    A história das relações entre escritores portugueses e brasileiros está ainda em parte por ser escrita - ao menos no que diz respeito aos contatos mantidos a partir das primeiras décadas do século XX. Arnaldo Saraiva apresentou há cerca de quinze anos o que pode ser tomado como um bom começo de conversa, 1 centrado nas décadas de dez e vinte. Mas, ao que tudo indica, apenas a partir dos anos trinta estas relações se desenvolveriam plenamente, dando aos contatos anteriores sua real dimensão. Ainda que mais extensos e menos eventuais do que fora sugerido, estes serão, de certa forma, integrados nessa nova etapa das relações, quando os brasileiros terão como interlocutores principais os escritores da geração da presença

Reperfusion injury hampers myocardial recovery in a clinical setting and its management is an unmet need. We investigated the efficacy of intracoronary (IC) delivery of xenogeneic hUCM-MSC as reperfusion-adjuvant therapy in a translational model of AMI in swine. Methods: In a placebo-controlled trial, pot-belied pigs were randomly assigned to a sham-control group (vehicle-injection; n = 8), AMI + vehicle (n = 12) or AMI + IC-injection (n = 11) of 5 × 105 hUCM-MSC/Kg, within 30 min of reperfusion. AMI was created percutaneously by balloon occlusion of the mid-LAD. Left-ventricular function was blindly evaluated at 8-weeks by invasive pressure-volume loop analysis (primary endpoint). Mechanistic readouts included histology, strength-length relationship in skinned cardiomyocytes and gene expression analysis by RNA-sequencing. Results: As compared to vehicle, hUCM-MSC enhanced systolic function as shown by higher ejection fraction (65 ± 6% vs. 43 ± 4%; p = 0.0048), cardiac index (4.1 ± 0.4 vs. 3.1 ± 0.2 L/min/m2; p = 0.0378), preload recruitable stroke work (75 ± 13 vs. 36 ± 4 mmHg; p = 0.0256) and end-systolic elastance (2.8 ± 0.7 vs. 2.1 ± 0.4 mmHg*m2/ml; p = 0.0663). Infarct size was non-significantly lower in cell-treated animals (13.7 ± 2.2% vs. 15.9 ± 2.7%; Δ = −2.2%; p = 0.23), as was interstitial fibrosis and cardiomyocyte hypertrophy in the remote myocardium. Sarcomere active tension improved, and genes related to extracellular matrix remodelling (including MMP9, TIMP1 and PAI1), collagen fibril organization and glycosaminoglycan biosynthesis were downregulated in animals treated with hUCM-MSC. Conclusion: Intracoronary transfer of xenogeneic hUCM-MSC shortly after reperfusion improved left-ventricular systolic function, which could not be explained by the observed extent of infarct size reduction alone. Combined contributions of favourable modification of myocardial interstitial fibrosis, matrix remodelling and enhanced cardiomyocyte contractility in the remote myocardium may provide mechanistic insight for the biological effect.publishersversionpublishe

