    Krebspatienten sind durch die Diagnose und der damit verbundenen Therapie enormen Belastungen ausgesetzt. Die gesundheitsbezogene Lebensqualität (LQ) hat sich als relevanter patientenberichteter Endpunkt (patient-reported outcome, PRO) in der klinischen Forschung und Patientenversorgung durchgesetzt. Ein etabliertes Instrument zur Erfassung der LQ von Krebspatienten ist der EORTC QLQ-C30. Dieses Projekt verfolgte mehrere Ziele: Untersuchung des Einflusses der Response-Skala sowie positiver und negativer Affekte auf die berichtete LQ. Frühere Forschungsergebnisse wiesen darauf hin, dass die Response-Kategorie 3 „mäßig“ die Intervallskalierung der 4-Punkt-Likert Skala des QLQ-C30 verletzt, was sich folgend auf die berichteten Symptom- und Funktionswerte auswirken könnte (Scaling-Effekt). In einem Set von drei Studien zur Überprüfung der Intervalleigenschaften von „mäßig“ wurden insgesamt 334 Teilnehmer eingeschlossen. Unter Beibehaltung der 4-Punkt-Likert-Skala hat sich „ziemlich“ als geeigneterer Begriff für die Response-Kategorie 3 herauskristallisiert, was zur Erstellung einer aktualisierten QLQ-C30 Version führte. In anschließenden Studien an 450 deutschsprachigen Krebspatienten und 2033 Probanden der deutschen Normalbevölkerung bestätigte sich ein Scaling-Effekt. Wie vermutet, waren die Funktionswerte in der aktualisierten „ziemlich“ Version tendenziell höher und die Symptomwerte tendenziell geringer als in der ursprünglichen „mäßig“ Version. Dieser Effekt war innerhalb von Patienten mit eingeschränkter LQ stärker. Folgend wurde der aktualisierte EORTC QLQ-C30 in Deutschland eingeführt. Der Zusammenhang zwischen Affekt (PANAS), Coping (Brief COPE), psychoonkologischen Betreuungsbedarf (HSI) und LQ (EORTC QLQ-C30 „ziemlich“ Version) ist komplex und noch nicht vollständig erfasst. In dieser Arbeit wurden zwei theoretische Wirkungsweisen (Moderation und Mediation) der aktuellen Stimmung betrachtet. Affekte wirken innerhalb von stressvollen Situationen prädisponierend auf die Einschätzung der Situationen sowie auf die Wahl der Coping-Strategie. Affekte haben zudem Einflüsse auf die berichtete LQ, speziell von Symptomen. Die vermittelnde Rolle von Affekten im Zusammenspiel der Konstrukte konnte teilweise gezeigt werden. Dahingegen ist eine moderierende Rolle von Affekten auszuschließen. Im Rahmen einer Krebsbehandlung sollte der Fokus nicht nur auf Symptom-Ebene gelegt werden. Den Patienten sollten aktiv psychoonkologische Unterstützung in einem ganzheitlichen Ansatz angeboten werden, der auch Affekte einschließt

    Amyotrophic lateral sclerosis (ALS) is a progressive motor neuronal disorder characterized by neuronal degeneration and currently no effective cure is available to stop or delay the disease from progression. Transplantation of murine glial-restricted precursors (mGRPs) is an attractive strategy to modulate ALS development and advancements such as the use of immune modulators could potentially extend graft survival and function. Using a well-established ALS transgenic mouse model (SOD1G93A), we tested mGRPs in combination with the immune modulators synthetic PreImplantation Factor (sPIF), Tacrolimus (Tac), and Costimulatory Blockade (CB). We report that transplantation of mGRPs into the cisterna magna did not result in increased mice survival. The addition of immunomodulatory regimes again did not increase mice lifespan but improved motor functions and sPIF was superior compared to other immune modulators. Immune modulators did not affect mGRPs engraftment significantly but reduced pro-inflammatory cytokine production. Finally, sPIF and CB reduced the number of microglial cells and prevented neuronal number loss. Given the safety profile and a neuroprotective potential of sPIF, we envision its clinical application in near future

    Revising quantum optical phenomena in adatoms coupled to graphene nanoantennas

    Graphene flakes acting as photonic nanoantennas may sustain strong electromagnetic field localization and enhancement. To exploit the field enhancement, quantum emitters such as atoms or molecules should be positioned in such close proximity to the flake that electron tunneling might influence the optical and electronic properties of the system. However, tunneling is usually not considered if the optical coupling mechanism between quantum emitters and nanoantennas is at focus. This work presents a framework for describing the electron dynamics in hybrid systems consisting of graphene nanoflakes coupled both electronically and optically to adatoms and subject to external illumination. Our framework combines the single-particle tight-binding approach with a nonlinear master equation formalism that captures both optical and electronic interactions. We apply the framework to demonstrate the impact of electron tunneling between the adatom and the flake on emblematic quantum optical phenomena: degradation of coherent Rabi oscillations and quenching of Purcell spontaneous emission enhancement in two-level adatoms in proximity of triangular graphene nanoflakes

