    Childhood obesity, thyroid function, and insulin resistance – is there a link? A longitudinal study

    Serum thyroid stimulating hormone (TSH) levels are frequently elevated in obese children and are most likely to be associated with insulin resistance. However, clinical relevance of this association remains unclear. OBJECTIVES: To assess the prevalence of hyperthyrotropinemia; to analyze the relationship between TSH and homeostasis model assessment - insulin resistance (HOMA-IR); and to verify whether TSH levels and HOMA-IR vary with weight loss in obese children. SUBJECTS AND METHODS: Retrospective longitudinal study with data from baseline and 1 year after lifestyle intervention in a pediatric obese group (344 children were recruited and 100 among them completed follow-up). For postintervention analysis, three groups were considered according to body mass index-standard deviation score (BMI-SDS) variations: ≤-0.5 (significant weight loss); 0.5-0 (weight loss); and >0 (weight gain). Statistical analysis was performed using SPSS 19.0®. RESULTS: The prevalence of increased TSH levels was 9.3%. At baseline TSH (p=0.007), fT4 (p=0.006), and HOMA-IR (p<0.001) were positively correlated to BMI-SDS (n=344). Weight reduction was verified in 67 out of 100 cases but significant loss was present in only 21 cases. Decreases in both TSH and BMI-SDS were independently associated with decreases in HOMA-IR (p=0.005 and p=0.016, respectively). There was no correlation between TSH and BMI-SDS variation. Significant decreases in the HOMA-IR (p=0.006) were only achieved in the significant weight loss group. CONCLUSIONS: The prevalence of hyperthyrotropinemia was lower than previously reported. However, cutoff values were adjusted to pubertal stage, suggesting an over report in other studies. Insulin resistance and TSH were positively correlated, independent of body status.     Trace elements in an adjacent channel of an anthropized area: a case study of Baixada Santista, Southeastern Brazil

    In recent decades, due to intensive industrialization, the Baixada Santista has undergone an intense process of environmental degradation. The metals are toxic and persistent in varying concentrations and oxidation states and may be incorporated in sediments and biota. Thus, understanding the importance of this contamination is necessary for coastal planning. This study provides a basis for understanding the levels of metal and As contamination in the Bertioga Channel (SP). The levels of Al, As, Cd, Cr, Cu, Fe, Mn, Ni, Pb, Sc, V and Zn in superficial sediment samples were determined by ICP-OES. The degree of sediment contamination was evaluated according to the sediment quality standards set by the Canadian environmental agency (ISQG and PEL) and by statistical tests. All values were below PEL, and most of the sample values were below ISQG, except for As, Cu and Pb. From a cluster analysis, it was possible to differentiate eastern and western parts of the channel due to their distinct hydrodynamic patterns. Furthermore, it was possible to separate the trace elements by geochemical behavior, in which Cu, Pb and Zn were linked to a small anthropogenic contribution. Thus, this study detected small anthropogenic contributions from an adjacent channel of an anthropized area, but most of the results were linked to natural geochemical processes

    Cadmium exposure and breast cancer risk: a systematic review and dose-response meta-analysis of cohort studies

    Background and aim: Cadmium is a heavy metal which has been implicated in breast cancer etiology because of its toxic properties such as endocrine disruption. The general population is exposed to cadmium through dietary intake, cigarette smoking, emissions of motorized traffic and industrial facilities. We carried out a systematic review and dose-response meta-analysis of the cohort studies investigating the association between cadmium exposure and breast cancer risk, for which inconsistent results have been reported in the literature. Methods: Following online database search up to January 2019, we carried out a dose-response meta-analysis to identify the relation between cadmium exposure and disease risk. We used a restricted cubic spline model and the \u2018one-stage\u2019 approach, stratifying for exposure assessment method and menopausal status. Results: We identified 12 studies, 9 using breast cancer incidence and 3 mortality as an outcome. In six studies cadmium exposure was assessed through dietary questionnaires, in five through urinary excretion levels, and in one based on environmental air levels. Seven studies included post-menopausal women only. Overall, we observed a positive linear relation between breast cancer risk and dietary cadmium intake (relative risk (RR) 1.04, 95% confidence interval (CI) 0.81-1.33 at 10 \ub5g/day, and RR 1.12, 95% CI 0.80-1.56 at 20 \ub5g/day). On the converse, risk was not associated with urinary excretion. Analysis restricted to post-menopausal women showed a positive association between cadmium exposure assessed through either dietary intake and urinary excretion, for levels higher than 20 \ub5g/day and 1.65 \ub5g/g creatinine, respectively

