    Krebspatienten sind durch die Diagnose und der damit verbundenen Therapie enormen Belastungen ausgesetzt. Die gesundheitsbezogene Lebensqualität (LQ) hat sich als relevanter patientenberichteter Endpunkt (patient-reported outcome, PRO) in der klinischen Forschung und Patientenversorgung durchgesetzt. Ein etabliertes Instrument zur Erfassung der LQ von Krebspatienten ist der EORTC QLQ-C30. Dieses Projekt verfolgte mehrere Ziele: Untersuchung des Einflusses der Response-Skala sowie positiver und negativer Affekte auf die berichtete LQ. Frühere Forschungsergebnisse wiesen darauf hin, dass die Response-Kategorie 3 „mäßig“ die Intervallskalierung der 4-Punkt-Likert Skala des QLQ-C30 verletzt, was sich folgend auf die berichteten Symptom- und Funktionswerte auswirken könnte (Scaling-Effekt). In einem Set von drei Studien zur Überprüfung der Intervalleigenschaften von „mäßig“ wurden insgesamt 334 Teilnehmer eingeschlossen. Unter Beibehaltung der 4-Punkt-Likert-Skala hat sich „ziemlich“ als geeigneterer Begriff für die Response-Kategorie 3 herauskristallisiert, was zur Erstellung einer aktualisierten QLQ-C30 Version führte. In anschließenden Studien an 450 deutschsprachigen Krebspatienten und 2033 Probanden der deutschen Normalbevölkerung bestätigte sich ein Scaling-Effekt. Wie vermutet, waren die Funktionswerte in der aktualisierten „ziemlich“ Version tendenziell höher und die Symptomwerte tendenziell geringer als in der ursprünglichen „mäßig“ Version. Dieser Effekt war innerhalb von Patienten mit eingeschränkter LQ stärker. Folgend wurde der aktualisierte EORTC QLQ-C30 in Deutschland eingeführt. Der Zusammenhang zwischen Affekt (PANAS), Coping (Brief COPE), psychoonkologischen Betreuungsbedarf (HSI) und LQ (EORTC QLQ-C30 „ziemlich“ Version) ist komplex und noch nicht vollständig erfasst. In dieser Arbeit wurden zwei theoretische Wirkungsweisen (Moderation und Mediation) der aktuellen Stimmung betrachtet. Affekte wirken innerhalb von stressvollen Situationen prädisponierend auf die Einschätzung der Situationen sowie auf die Wahl der Coping-Strategie. Affekte haben zudem Einflüsse auf die berichtete LQ, speziell von Symptomen. Die vermittelnde Rolle von Affekten im Zusammenspiel der Konstrukte konnte teilweise gezeigt werden. Dahingegen ist eine moderierende Rolle von Affekten auszuschließen. Im Rahmen einer Krebsbehandlung sollte der Fokus nicht nur auf Symptom-Ebene gelegt werden. Den Patienten sollten aktiv psychoonkologische Unterstützung in einem ganzheitlichen Ansatz angeboten werden, der auch Affekte einschließt

    Revising quantum optical phenomena in adatoms coupled to graphene nanoantennas

    Graphene flakes acting as photonic nanoantennas may sustain strong electromagnetic field localization and enhancement. To exploit the field enhancement, quantum emitters such as atoms or molecules should be positioned in such close proximity to the flake that electron tunneling might influence the optical and electronic properties of the system. However, tunneling is usually not considered if the optical coupling mechanism between quantum emitters and nanoantennas is at focus. This work presents a framework for describing the electron dynamics in hybrid systems consisting of graphene nanoflakes coupled both electronically and optically to adatoms and subject to external illumination. Our framework combines the single-particle tight-binding approach with a nonlinear master equation formalism that captures both optical and electronic interactions. We apply the framework to demonstrate the impact of electron tunneling between the adatom and the flake on emblematic quantum optical phenomena: degradation of coherent Rabi oscillations and quenching of Purcell spontaneous emission enhancement in two-level adatoms in proximity of triangular graphene nanoflakes

    From single-particle-like to interaction-mediated plasmonic resonances in graphene nanoantennas