    From single-particle-like to interaction-mediated plasmonic resonances in graphene nanoantennas

    Plasmonic nanostructures attract tremendous attention as they confine electromagnetic fields well below the diffraction limit while simultaneously sustaining extreme local field enhancements. To fully exploit these properties, the identification and classification of resonances in such nanostructures is crucial. Recently, a novel figure of merit for resonance classification has been proposed 1 and its applicability was demonstrated mostly to toy model systems. This novel measure, the energy-based plasmonicity index (EPI), characterizes the nature of resonances in molecular nanostructures. The EPI distinguishes between either a single-particle-like or a plasmonic nature of resonances based on the energy space coherence dynamics of the excitation. To advance the further development of this newly established measure, we present here its exemplary application to characterize the resonances of graphene nanoantennas. In particular, we focus on resonances in a doped nanoantenna. The structure is of interest, as a consideration of the electron dynamics in real space might suggest a plasmonic nature of selected resonances in the low doping limit but our analysis reveals the opposite. We find that in the undoped and moderately doped nanoantenna, the EPI classifies all emerging resonances as predominantly single-particle-like and only after doping the structure heavily, the EPI observes plasmonic response.Comment: The following article has been submitted to the Journal of Applied Physic

    A modern look at a medieval bilayer metal leaf: nanotomography of Zwischgold

    Many European sculptures and altarpieces from the Middle Ages were decorated with Zwischgold, a bilayer metal leaf with an ultra-thin gold face backed by silver. Zwischgold corrodes quickly when exposed to air, causing the surface of the artefact to darken and lose gloss. The conservation of such Zwischgold applied artefacts has been an obstinate problem. We have acquired quantitative, 3D nanoscale images of Zwischgold samples from 15th century artefacts and modern materials using ptychographic X-ray computed tomography (PXCT), a recently developed coherent diffractive imaging technique, to investigate the leaf structure and chemical state of Zwischgold. The measurements clearly demonstrate decreasing density (increasing porosity) of the leaf materials and their corrosion products, as well as delamination of the leaves from their substrate. Each of these effects speak to typically observed issues in the conservation of such Zwischgold applied artefacts. Further, a rare variant of Zwischgold that contains extremely thin multiple gold layers and an overlapping phenomenon of Zwischgold with other metal leaves are observed through PXCT.     A Prospective, Randomised, Controlled, Split-Face Clinical Trial to Assess the Safety and the Efficacy of Cold Atmospheric Plasma in the Treatment of Acne Vulgaris

    The increase in antibiotic resistance requires effective non-antibiotic therapies for acne. Cold atmospheric plasma (CAP) inactivates bacteria and improves wound healing, but its effect on acne has not been investigated. The objective of this controlled split-face study was to assess safety and efficacy of CAP in moderate acne. One side of the face received 8–10 treatments with cold helium plasma within 4–6 weeks; follow-up was two and four weeks thereafter. Acne lesions were counted, followed by global acne severity ratings. Of the 34 patients included, 29 completed the study. No serious adverse events occurred. The two facial sides did not significantly differ in the number of inflammatory and non-inflammatory lesions. An interaction effect of number and type of treatment was found for inflammatory lesions. Lesion reduction after 10 treatments was significantly higher on the treated than on the untreated side. Percentage of patients reporting improved aesthetics was higher for the treated than for the untreated side after treatment completion (79% vs. 45%) and at the two- (72% vs. 45%) and four-week follow-up (79% vs. 52%). In conclusion, CAP was safe with excellent tolerability, showed moderate reduction in acne lesions and led to higher patient-based ratings of aesthetics than non-treatment

    Neuronal Redox-Imbalance in Rett Syndrome Affects Mitochondria as Well as Cytosol, and Is Accompanied by Intensified Mitochondrial O2 Consumption and ROS Release

