    Understanding the optimal learning environment in palliative care

    The learning experiences of student nurses undertaking clinical placement are reported widely, however little is known about the learning experiences of health professionals undertaking continuing professional development (CPD) in a clinical setting, especially in palliative care. The aim of this study, which was conducted as part of the national evaluation of a professional development program involving clinical attachments with palliative care services (The Program of Experience in the Palliative Approach [PEPA]), was to explore factors influencing the learning experiences of participants over time. Thirteen semi-structured, one-to-one telephone interviews were conducted with five participants throughout their PEPA experience. The analysis was informed by the traditions of adult, social and psychological learning theories and relevant literature. The participants' learning was enhanced by engaging interactively with host site staff and patients, and by the validation of their personal and professional life experiences together with the reciprocation of their knowledge with host site staff. Self-directed learning strategies maximised the participants' learning outcomes. Inclusion in team activities aided the participants to feel accepted within the host site. Personal interactions with host site staff and patients shaped this social/cultural environment of the host site. Optimal learning was promoted when participants were actively engaged, felt accepted and supported by, and experienced positive interpersonal interactions with, the host site staff

    A Decision Tool that Combines Discrete Event Software Process Models with System Dynamics Pieces for Software Development Cost Estimation and Analysis

    The development process for a large software development project is very complex and dependent on many variables that are dynamic and interrelated. Factors such as size, productivity and defect injection rates will have substantial impact on the project in terms of cost and schedule. These factors can be affected by the intricacies of the process itself as well as human behavior because the process is very labor intensive. The complex nature of the development process can be investigated with software development process models that utilize discrete event simulation to analyze the effects of process changes. The organizational environment and its effects on the workforce can be analyzed with system dynamics that utilizes continuous simulation. Each has unique strengths and the benefits of both types can be exploited by combining a system dynamics model and a discrete event process model. This paper will demonstrate how the two types of models can be combined to investigate the impacts of human resource interactions on productivity and ultimately on cost and schedule

    Dietary Patterns and Cognitive Function among Older Community-Dwelling Adults.

    Diet may be an important modifiable risk factor for maintenance of cognitive health in later life. This study aimed at examining associations between common dietary indices and dietary patterns defined by factor analysis and cognitive function in older community-dwelling adults. Dietary information for 1499 participants from the Rancho Bernardo Study was collected in 1988⁻1992 and used to calculate the alternate Mediterranean diet score, Alternate Healthy Eating Index (AHEI)-2010 score and factor scores derived from factor analysis of nutrients. Global cognitive function, executive function, verbal fluency and episodic memory were assessed at approximate four-year intervals from 1988⁻2016. Linear mixed models were used to examine associations between dietary patterns and cognitive trajectories. Estimates for the highest vs. lowest tertile in models adjusting for age, sex, education, energy intake, lifestyle variables and retest effect showed greater adherence to the Mediterranean score was associated with better baseline global cognitive function (β (95% CI) = 0.33 (0.11, 0.55)). The AHEI-2010 score was not significantly associated with cognitive performance. Higher loading on a plant polyunsaturated fatty acid (PUFA)/vitamin E factor was associated with better baseline global cognitive function and executive function (β = 0.22 (0.02, 0.42) and β = -7.85 (-13.20, -2.47)). A sugar/low protein factor was associated with poorer baseline cognitive function across multiple domains. Dietary patterns were not associated with cognitive decline over time. Adherence to a healthy diet with foods high in PUFA and vitamin E and a low sugar to protein ratio, as typified by a Mediterranean diet, may be beneficial for cognitive health in late life

    Relative impact of aerosol, soil moisture, and orography perturbations on deep convection

    The predictability of deep moist convection depends on many factors, such as the synoptic-scale flow, the geographical region (i.e., the presence of mountains), and land surface–atmosphere as well as aerosol–cloud interactions. This study addresses all these factors by investigating the relative impact of orography, soil moisture, and aerosols on precipitation over Germany in different weather regimes. To this end, we conduct numerical sensitivity studies with the COnsortium for Small-sale MOdelling (COSMO) model at high spatial resolution (500 m grid spacing) for 6 days with weak and strong synoptic forcing. The numerical experiments consist of (i) successive smoothing of topographical features, (ii) systematic changes in the initial soil moisture fields (spatially homogeneous increase/decrease, horizontal uniform soil moisture, different realizations of dry/wet patches), and (iii) different assumptions about the ambient aerosol concentration (spatially homogeneous and heterogeneous fields). Our results show that the impact of these perturbations on precipitation is on average higher for weak than for strong synoptic forcing. Soil moisture and aerosols are each responsible for the maximum precipitation response for three of the cases, while the sensitivity to terrain forcing always shows the smallest spread. For the majority of the analyzed cases, the model produces a positive soil moisture–precipitation feedback when averaged over the entire model domain. Furthermore, the amount of soil moisture affects precipitation more strongly than its spatial distribution. The precipitation response to changes in the CCN concentration is more complex and case dependent. The smoothing of terrain shows weaker impacts on days with strong synoptic forcing because surface fluxes are less important and orographic ascent is still simulated reasonably well, despite missing fine-scale orographic features. We apply an object-based characterization to identify whether and how the perturbations affect the structure, location, timing, and intensity of precipitation. These diagnostics reveal that the structure component, comparing the size and shape of precipitating objects to the reference simulation, is on average highest in the soil moisture and aerosol simulations, often due to changes in the maximum precipitation amounts. This indicates that the dominant mechanisms for convection initiation remain but that precipitation amounts depend on the strength of the trigger mechanisms. Location and amplitude parameters both vary over a much smaller range. Still, the temporal evolution of the amplitude component correlates well with the rain rate. Our results suggest that for quantitative precipitation forecasting, both aerosols and soil moisture are of similar importance and that their inclusion in convective-scale ensemble forecasting containing classical sources of uncertainty should be assessed in the future

    Building a Model of Collaboration Between Historically Black and Historically White Universities

    Despite increases over the last two decades in the number of degrees awarded to students from underrepresented groups in science, technology, engineering, and mathematics (STEM) disciplines, enhancing diversity in these disciplines remains a challenge. This article describes a strategic approach to this challenge—the development of a collaborative partnership between two universities: the historically Black Elizabeth City State University and the historically White University of New Hampshire. The partnership, a type of learning organization built on three mutually agreed upon principles, strives to enhance opportunities for underrepresented students to pursue careers in the STEM disciplines. This article further describes six promising practices that framed the partnership, which resulted in the submission of nine proposals to federal agencies and the funding of four grants that led to the implementation, research, learning, and evaluation that followed
