    A review of Italian research on free-living marine nematodes and the future perspectives on their use as Ecological Indicators (EcoInds)

    The use of free-living marine nematodes as ecological indicators (EcoInds) of human impacts has increased greatly in Italy since 1990. This paper is a summary of the Italian research experience in the study of nematode assemblages of shallow water habitats, and provides a breakdown of the most important insights that have been obtained so far. Although nematodes are among the best candidates for the Ecological Quality (EcoQ) assessment in the benthic domain, many guidelines need to be developed and limits overcome. Italian research has certainly contributed to the achievement of this purpose with highly focused local investigations on the effects of specific stressors (riverine and sewage discharge, aquaculture, trace elements and hydrocarbons), but also to a large extent with wider analyses aimed at finding new and valuable tools for monitoring programs and useful nematode descriptors, in line with the European Directives. Currently, the Italian and foreign experiences in this field draw light to the fact that the best nematode descriptors for the EcoQ assessment are the taxonomic composition and life strategy traits. However, nematode worldwide research is running the risk of being marginalized because of the relatively narrow scope of most contemporary studies. Nematode researchers should work in a nematologist community in order to better promote the use of nematodes such as EcoInds in the era of the Water Framework Directive (WFD) and Marine Strategy Framework Directive (MSFD).     This review illustrates various types of polymer and nanocomposite polymeric based sensors used in a wide variety of devices. Moreover, it provides an overview of the trends and challenges in sensor research. As fundamental components of new devices, polymers play an important role in sensing applications. Indeed, polymers offer many advantages for sensor technologies: their manufacturing methods are pretty simple, they are relatively low-cost materials, and they can be functionalized and placed on different substrates. Polymers can participate in sensing mechanisms or act as supports for the sensing units. Another good quality of polymer-based materials is that their chemical structure can be modified to enhance their reactivity, biocompatibility, resistance to degradation, and flexibility

    New Rheumatoid Arthritis Treatments for ‘Old’ Patients : Results of a Systematic Review

    Introduction: In the last 20 years, biologic and targeted synthetic disease-modifying antirheumatic drugs (DMARDs) have become available for treating rheumatoid arthritis (RA), and a treat-to-target strategy has been introduced. We hypothesise that these advances should have resulted in changes to the characteristics of patients with RA participating in clinical trials of the newest therapies. This study determined whether the baseline characteristics of patients with RA enrolled in clinical trials have changed in the past decade versus patients participating in earlier RA studies. Methods: This secondary analysis was based on randomised controlled trials (RCTs) identified in a systematic literature review. Baseline characteristics of patients with RA with inadequate response to conventional synthetic DMARDs were compared between RCTs published in 1999\u20132009 and those published in 2010\u20132017 using random-effects meta-analyses. Results: Forty RCTs were analysed: 22 from 1999\u20132009 and 18 from 2010\u20132017. No significant difference between the two timeframes and no obvious trend over time were observed for age, gender, disease duration, rheumatoid factor status, tender and swollen joint counts, physician and patient global assessments of disease activity, and pain scores. Variability between RCTs was high. Similar results were observed for Disease Activity Scores and Health Assessment Questionnaire-Disability Index scores, but with low variability between RCTs. Conclusion: The baseline characteristics of patients with RA participating in RCTs do not appear to have changed in the last decade despite the availability of new treatments and a different treatment approach. Further research should determine the impact of baseline patient characteristics on patients\u2019 response to RA treatments

    Sessile macrobenthos (Ochrophyta) drives seasonal change of meiofaunal community structure on temperate rocky reefs

    none7noUnlike the soft bottom meiofauna, meiofauna associated to hard substrata is poorly studied, despite its ecological relevance. Since communities of hard substrata are usually characterized by species with different life cycles and strategies from those of soft bottom assemblages, information on hard substrata meiofauna is still needed. In this study, sessile macrobenthos and the associated meiofaunal assemblages of two sites of Portofino (NW Mediterranean) were investigated in two seasons at three different depths on both sub-vertical and inclined reefs. The study aimed to assess the abundance, diversity and composition of the meiofauna and the factors structuring its assemblages. Moreover, as meiofauna is known to be dependent upon the substrate characteristics, the study investigated whether the meiofaunal patterns could be related to the sessile macrobenthos structure and composition, and to which extent. Macroalgae dominated the sessile macrobenthic assemblages, while Nematoda and Copepoda were the main meiofaunal groups. Meiofaunal higher-taxa richness and diversity resulted very high, due to the large number of different microhabitats offered by macroalgae. Macrobenthic assemblages were dominated by Rodophyta and Ochrophyta in summer, the latter dramatically collapsing in winter. The meiofaunal abundance and composition changed significantly with the season, consistently with the sessile macrobenthic assemblages, and resulted strongly correlated with Ochrophyta.     The use of nematodes in assessing ecological conditions in shallow waters surrounding a Mediterranean harbour facility