    Plasmonic nanostructures attract tremendous attention as they confine electromagnetic fields well below the diffraction limit while simultaneously sustaining extreme local field enhancements. To fully exploit these properties, the identification and classification of resonances in such nanostructures is crucial. Recently, a novel figure of merit for resonance classification has been proposed 1 and its applicability was demonstrated mostly to toy model systems. This novel measure, the energy-based plasmonicity index (EPI), characterizes the nature of resonances in molecular nanostructures. The EPI distinguishes between either a single-particle-like or a plasmonic nature of resonances based on the energy space coherence dynamics of the excitation. To advance the further development of this newly established measure, we present here its exemplary application to characterize the resonances of graphene nanoantennas. In particular, we focus on resonances in a doped nanoantenna. The structure is of interest, as a consideration of the electron dynamics in real space might suggest a plasmonic nature of selected resonances in the low doping limit but our analysis reveals the opposite. We find that in the undoped and moderately doped nanoantenna, the EPI classifies all emerging resonances as predominantly single-particle-like and only after doping the structure heavily, the EPI observes plasmonic response.Comment: The following article has been submitted to the Journal of Applied Physic

    A Prospective, Randomised, Controlled, Split-Face Clinical Trial to Assess the Safety and the Efficacy of Cold Atmospheric Plasma in the Treatment of Acne Vulgaris

    The increase in antibiotic resistance requires effective non-antibiotic therapies for acne. Cold atmospheric plasma (CAP) inactivates bacteria and improves wound healing, but its effect on acne has not been investigated. The objective of this controlled split-face study was to assess safety and efficacy of CAP in moderate acne. One side of the face received 8–10 treatments with cold helium plasma within 4–6 weeks; follow-up was two and four weeks thereafter. Acne lesions were counted, followed by global acne severity ratings. Of the 34 patients included, 29 completed the study. No serious adverse events occurred. The two facial sides did not significantly differ in the number of inflammatory and non-inflammatory lesions. An interaction effect of number and type of treatment was found for inflammatory lesions. Lesion reduction after 10 treatments was significantly higher on the treated than on the untreated side. Percentage of patients reporting improved aesthetics was higher for the treated than for the untreated side after treatment completion (79% vs. 45%) and at the two- (72% vs. 45%) and four-week follow-up (79% vs. 52%). In conclusion, CAP was safe with excellent tolerability, showed moderate reduction in acne lesions and led to higher patient-based ratings of aesthetics than non-treatment

    Dickkopf 3—A New Indicator for the Deterioration of Allograft Function After Kidney Transplantation

    Evidence of tubular atrophy and interstitial fibrosis is prognostically unfavorable and associated with a premature graft loss after kidney transplantation. Recently, Dickkopf 3 (DKK3), a profibrotic glycoprotein released by stressed tubular epithelial cells, has been identified to cause IF/TA by regulating the Wnt/β-catenin signaling and seems to engage a T-cell response. The aim of our study was to determine if a correlation between DKK3 and graft function exists and if DKK3 could be a new indicator to identify patients at risk for a deterioration in graft function. Patients, transplanted between 2016 and 2018, were analyzed with regard to DKK3 in the urine and graft function (creatinine, eGFR, albuminuria). Multivariable analyzes were used including known factors influencing graft function (PRA, donor age) to stress robustness of DKK3. The 3 and 12 month DKK3 values were significant predictors for subsequent graft function up to 36 months. An increase of DKK3 from month 3 to 12 of ≥ 25% showed a higher risk of an impaired graft function, with, e.g., a reduction in eGFR of about 9–10 ml/min in contrast to patients without intensified DKK3 increase. Induction therapy has an influence on DKK3 as patients induced with a T-cell depleting therapy showed a trend toward lower DKK3 values. In summary, our study is the first investigation of DKK3 in kidney transplant recipients and was able to show that DKK3 could forecast graft function. It is recommended to investigate the potential of DKK3 as a predictor of kidney function after transplantation in further studies

    Immunolocalization of dually phosphorylated MAPKs in dividing root meristem cells of Vicia faba, Pisum sativum, Lupinus luteus and Lycopersicon esculentum

    Key message In plants, phosphorylated MAPKs display constitutive nuclear localization; however, not all studied plant species show co-localization of activated MAPKs to mitotic microtubules. Abstract The mitogen-activated protein kinase (MAPK) signaling pathway is involved not only in the cellular response to biotic and abiotic stress but also in the regulation of cell cycle and plant development. The role of MAPKs in the formation of a mitotic spindle has been widely studied and the MAPK signaling pathway was found to be indispensable for the unperturbed course of cell division. Here we show cellular localization of activated MAPKs (dually phosphorylated at their TXY motifs) in both interphase and mitotic root meristem cells of Lupinus luteus, Pisum sativum, Vicia faba (Fabaceae) and Lycopersicon esculentum (Solanaceae). Nuclear localization of activated MAPKs has been found in all species. Colocalization of these kinases to mitotic microtubules was most evident in L. esculentum, while only about 50 % of mitotic cells in the root meristems of P. sativum and V. faba displayed activated MAPKs localized to microtubules during mitosis. Unexpectedly, no evident immunofluorescence signals at spindle microtubules and phragmoplast were noted in L. luteus. Considering immunocytochemical analyses and studies on the impact of FR180204 (an inhibitor of animal ERK1/2) on mitotic cells, we hypothesize that MAPKs may not play prominent role in the regulation of microtubule dynamics in all plant species