    Mycobacterium bovis: polymerase chain reaction identification in bovine Lymphonode biopsies and genotyping in isolates from Southeast Brazil by spolygotyping and restriction fragment length polymorphism

    Diagnosis of the Mycobacterium tuberculosis complex by direct PCR of mediastinal lymphnode DNA and microbiological tests were compared in cattle suspicious of bearing tuberculous-like lesions detected during slaughter. The PCR procedure applied on DNA samples (n=54) obtained by adding alpha -casein into the thiocyanate extraction mix was positive in 70% of the samples. PCR confirmed the identification of 23 samples (100%) that grew in culture, 9 samples (60%) that failed to grow in culture, plus 6 (37.5%) samples that resulted in growth of bacterial contaminants. Genotyping by IS6110-RFLP and DR-spoligotyping analysis of seven samples revealed the presence of several polimorphisms.     Potassium Intake and Blood Pressure: A Dose-Response Meta-Analysis of Randomized Controlled Trials

    BACKGROUND: Epidemiologic studies, including trials, suggest an association between potassium intake and blood pressure (BP). However, the strength and shape of this relationship is uncertain. METHODS AND RESULTS: We performed a meta-analysis to explore the dose-response relationship between potassium supplementation and BP in randomized-controlled trials with a duration >= 4 weeks using the recently developed 1-stage cubic spline regression model. This model allows use of trials with at least 2 exposure categories. We identified 32 eligible trials. Most were conducted in adults with hypertension using a crossover design and potassium supplementation doses that ranged from 30 to 140 mmol/d. We observed a U-shaped relationship between 24-hour active and control arm differences in potassium excretion and BP levels, with weakening of the BP reduction effect above differences of 30 mmol/d and a BP increase above differences similar to 80 mmol/d. Achieved potassium excretion analysis also identified a U-shaped relationship. The BP-lowering effects of potassium supplementation were stronger in participants with hypertension and at higher levels of sodium intake. The BP increase with high potassium excretion was noted in participants with antihypertensive drug-treated hypertension but not in their untreated counterparts. CONCLUSIONS: We identified a nonlinear relationship between potassium intake and both systolic and diastolic BP, although estimates for BP effects of high potassium intakes should be interpreted with caution because of limited availability of trials. Our findings indicate an adequate intake of potassium is desirable to achieve a lower BP level but suggest excessive potassium supplementation should be avoided, particularly in specific subgroups

    Use of point of view cameras for welfare management in extensive grazing systems

    Grazing patterns can be a major indicator for welfare assessment in extensive systems. The aim of this study was to test animal-borne video as a method to evaluate sheep grazing behavior. To test the changes in grazing duration, grazing frequency and grazing rate, we used wide angle cameras (Go-Pro and infra-red night vision) to access diurnal and nocturnal grazing activities of ewes in a native sward, during two weeks in late spring, the season with greatest changes in weather and vegetative growth. Three ewes were chosen from a flock of 15 animals for focal observation. The foraging behaviour of the entire flock were recorded by scan sampling with a 10-minute interval. Grazing bout duration increased significantly between the first (44.6 ± 41.4 s) and the second week (94.0 ± 57.0 s) (F = 10.295, P<0.05). Conversely, grazing bout duration differed among grazing hours (F = 4.523, P<0.05), with a peak for the dusk grazing period (128.1 ± 88.8 s). Nocturnal grazing did not differ significantly for any of the grazing parameters, although a general trend for longer grazing duration through time was observed. Sheep spend the hot part of the day lying in shade under trees and use more than one tree as resting areas. A significant correlation was shown to exist between the grazing behaviour estimated from focal sampling with video cameras and the scan sampling (r2=0.83). We concluded that animal-borne video cameras are a method appropriate for assessing sheep variations of grazing behaviour structure allowing to understand differences of adaptive grazing patterns that can be useful for welfare management in extensive systems

    Inbreeding levels in Northeast Brazil: Strategies for the prospecting of new genetic disorders