    Elaboração e validação de um vídeo sobre banho no leito

    Objetivo: elaborar e validar um vídeo sobre a técnica do banho no leito direcionado aos profissionais e estudantes de enfermagem. Método: o vídeo foi baseado na literatura e apresenta a definição de banho no leito, indicações, passos para sua realização e possíveis complicações. A validação foi conduzida por docentes de enfermagem e enfermeiros, que avaliaram a relevância do conteúdo, clareza e linguagem utilizada no vídeo. A Técnica de Delphi foi usada nesta fase. Depois da gravação, o vídeo educacional foi avaliado por três docentes de enfermagem e estudantes do Curso de Graduação em Enfermagem de uma universidade pública. Primeiramente os docentes assistiram ao vídeo e sugeriram alterações e, posteriormente, após os ajustes solicitados, os alunos assistiram ao vídeo. Resultados: seis rodadas foram necessárias para os especialistas validarem o roteiro do vídeo usando a Técnica de Delphi. Depois de assistir ao vídeo, os alunos de graduação o consideraram compreensível. Conclusão: o roteiro do vídeo, composto por quatro tópicos, foi validado por especialistas depois de seis rodadas. O vídeo foi avaliado por docentes e alunos de graduação, que consideraram os tópicos, e o vídeo como um todo, compreensíveis. Espera-se que este vídeo possa contribuir para a qualificação profissional e aprimoramento do conhecimento e habilidades de estudantes de enfermagem.Objetivo: elaborar y validar un video sobre la técnica del baño en la cama dirigido a los profesionales y estudiantes de enfermería. Método: el video fue basado en la literatura y presenta la definición de baño en la cama, indicaciones, pasos para su realización y posibles complicaciones. La validez fue realizada por docentes de enfermería y enfermeros, los que evaluaron la relevancia de contenido, clareza y lenguaje, utilizados en el video. La Técnica de Delphi fue utilizada en esta fase. Después de la grabación, el video educacional fue evaluado por tres docentes de enfermería y por estudiantes del curso de graduación en enfermería de una universidad pública. Primero, los docentes presenciaron el video y sugirieron hacer alteraciones; posteriormente, después de los ajustes solicitados los estudiantes presenciaron el video. Resultados: seis rondas fueron necesarias para que los especialistas validasen el guión del video, usando la Técnica de Delphi. Después de ver el video, los estudiantes de graduación lo consideraron comprensible. Conclusión: el guión del video, compuesto por cuatro tópicos, fue validado por especialistas después de seis rondas de análisis. El video fue evaluado por docentes y estudiantes de graduación, los que consideraron los tópicos y el video como siendo un todo comprensible. Se espera que este video pueda contribuir para la calificación profesional y el perfeccionamiento del conocimiento y de habilidades de los estudiantes de enfermería.Objective: develop and validate a video on bed bathing directed to nursing professionals and students. Method: the video was based on the literature and presents the definition of bed bath, indications for its performance, steps to perform it, and potential complications. Nursing professors and nurses validated it. They assessed the pertinence of content, clarity, and language of the video scenes. The Delphi Technique was used in this phase. After recording, three nursing professors, along with undergraduate nursing students from a public university, assessed the educational video. The professors first watched the video and suggested changes, and then the students watched the video after the changes were implemented. Results: six rounds were needed for experts to validate the video script using the Delphi Technique. After the video recording, undergraduate students considered the video of easy understanding. Conclusion: the video script was composed of four topics and was validated by experts after six rounds. The video was assessed by the professors and nursing undergraduate students, who considered the topics and the video as a whole as apprehensible. This study is expected to contribute to professional training and improvement of the knowledge and skills of nursing students

    A Prospective Study from the IoMum Cohort

    Cobalt (Co), copper (Cu), manganese (Mn), molybdenum (Mo), and zinc (Zn) are essential trace elements (ETEs) and important cofactors for intermediary metabolism or redox balance. These ETEs are crucial during pregnancy, their role on specific pregnancy outcomes is largely unknown. This prospective study (#NCT04010708) aimed to assess urinary levels of these ETEs in pregnancy and to evaluate their association with pregnancy outcomes. First trimester pregnant women of Porto and Lisbon provided a random spot urine sample, and sociodemographic and lifestyle data. Clinical data were obtained from clinical records. Urinary ETEs were quantified by inductively coupled plasma mass spectrometry (ICP-MS). A total of 635 mother:child pairs were included. Having urinary Zn levels above the 50th percentile (P50) was an independent risk factor for pre-eclampsia (PE) (aOR [95% CI]: 5.350 [1.044-27.423], p = 0.044). Urinary Zn levels above the P50 decreased the risk of small for gestational age (SGA) birth head circumference (aOR [95% CI]: 0.315 [0.113-0.883], p = 0.028), but it increased the risk SGA length (aOR [95% CI]: 2.531 [1.057-6.062], p = 0.037). This study may provide valuable information for public health policies related to prenatal nutrition, while informing future efforts to de-fine urinary reference intervals for ETEs in pregnant women.publishersversionpublishe