    Rett syndrome (RTT), an X chromosome-linked neurodevelopmental disorder affecting almost exclusively females, is associated with various mitochondrial alterations. Mitochondria are swollen, show altered respiratory rates, and their inner membrane is leaking protons. To advance the understanding of these disturbances and clarify their link to redox impairment and oxidative stress, we assessed mitochondrial respiration in defined brain regions and cardiac tissue of male wildtype (WT) and MeCP2-deficient (Mecp2-/y) mice. Also, we quantified for the first time neuronal redox-balance with subcellular resolution in cytosol and mitochondrial matrix. Quantitative roGFP1 redox imaging revealed more oxidized conditions in the cytosol of Mecp2-/y hippocampal neurons than in WT neurons. Furthermore, cytosol and mitochondria of Mecp2-/y neurons showed exaggerated redox-responses to hypoxia and cell-endogenous reactive oxygen species (ROS) formation. Biochemical analyzes exclude disease-related increases in mitochondrial mass in Mecp2-/y hippocampus and cortex. Protein levels of complex I core constituents were slightly lower in Mecp2-/y hippocampus and cortex than in WT; those of complex V were lower in Mecp2-/y cortex. Respiratory supercomplex-formation did not differ among genotypes. Yet, supplied with the complex II substrate succinate, mitochondria of Mecp2-/y cortex and hippocampus consumed more O2 than WT. Furthermore, mitochondria from Mecp2-/y hippocampus and cortex mediated an enhanced oxidative burden. In conclusion, we further advanced the molecular understanding of mitochondrial dysfunction in RTT. Intensified mitochondrial O2 consumption, increased mitochondrial ROS generation and disturbed redox balance in mitochondria and cytosol may represent a causal chain, which provokes dysregulated proteins, oxidative tissue damage, and contributes to neuronal network dysfunction in RTT

    Side effects by oral application of atmospheric pressure plasma on the mucosa in mice

    Cold atmospheric pressure plasma (CAP) has been investigated with promising results for peri-implant diseases treatment. However, prior to in-vivo applications of CAP sources in humans, short-term harmful mucosal damage or other unwanted side effects have to be reviewed. 180 male mice (B6C3F1) were divided into twelve treatment groups (n = 15). The right buccal cheek mucosa was treated with CAP. The first and second group each received continuous 10 sec irradiation with 2 different plasma sources (kINPen09, PS-MWM). The third group was treated with the kINPen09 for one minute. Control groups were treated with a corresponding dose of ultraviolet light for 8 seconds or 48 seconds and the other one was left untreated. The animals were weighed before and after treatment. The animals were sacrificed one day or one week after exposure. Stained tissue samples were histologically examined for tissue damage independently by two experienced pathologists. One day after CAP treatment histological analysis showed focal mucosal erosion with superficial ulceration and necrosis accompanied by a mild inflammatory reaction. One week after CAP treatment, the mucosal defects were completely re-epithelialized, associated with remnants of granulation tissue in the stroma irrespective of treatment duration. Furthermore, no cytological atypia was found and no severe weight loss occurred. The control groups did not show any alterations at all. CAP treatment led to a superficial mucosal damage that healed within few days. Nonetheless, further long-term experiments are necessary to exclude undesirable side effects after longer observation time. Particularly, potential carcinogenic effects must be ruled out prior to the application of CAP treatment in daily dental practice

    Role of clinical and CT findings in the identification of adult small-bowel intussusception requiring surgical intervention

    Background In adults, intussusception has been considered traditionally to have an underlying aetiology. The aim of this study was to determine CT and clinical features of small-bowel intussusceptions that required surgical intervention. Methods Adult patients were identified in whom small-bowel intussusceptions were noted on CT images. The appearance, number, type (enteroenteric versus enterocolic), length and maximum short-axis diameter of intussusceptions, and presence of bowel obstruction (short-axis diameter of proximal small bowel greater than 3 cm) were analysed. The outcome was defined as surgical (complicated) or self-limiting (uncomplicated). Associations between complicated and uncomplicated intussusceptions and patient characteristics were investigated. Results Among 75 patients (56 male) with a mean age of 45 years, 103 intussusceptions were identified, of which 98 (95 per cent) were enteroenteric and 5 (5 per cent) enterocolic. Only 12 of 103 intussusceptions (12 per cent) in 12 of 75 (16 per cent) patients required surgical therapy and were considered to be complicated, with half of these having a neoplastic lead point. Length (P < 0.001), diameter (P < 0.001) and type (P = 0.002) of intussusception as well as presence of vessels (P = 0.023) within an intussusception on a CT scan, clinical symptoms (P = 0.007) and signs of bowel obstruction (P < 0.001) were associated with a surgical outcome. Conclusion Clinical symptoms, signs of bowel obstruction, type and length of intussusception, and a visible tumour within an intussusception on CT scan were critical signs of complicated intussusception, requiring surgical intervention
