    A new sea ice albedo scheme including melt ponds for ECHAM5 general circulation model

    Today we experience an accelerated melting of sea ice in the Arctic which global circulation models are inadequate to predict. We believe one of the reasons is the shortcomings in the sea ice albedo schemes for these models. This paper investigates a physically based sea ice albedo scheme for ECHAM5 GCM, which separates between snow-covered sea ice, bare sea ice, melt ponds, and open water (separately for the albedos and albedo fractions). The new albedo scheme includes important components such as albedo decay due to snow aging, bare sea ice albedo dependent on the ice thickness, and a melt pond albedo dependent on the melt pond depth. The explicit treatment of melt pond albedos has, to our knowledge, not been included in general circulation models before and represents a substantial improvement when simulating the annual cycle of sea ice albedo. The new albedo scheme overall reduces the sea ice albedo both in winter, because of snow aging, and in summer, because of melt ponds. The reduced sea ice albedo leads to overall reduced sea ice thickness, concentration, and volume, with large temporal and spatial variations. In the Northern Hemisphere in March, some areas experience increased albedo, resulting in thicker sea ice and higher ice concentration, but in August the pattern is spatially homogeneous, with reduced albedo, thickness, and concentrations for all areas where the new scheme has a significant effect.     Incidence and mortality of hip fracture among the elderly population in South Korea: a population-based study using the National Health Insurance claims data

    <p>Abstract</p> <p>Background</p> <p>The lack of epidemiologic information on osteoporotic hip fractures hampers the development of preventive or curative measures against osteoporosis in South Korea. We conducted a population-based study to estimate the annual incidence of hip fractures. Also, we examined factors associated with post-fracture mortality among Korean elderly to evaluate the impact of osteoporosis on our society and to identify high-risk populations.</p> <p>Methods</p> <p>The Korean National Health Insurance (NHI) claims database was used to identify the incidence of hip fractures, defined as patients having a claim record with a diagnosis of hip fracture and a hip fracture-related operation during 2003. The 6-month period prior to 2003 was set as a 'window period,' such that patients were defined as incident cases only if their first record of fracture was observed after the window period. Cox's proportional hazards model was used to investigate the relationship between survival time and baseline patient and provider characteristics available from the NHI data.</p> <p>Results</p> <p>The age-standardized annual incidence rate of hip fractures requiring operation over 50 years of age was 146.38 per 100,000 women and 61.72 per 100,000 men, yielding a female to male ratio of 2.37. The 1-year mortality was 16.55%, which is 2.85 times higher than the mortality rate for the general population (5.8%) in this age group. The risk of post-fracture mortality at one year is significantly higher for males and for persons having lower socioeconomic status, living in places other than the capital city, not taking anti-osteoporosis pharmacologic therapy following fracture, or receiving fracture-associated operations from more advanced hospitals such as general or tertiary hospitals.</p> <p>Conclusion</p> <p>This national epidemiological study will help raise awareness of osteoporotic hip fractures among the elderly population and hopefully motivate public health policy makers to develop effective national prevention strategies against osteoporosis to prevent hip fractures.</p

    Persistence and compliance to antidepressant treatment in patients with depression: A chart review

    <p>Abstract</p> <p>Background</p> <p>Adherence has recently been suggested to be divided into these two components: persistence (i.e., whether patients continue treatment or not) and compliance (i.e., whether patients take doses as instructed). However, no study has yet assessed these two clinically relevant components at the same time in adherence to antidepressant treatment in the clinical outpatient setting.</p> <p>Methods</p> <p>In this retrospective chart-review, 6-month adherence to antidepressants was examined in 367 outpatients with a major depressive disorder (ICD-10) (170 males; mean ± SD age 37.6 ± 13.9 years), who started antidepressant treatment from April 2006 through March 2007. Additionally, we evaluated Medication Possession Rate (MPR), defined as the total days a medication was dispensed to patients divided by the treatment period.</p> <p>Results</p> <p>Only 161 patients (44.3%) continued antidepressant treatment for 6 months. Among 252 patients who discontinued their initial antidepressant, 63.1% of these patients did so without consulting their physicians. Sertraline use was associated with a higher persistence rate at month 6 (odds ratio 2.59 in comparison with sulpiride), and the use of anxiolytic benzodiazepines had a positive effect on persistence to antidepressant treatment only at month 1 (odds ratio 2.14). An overall MPR was 0.77; 55.6% of patients were considered compliant (i.e., a MPR of ≥ 0.8).</p> <p>Conclusion</p> <p>Given a high rate of antidepressant discontinuation without consulting their physicians, closer communication between patients and their physicians should be encouraged. Although the use of anxiolytic benzodiazepines was associated with a higher persistence to antidepressant treatment at month 1, the use of these drugs should be avoided as a rule, given their well-known serious adverse effects.</p

