minimal equipment. Aims: To compare completion rates and outcomes in COPD patients undergoing PR in a community setting with minimal equipment(PR-min)with a matched sample undergoing PR in a gym setting with specialist aerobic and resistance equipment(PR-gym). Methods: Using propensity score matching,318 patients with COPD referred for 8 weeks of PR-min were matched 1:1 with a control group of 318 patients who undertook 8 weeks of PR-gym. Completion rate(attendance≥8 supervised sessions)and changes in incremental shuttle walk(ISW),Chronic Respiratory Disease Questionnaire(CRQ)and quadriceps maximal voluntary contraction(QMVC)were compared. Results: Groups were matched for age(70.8v70.7years),FEV1%predicted(46.8v45.8),ISW(192v195m),%current smoking status(19v20)and depression scores(6.5v6.6).No between group differences were seen in ISW,CRQ or QMVC change. Completion rates were better in the PR-gym(73%)as opposed to the PR-min(64%);p=0.01. Conclusion: This case-control study shows that PR-min had similar benefits to PR-gym.A randomised non-inferiority trial is needed to confirm the findings of this study

The rate at which bamboos are being planted in South Africa is a cause for concern because of the impacts of bamboo invasions in other parts of the world. To understand the risks associated with new introductions and new plantings, we assess the outcomes of past introductions of bamboos into South Africa. To this end we: (1) produce an inventory of alien bamboo taxa; (2) assess the distribution of bamboos; (3) determine the rate of spread of bamboo at a site with a high density of naturalised stands; and (4) evaluate the current regulatory status of alien bamboos in South Africa. We used a combination of expert opinion, literature, historical records of populations, and public participation to produce a species list and locate populations of alien bamboos. We also attempted to confirm species identities using DNA barcoding. We found that 28 currently-accepted species of bamboo have been recorded in South Africa. However, we have little confidence in this estimate, as 20 of the species could not be confirmed or identified as present in the country. Bamboos are an inherently challenging group to identify using vegetative material, and DNA barcoding was inconclusive. The distribution of bamboos across the country varied with the type or lineage (e.g. herbaceous, tropical or temperate) and the source of information (e.g. herbarium records, in-field observation or public contribution). Although alien bamboos are naturalised at several sites, we found no large invasive stands nor evidence of widespread negative environmental impacts. Nonetheless, we recommend caution regarding future introductions of bamboos for commercial cultivation, as the nature of the plantings will likely differ from the historical situation in both the location, configuration, and the scale of cultivation, and as new species are likely to be introduced.     Causes of death in people with coeliac disease in England compared with the general population: a competing risk analysis.

    INTRODUCTION: Quantifying excess cause-specific mortality among people with coeliac disease (CD) compared with the general population accounting for competing risks will allow accurate information to be given on risk of death from specific causes. METHOD: We identified from the Clinical Practice Research Datalink all patients with CD linked to Office for National Statistics between 1998 and 2012. We selected controls by frequency matching from the registered general practice population within 10-year age bands. We calculated the adjusted cumulative incidence (including adjustment for competing risks) and excess cumulative incidence for different causes of death up to 10 years from diagnosis. RESULTS: Of the 10 825 patients with CD, 773 died within the study period. The overall mortality rate among patients with CD was 128/10 000 person years compared with 153/10 000 in controls (HR=0.94 95% CI 0.84 to 1.01). We found no overall difference in the cumulative incidence of respiratory disease, digestive disease or cancer related death among cases and controls. The adjusted cumulative incidence of death from cardiovascular deaths was slightly lower compared with those without CD diagnosis (CD 0.32% vs controls 0.41%) with a corresponding excess cumulative incidence of -0.08% (95% CI -0.13 to -0.04). However, patients with CD had 0.15% excess risk (95% CI 0.03 to 0.27) of deaths from non-Hodgkin's lymphoma from the general population baseline risk. CONCLUSIONS: Overall, people with CD have no major excess risk of cancer, digestive disease or respiratory disease related or cardiovascular mortality compared with the general population. These findings should be reassuring to patients with CD and clinicians managing their care

    Use of Cold Radiometers in Several Thermal/Vacuum Tests

    We have developed a low cost low temperature broadband radiometer for use with low temperature tests as a diagnostic tool for measuring stray thermal radiation and remote measurement of material properties. So far these radiometers have been used in two large thermal/vacuum tests for the James Webb Space Telescope (JWST) Project. In the first test the radiometers measured stray radiation in a test of part of the JWST sunshield, and in the second test the radiometers were used to measure the reflectivity and specularity of black Z307 painted aluminum walls on a 25 K cooled shroud. These results will be presented as well as plans for future tests to measure the residual energy through a baffled aperture in the shroud and other stray thermal energy measurements

    Phylogenetic analyses reveal multiple new stem-boring Tetramesa taxa (Hymenoptera: Eurytomidae): implications for the biological control of invasive African grasses

    Many native South African grass species have become invasive elsewhere in the world. The application of biological control to invasive grasses has been approached with trepidation in the past, primarily due to concerns of a perceived lack of host specific herbivores. This has changed in recent times, and grasses are now considered suitable candidates. The Tetramesa Walker genus (Hymenoptera: Eurytomidae) has been found to contain species that are largely host specific to a particular grass species, or complex of closely related congeners. Very little taxonomic work exists for Tetramesa in the southern hemisphere, and the lack of morphological variability between many Tetramesa species has made identification difficult. This limits the ability to assess the genus for potential biological control agents. Species delimitation analyses indicated 16 putative novel southern African Tetramesa taxa. Ten of these were putative Tetramesa associated with Eragrostis curvula (Schrad.) Nees and Sporobolus pyramidalis Beauv. and S. natalensis Steud., which are alien invasive weeds in Australia. Of these ten Tetramesa taxa, eight were only found on a single host plant, while two taxa were associated with multiple species in a single grass genus. The Tetramesa spp. on S. pyramidalis and S. africanus were deemed suitably host-specific to be used as biological control agents. Field host range data for the Tetramesa species on E. curvula revealed that the wasp may not be suitably host specific for use as a biological control agent. However, further host specificity testing on non-target native Australian species is required