    Influence of EPICardial adipose tissue in HEART diseases (EPICHEART) study: Protocol for a translational study in coronary atherosclerosis

    Accumulation of epicardial adipose tissue (EAT) is associated with coronary artery disease (CAD) and increased risk of coronary events in asymptomatic subjects and low-risk patients, suggesting that EAT promotes atherosclerosis in its early stage. Recent studies have shown that the presence of CAD affects the properties of adjacent EAT, leading to dynamic changes in the molecular players involved in the interplay between EAT and the coronary arteries over the history of the disease. The role of EAT in late-stage CAD has not been investigated.coronárioResumoIntroduc¸ão:Acumulac¸ão de tecido adiposo epicárdico (TAE) tem sido associado a doenc¸acoronária aterosclerótica (DC) e aumento do risco de eventos coronários em indivíduos ass-intomáticos e doentes de baixo risco, sugerindo que o TAE pode promover fases precoces daDC. Estudo recentes mostraram que a presenc¸a de DC afeta as características do TAE adja-cente levando a modificac¸ões dinâmicas nos mediadores envolvidos na comunicac¸ão entre oTAE e as artérias coronárias ao longo da história da DC. O papel doTAE nas fases avanc¸adas daaterosclerose coronária não foi investigado.Objetivos: Através de análise comparativa com o tecido adiposo mediastínico e subcutâneo,pretendemos investigar se o volume do TAE, avaliado por tomografia computadorizada (TC), eo seu proteoma, avaliado por espectrometria de massa técnica de SWATH, estão associados aestadios avanc¸ados da DC numa coorte de estenose aórtica grave.Métodos: O estudo EPICHEART (NCT03280433) é um estudo prospetivo que inclui doentescom estenose aórtica grave referenciados para substituic¸ão eletiva da válvula aórtica, cujoprotocolo envolve avaliac¸ão pré-operatória clínica, nutricional, ecocardiográfica, por TC eangiografia coronária invasiva. Durante a cirurgia cardíaca, colhemos amostras de tecido adi-poso epicárdico, mediastínico e subcutâneo para análise do seu proteoma por espectrometriade massa técnica de SWATH. Adicionalmente, colhemos líquido pericárdico, sangue venoso per-iférico e do seio coronário para investigar mediadores de DC derivados do TAE na circulac¸ãosistémica e local.Conclusão: Desenhámos um estudo de translac¸ão para explorar a associac¸ão da quantidade equalidade do TAE com a DC tardia. Esperamos identificar mediadores da comunicac¸ão recíprocaentre o TAE e as artérias coronárias que estão envolvidos na patogénese das fases avanc¸adas daDC, especialmente, calcificac¸ão coronária, os quais podem servir como novos alvos terapêuticose soluc¸ões de engenharia biomédica para visualizac¸ão da DC

    The germline mutational landscape of BRCA1 and BRCA2 in Brazil

    The detection of germline mutations in BRCA1 and BRCA2 is essential to the formulation of clinical management strategies, and in Brazil, there is limited access to these services, mainly due to the costs/availability of genetic testing. Aiming at the identification of recurrent mutations that could be included in a low-cost mutation panel, used as a first screening approach, we compiled the testing reports of 649 probands with pathogenic/likely pathogenic variants referred to 28 public and private health care centers distributed across 11 Brazilian States. Overall, 126 and 103 distinct mutations were identified in BRCA1 and BRCA2, respectively. Twenty-six novel variants were reported from both genes, and BRCA2 showed higher mutational heterogeneity. Some recurrent mutations were reported exclusively in certain geographic regions, suggesting a founder effect. Our findings confirm that there is significant molecular heterogeneity in these genes among Brazilian carriers, while also suggesting that this heterogeneity precludes the use of screening protocols that include recurrent mutation testing only. This is the first study to show that profiles of recurrent mutations may be unique to different Brazilian regions.
