    Practitioner’s Keypoints Based on the reviewed research findings, this paper supports the following arguments: · overall, children adopted internationally can and do thrive in their adoptive countries; · such success is guaranteed by the high commitment of adoptive parents and the availability of post-adoption support services; · there is, however, a lack of evidence-based practice relating to interventions best suited to the needs of these children post-international adoption; · following a globally standardised process and strict protocols may help alleviate illicit activity in international adoptions; · existing guidelines must be tightened to ensure that children are the central focus in intercountry adoption and that their development is not affected by cross-border movement

    Ten Misconceptions from the History of Analysis and Their Debunking

    The widespread idea that infinitesimals were "eliminated" by the "great triumvirate" of Cantor, Dedekind, and Weierstrass is refuted by an uninterrupted chain of work on infinitesimal-enriched number systems. The elimination claim is an oversimplification created by triumvirate followers, who tend to view the history of analysis as a pre-ordained march toward the radiant future of Weierstrassian epsilontics. In the present text, we document distortions of the history of analysis stemming from the triumvirate ideology of ontological minimalism, which identified the continuum with a single number system.     Disparity between skin perfusion and sublingual microcirculatory alterations in severe sepsis and septic shock: a prospective observational study

    Objective: Measurement of central-to-toe temperature difference has been advocated as an index of severity of shock and as a guide for circulatory therapy in critically ill patients. However, septic shock, in contrast to other forms of shock, is associated with a distributive malfunction resulting in a disparity between vascular compartments. Although this disparity has been established between systemic and microcirculatory parameters, it is unclear whether such disparity exists between skin perfusion and microcirculation. To test this hypothesis of disparity, we simultaneously measured parameters of the two vascular compartments, in the early phase of sepsis. Design: Prospective observational study in patients with severe sepsis/septic shock in the first 6 h of ICU admission. Simultaneous measurements of central-to-toe temperature difference and sublingual microcirculatory orthogonal polarization spectral imaging, together with parameters of systemic hemodynamics. Setting: 22 bed mixed-ICU in a tertiary teaching hospital. Patients: 35 consecutive patients in a 12-month period. Measurements and results: In 35 septic patients and a median APACHE II score of 20, no correlation between central-to-toe temperature gradient and microvascular flow index was observed (r(s) =-0.08, p = 0.65). Also no significant correlation between temperature gradient/microvascular flow index and systemic hemodynamic parameters could be demonstrated. Conclusions: During the early phase of resuscitated severe sepsis and septic shock there appears to be no correlation between sublingual microcirculatory alterations and the central-to-toe temperature difference. This finding adds to the concept of a dispersive nature of blood flow under conditions of sepsis between microcirculatory and systemic hemodynamic

    In March 2012, a group of researchers met to discuss emerging topics in ceramic science and to identify grand challenges in the field. By the end of the workshop, the group reached a consensus on eight challenges for the future:—understanding rare events in ceramic microstructures, understanding the phase-like behavior of interfaces, predicting and controlling heterogeneous microstructures with unprecedented functionalities, controlling the properties of oxide electronics, understanding defects in the vicinity of interfaces, controlling ceramics far from equilibrium, accelerating the development of new ceramic materials, and harnessing order within disorder in glasses. This paper reports the outcomes of the workshop and provides descriptions of these challenges

    Functional characterization of the human myosin-7a motor domain

    Myosin-7a participates in auditory and visual processes. Defects in MYO7A, the gene encoding the myosin-7a heavy chain, are causative for Usher syndrome 1B, the most frequent cause of deaf-blindness in humans. In the present study, we performed a detailed kinetic and functional characterization of the isolated human myosin-7a motor domain to elucidate the details of chemomechanical coupling and the regulation of motor function. A rate-limiting, slow ADP release step causes long lifetimes of strong actin-binding intermediates and results in a high duty ratio. Moreover, our results reveal a Mg2+-sensitive regulatory mechanism tuning the kinetic and mechanical properties of the myosin-7a motor domain. We obtained direct evidence that changes in the concentration of free Mg2+ ions affect the motor properties of human myosin-7a using an in vitro motility assay system. Our results suggest that in a cellular environment, compartment-specific fluctuations in free Mg2+ ions can mediate the conditional switching of myosin-7a between cargo moving and tension bearing modes

    Antisemitism and anti-Zionism in Iran: the effects of identity, threat, and political trust

    Antisemitism and anti-Zionism constitute two important ideological building blocks of the Islamic Republic of Iran. This article is the first to present quantitative empirical survey data elucidating attitudes towards Jews and Israel among an opportunity sample of Iranians. This study examines the correlates of antisemitism and anti-Zionism in Iran with particular attention to the effects of identity, threat, and political trust. Consistent with emerging research into antisemitism and anti-Zionism, there was a positive association between both forms of prejudice, suggesting social psychological overlap between the constructs. Given the pervasiveness of antisemitic and anti-Zionist representations in Iran, there were no significant differences in levels of anti-Zionism or antisemitism on the basis of gender and educational orientations. Political conservatives did manifest greater antisemitism and anti-Zionism than political reformists, although both groups scored high on these scales. There was a significant interaction effect of Iranian national identity and political trust on anti-Zionism, and a significant interaction effect of Muslim religious identity and political trust on antisemitism. Political trust was by far the most powerful predictor of both forms of prejudice, followed by the perception of identity threat.     Hemodynamic parameters to guide fluid therapy

    The clinical determination of the intravascular volume can be extremely difficult in critically ill and injured patients as well as those undergoing major surgery. This is problematic because fluid loading is considered the first step in the resuscitation of hemodynamically unstable patients. Yet, multiple studies have demonstrated that only approximately 50% of hemodynamically unstable patients in the intensive care unit and operating room respond to a fluid challenge. Whereas under-resuscitation results in inadequate organ perfusion, accumulating data suggest that over-resuscitation increases the morbidity and mortality of critically ill patients. Cardiac filling pressures, including the central venous pressure and pulmonary artery occlusion pressure, have been traditionally used to guide fluid management. However, studies performed during the past 30 years have demonstrated that cardiac filling pressures are unable to predict fluid responsiveness. During the past decade, a number of dynamic tests of volume responsiveness have been reported. These tests dynamically monitor the change in stroke volume after a maneuver that increases or decreases venous return (preload) and challenges the patients' Frank-Starling curve. These dynamic tests use the change in stroke volume during mechanical ventilation or after a passive leg raising maneuver to assess fluid responsiveness. The stroke volume is measured continuously and in real-time by minimally invasive or noninvasive technologies, including Doppler methods, pulse contour analysis, and bioreactance
