    Investigations of the impact of powerformerℱ on composite power system reliability

    This paper examines the impact of a high voltage generator Powerformerℱ on the composite system reliability. The Powerformerℱ is directly connected to the high voltage side of the grid; it has higher availability, more reactive power margin, and extra short term overloading capacity. Its impacts are compared with the existing conventional generators using the 24-bus IEEE-RTS test system. A number of sensitivity analyses are performed and the results are presented. It has been observed that the improvement in the steady state adequacy is largely attributed to its higher availabilities. Importantly its impacts depend on its location, load level and the system topologies. In some cases system indices may improve while load point indices may not or the vice versa. From its presence the buses with major loads and networked in mesh benefit better than the one connected radially and with little load. Its extra reactive power margin has relatively less impact on the adequacy but reduces number of voltage violations

    UK psychiatrists' experience of withdrawal of antipsychotics prescribed for challenging behaviours in adults with intellectual disabilities and/or autism.

    BACKGROUND: A high proportion of adults with intellectual disabilities are prescribed off-licence antipsychotics in the absence of a psychiatric illness. The National Health Service in England launched an initiative in 2016, 'Stopping over-medication of people with a learning disability [intellectual disability], autism or both' (STOMP), to address this major public health concern. AIMS: To gain understanding from UK psychiatrists working with adults with intellectual disabilities on the successes and challenges of withdrawing antipsychotics for challenging behaviours. METHOD: An online questionnaire was sent to all UK psychiatrists working in the field of intellectual disability (estimated 225). RESULTS: Half of the 88 respondents stated that they started withdrawing antipsychotics over 5 years ago and 52.3% stated that they are less likely to initiate an antipsychotic since the launch of STOMP. However, since then, 46.6% are prescribing other classes of psychotropic medication instead of antipsychotics for challenging behaviours, most frequently the antidepressants. Complete antipsychotic discontinuation in over 50% of patients treated with antipsychotics was achieved by only 4.5% of respondents (n = 4); 11.4% reported deterioration in challenging behaviours in over 50% of patients on withdrawal and the same proportion (11.4%) reported no deterioration. Only 32% of respondents made the diagnosis of psychiatric illness in all their patients themselves. Family and paid carers' concern, lack of multi-agency and multidisciplinary input and unavailability of non-medical psychosocial intervention are key reported factors hampering the withdrawal attempt. CONCLUSIONS: There is an urgent need to develop national guidelines to provide a framework for systematic psychotropic drug reviews and withdrawal where possible

    Grain size-dependent thermal conductivity of polycrystalline twisted bilayer graphene

    We report the room temperature thermal conductivity of polycrystalline twisted bilayer graphene (tBLG) as a function of grain size measured by employing a noncontact optical technique based on micro-Raman spectroscopy. Polycrystalline tBLG sheets of different grain sizes were synthesized on copper by hot filament chemical vapor deposition. The thermal conductivity values are 1305±122, 971±73, and 657±42 Wm−1K−1 for polycrystalline tBLG with average grain sizes of 54, 21, and 8 nm, respectively. Based on these thermal conductivity values, we also estimated the grain boundary conductance, 14.43±1.21×1010Wm−2K−1, and the thermal conductivity for single crystal tBLG, 1510±103Wm−1K−1. Our results show that the relative degradation of thermal conductivity due to grain boundaries is smaller in bilayer than in monolayer graphene. Molecular dynamics simulations indicate that interlayer interactions play an important role in the heat conductivity of polycrystalline bilayer graphene.     Indigenous adult women, learning and social justice: Challenging deficit discourses in the current policy environment

    Indigenous education engages directly with an overtly politicised process of knowledge construction, recognising and building on existing skills and informal learning practices within communities. Given the 2030 Sustainable Development Agenda’s emphasis on social justice and gender equality, this paper sets out to explore what indigenous movements can offer in terms of developing an alternative approach to adult learning based on a rights perspective. The article compares documentary analysis of policy on indigenous women and adult education internationally with a case study of indigenous movements and government policy in Nepal. The analysis reveals that international policy recognises indigenous women as a particularly marginalised group, but is not informed by a transformative notion of empowerment nor consideration of the implications of indigenous knowledge for mainstream education. In Nepal, indigenous federations and the government non formal education programmes similarly aim to impart skills for a modernised economy. However women’s indigenous movements are committed to developing capabilities and creating new spaces for indigenous women to engage in political debate and representation. This politicised informal learning offers insights for developing the cross-sectoral rights-based adult education envisaged in the 2030 Sustainable Development Agenda