    A new autosomal recessive genetic condition, the SPOAN syndrome (an acronym for spastic paraplegia, optic atrophy and neuropathy syndrome), was recently discovered in an isolated region of the State of Rio Grande do Norte in Northeast Brazil, in a population that was identified by the IBGE (Brazilian Institute of Geography and Statistics) as belonging to the Brazilian communities with the highest rates of “deficiencies” (Neri, 2003), a term used to describe diseases, malformations, and handicaps in general. This prompted us to conduct a study of consanguinity levels in five of its municipal districts by directly interviewing their inhabitants. Information on 7,639 couples (corresponding to about 40% of the whole population of the studied districts) was obtained. The research disclosed the existence of very high frequencies of consanguineous marriages, which varied from about 9% to 32%, suggesting the presence of a direct association between genetic diseases such as the SPOAN syndrome, genetic drift and inbreeding levels. This fact calls for the introduction of educational programs for the local populations, as well as for further studies aiming to identify and characterize other genetic conditions. Epidemiological strategies developed to collect inbreeding data, with the collaboration of health systems available in the region, might be very successful in the prospecting of genetic disorders

    Efficacy of prolonged application of low-level laser therapy combined with exercise in knee osteoarthritis: A randomized controlled double-blind study.

    OBJECTIVES: To investigate the effect of prolonged low-level laser therapy application combined with exercise on pain and disability in patients with osteoarthritis of the knee. DESIGN: A randomized controlled trial. SETTING: Special rehabilitation services. SUBJECTS: Forty-three participants with knee osteoarthritis. INTERVENTION: Following initial assessment, participants were randomly allocated to the Laser group (n = 22, 44 knees) and received low-level laser therapy while the Placebo group (n = 21, 42 knees) received placebo therapy three times a week for 3 weeks. Both groups then received low-level laser therapy combined with exercise three times a week for the following 8 weeks. MAIN OUTCOME MEASURES: The primary outcome was change in knee pain and disability (Lequesne). Secondary outcomes included change in mobility (Timed Up and Go test), range of motion (goniometer), muscular strength (dynamometer), activity (Western Ontario and McMaster Universities Osteoarthritis questionnaire), and medication intake and relief. RESULTS: Mean (SD) age of participants was 63.02 (9.9) years. Pain scores at baseline, 3 weeks, 11 weeks, and 6 months follow-up were 9.1 (1.3), 2.6 (2.3), 0.2 (0.9), and 0.2 (0.8) for the Laser group and 9.5 (8.0), 7.7 (5.3), 5.6 (2.4), and 7.4 (5.0) for the Placebo group, respectively. Disability scores at baseline, 3 weeks, 11 weeks, and 6 months follow-up were 14.9 (4.7), 7.6 (4.8), 3.9 (4.2), and 3.5 (4.1) for the Laser group and 17.8 (14.7), 15.2 (11.5), 11.6 (6.4), and 15.8 (11.9) for the Placebo Group, respectively. CONCLUSION: In participants with osteoarthritis of the knee, the isolated application of low-level laser therapy in the initial 3 weeks and combined with exercises in the final 8 weeks reduced pain, disability, and intake of medication over a 6-month period

    Control of oceanic circulation on sediment distribution in the southwestern Atlantic margin (23 to 55º S)

    In this study, we interpret the role played by ocean circulation in sediment distribution on the southwestern Atlantic margin using radiogenic Nd and Pb isotopes. The latitudinal trends for Pb and Nd isotopes reflect the different current systems acting on the margin. The utilization of the sediment fingerprinting method allowed us to associate the isotopic signatures with the main oceanographic features in the area. We recognized differences between Nd and Pb sources to the Argentinean shelf (carried by the flow of Subantarctic Shelf Water) and slopes (transported by deeper flows). Sediments from Antarctica extend up to the Uruguayan margin, carried by the Upper and Lower Circumpolar Deep Water. Our data confirm that, for shelf and intermediate areas (the upper 1200 m), the transfer of sediments from the Argentinean margin to the north of 35∘ S is limited by the Subtropical Shelf Front and the basin-wide recirculated Antarctic Intermediate Water. On the southern Brazilian inner and middle shelf, it is possible to recognize the northward influence of the Río de la Plata sediments carried by the Plata Plume Water. Another flow responsible for sediment transport and deposition on the outer shelf and slope is the southward flow of the Brazil Current. Finally, we propose that the Brazil–Malvinas Confluence and the Santos Bifurcation act as boundaries of geochemical provinces in the area.
