    Elastofibrome dorsal: étude rétrospective de 21 cas et revue de littérature

    L'élastofibrome est une tumeur bénigne du tissu mou survient  essentiellement chez les personnes âgées de plus de 55 ans avec une prédominance féminine. Il survient électivement à l'angle caudal de la scapula (99%). Il est bilatéral dans 10% à 66% des cas. L'etiopathogenie de l'EF reste  encore non élucidée. Il s'agissait d'une étude rétrospective au niveau du laboratoire d'anatomie pathologique d'Ibn sina de Rabat durant une période de cinq ans (2007- 2011), concernant 21 cas d'EF. Toutes ces lésions ont été diagnostiquées après coloration standard à l'HE et à partir des dossiers médicaux des patients, nous avons recueilli leurs données cliniques, radiologiques, leur prise en charge thérapeutique et leur suivi. Quizze des patients étaient de sexe féminin, 6 cas étaient de sexe masculin. Au moment du traitement l'âge moyen était de 57,6 ans. Chez 10 patients, la localisation de la tumeur était du côté droit, 6 cas du côté gauche et bilatérale chez 5 patients. La plupart des patients étaientasymptomatiques. La taille des tumeurs variait entre 5 et 14 cm de grand axe. En IRM, leur aspect était caractéristique et l'étude   anatomopathologique avait confirmé le diagnostic chez tous les patients. L'elastofibrome est le diagnostic le plus probable quand il s'agit d'une localisation infra-scapulaire d'une masse du tissu mou. L'IRM est l'examen clé du diagnostic. Une éventuelle biopsie permettra d'exclure un processus tumoral malin et rassurer les patients asymptomatiques qu'aucun  traitement chirurgical n'est nécessaire

    Überprüfung der Bezugskurven der Schweizer Version des Freiburger Zahlen- und Einsilbertests

    Zusammenfassung: Hintergrund: Die Schweizer Ausgabe des Freiburger Sprachverständlichkeitstests unterscheidet sich von der deutschen Version u.a. durch den Ersatz von 5 in der Schweiz ungebräuchlichen einsilbigen Testworten. Zudem wird keine Freifeldentzerrung benutzt und die die sich aus dem Freifeldübertragungsmaß ergebende Differenz von 6dB zwischen Lautsprecher und Kopfhörer wird bei der Kalibrierung nicht berücksichtigt. Der vor einiger Zeit eingeführte Pegelausgleich zwischen den Prüfworten und Anpassungen in der Kalibrierung veranlassten uns zur Überprüfung der Bezugskurven. Patienten/Methoden: Bei 20 normal hörenden Versuchpersonen wurde das Sprachverstehen von Zahlen und einsilbigen Wörtern mit Kopfhörern und Lautsprechern geprüft. Ergebnisse: Der Pegel für 50% Sprachverstehen lag bei Lautsprecherdarbietung im Mittel um 7,5dB niedriger ist als bei Kopfhörerdarbietung. Die mittlere Pegeldifferenz zwischen Zahlen und Einsilbern betrug 9,6dB, liegt also deutlich unter den 14dB der heute in der Schweiz empfohlenen Bezugskurven. Fazit: Die heute in der Schweiz empfohlenen Bezugskurven stimmen für Einsilber bei Kopfhörerdarbietung gut mit unseren Messungen überein, nicht aber für Zahlen und bei Lautsprecherdarbietun

    Perilymphatic fistula in guinea pigs: natural evolution versus surgical treatment

    Perilymphatic fistulas still represent a major treatment challenge. In some cases, its surgical closure can reduce auditory and vestibular sequelae. AIM: to compare the behavior of cochlear window perilymphatic fistulas in guinea pigs as to their natural evolution and immediate surgical closure. MATERIALS AND METHODS: Experimental study. Forty guinea pigs were submitted to cochlear window membrane lesion and randomly broken down into two groups: open fistula (OF) and surgically closed fistula (SCF). We found the summation potential (SP) and action potential (AP) latencies and amplitudes and the SP/AP ratio at three times: pre-fistula (PRE), immediate post-fistula (IPF) and late post-fistula (LPF). RESULTS: There was a significant drop in amplitudes and raise in SP and AP latencies among the times studied. As to the SP/AP ratios, there was a reduction between PRE and IPF, both were significant. There was no behavior difference between the OF and SCF. CONCLUSIONS: Within the time frame considered, guinea pigs submitted to cochlear window membrane lesions evolved with a worsening in potentials and latencies. Despite the partial improvement in electrophysiological parameters, surgical closure did not prove statistically more effective than natural evolution.As fístulas perilinfáticas ainda constituem um grande desafio quanto ao seu tratamento. Em alguns casos, seu fechamento cirúrgico pode reduzir as sequelas auditivas e vestibulares. OBJETIVO: Comparar o comportamento de fístulas perilinfáticas de janela coclear em cobaias quanto à evolução natural e fechamento cirúrgico imediato. MATERIAL E MÉTODOS: Estudo experimental. Quarenta cobaias foram submetidas à lesão da membrana da janela coclear e divididas em dois grupos aleatoriamente: fístula aberta (FA) e fístula fechada cirurgicamente (FF). Foram determinadas as amplitudes e latências do potencial de somação (PS) e do potencial de ação (PA) e da relação PS/PA em três momentos: pré-fístula (PRÉ), pós-fístula imediato (PFI) e pós-fístula tardio (PFT). RESULTADOS: Observou-se significativa queda das amplitudes e aumento das latências do PS e PA entre os momentos considerados. Quanto à relação PS/PA, houve diminuição entre PRÉ e PFI mas aumento entre PFI e PFT, ambos significantes. Não houve diferença de comportamento entre os grupos FA e FF. CONCLUSÕES: No período considerado, cobaias submetidas a lesões da membrana da janela coclear evoluíram com piora dos potenciais e latências.     The vulgar, egotistic junkman Harry Brock has come to a swanky hotel in Washington to make crooked deals with government big-wigs. He has brought with him the charming but dumb ex-chorus girl Billie, whose lack of social graces embarrasses even Harry. Billie must be taught some of the amenities, and a few basic bits of information. The young, idealistic magazine reporter Paul Verrall, who has been investigating political skullduggery and is interested in Brock\u27s activities, agrees for a salary, to educate Billie. He finds Billie has a natural honesty and a frank streak in her, and she begins to learn about history, politics, and what Harry really is and what he wants. At a dramatic moment she rebels against being merely a tool in Harry\u27s crooked schemes and refuses to sign the documents which she has come to learn are part of an ambitious effort to defraud the public. This precipitates a crisis, as Billie readies to leave Harry for a new life of her own. Harry\u27s reaction takes the only form he knows: physical violence. Billie now knows that she can no longer have anything to do with Brock, and realizes she and Paul have fallen genuinely in love. Just before she leaves Harry, she helps Paul get hold of incriminating documents of Harry\u27s which will result in scandal and disaster.
