    Les émotions au travail (scientifique): enjeux éthiques et stratégies méthodologiques d'une enquête en terrain intime

    Based on a qualitative approach, the article proposes to illustrate the different stages of the elaboration of a methodological package, focusing on emotions that are actively put at work. The investigation was conducted on an intimate field, as it deals with themes such sexuality and HIV. The principal aim of the study is to understand how immigrant women of sub-Saharan origin living in Switzerland manage the secret of their HIV / AIDS positive status. Facing multiple vulnerabilities, the studied population is particularly precarious. This raises several ethical questions that are explored in the text. In particular, the emotional implications for the investigators and the impact of the research on the interviewee’s life are questioned. Reflecting on the mobilization of emotions in the process of research, one epistemological question emerges: could knowing subjects be emotional subjects and vice versa? Finally, the emotions mobilized both by the investigators and the interviewees constitute the raw material on which a situated, felt and committed methodology take place

    Les femmes migrantes d'Afrique subsaharienne et séropositives cumulent les vulnérabilités. Fondé sur une recherche qualitative menée auprès de 30 femmes atteintes du VIH, cet article se propose d'analyser comment elles investissent les diverses sphères sociales (familiales, professionnelles, communautaires, etc.) et créent différents types de liens. Si le statut légal, l'expérience migratoire, l'origine, la précarité sociale ou le genre tendent à orienter les pratiques de sociabilité, nous montrerons que l'usage du secret comme stratégie de gestion de l'information relative au VIH reste l'élément déterminant dans l'organi­sation des relations sociales des femmes interrogées. La proximité sociale et l'investissement affectif sont forts dans le milieu médical et associatif, alors que par crainte de stigmatisation, ils le sont moins au sein des communautés africaines, ce qui réduit les possibilités d'entraide et de solidarité. Fortement liée aux représentations négatives du VIH/sida, la pratique du secret conditionne également les relations au sein de la famille et du couple où se mêlent soutien et violence

    Multimodality treatment of unresectable hepatic metastases from pancreatic glucagonoma

    Glucagonomas are pancreatic islet cell tumors arising from the alpha cells which belong to neuroendocrine tumors. They frequently metastasize to the liver. We report the case of a 52- year old man with a pancreatic glucagonoma with synchronous multiple liver metastases treated by surgery, transarterial chemoembolization, percutaneous radiofrequency thermal ablation and long-acting octreotide. Our report confirms that a multimodal approach is very effective in patients with unresectable liver metastases from pancreatic endocrine tumors providing long-lasting palliation and probably prolonging survival

    A Proposal for Evading the Measurement Uncertainty in Classical and Quantum Computing: Application to a Resonant Tunneling Diode and a Mach-Zehnder Interferometer

    Measuring properties of quantum systems is governed by a stochastic (collapse or state-reduction) law that unavoidably yields an uncertainty (variance) associated with the corresponding mean values. This non-classical source of uncertainty is known to be manifested as noise in the electrical current of nanoscale electron devices, and hence it can flaw the good performance of more complex quantum gates. We propose a protocol to alleviate this quantum uncertainty that consists of (i) redesigning the device to accommodate a large number of electrons inside the active region, either by enlarging the lateral or longitudinal areas of the device and (ii) re-normalizing the total current to the number of electrons. How the above two steps can be accommodated using the present semiconductor technology has been discussed and numerically studied for a resonant tunneling diode and a Mach-Zehnder interferometer, for classical and quantum computations, respectively. It is shown that the resulting protocol formally resembles the so-called collective measurements, although, its practical implementation is substantially different

    Hodgkin's disease as unusual presentation of post-transplant lymphoproliferative disorder after autologous hematopoietic cell transplantation for malignant glioma

    BACKGROUND: Post-transplant lymphoproliferative disorder (PTLD) is a complication of solid organ and allogeneic hematopoietic stem cell transplantation (HSCT); following autologous HSCT only rare cases of PTLD have been reported. Here, a case of Hodgkin's disease (HD), as unusual presentation of PTLD after autologous HSCT for malignant glioma is described. CASE PRESENTATION: 60-years old man affected by cerebral anaplastic astrocytoma underwent subtotal neurosurgical excision and subsequent high-dose chemotherapy followed by autologous HSCT. During the post HSCT course, cranial irradiation and corticosteroids were administered as completion of therapeutic program. At day +105 after HSCT, the patient developed HD, nodular sclerosis type, with polymorphic HD-like skin infiltration. CONCLUSION: The clinical and pathological findings were consistent with the diagnosis of PTLD

    Treatment of Chronic Hepatitis C in a Patient Affected by Systemic Sclerosis

    The currently recommended treatment for patients infected with hepatitis C virus (HCV) is pegilated interferon α (IFN α) plus ribavirin. Despite the numerous benefits of this therapy, there is an increasing concern regarding his tolerance. Among the most common side effects, interferon may trigger the onset or exacerbation of autoimmune diseases. When chronic hepatitis C coexists with an autoimmune disorder, it is not clear whether using interferon is better than avoiding it. We evaluated the disease state of a 55-year old female affected by sistemic sclerosis (SSc), during and after therapy with IFNα pegilated plus ribavirin for chronic HCV infection. We were worried about the potential worsening of the autoimmune disease during the therapy, but we were confident that we would give our patient a short course of peginterferon and ribavirin. A mild, asymptomatic worsening of lung SSc was observed during IFN administration, without life threatening symptoms. After 24 months follow up we observed the maintenance of the virological response and a good control of the rheumatological disease. Thus, in liver disease at high risk of progression and concomitant SSc, the antiviral therapy with IFNα is a feasible approach

    A relevance index method to infer global properties of biological networks

    Many complex systems, both natural and artificial, may be represented by networks of interacting nodes. Nevertheless, it is often difficult to find meaningful correspondences between the dynamics expressed by these systems and the topological description of their networks. In contrast, many of these systems may be well described in terms of coordinated behavior of their dynamically relevant parts. In this paper we use the recently proposed Relevance Index approach, based on information-theoretic measures. Starting from the observation of the dynamical states of any system, the Relevance Index is able to provide information about its organization. Moreover, we show how the application of the proposed approach leads to novel and effective interpretations in the T helper network case study

    Starting from a qualitative research concerning the management of the secret of the HIV among immigrant women from sub-Saharan origins living in Switzerland, this article focuses on the methodological and ethical issues requested from such kind of survey. The notions of posture, commitment and participation are here explored. In order to illustrate the ethical dimensions and methodological difficulties, the article is organized in two parts. The first one analyses the institutional constraints due to the research in a public hospital. The second one concerns the political dimension during the delicate stage of the results’ feedback and its impact on women life. Here, the notion of participation in the production of scientific (sociological) data shows the emergence of the concept of scientific citizenship.A partir d’une recherche qualitative portant sur la gestion du secret de la séropositivité chez des femmes migrantes originaires d’Afrique subsaharienne et séropositives vivant en Suisse, cet article s’intéresse aux dimensions méthodologiques et éthiques liées aux enquêtes sur des terrains sensibles. Les notions de posture, d’engagement et de participation sont ici explorées. Afin d’illustrer les enjeux éthiques, les difficultés et les choix méthodologiques que ce type d’enquête soulève, le texte se structure en deux parties. La première partie propose une réflexion autour des contraintes institutionnelles liées à une recherche en milieu hospitalier. La deuxième partie porte sur la dimension politique de la recherche: ses retombées (réelles, imaginées ou espérées) ont une portée pratique dans la vie des femmes interviewées. La notion de participation révèle l’émergence d’une forme de citoyenneté dans la production et la restitution des résultats
