La clínica tiene algunos aspectos peculiares, como puede ser la aparición de una sinovitis reactiva por la proximidad de la lesión a la articulación, con disminución de los arcos de movilidad normales. La radiología muestra el "nidus" típico de esta tumoración benigna, y ausencia de la esclerosis reactiva propia de las localizaciones metafisodiafisarias in tracorticales. La resección de la tumoración ha sido el tratamiento que resultó curativo
Intraoperative radiotherapy electron boost followed by moderate doses of external beam radiotherapy in resected soft-tissue sarcoma of the extremities
To analyze the patterns of failure and the toxicity profile of
intraoperative electron beam radiotherapy (IOERT) after resection of soft tissue
sarcomas of the extremities (STS). PATIENTS AND METHODS: Forty-five patients with
extremity STS were treated with IOERT and moderate-dose postoperative
radiotherapy (45-50 Gy). Twenty-six patients were treated for primary disease
(PD) and 19 patients for an isolated recurrence (ILR). Tumor size was >5 cm
(maximum diameter) in 36 patients (80%), and high-grade histology in PD patients
was present in 14 patients (54%). In nine patients, IOERT was used alone, due to
previous irradiation or patient refusal. Chemotherapy (neoadjuvant and/or
adjuvant) was mainly given to high-grade tumors. RESULTS: Nine patients relapsed
in the extremity (20%), and 12 patients in distant sites (28%). Actuarial local
control at 5 years was 88% for patients with negative/close margins and 57% for
patients presenting positive margins (P=0.04). Five patients (11%) developed
neuropathy associated with the treatment. Extremity preservation was achieved in
40 patients (88%). With a median follow-up of 93 months (range: 27-143 months)
for the patients at risk, 25 patients remain alive (a 7-year actuarial survival
rate of 75% for PD and 47% for ILR; P=0.01). CONCLUSIONS: IOERT combined with
moderate doses of external beam irradiation yields high local control and
extremity preservation rates in resected extremity STS. Peripheral nerves in the
IOERT field are dose-limiting structures requiring a dose compromise in the IOERT
component to avoid severe neurological damage
MSH3 polymorphisms and protein levels affect CAG repeat instability in huntington's disease mice
Expansions of trinucleotide CAG/CTG repeats in somatic tissues are thought to contribute to ongoing disease progression through an affected individual's life with Huntington's disease or myotonic dystrophy. Broad ranges of repeat instability arise between individuals with expanded repeats, suggesting the existence of modifiers of repeat instability. Mice with expanded CAG/CTG repeats show variable levels of instability depending upon mouse strain. However, to date the genetic modifiers underlying these differences have not been identified. We show that in liver and striatum the R6/1 Huntington's disease (HD) (CAG)~100 transgene, when present in a congenic C57BL/6J (B6) background, incurred expansion-biased repeat mutations, whereas the repeat was stable in a congenic BALB/cByJ (CBy) background. Reciprocal congenic mice revealed the Msh3 gene as the determinant for the differences in repeat instability. Expansion bias was observed in congenic mice homozygous for the B6 Msh3 gene on a CBy background, while the CAG tract was stabilized in congenics homozygous for the CBy Msh3 gene on a B6 background. The CAG stabilization was as dramatic as genetic deficiency of Msh2. The B6 and CBy Msh3 genes had identical promoters but differed in coding regions and showed strikingly different protein levels. B6 MSH3 variant protein is highly expressed and associated with CAG expansions, while the CBy MSH3 variant protein is expressed at barely detectable levels, associating with CAG stability. The DHFR protein, which is divergently transcribed from a promoter shared by the Msh3 gene, did not show varied levels between mouse strains. Thus, naturally occurring MSH3 protein polymorphisms are modifiers of CAG repeat instability, likely through variable MSH3 protein stability. Since evidence supports that somatic CAG instability is a modifier and predictor of disease, our data are consistent with the hypothesis that variable levels of CAG instability associated with polymorphisms of DNA repair genes may have prognostic implications for various repeat-associated diseases
InforMing the PAthway of COPD Treatment (IMPACT Trial) Single-Inhaler Triple Therapy (Fluticasone Furoate/Umeclidinium/Vilanterol) Versus Fluticasone Furoate/Vilanterol and Umeclidinium/Vilanterol in Patients With COPD: Analysis o the Western Europe and North America Regions
Chronic obstructive pulmonary disease (COPD) is a lung disease characterized by airflow limitation and progressive respiratory symptoms.1 Global public health trends estimate that the COPD burden will continue to rise, with COPD deaths estimated to increase to 4.4% of all deaths in Europe and 6.3% in the World Health Organization-defined region of the Americas by 2060.2 There are differences in the COPD burden in different regions reflecting variations in etiology,3,4 disease severity,5 symptoms,6 medication use,7 and health care systems and utilization.7 These differences may help inform therapeutic strategies to optimize therapeutic approaches to reducing symptoms and exacerbation risk.1
In the global InforMing the PAthway of COPD Treatment (IMPACT) trial, single-inhaler triple therapy fluticasone furoate/umeclidinium/vilanterol (FF/UMEC/VI) reduced moderate/severe exacerbation rates and improved lung function and health-related quality of life versus FF/VI or UMEC/VI dual therapy in patients ≥40 years of age with symptomatic COPD and a history of exacerbations.8 Within trial populations, regional differences such as patient characteristics, treatment patterns, access to care and cultural/socioeconomic factors may dictate treatment choices and influence disease severity and progression in particular geographical locations. For example, a meta-analysis conducted in 2015 comprising 123 studies between 1990 and 2010 found that the overall prevalence of COPD as well as the rate of increase was higher in the Americas (including both North and South America) compared with Europe.9 Furthermore, a cross-sectional study assessing the burden of COPD symptoms in the United States and Europe found variations between patients across countries who had experienced at least 1 symptom of COPD.10 In Europe, patients with more frequent symptoms were more likely to experience worsening of symptoms and unexpected hospitalization. Whereas in the United States, patients with more frequent symptoms were not only more likely to experience worsening of symptoms but also longer lasting symptoms and a longer length of exacerbations.10 A further difference was that treatment adherence was higher in the United States than Europe, however, adherence was consistent across patients in Europe when assessed by modified Global initiative for chronic Obstructive Lung Disease (GOLD) 2014 groups11 but varied in the United States with adherence highest in the GOLD Group C and lowest in Group A.10 Therefore, it is important to evaluate how overall population results pertain to patients treated in particular regions. As IMPACT is one of the largest trials conducted in patients with COPD to date, we have the unique opportunity to analyze study outcomes in patients enrolled in Western Europe and North America, the 2 main regions from an enrollment perspective
Highlights from the Pierre Auger Observatory
The Pierre Auger Observatory is the world's largest cosmic ray observatory.
Our current exposure reaches nearly 40,000 km str and provides us with an
unprecedented quality data set. The performance and stability of the detectors
and their enhancements are described. Data analyses have led to a number of
major breakthroughs. Among these we discuss the energy spectrum and the
searches for large-scale anisotropies. We present analyses of our X
data and show how it can be interpreted in terms of mass composition. We also
describe some new analyses that extract mass sensitive parameters from the 100%
duty cycle SD data. A coherent interpretation of all these recent results opens
new directions. The consequences regarding the cosmic ray composition and the