    The spatial distribution and structure of nematode assemblages in the area surrounding the harbour of Vado Ligure (Savona, NW Mediterranean) were studied in relation to the influence of natural and anthropogenic environmental factors. Stations were selected following an “anthropogenic gradient” from sites located near the city centre and its harbour to more pristine and distant sites. Sediment quality was determined by considering both sediment granulometric and chemical parameters (hydrocarbons, heavy metals, total organic matter, proteins, carbohydrates) as well as nematode abundance, diversity, life strategies, trophic structure and assemblage composition. A high correlation between environmental characteristics and the nematode response was found. On the basis of the comparison of these results, which identified three distinct sub-areas associated with different levels of environmental quality, a set of nematode indicator genera was selected for the future evaluation of quality status

    Depressed Myocardial Energetic Efficiency Increases Risk of Incident Heart Failure: The Strong Heart Study

    An estimation of myocardial mechano-energetic efficiency (MEE) per unit of left ventricular (LV) mass (MEEi) can significantly predict composite cardiovascular (CV) events in treated hypertensive patients with normal ejection fraction (EF), after adjustment for LV hypertrophy (LVH). We have tested whether MEEi predicts incident heart failure (HF), after adjustment for LVH, in the population-based cohort of a "Strong Heart Study" (SHS) with normal EF. We included 1,912 SHS participants (age 59 ± 8 years; 64% women) with preserved EF (≄50%) and without prevalent CV disease. MEE was estimated as the ratio of stroke work to the "double product" of heart rate times systolic blood pressure. MEEi was calculated as MEE/LV mass, and analyzed in quartiles. During a follow-up study of 9.2 ± 2.3 years, 126 participants developed HF (7%). HF was preceded by acute myocardial infarction (AMI) in 94 participants. A Kaplan-Meier plot, in quartiles of MEEi, demonstrated significant differences, substantially due to the deviation of the lowest quartile (p < 0.0001). Using AMI as a competing risk event, sequential models of Cox regression for incident HF (including significant confounders), demonstrated that low MEEi predicted incident HF not due to AMI (p = 0.026), after adjustment for significant effect of age, LVH, prolonged LV relaxation, diabetes, and smoking habits with negligible effects for sex, hypertension, antihypertensive therapy, obesity, and hyperlipemia. Carbon with sp-hybridization, the foundation of the elusive allotrope carbyne, offers vast opportunities for functionalized molecules in the form of linear carbon atomic wires (CAWs), with intriguing and even superior predicted electronic properties. While CAWs represent a vibrant field of research, to date, they have only been applied sparingly to molecular devices. The recent observation of the field-effect in microcrystalline cumulenes suggests their potential applications in solution-processed thin-film transistors but concerns surrounding the stability and electronic performance have precluded developments in this direction. In the present study, ideal field-effect characteristics are demonstrated for solution-processed thin films of tetraphenyl[3]cumulene, the shortest semiconducting CAW. Films are deposited through a scalable, large-area, meniscus-coating technique, providing transistors with hole mobilities in excess of 0.1&nbsp;cm2V−1s−1, as well as promising operational stability under dark conditions.     Quantitative flow ratio-based outcomes in patients undergoing transcatheter aortic valve implantation quaestio study

    Background: Coronary artery disease (CAD) is common in patients with aortic valve stenosis (AS) ranging from 60% to 80%. The clinical and prognostic role of coronary artery lesions in patients undergoing Transcatheter Aortic Valve Implantation (TAVI) remains unclear. The aim of the present observational study was to estimate long-term clinical outcomes by Quantitative Flow Ratio (QFR) characterization of CAD in a well-represented cohort of patients affected by severe AS treated by TAVI. Methods: A total of 439 invasive coronary angiographies of patients deemed eligible for TAVI by local Heart Teams with symptomatic severe AS were retrospectively screened for QFR analysis. The primary endpoint of the study was all-cause mortality. The secondary endpoint was a composite of cardiovascular mortality, stroke/transient ischemic attack (TIA), acute myocardial infarction (AMI), and any hospitalization after TAVI. Results: After exclusion of patients with no follow-up data, coronary angiography not feasible for QFR analysis and previous surgical myocardial revascularization (CABG) 48/239 (20.1%) patients had a QFR value lower or equal to 0.80 (QFR + value), while the remaining 191/239 (79.9%) did not present any vessel with a QFR positive value. In the adjusted Cox regression analysis, patients with positive QFR were independently associated with an increased risk of all-casual mortality (Model 1, HR 3.47, 95% CI, 2.35−5.12; Model 2, HR 5.01, 95% CI, 3.17−7.90). In the adjusted covariate analysis, QFR+ involving LAD (37/48, 77,1%) was associated with the higher risk of the composite outcome compared to patients without any positive value of QFR or non-LAD QFR positive value (11/48, 22.9%). Conclusions: Pre-TAVI QFR analysis can be used for a safe, simple, wireless functional assessment of CAD. QFR permits to identify patients at high risk of cardiovascular mortality or MACE, and it could be considered by local Heart Teams
