    Amyloidogenesis of proteolytic fragments of human elastin

    Some polypeptides encoded by the C-terminal region of human tropoelastin gene have been demonstrated to be amyloidogenic in vitro. The biological relevance of this finding is still under investigation given that only limited evidence concerning the involvement of elastin in amyloidosis exists. Recent studies identified, by mass spectrometry, several elastin fragments produced from the cleavages made by some elastases in human elastin substrate. Some of these fragments are contained into the same polypeptide sequences previously demonstrated to be amyloidogenic. Our hypothesis is that the upregulation of elastases in inflammatory processes triggered, for example, by aging induces the release of elastin fragments potentially amyloidogenic. Therefore our aim in this study is to demonstrate if any of these fragments is amyloidogenic in vitro. At molecular level, CD, NMR, FTIR spectroscopies and MD simulations were used, while, at supramolecular level, Congo red binding assay and ThT fluorescence spectroscopy complemented with AFM microscopy were carried out. Our results show that the longest peptide, among those synthesized and mimicking the elastin fragments produced by elastases on human elastin, constituted of 22 residues, is able to aggregate into amyloid-like fibres.     Effects of polluted air-masses advection on atmospheric particles in a semi-rural site in south Italy by SEM-EDX analysis

    The variation of aerosol properties under polluted air masses advection was studied in a semi-rural site in South Italy, by means of SEM-EDX analysis performed on particles collected on 13-stages impactor filters. Radiometric measurements, HYSPLIT back-trajectories and NAAPS maps helped to choose four measurement days of polluted air mass circulation, two of them collected in the warm season (31 July 2008, 16 September 2010) and two in the cold one (16 April 2009, 18 March 2010). Polluted aerosol characteristics were compared to those under background (BG) conditions (8 February 2011), highlighting differences in the particles chemical and morphological properties. One of the signatures of the air mass transport in the coarse fraction was the higher content of particles containing S, i.e., S-reacted, (27.5% on average) in comparison with BG conditions (1%). Two main sources of transported aerosols were identified: industrial processes and biomass burning, with fly ash, metal and S-rich particles in the first case, and K-salts and nitrate-coated Ca-bearingparticles in the second. Single particle analysis on the coarse fraction allowed large agglomerates of soot to be identified, with inclusions of silicate particles rich in Cu and Zn, Ca-S, fly ash and metals particles that are a clear indication of extensive modifications of aerosol size, chemical composition and, likely, radiative properties. Assim, pretende-se com este guia fornecer subsídios para a conduta dos trabalhadores em relação à exposição biológica, contribuindo para a adesão ao protocolo de monitoramento.Health workers are exposed to occupational hazards at the workplace, among which we highlight the biological risk. Information and orientation should be provided regarding the conduct that workers are expected to follow in the case becoming exposed. Thus, this guide is intended to provide the elements to orient workers' conduct in cases of biological exposure, improving adherence to the monitoring protocol.Los trabajadores de la salud están expuestos a riesgos ocupacionales en su ambiente de trabajo, entre ellos se destaca el riesgo biológico.     Knowledge and attitudes towards clinical trials among women with ovarian cancer: results of the ACTO study

    Background Despite several initiatives by research groups, regulatory authorities, and scientific associations to engage citizens/patients in clinical research, there are still obstacles to participation. Among the main discouraging aspects are incomplete understanding of the concepts related to a clinical trial, and the scant, sometimes confused, explanations given. This observational, cross-sectional multicenter study investigated knowledge, attitudes and trust in clinical research. We conducted a survey among women with ovarian cancer at their first follow-up visit or first therapy session, treated in centers belonging to the Mario Negri Gynecologic Oncology (MaNGO) and Multicenter Italian Trials in Ovarian Cancer (MITO) groups. A questionnaire on knowledge, attitudes and experience was assembled ad hoc after a literature review and a validation process involving patients of the Alliance against Ovarian Cancer (ACTO). Results From 25 centers 348 questionnaire were collected; 73.5% of responders were 56 years or older, 54.8% had a high level of education, more than 80% had no experience of trial participation. Among participants 59% knew what clinical trials were and 71% what informed consent was. However, more than half did not know the meaning of the term randomization. More than half (56%) were in favor of participating in a clinical trial, but 35% were not certain. Almost all responders acknowledged the doctor's importance in decision-making. Patients' associations were recognized as having a powerful role in the design and planning of clinical trials. Conclusions This study helps depict the knowledge and attitudes of women with ovarian cancer in relation to clinical trials, suggesting measures aimed at improving trial "culture", literacy and compliance, and fresh ways of communication between doctors and patients

    COVID-19 Infection in Children and Infants: Current Status on Therapies and Vaccines