Determination of the b quark mass at the M_Z scale with the DELPHI detector at LEP
An experimental study of the normalized three-jet rate of b quark events with
respect to light quarks events (light= \ell \equiv u,d,s) has been performed
using the CAMBRIDGE and DURHAM jet algorithms. The data used were collected by
the DELPHI experiment at LEP on the Z peak from 1994 to 2000. The results are
found to agree with theoretical predictions treating mass corrections at
next-to-leading order. Measurements of the b quark mass have also been
performed for both the b pole mass: M_b and the b running mass: m_b(M_Z). Data
are found to be better described when using the running mass. The measurement
yields: m_b(M_Z) = 2.85 +/- 0.18 (stat) +/- 0.13 (exp) +/- 0.19 (had) +/- 0.12
(theo) GeV/c^2 for the CAMBRIDGE algorithm. This result is the most precise
measurement of the b mass derived from a high energy process. When compared to
other b mass determinations by experiments at lower energy scales, this value
agrees with the prediction of Quantum Chromodynamics for the energy evolution
of the running mass. The mass measurement is equivalent to a test of the
flavour independence of the strong coupling constant with an accuracy of 7

Measurement and Interpretation of Fermion-Pair Production at LEP energies above the Z Resonance
This paper presents DELPHI measurements and interpretations of
cross-sections, forward-backward asymmetries, and angular distributions, for
the e+e- -> ffbar process for centre-of-mass energies above the Z resonance,
from sqrt(s) ~ 130 - 207 GeV at the LEP collider. The measurements are
consistent with the predictions of the Standard Model and are used to study a
variety of models including the S-Matrix ansatz for e+e- -> ffbar scattering
and several models which include physics beyond the Standard Model: the
exchange of Z' bosons, contact interactions between fermions, the exchange of
gravitons in large extra dimensions and the exchange of sneutrino in R-parity

A Determination of the Centre-of-Mass Energy at LEP2 using Radiative 2-fermion Events
Using e+e- -> mu+mu-(gamma) and e+e- -> qqbar(gamma) events radiative to the
Z pole, DELPHI has determined the centre-of-mass energy, sqrt{s}, using energy
and momentum constraint methods. The results are expressed as deviations from
the nominal LEP centre-of-mass energy, measured using other techniques. The
results are found to be compatible with the LEP Energy Working Group estimates

Study of Tau-pair Production in Photon-Photon Collisions at LEP and Limits on the Anomalous Electromagnetic Moments of the Tau Lepton
Tau-pair production in the process e+e- -> e+e-tau+tau- was studied using
data collected by the DELPHI experiment at LEP2 during the years 1997 - 2000.
The corresponding integrated luminosity is 650 pb^{-1}. The values of the
cross-section obtained are found to be in agreement with QED predictions.
Limits on the anomalous magnetic and electric dipole moments of the tau lepton