    Expression of Low Affinity Nerve Growth Factor Receptor p75 in Classic Bladder Exstrophy

    Successful primary closure of classic bladder exstrophy (BE) is crucial for development of bladder capacity and voided continence. It is universally agreed that an intensive pain management including the use of caudal epidural anesthesia is an essential cornerstone for the outcome of this complex surgery. Whether and to what extent pain is caused by structural or functional changes is not yet known. The nerve growth factor (NGF) is regarded as a marker for pain in different bladder disorders. This prospective study investigated the role of histological alterations and NGF in patients with BE including 34 patients with BE and 6 patients with congenital vesicoureterorenal reflux (VUR) who served as controls. Between January 2015 and April 2020 transmural bladder biopsies were taken from the posterior bladder wall during delayed primary bladder closure. The samples were stained for histological evaluation and subjected to immunohistochemistry to analyze NGFR p75. Differences in histological alterations were examined with Fisher's exact test, and Mann-Whitney-U-test was used to compare the NGFR p75 staining intensity between patients with BE and controls. Patients with BE showed significantly more often acute inflammation (p < 0.001), squamous metaplasia (p = 0.002), and cystitis glandularis (p = 0.005) as well as NGFR p75 in the urothelium (p = 0.003) than patients with VUR. A limitation of this study is the small number of participants due to the rare disease entity. Similar to other painful bladder disorders, pain transmission in BE after intitial closure may in part be facilitated by elevated NGF signaling through its receptor

    Repeated exposure of the oral mucosa over 12 months with cold plasma is not carcinogenic in mice

    Peri-implantitis may result in the loss of dental implants. Cold atmospheric pressure plasma (CAP) was suggested to promote re-osseointegration, decrease antimicrobial burden, and support wound healing. However, the long-term risk assessment of CAP treatment in the oral cavity has not been addressed. Treatment with two different CAP devices was compared against UV radiation, carcinogen administration, and untreated conditions over 12 months. Histological analysis of 406 animals revealed that repeated CAP exposure did not foster non-invasive lesions or squamous cell carcinoma (SCCs). Carcinogen administration promoted non-invasive lesions and SCCs. Molecular analysis by a qPCR screening of 144 transcripts revealed distinct inflammatory profiles associated with each treatment regimen. Interestingly, CAP treatment of carcinogen-challenged mucosa did not promote but instead left unchanged or reduced the proportion of non-invasive lesions and SCC formation. In conclusion, repeated CAP exposure of murine oral mucosa was well tolerated, and carcinogenic effects did not occur, motivating CAP applications in patients for dental and implant treatments in the future

    The discovery of the possible reasons for delayed graduation and dropout in the light of a qualitative research study

    Higher educational dropout is a significant area of education policy in Hungary. First, the proportion of graduated higher educational students is low when compared to the OECD average, which may be caused by dropout from higher educational courses. On the other hand, although the phenomenon of dropout has been closely investigated in several international research papers, the methodology used to determine the dropout ratio is unsatisfactory, mainly due to the lack of expert consensus. As a consequence, we do not have precise data regarding the dropout ratios, which make investigations related to this area even more necessary. The aim of this study was to measure the possible reasons for delayed graduation and dropout, and it was carried out as a qualitative study based on existing theories. In our investigation, the role of the sociocultural background; the years prior to the time spent in higher education; and the motivation of the choice of institution, employment, sports, and social activities were measured through an analysis of seven individual interviews and one focus group conversation involving 10 participants based on a semi-structured interview methodology. The causes of delayed graduation and dropout, which are more difficult to observe, are an inappropriately chosen institution and/or course, employment while studying intensively in a higher education institution, competitive sport and friends with a negative attitude toward learning. Our analysis provides a stable basis for a wider questionnaire-based investigation on a representative sample and its main units have been developed according to the research blocks of the interview analysis
