Gas exchange calculation may estimate changes in pulmonary blood flow during veno-arterial extracorporeal membrane oxygenation in a porcine model.
BACKGROUND
Veno-arterial extracorporeal membrane oxygenation (VA-ECMO) is used as rescue for severe cardiopulmonary failure. We tested whether the ratio of CO2 elimination at the lung and the ECMO (VCO2ECMO/VCO2Lung) would reflect the ratio of respective blood flows and could be used to estimate changes in pulmonary blood flow (QLUNG), i. e. native cardiac output.
METHODS
Four healthy pigs were centrally cannulated for VA-ECMO. We measured blood flows with an ultrasonic flow probes. VCO2ECMO and VCO2Lung were calculated from sidestream capnographs under constant pulmonary ventilation during ECMO weaning with changing sweep gas and/or ECMO blood flow. If ventilation/perfusion (V/Q) ratio of ECMO was not one, the VCO2ECMO was normalized to V/Q=1 (VCO2ECMONORM). Changes in pulmonary blood flow were calculated using the relationship between changes in CO2 elimination and ECMO blood flow.
RESULTS
QECMO correlated strongly with VCO2ECMONORM (r2 0.95 - 0.99). QLUNG correlated well with VCO2LUNG (r2 0.65 - 0.89, p<=0.002). Absolute QLung could not be calculated in a non-steady state. Calculated pulmonary blood flow changes had a bias of 76 (-266 to 418) ml/min and correlated with measured QLUNG (r2 0.974 - 1.000, p = 0.1 to 0.006) for cumulative ECMO flow reductions.
CONCLUSIONS
VCO2 of the lung correlated strongly with pulmonary blood flow. Our model could predict pulmonary blood flow changes within clinically acceptable margins of error. The prediction is made possible with a normalization to a V/Q of 1 for ECMO. This approach depends on measurements readily available and may allow immediate assessment of the cardiac output response
composition by different sodium hypochlorite concentrations
S odium hypochlorite (NaOCl) remains the most used irrigation solution during root canal preparation because of characteristics such as wide-spectrum antimicrobial activity and organic tissue dissolution capacity. However, these solutions can alter dentin composition and there is no consensus on the optimal concentration of NaOCl to be used. Objectives: To determine the organic matter dissolution and changes in dentin chemical composition promoted by different concentrations of NaOCl over time. Material and Methods: Fragments of bovine muscle tissue were weighed before and after 5, 10, and 15 min of immersion in the groups (n=10): G1-0.9% saline solution; G2-1% NaOCl; G3-2.5% NaOCl; and G4-5% NaOCl. Bovine dentin fragments were subjected to the same irrigants and absorption Spectroscopy (ATR-FTIR) before and after 0,5, 1, 2, 3, 5, 8, and 10 min of immersion in the solutions. The ratios of the amide III/phosphate and carbonate/phosphate absorption bands were determined. The tissue dissolution and carbonate/phosphate ratios were submitted to the two-way analysis of variance (ANOVA) with Tukey's multiple-comparison test ( <0.05) and to the one-way analysis of variance with Tukey's ( <0.05). The amide III/phosphate ratio was analyzed by Friedman test ( <0.05) and the Kruskal-Wallis test with Dunn's post-hoc ( <0.05). Results: The increase in NaOCl concentration and contact observed in the dissolution of organic matter at 10 min and in the amide III/phosphate ratio between the saline solution and 5% NaOCl at 5 min. The carbonate/phosphate ratio alterations did not occur in the subsequent periods (p>0.05). Intergroup differences were not observed in this ratio (p>0.05). Conclusions: The increase in the exposure time and in the concentration of NaOCl solution lead to an increase in the tissue dissolution and dentin collagen deproteination. Furthermore, some carbonate ions are removed from the dentin inorganic phase by the NaOCl

Antimicrobial Photodynamic Therapy with Phenothiazinium Photosensitizers in non-vertebrate model Galleria mellonella infected with Fusarium keratoplasticum and Fusarium moniliforme.
Fusarium keratoplasticum and Fusarium moniliforme are filamentous fungi common in the environment and cause mycosis in both animals and plants. Human infections include mycetoma, keratitis and onychomycosis, while deeper mycosis occurs in immunocompromised patients. Most of the Fusarium spp. are frequently resistant to treatment with currently used antifungals. The frequent occurrence of antifungal resistance has motivated the study of antimicrobial photodynamic therapy as an alternative treatment for fungal infections. Many studies have investigated the in vitro use of antimicrobial photodynamic therapy to kill fungi, but rarely in animal models of infection. Thus, here we employed the invertebrate wax moth Galleria mellonella to study the in vivo effects of antimicrobial photodynamic therapy with three different phenothiazinium photosensitizers, methylene blue, new methylene blue N and the pentacyclic S137 against infection with microconidia of Fusarium keratoplasticum and Fusarium moniliforme. The effect of antimicrobial photodynamic therapy using these photosensitizers and light-emitting diodes with an emission peak at 635ânm and an integrated irradiance from 570 to 670ânm of 9.8âmW cm-2 was investigated regarding the toxicity, fungal burden, larval survival and cellular immune response. The results from this model indicate that antimicrobial photodynamic therapy with methylene blue, new methylene blue N and S137 is efficient for the treatment of infection with F. keratoplasticum and F. moniliforme. The efficiency can be attributed to the fungal cell damage caused by antimicrobial photodynamic therapy which facilitates the action of the host immune response
Multidifferential study of identified charged hadron distributions in -tagged jets in proton-proton collisions at 13 TeV
Jet fragmentation functions are measured for the first time in proton-proton
collisions for charged pions, kaons, and protons within jets recoiling against
a boson. The charged-hadron distributions are studied longitudinally and
transversely to the jet direction for jets with transverse momentum 20 GeV and in the pseudorapidity range . The
data sample was collected with the LHCb experiment at a center-of-mass energy
of 13 TeV, corresponding to an integrated luminosity of 1.64 fb. Triple
differential distributions as a function of the hadron longitudinal momentum
fraction, hadron transverse momentum, and jet transverse momentum are also
measured for the first time. This helps constrain transverse-momentum-dependent
fragmentation functions. Differences in the shapes and magnitudes of the
measured distributions for the different hadron species provide insights into
the hadronization process for jets predominantly initiated by light quarks.Comment: All figures and tables, along with machine-readable versions and any