    CD39 and control of cellular immune responses

    CD39 is the cell surface-located prototypic member of the ecto-nucleoside triphosphate diphosphohydrolase (E-NTPDase) family. Biological actions of CD39 are a consequence (at least in part) of the regulated phosphohydrolytic activity on extracellular nucleotides. This ecto-enzymatic cascade in tandem with CD73 (ecto-5–nucleotidase) also generates adenosine and has major effects on both P2 and adenosine receptor signalling. Despite the early recognition of CD39 as a B lymphocyte activation marker, little is known of the role of CD39 in humoral or cellular immune responses. There is preliminary evidence to suggest that CD39 may impact upon antibody affinity maturation. Pericellular nucleotide/nucleoside fluxes caused by dendritic cell expressed CD39 are also involved in the recruitment, activation and polarization of naïve T cells. We have recently explored the patterns of CD39 expression and the functional role of this ecto-nucleotidase within quiescent and activated T cell subsets. Our data indicate that CD39, together with CD73, efficiently distinguishes T regulatory cells (Treg) from other resting or activated T cells in mice (and humans). Furthermore, CD39 serves as an integral component of the suppressive machinery of Treg, acting, at least in part, through the modulation of pericellular levels of adenosine. We have also shown that the coordinated regulation of CD39/CD73 expression and of the adenosine receptor A2A activates an immunoinhibitory loop that differentially regulates Th1 and Th2 responses. The in vivo relevance of this network is manifest in the phenotype of Cd39-null mice that spontaneously develop features of autoimmune diseases associated with Th1 immune deviation. These data indicate the potential of CD39 and modulated purinergic signalling in the co-ordination of immunoregulatory functions of dendritic and Treg cells. Our findings also suggest novel therapeutic strategies for immune-mediated diseases

    CD83 Modulates B Cell Function In Vitro: Increased IL-10 and Reduced Ig Secretion by CD83Tg B Cells

    The murine transmembrane glycoprotein CD83 is an important regulator for both thymic T cell maturation and peripheral T cell responses. Recently, we reported that CD83 also has a function on B cells: Ubiquitous transgenic (Tg) expression of CD83 interfered with the immunoglobulin (Ig) response to infectious agents and to T cell dependent as well as T cell independent model antigen immunization. Here we compare the function of CD83Tg B cells that overexpress CD83 and CD83 mutant (CD83mu) B cells that display a drastically reduced CD83 expression. Correlating with CD83 expression, the basic as well as the lipopolysaccharide (LPS) induced expression of the activation markers CD86 and MHC-II are significantly increased in CD83Tg B cells and reciprocally decreased in CD83mu B cells. Wild-type B cells rapidly upregulate CD83 within three hours post BCR or TLR engagement by de novo protein synthesis. The forced premature overexpression of CD83 on the CD83Tg B cells results in reduced calcium signaling, reduced Ig secretion and a reciprocally increased IL-10 production upon in vitro activation. This altered phenotype is mediated by CD83 expressed on the B cells themselves, since it is observed in the absence of accessory cells. In line with this finding, purified CD83mu B cells displayed a reduced IL-10 production and slightly increased Ig secretion upon LPS stimulation in vitro. Taken together, our data strongly suggest that CD83 is expressed by B cells upon activation and contributes to the regulation of B cell function

    The piano music of Sterndale Bennett in the context of nineteenth-century pianism : a practice-based interpretive study with critical commentary

    Sterndale Bennett (1816 - 75) made a significant contribution to piano music and pianism in London during the nineteenth-century, as evidenced by his substantial work list (see Appendix A). The aim of this thesis is to show how a knowledge of the performance practices of his time and of his own approach to style and interpretation can illuminate the performance of this repertoire. A secondary aim is to set this study within a clear historical framework and hence to make a strong connection between contextual and textual studies. An examination of his piano music and contemporary accounts of his piano playing reveal a conservative approach compared to other performers. The picture is amplified by an account of practices described in nineteenth-century writings on performance and of the differences between English and Viennese pianos. In the recordings, music by Sterndale Bennett is juxtaposed with music by selected predecessors and contemporaries, not only to show how his music relates to the nineteenth-century continuum, but also to present in sharp relief his special stylistic qualities. Some of the recordings reflect the work of members of the London Pianoforte School. The justification for this twentieth-century grouping is discussed in Chapter 1 in the context of London musical life and pianism in the nineteenthcentury, with reference to contemporary opinion-formers. The influence of Mozart and of the revival of Baroque keyboard music on Sterndale Bennett are also discussed. Publishing practices of the period are examined in Chapter 2, leading to a survey of Sterndale Bennett's sources and publications. Chapter 3 investigates approaches to nineteenth-century pianism, drawing on contemporary documents and secondary sources, comparing them with the preserved evidence we have regarding Sterndale Bennett's own stance on these matters. This process reveals, in many cases, that Sterndale Bennett represented a more scholarly and less commercial approach to piano playing than was prevalent among contemporaries such as Kalkbrenner, Thalberg and others.
