Analysis of the Two-Level NO PLIF Model for Low-Temperature High-Speed Flow Applications
The current work compares experimentally obtained nitric oxide (NO) laser-induced fluorescence (LIF) spectra with the equivalent spectra obtained analytically. The experimental spectra are computed from captured images of fluorescence in a gas cell and from a laser sheet passing through the fuel-air mixing flowfield produced by a high-speed fuel injector. The fuel injector is a slender strut that is currently being studied as a part of the Enhanced Injection and Mixing Project (EIMP) at the NASA Langley Research Center. This injector is placed downstream of a Mach 6 facility nozzle, which simulates the high Mach number airflow at the entrance of a scramjet combustor, and injects helium, which is used as an inert substitute for hydrogen fuel. Experimental planar (P) LIF is obtained by using a UV laser to excite fluorescence from the NO molecules that are present in either a gas cell or the facility air used for the EIMP experiments. The experimental data are obtained for several segments of the NO fluorescence spectrum. The selected segments encompass LIF lines with rotational quantum numbers appropriate for low-to-moderate temperature flows similar to those corresponding to the nominal experimental flow conditions. The experimental LIF spectra are then evaluated from the data and compared with those obtained from the theoretical models. The theoretical spectra are obtained from LIFBASE and LINUS software, and from a simplified version of the two-level fluorescence model. The equivalent analytic PLIF images are also obtained by applying only the simplified model to the results of the Reynolds-averaged simulations (RAS) of the mixing flowfield. Good agreement between the experimental and theoretical results provides increased confidence in both the simplified LIF modeling and CFD simulations for further investigations of high-speed injector performance using this approach

TGF-beta(2)- and H2O2-Induced Biological Changes in Optic Nerve Head Astrocytes Are Reduced by the Antioxidant Alpha-Lipoic Acid
Background/Aims: The goal of the present study was to determine whether transforming growth factor-beta(2) (TGF-beta(2))- and oxidative stress-induced cellular changes in cultured human optic nerve head (ONH) astrocytes could be reduced by pretreatment with the antioxidant alpha-lipoic acid (LA). Methods: Cultured ONH astrocytes were treated with 1.0 ng/ml TGF-beta(2) for 24 h or 200 mu M hydrogen peroxide (H2O2) for 1 h. Lipid peroxidation was measured by a decrease in cis-pari-naric acid fluorescence. Additionally, cells were pretreated with different concentrations of LA before TGF-beta 2 or H2O2 exposure. Expressions of the heat shock protein (Hsp) alpha B-crystallin and Hsp27, the extracellular matrix (ECM) component fibronectin and the ECM-modulating protein connective tissue growth factor (CTGF) were examined with immunohistochemistry and real-time PCR analysis. Results: Both TGF-beta(2) and H2O2 increased lipid peroxidation. Treatment of astrocytes with TGF-beta(2) and H2O2 upregulated the expression of alpha B-crystallin, Hsp27, fibronectin and CTGF. Pretreatment with different concentrations of LA reduced the TGF-beta(2)- and H2O2-stimulated gene expressions. Conclusion: We showed that TGF-beta(2)- and H2O2-stimulated gene expressions could be prevented by pretreatment with the antioxidant LA in cultured human ONH astrocytes. How residents and interns utilise and perceive the personal digital assistant and UpToDate
<p>Abstract</p> <p>Background</p> <p>In this era of evidence-based medicine, doctors are increasingly using information technology to acquire medical knowledge. This study evaluates how residents and interns utilise and perceive the personal digital assistant (PDA) and the online resource UpToDate.</p> <p>Methods</p> <p>This is a questionnaire survey of all residents and interns in a tertiary teaching hospital.</p> <p>Results</p> <p>Out of 168 doctors, 134 (79.8%) responded to the questionnaire. Only 54 doctors (40.3%) owned a PDA. Although these owners perceived that the PDA was most useful for providing drug information, followed by medical references, scheduling and medical calculators, the majority of them did not actually have medical software applications downloaded on their PDAs. The greatest concerns highlighted for the PDA were the fear of loss and breakage, and the preference for working with desktop computers and paper. Meanwhile, only 76 doctors (56.7%) used UpToDate, even though the hospital had an institutional subscription for it. Although 93.4% of these users would recommend UpToDate to a colleague, only 57.9% stated that the use of UpToDate had led to a change in their management of patients.</p> <p>Conclusion</p> <p>Although UpToDate and various PDA software applications were deemed useful by some of the residents and interns in our study, both digital tools were under-utilised. More should be done to facilitate the use of medical software applications on PDAs, to promote awareness of tools for evidence-based medicine such as UpToDate, and to facilitate the application of evidence-based medicine in daily clinical practice.</p
Daptomycin versus standard therapy for bacteremia and endocarditis caused by Staphylococcus aureus.
BACKGROUND: Alternative therapies for Staphylococcus aureus bacteremia and endocarditis are needed.
METHODS: We randomly assigned 124 patients with S. aureus bacteremia with or without endocarditis to receive 6 mg of daptomycin intravenously per kilogram of body weight daily and 122 to receive initial low-dose gentamicin plus either an antistaphylococcal penicillin or vancomycin. The primary efficacy end point was treatment success 42 days after the end of therapy.
