    Environmental Management and Data for the SDGs

    Ambient intelligence data is key for the global scope of climate, biodiversity, water, air pollution, and general environmental control. The account of carbon emissions, water and air pollutants, sea and freshwater life, and earth nature quality, are key for a healthy environment, in local or global concerns. Controlling initiatives and data management for the environment are part of sustainability strategies for governments, businesses, and entities in general. Such initiatives are increasingly connected to key mankind issues such as poverty, hunger, health, education, gender and equality, among others. Altogether, they are clearly addressed in the United Nations Sustainable Development Goals, to be met in 2030. The Sustainable Development Goals (SDGs) provide a relevant connection of environmental intelligence systems with public policies, research, international cooperation, and technology transfers. In a detailed analysis of their targets, they offer the stimulus for a vast array of technical applications linking sustainability issues with Ambient Intelligence technologies. The SDGs should be considered the benchmark for action towards a healthy environment given their global concern and interconnectivity of multiple issues related to a better quality of life. The SDGs should become, ultimately, the main driver for the spread of present and new technologies to promote better monitoring and control of the environment. The growth and advances of Ambient Intelligence technologies to new areas of knowledge should be enhanced by their connectivity with the SDGs

    Perspective Chapter: Cryptocurrencies Effectiveness for Nature

    The rise of cryptocurrencies based on Blockchain platforms have provided multiple solutions for social and nature projects supported by concerned investors with sustainable development initiatives. Speculation and unclear uses of a cryptocurrency plays a negative role for the projects they claim to support. A positive relationship between coin investors and supported projects must position the coin value on the scale of the community involvement among the coin and project issues, thus placing the project results above speculative moves. Coin nature and social based projects may include a decentralized autonomous organization (DAO), combined with a digital currency to contribute to social and nature improvements. This organization provides a framework for the engagement of investors, beneficiaries, and implementation partners, with results measured by reliable third parties. The potential funding from non fiduciary sources for sustainable development targets may be framed under the fundraising and financial solutions models, addressing the cryptocurrency volatility risks with responsible tokenomics in attention to transaction and regulatory issues. Overall, the more clear are the object and transaction issues of a nature conservation project supported by a currency, the more successful it will be in terms of nature and social improvements and the currency valuation for all parties involved

    A review of Cushing’s disease treatment by the Department of Neuroendocrinology of the Brazilian Society of Endocrinology and Metabolism

    The treatment objectives for a patient with Cushing’s disease (CD) are remission of hypercortisolism, adequate management of co-morbidities, restoration of the hypothalamic-pituitary-adrenal axis, preservation of fertility and pituitary function, and improvement of visual defects in cases of macroadenomas with suprasellar extension. Transsphenoidal pituitary surgery is the main treatment option for the majority of cases, even in macroadenomas with low probability of remission. In cases of surgical failure, another subsequent pituitary surgery might be indicated in cases with persistent tumor imaging at post surgical magnetic resonance imaging (MRI) and/or pathology analysis of adrenocorticotropic hormone-positive (ACTH+) positive pituitary adenoma in the first procedure. Medical treatment, radiotherapy and adrenalectomy are the other options when transsphenoidal pituitary surgery fails.There are several options of medical treatment, although cabergoline and ketoconazole are the most commonly used alone or in combination. Novel treatments are also addressed in this review. Different therapeutic approaches are frequently needed on an individual basis, both before and, particularly, after surgery, and they should be individualized. The objective of the present review is to provide the necessary information to achieve a more effective treatment for CD. It is recommended that patients with CD be followed at tertiary care centers with experience in treating this condition

    International multicenter observational study on assessment of ventilatory management during general anaesthesia for robotic surgery and its effects on postoperative pulmonary complication (AVATaR) : study protocol and statistical analysis plan

    Introduction: Robotic-assisted surgery (RAS) has emerged as an alternative minimally invasive surgical option. Despite its growing applicability, the frequent need for pneumoperitoneum and Trendelenburg position could significantly affect respiratory mechanics during RAS. AVATaR is an international multicenter observational study aiming to assess the incidence of postoperative pulmonary complications (PPC), to characterise current practices of mechanical ventilation (MV) and to evaluate a possible association between ventilatory parameters and PPC in patients undergoing RAS. Methods and analysis: AVATaR is an observational study of surgical patients undergoing MV for general anaesthesia for RAS. The primary outcome is the incidence of PPC during the first five postoperative days. Secondary outcomes include practice of MV, effect of surgical positioning on MV, effect of MV on clinical outcome and intraoperative complications. Ethics and dissemination: This study was approved by the Institutional Review Board of the Hospital Israelita Albert Einstein.     Guidelines for the management of neuroendocrine tumours by the Brazilian gastrointestinal tumour group

    Neuroendocrine tumours are a heterogeneous group of diseases with a significant variety of diagnostic tests and treatment modalities. Guidelines were developed by North American and European groups to recommend their best management. However, local particularities and relativisms found worldwide led us to create Brazilian guidelines. Our consensus considered the best feasible strategies in an environment involving more limited resources.