    Since the beginning in December 2019, the SARS-CoV-2 outbreak appeared to affect mostly the adult population, sparing the vast majority of children who only showed mild symptoms. The purpose of this investigation is to assess the status on the mechanisms that give children and infants this variation in epidemiology compared to the adult population and its impact on therapies and vaccines that are aimed towards them. A literature review, including in vitro studies, reviews, published guidelines and clinical trials was performed. Clinical trials concerned topics that allowed a descriptive synthesis to be produced. Four underlying mechanisms were found that may play a key role in providing COVID-19 protection in babies. No guidelines are available yet for therapy due to insufficient data; support therapy remains the most used. Only two vaccines are approved by the World Health Organization to be used in children from 12 years of age, and there are currently no efficacy or safety data for children below the age of 12 years. The COVID-19 clinical frame infection is milder in children and adolescents. This section of the population can act as vectors and reservoirs and play a key role in the transmission of the infection; therefore, vaccines are paramount. More evidence is required to guide safely the vaccination campaign

    ECMO for COVID-19 patients in Europe and Israel

    Since March 15th, 2020, 177 centres from Europe and Israel have joined the study, routinely reporting on the ECMO support they provide to COVID-19 patients. The mean annual number of cases treated with ECMO in the participating centres before the pandemic (2019) was 55. The number of COVID-19 patients has increased rapidly each week reaching 1531 treated patients as of September 14th. The greatest number of cases has been reported from France (n = 385), UK (n = 193), Germany (n = 176), Spain (n = 166), and Italy (n = 136) .The mean age of treated patients was 52.6 years (range 16–80), 79% were male. The ECMO configuration used was VV in 91% of cases, VA in 5% and other in 4%. The mean PaO2 before ECMO implantation was 65 mmHg. The mean duration of ECMO support thus far has been 18 days and the mean ICU length of stay of these patients was 33 days. As of the 14th September, overall 841 patients have been weaned from ECMO support, 601 died during ECMO support, 71 died after withdrawal of ECMO, 79 are still receiving ECMO support and for 10 patients status n.a. . Our preliminary data suggest that patients placed on ECMO with severe refractory respiratory or cardiac failure secondary to COVID-19 have a reasonable (55%) chance of survival. Further extensive data analysis is expected to provide invaluable information on the demographics, severity of illness, indications and different ECMO management strategies in these patients

    The Exhibition Library reimagines the medium of art exhibition as well as that of art catalogue. Catalogues carry exhibitions through time and space, figuring as tropes for imagining arrangements and the course of works and settings they describe. However, they rarely give us a clue about what really happened, since they are often made before the show opens. Rather than documenting it, they often stand on their own, almost as if another work on display, truly as an artistic medium on its own. For this work, artists, designers, curators, poets and collectives created thirty catalogues of imaginary exhibitions. Exploring both the potential and impossible in art, the resulting exhibition library also serves as a “library of exhibitions.

    Effectiveness of lithium in subjects with treatment-resistant depression and suicide risk: results and lessons of an underpowered randomised clinical trial

    BACKGROUND: As lithium treatment might be effective in reducing the risk of deliberate self-harm (DSH) in adult patients with unipolar affective disorders, we designed a pragmatic randomised trial to assess its efficacy in more than 200 patients with treatment-resistant depression. However, we randomised 56 patients only. The aim of this report is therefore twofold: first, to disseminate the results of this underpowered study which may be incorporated into future meta-analytical reviews; second, to analyse some critical aspects of the study which might explain failure to reach the target sample size.METHODS: We carried out a randomised, parallel group, assessor-blinded superiority clinical trial. Adults with a diagnosis of major depression, an episode of DSH in the previous 12 months and inadequate response to at least two antidepressants given sequentially at an adequate dose for an adequate time for the current depressive episode were allocated to add lithium to usual care (intervention arm) versus usual care alone (control arm). Suicide completion and acts of DSH during the 12 months of follow-up constituted the composite primary outcome.RESULTS: Of 58 patients screened for inclusion, 29 were allocated to lithium plus usual care and 27 were assigned to usual care without lithium. Six patients in the lithium plus usual care group and seven in the usual care group committed acts of DSH during the follow-up phase. The survival probability did not differ between the two treatment arms (Chi2 = 0.17, p =0.676). With regard to changes in the severity of depressive symptomatology from baseline to endpoint, no significant differences were detected.CONCLUSIONS: The present study failed to achieve the minimum sample size needed to detect a clinically meaningful difference between the two treatment arms. Consequently, the finding that lithium, in addition to usual care, did not exert a positive effect in terms of reduction of DSH after 12 months of follow-up is likely due to the lack of sufficient statistical power to detect a difference, if a difference existed. The dissemination of the results of this underpowered study will inform future meta-analytical reviews on lithium and suicide-related outcomes.TRIAL REGISTRATION: ClinicalTrials.gov identifier: NCT00927550