RESULTS: Forty-two days after the end of therapy in the modified intention-to-treat analysis, a successful outcome was documented for 53 of 120 patients who received daptomycin as compared with 48 of 115 patients who received standard therapy (44.2 percent vs. 41.7 percent; absolute difference, 2.4 percent; 95 percent confidence interval, -10.2 to 15.1 percent). Our results met prespecified criteria for the noninferiority of daptomycin. The success rates were similar in subgroups of patients with complicated bacteremia, right-sided endocarditis, and methicillin-resistant S. aureus. Daptomycin therapy was associated with a higher rate of microbiologic failure than was standard therapy (19 vs. 11 patients, P=0.17). In 6 of the 19 patients with microbiologic failure in the daptomycin group, isolates with reduced susceptibility to daptomycin emerged; similarly, a reduced susceptibility to vancomycin was noted in isolates from patients treated with vancomycin. As compared with daptomycin therapy, standard therapy was associated with a nonsignificantly higher rate of adverse events that led to treatment failure due to the discontinuation of therapy (17 vs. 8, P=0.06). Clinically significant renal dysfunction occurred in 11.0 percent of patients who received daptomycin and in 26.3 percent of patients who received standard therapy (P=0.004).
CONCLUSIONS: Daptomycin (6 mg per kilogram daily) is not inferior to standard therapy for S. aureus bacteremia and right-sided endocarditis. (ClinicalTrials.gov number, NCT00093067 [ClinicalTrials.gov].)
Prognostic factors in left-sided endocarditis: results from the andalusian multicenter cohort
<p>Abstract</p> <p>Background</p> <p>Despite medical advances, mortality in infective endocarditis (IE) is still very high. Previous studies on prognosis in IE have observed conflicting results. The aim of this study was to identify predictors of in-hospital mortality in a large multicenter cohort of left-sided IE.</p> <p>Methods</p> <p>An observational multicenter study was conducted from January 1984 to December 2006 in seven hospitals in Andalusia, Spain. Seven hundred and five left-side IE patients were included. The main outcome measure was in-hospital mortality. Several prognostic factors were analysed by univariate tests and then by multilogistic regression model.</p> <p>Results</p> <p>The overall mortality was 29.5% (25.5% from 1984 to 1995 and 31.9% from 1996 to 2006; Odds Ratio 1.25; 95% Confidence Interval: 0.97-1.60; p = 0.07). In univariate analysis, age, comorbidity, especially chronic liver disease, prosthetic valve, virulent microorganism such as <it>Staphylococcus aureus</it>, <it>Streptococcus agalactiae </it>and fungi, and complications (septic shock, severe heart failure, renal insufficiency, neurologic manifestations and perivalvular extension) were related with higher mortality. Independent factors for mortality in multivariate analysis were: Charlson comorbidity score (OR: 1.2; 95% CI: 1.1-1.3), prosthetic endocarditis (OR: 1.9; CI: 1.2-3.1), <it>Staphylococcus aureus </it>aetiology (OR: 2.1; CI: 1.3-3.5), severe heart failure (OR: 5.4; CI: 3.3-8.8), neurologic manifestations (OR: 1.9; CI: 1.2-2.9), septic shock (OR: 4.2; CI: 2.3-7.7), perivalvular extension (OR: 2.4; CI: 1.3-4.5) and acute renal failure (OR: 1.69; CI: 1.0-2.6). Conversely, <it>Streptococcus viridans </it>group etiology (OR: 0.4; CI: 0.2-0.7) and surgical treatment (OR: 0.5; CI: 0.3-0.8) were protective factors.</p> <p>Conclusions</p> <p>Several characteristics of left-sided endocarditis enable selection of a patient group at higher risk of mortality. This group may benefit from more specialised attention in referral centers and should help to identify those patients who might benefit from more aggressive diagnostic and/or therapeutic procedures.</p
How effective are on-farm conservation land management strategies for preserving ecosystem services in developing countries? A systematic map protocol
Background
An extensive body of literature in the field of agro-ecology claims to show the positive effects that maintenance of ecosystem services can have on sustainably meeting future food demand, by making farms more productive and resilient, and contributing to better nutrition and livelihoods of farmers. In Africa alone, some research has estimated a two-fold yield increase if food producers capitalize on new and existing knowledge from science and technology. Site-specific strategies adopted with the aim of improving ecosystem services may incorporate principles of multifunctional agriculture, sustainable intensification and conservation agriculture. However, a coherent synthesis and review of the evidence of these claims is largely absent, and the quality of much of this literature is questionable. Moreover, inconsistent effects have commonly been reported, while empirical evidence to support assumed improvements is largely lacking.
Objectives
This systematic map is stimulated by an interest to (1) collate evidence on the effectiveness of on-farm conservation land management for preserving and enhancing ecosystem services in agricultural landscapes, by drawing together the currently fragmented and multidisciplinary literature base, and (2) geographically map what indicators have been used to assess on-farm conservation land management. For both questions, we will focus on 74 low-income and developing countries, where much of the world’s agricultural expansion is occurring, yet 80% of arable land is already used and croplands are yielding well below their potential.
Methods/Design
To this end, reviewers will systematically search bibliographic databases for peer-reviewed research from Web of Science, SCOPUS, AGRICOLA, AGRIS databases and CAB abstracts, and grey literature from Google Scholar, and 22 subject-specific or institutional websites. Boolean search operators will be used to create search strings where applicable. Ecosystem services included in the study are pollination services; pest-, carbon-, soil-, and water-regulation; nutrient cycling; medicinal and aromatic plants; fuel wood and cultural services. Outputs of the systematic map will include a database, technical report and an online interactive map, searchable by topic. The results of this map are expected to provide clarity about synergistic outcomes of conservation land management, which will help support local decision-making
