Adjunctive buccal and palatal corticotomy for adult maxillary expansion in an animal model
Objective: This study aimed to explore the usefulness of adjunctive buccal and palatal corticotomy for adult maxillary expansion in an animal model using cone-beam computed tomography (CBCT). Methods: Twelve adult sheep were randomly divided into two groups (each n = 6): a control group, where no treatment was administered, and a treatment group, where buccal and palatal corticotomy-assisted maxillary expansion was performed. CBCT scans were taken before (T1) and after (T2) treatment. Differences in all transverse dental and alveolar dimensions, alveolar width at crest level, hard palate level, horizontal bone loss, interdental cusp width and inter-root apex were assessed using Wilcoxon signed-rank and Mann-Whitney U-tests. Kruskal-Wallis tests and pairwise comparisons were used to detect the significance of differences among the inter-premolar and inter-molar widths. Results: CBCT data revealed significant changes in all transverse dental and alveolar dimensions. The mean interpremolar alveolar width showed an increase of 2.29 to 3.62 mm at the hard palate level, 3.89 to 4.38 mm at the alveolar crest level, and 9.17 to 10.42 mm at the buccal cusp level. Dental changes in the vertical dimension were not significant. Conclusions: Our findings based on an adult animal model suggest that adjunctive buccal and palatal corticotomy can allow for both skeletal and dental expansion, with the amount of dental expansion exceeding that of skeletal expansion at alveolar crest and hard palate levels by two and three folds, respectively. Therefore, this treatment modality is potential to enhance the outcomes of maxillary expansion in adults
SARS Transmission among Hospital Workers in Hong Kong
Despite infection control measures, breakthrough transmission of severe acute respiratory syndrome (SARS) occurred for many hospital workers in Hong Kong. We conducted a case-control study of 72 hospital workers with SARS and 144 matched controls. Inconsistent use of goggles, gowns, gloves, and caps was associated with a higher risk for SARS infection (unadjusted odds ratio 2.42 to 20.54, p < 0.05). The likelihood of SARS infection was strongly associated with the amount of personal protection equipment perceived to be inadequate, having <2 hours of infection control training, and not understanding infection control procedures. No significant differences existed between the case and control groups in the proportion of workers who performed high-risk procedures, reported minor protection equipment problems, or had social contact with SARS-infected persons. Perceived inadequacy of personal protection equipment supply, infection control training <2 hours, and inconsistent use of personal protection equipment when in contact with SARS patients were significant independent risk factors for SARS infection
Iodine-123 metaiodobenzylguanidine scintigraphic assessment of the transplanted human heart: Evidence for late reinnervation
Objectives.This study attempted to determine whether cardiac sympathetic reinnervation occurs late after orthotopic heart transplantation.Background.Metaiodobenzylguanidine (MIBG) is taken up by myocardial sympathetic nerves. Iodine-123 (I-123) MIBG cardiac uptake reflects intact myocardial sympathetic innervation of the heart. Cardiac transplant recipients do not demonstrate I-123 MIBG cardiac uptake when studied <6 months from transplantation. However, physiologic and biochemical studies suggest that sympathetic reinnervation of the heart can occur >1 year after transplantation.Methods.We performed serial cardiac I-123 MIBG imaging in 23 cardiac transplant recipients early (<-1 year) and late (>1 year) after operation. In 16 subjects transmyocardial norepinephrine release was measured late after transplantation.Results.No subject had visible I-123 MIBG uptake on imaging <1 year after transplantation. However, 11 (48%) of 23 subjects developed visible cardiac I-123 MIBG uptake 1 to 2 years after transplantation. Only 3 (25%) of 12 subjects with a pretransplantation diagnosis of idiopathic cardiomyopathy demonstrated I-123 MIBG uptake compared with 8 (73%) of 11 with a pretransplantation diagnosis of ischemic or rheumatic heart disease (p = 0.04). All 10 subjects with a net myocardial release of norepinephrine had cardiac I-123 MIBG uptake; all 6 subjects without a net release of norepinephrine had no cardiac I-123 MIBG uptake.Conclusions.Sympathetic reinnervation of the transplanted human heart can occur >1 year after operation, as assessed by I-123 MIBG imaging and the transmyocardial release of norepinephrine. Reinnervation is less likely to occur in patients with a pretransplantation diagnosis of idiopathic cardiomyopathy than in those with other etiologies of congestive heart failure
Elucidating hepatic lipidosis in stray cats through serum biochemistry, liver histopathology and liver RNA expression of PPAR-δ and PPAR-γ
Early detection of feline hepatic lipidosis (FHL) with appropriate treatment can increase prognosis significantly. This study looks into the serum biochemistry and lipid composition of serum and liver samples in a group of stray cats (N=18) collected from pounds in Klang Valley, Malaysia. Alanine aminotransferase (ALT) in blood serum was used to detect for liver damage possibly due to FHL, confirmed through light microscopy, serum biochemistry (triglyceride, cholesterol, creatinine, and urea), liver triglyceride and cholesterol concentrations, and liver RNA expression of lipid droplet regulators peroxisome proliferator-activated receptors (PPARs). Differing severity of FHL in samples were divided and grouped using an adapted scoring method observing fatty change of liver (FCL) with trends between FCL groups investigated. Elevated serum ALT reflective of increasing FCL severity was observed with elevated concentrations of liver TAG and cholesterol levels. Serum TAG and cholesterol decreased with heightened FCL pointing to fatty acid oxidation and lipid restoration in the liver, supported by PPAR-γ expression which also propose macrophage activation for liver recovery alongside PPAR-δ for lipogenesis and inflammatory reactions. Elevated serum creatinine and urea levels with increasing FCL severity propose overall intact hepatic function in the stray cat samples
Determination of the Form Factors for the Decay B0 --> D*-l+nu_l and of the CKM Matrix Element |Vcb|
We present a combined measurement of the Cabibbo-Kobayashi-Maskawa matrix element and of the parameters , , and , which fully characterize the form factors of the decay in the framework of HQET, based on a sample of about 52,800 decays recorded by the BABAR detector. The kinematical information of the fully reconstructed decay is used to extract the following values for the parameters (where the first errors are statistical and the second systematic): , , , .