    Global Retinoblastoma Presentation and Analysis by National Income Level

    Importance: Early diagnosis of retinoblastoma, the most common intraocular cancer, can save both a child's life and vision. However, anecdotal evidence suggests that many children across the world are diagnosed late. To our knowledge, the clinical presentation of retinoblastoma has never been assessed on a global scale. Objectives: To report the retinoblastoma stage at diagnosis in patients across the world during a single year, to investigate associations between clinical variables and national income level, and to investigate risk factors for advanced disease at diagnosis. Design, Setting, and Participants: A total of 278 retinoblastoma treatment centers were recruited from June 2017 through December 2018 to participate in a cross-sectional analysis of treatment-naive patients with retinoblastoma who were diagnosed in 2017. Main Outcomes and Measures: Age at presentation, proportion of familial history of retinoblastoma, and tumor stage and metastasis. Results: The cohort included 4351 new patients from 153 countries; the median age at diagnosis was 30.5 (interquartile range, 18.3-45.9) months, and 1976 patients (45.4%) were female. Most patients (n = 3685 [84.7%]) were from low- A nd middle-income countries (LMICs). Globally, the most common indication for referral was leukocoria (n = 2638 [62.8%]), followed by strabismus (n = 429 [10.2%]) and proptosis (n = 309 [7.4%]). Patients from high-income countries (HICs) were diagnosed at a median age of 14.1 months, with 656 of 666 (98.5%) patients having intraocular retinoblastoma and 2 (0.3%) having metastasis. Patients from low-income countries were diagnosed at a median age of 30.5 months, with 256 of 521 (49.1%) having extraocular retinoblastoma and 94 of 498 (18.9%) having metastasis. Lower national income level was associated with older presentation age, higher proportion of locally advanced disease and distant metastasis, and smaller proportion of familial history of retinoblastoma. Advanced disease at diagnosis was more common in LMICs even after adjusting for age (odds ratio for low-income countries vs upper-middle-income countries and HICs, 17.92 [95% CI, 12.94-24.80], and for lower-middle-income countries vs upper-middle-income countries and HICs, 5.74 [95% CI, 4.30-7.68]). Conclusions and Relevance: This study is estimated to have included more than half of all new retinoblastoma cases worldwide in 2017. Children from LMICs, where the main global retinoblastoma burden lies, presented at an older age with more advanced disease and demonstrated a smaller proportion of familial history of retinoblastoma, likely because many do not reach a childbearing age. Given that retinoblastoma is curable, these data are concerning and mandate intervention at national and international levels. Further studies are needed to investigate factors, other than age at presentation, that may be associated with advanced disease in LMICs

    E-commerce ethics and its impact on buyer repurchase intentions and loyalty: an empirical study of small and medium Egyptian businesses

    The theoretical understanding of e-commerce has received much attention over the years; however, relatively little focus has been directed towards e-commerce ethics, especially the SMEs B2B e-commerce aspect. Therefore, the purpose of this paper is to develop and empirically test a framework that explains the impact of SMEs B2B e-commerce ethics on buyer repurchase intentions and loyalty. Using SEM to analyse the data collected from a sample of SME e-commerce firms in Egypt, the results indicate that buyers’ perceptions of supplier ethics construct is composed of six dimensions (security, non-deception, fulfilment/reliability, service recovery, shared value, and communication) and strongly predictive of online buyer repurchase intentions and loyalty. Furthermore, our results also show that reliability/fulfilment and non-deception are the most effective relationship-building dimensions. In addition, relationship quality has a positive effect on buyer repurchase intentions and loyalty. The results offer important implications for B2B e-commerce and are likely to stimulate further research in the area of relationship marketing
