    Evaluation of patients with fibrotic interstitial lung disease: Preliminary results from the Turk-UIP study

    OBJECTIVE: Differential diagnosis of idiopathic pulmonary fibrosis (IPF) is important among fibrotic interstitial lung diseases (ILD). This study aimed to evaluate the rate of IPF in patients with fibrotic ILD and to determine the clinical-laboratory features of patients with and without IPF that would provide the differential diagnosis of IPF. MATERIAL AND METHODS: The study included the patients with the usual interstitial pneumonia (UIP) pattern or possible UIP pattern on thorax high-resolution computed tomography, and/or UIP pattern, probable UIP or possible UIP pattern at lung biopsy according to the 2011 ATS/ERSARS/ALAT guidelines. Demographics and clinical and radiological data of the patients were recorded. All data recorded by researchers was evaluated by radiology and the clinical decision board. RESULTS: A total of 336 patients (253 men, 83 women, age 65.8 +/- 9.0 years) were evaluated. Of the patients with sufficient data for diag-nosis (n=300), the diagnosis was IPF in 121 (40.3%), unclassified idiopathic interstitial pneumonia in 50 (16.7%), combined pulmonary fibrosis and emphysema (CPFE) in 40 (13.3%), and lung involvement of connective tissue disease (CTD) in 16 (5.3%). When 29 patients with definite IPF features were added to the patients with CPFE, the total number of IPF patients reached 150 (50%). Rate of male sex (p<0.001), smoking history (p<0.001), and the presence of clubbing (p=0.001) were significantly high in patients with IPE None of the women <50 years and none of the men <50 years of age without a smoking history were diagnosed with IPE Presence of at least 1 of the symptoms suggestive of CTD, erythrocyte sedimentation rate (ESR), and antinuclear antibody (FANA) positivity rates were significantly higher in the non-IPF group (p<0.001, p=0.029, p=0.009, respectively). CONCLUSION: The rate of IPF among patients with fibrotic ILD was 50%. In the differential diagnosis of IPF, sex, smoking habits, and the presence of clubbing are important. The presence of symptoms related to CTD, ESR elevation, and EANA positivity reduce the likelihood of IPF

    The Erciş-Van earthquake with magnitude Mw:7.2 that struck at 13:41 local time on Sunday, October 23,2011 in northern part of Van province affected an area ofabout 13.500 square kilometer at the Eastern Turkey.After several days from the Erciş- Van earthquake,another quake with magnitude Mw: 5.6 occurred onNovember 9, 2011 at am 21:23 local time in southwest ofthe city centre of Van province. Thousands of aftershocks,many in the magnitude 4.0 to 5.0 range, occurred duringthe next few months. This investigation describes theeffects of the aforementioned earthquakes on structuraldamage.     TÜBİTAK MİSAG02.02.1998Bu çalışmada ODTÜ Kimya Mühendisliği Bölümü'nde kumlu olan 0.3 MW ısıl güçteki akışkan yataklı yakıcı üzerinde tipik bir Türk linyitini temsil eden kül, uçucu madde ve kükürt içeriği kaliteli kömürlere göre yüksek olan Beypazarı linyitinin yakılmasıyla ortaya çıkan NOX ve diğer baca gazlarının (CO2,CO,O2veSO2)(CO_2, CO, O_2 ve SO_2) emisyonları on-line ölçümlerle belirlenmiştir. NOx'in yakıcı boyunca dağılımının ve baca gazındaki emisyonunun fazla hava ve kireçtaşı katkısı ile değişimi incelenmiştir. Bulgular, NOx'in daha ziyade linyitin uçucu madde içeriğinden kaynaklandığını ve bu nedenle NOX oluşumunun çalışılan sıcaklık aralığında (857-918°C) sıcaklıktan etkilenmediğini göstermiştir. Baca gazındaki NOX emisyonunun hava fazlasıyla arttığı, kireçtaşı katkısıyla da azaldığı tesbit edilmiştir. Ölçümlerle belirlenen baca gazı emisyonları, günümüzdeki ulusal ve uluslararası standart aralıklarıyla karşılaştırılmış ve izin verilebilir değerlerin altında kaldığı görülmüştür

    Flywheel starter ring gear failures and hardness variation reduction in surface hardening process

    In general technical applications, truck heavy duty engine flywheel starter ring gears teeth is conventionally induction hardened and tempered in order to meet metallurgical aspects on specification. Especially for large sectioned starter ring gears (in this case diameter of the gear is 470 mm) the critical issue is to maintain a stable hardness distribution on gear cross sections. These hardness variations in process could yield to: 1. High scrap costs due to out of specification parts. 2. Increase in tact time in process, decrease in labor efficiency due to scrap rate and excessive hardness measurements. 3. Potential risk of failure on engine in low mileages. 4. 100% hardness measurements in ongoing process which leads to time and cost waste. This technical paper summarizes the case study conducted for truck flywheel starter ring gears with diameter of 470 mm in order to reduce hardness variation by optimization of induction hardening and tempering processes

    Accuracy of transrectal ultrasound guided prostate biopsy: Histopathological correlation to matched prostatectomy specimens

    WOS: 000176132200007PubMed: 12060438Background: The Gleason grading system is currently the world's most commonly used histological system for prostate cancer. It provides significant information about the prognosis. Therefore, Gleason score is accepted as an important factor in therapeutic decision-making for prostate cancer. This retrospective study assessed the correlation of transrectal ultrasound (TRUS) guided biopsy and radical prostatectomy specimens in terms of Gleason scores. Methods: We reviewed the records of 103 patients who underwent radical prostatectomy due to clinically localized prostate cancer. The Gleason scores of the TRUS biopsies were compared with the respective Gleason scores of surgical specimen. Results: In 28.7% of cases, the TRUS biopsy score was the same as that of the radical prostatectomy specimen. The most significant discordance was the upgrading of well-differentiated tumors after surgery in 71.7% of cases. However, in 81.8% of cases with high Gleason score on TRUS, biopsy was correlated with poorly differentiated tumor after surgery. Conclusions: Well-differentiated tumors on TRUS biopsy did not correlate with the grades of final pathology in the majority of cases; however, a high Gleason score on TRUS biopsy usually indicated a poorly differentiated tumor on prostatectomy specimen. Therefore, the treatment algorithms for particularly well-differentiated tumors should not be deduced from biopsy histology alone

    Adenoid Basal Carcinoma of Uterine Cervix: A Case Report

    Adenoid basal carcinoma of uterine cervix is a very rare entity that has been reported less than 100 cases in the literature. Although the origin of the carcinoma is controversial; the multipotential basal cell layer of cervix is the believed part for tumor growth. A 54 years-old white postmenopausal woman was referred to our clinic with the complaint of postmenopausal bleeding. Her pelvic examination revealed a tumor lesion of 1.5cm on the ectocervix. Her cervical and endometrial biopsies revealed an epithelial carcinoma and we performed a radical hysterectomy with bilateral salpingo-oophorectomy and bilateral pelvic-paraaortic lymph node dissection. The final pathology result revealed adenoid basal carcinoma. Distinction of adenoid basal carcinoma from adenoid cystic carcinoma and other aggressive basaloid pathologies of cervix is important for the clinical management of patients since adenoid basal carcinoma is benign in behaviour despite being a carcinoma
