Patterns and predictors of engagement in peer support among homeless veterans with mental health conditions and substance use histories
OBJECTIVES: Patterns and predictors of engagement in peer support services were examined among 50 previously homeless veterans with co-occurring mental health conditions and substance use histories receiving services from the Veterans Health Administration supported housing program.
METHOD: Veteran peer specialists were trained to deliver sessions focusing on mental health and substance use recovery to veterans for an intended 1-hr weekly contact over 9 months. Trajectories of peer engagement over the study\u27s duration are summarized. A mixed-effects log-linear model of the rate of peer engagement is tested with three sets of covariates representing characteristics of the veterans. These sets were demographics, mental health and substance use status, and indicators of community participation and support.
RESULTS: Data indicate that veterans engaged with peers about once per month rather than the intended once per week. However, frequency of contacts varied greatly. The best predictor of engagement was time, with most contacts occurring within the first 6 months. No other veteran characteristic was a statistically significant predictor of engagement. Older veterans tended to have higher rates of engagement with peer supporters.
CONCLUSIONS AND IMPLICATIONS FOR PRACTICE: Planners of peer support services could consider yardsticks of monthly services up to 6 months. Peer support services need a flexible strategy with varying levels of intensity according to need. Peer support services will need to be tailored to better engage younger veterans. Future research should consider other sources of variation in engagement with peer support such as characteristics of the peer supporters and service content and setting
Probing ISM Structure in Trumpler 14 & Carina I Using The Stratospheric Terahertz Observatory 2
We present observations of the Trumpler 14/Carina I region carried out using
the Stratospheric Terahertz Observatory 2 (STO2). The Trumpler 14/Carina I
region is in the west part of the Carina Nebula Complex, which is one of the
most extreme star-forming regions in the Milky Way. We observed Trumpler
14/Carina I in the 158 m transition of [C\,{\sc ii}] with a spatial
resolution of 48 and a velocity resolution of 0.17 km s. The
observations cover a 0.25 by 0.28 area with central position
{\it l} = 297.34, {\it b} = -0.60. The kinematics show that
bright [C\,{\sc ii}] structures are spatially and spectrally correlated with
the surfaces of CO clouds, tracing the photodissociation region and ionization
front of each molecular cloud. Along 7 lines of sight that traverse Tr 14 into
the dark ridge to the southwest, we find that the [C\,{\sc ii}] luminosity from
the HII region is 3.7 times that from the PDR. In same los we find in the PDRs
an average ratio of 1:4.1:5.6 for the mass in atomic gas:dark-CO gas: molecular
gas traced by CO. Comparing multiple gas tracers including HI 21cm, [C\,{\sc
ii}], CO, and radio recombination lines, we find that the HII regions of the
Carina Nebula Complex are well-described as HII regions with one-side freely
expanding towards us, consistent with the champagne model of ionized gas
evolution. The dispersal of the GMC in this region is dominated by EUV
photoevaporation; the dispersal timescale is 20-30 Myr.Comment: ApJ accepte
A tunable delivery platform to provide local chemotherapy for pancreatic ductal adenocarcinoma
Pancreatic ductal adenocarcinoma (PDAC) is one of the most devastating and painful cancers. It is often highly resistant to therapy owing to inherent chemoresistance and the desmoplastic response that creates a barrier of fibrous tissue preventing transport of chemotherapeutics into the tumor. The growth of the tumor in pancreatic cancer often leads to invasion of other organs and partial or complete biliary obstruction, inducing intense pain for patients and necessitating tumor resection or repeated stenting. Here, we have developed a delivery device to provide enhanced palliative therapy for pancreatic cancer patients by providing high concentrations of chemotherapeutic compounds locally at the tumor site. This treatment could reduce the need for repeated procedures in advanced PDAC patients to debulk the tumor mass or stent the obstructed bile duct. To facilitate clinical translation, we created the device out of currently approved materials and drugs. We engineered an implantable poly(lactic-co-glycolic)-based biodegradable device that is able to linearly release high doses of chemotherapeutic drugs for up to 60 days. We created five patient-derived PDAC cell lines and tested their sensitivity to approved chemotherapeutic compounds. These in vitro experiments showed that paclitaxel was the most effective single agent across all cell lines. We compared the efficacy of systemic and local paclitaxel therapy on the patient-derived cell lines in an orthotopic xenograft model in mice (PDX). In this model, we found up to a 12-fold increase in suppression of tumor growth by local therapy in comparison to systemic administration and reduce retention into off-target organs. Herein, we highlight the efficacy of a local therapeutic approach to overcome PDAC chemoresistance and reduce the need for repeated interventions and biliary obstruction by preventing local tumor growth. Our results underscore the urgent need for an implantable drug-eluting platform to deliver cytotoxic agents directly within the tumor mass as a novel therapeutic strategy for patients with pancreatic cancer. HIV-1 Vpu is a potent transcriptional suppressor of NF-ÎșB-elicited antiviral immune responses
Many viral pathogens target innate sensing cascades and/or cellular transcription factors to suppress antiviral immune responses. Here, we show that the accessory viral protein U (Vpu) of HIV-1 exerts broad immunosuppressive effects by inhibiting activation of the transcription factor NF-ÎșB. Global transcriptional profiling of infected CD4 +T cells revealed that vpu-deficient HIV-1 strains induce substantially stronger immune responses than the respective wild type viruses. Gene set enrichment analyses and cytokine arrays showed that Vpu suppresses the expression of NF-ÎșB targets including interferons and restriction factors. Mutational analyses demonstrated that this immunosuppressive activity of Vpu is independent of its ability to counteract the restriction factor and innate sensor tetherin. However, Vpu-mediated inhibition of immune activation required an arginine residue in the cytoplasmic domain that is critical for blocking NF-ÎșB signaling downstream of tetherin. Urban Sustainability : Recovering and Utilizing Urban Excess Heat
Urban heat sources from urban infrastructure and buildings could meet ~10% of the European building heating demand. There is, however, limited information on how to use them. The EU project ReUseHeat has generated much of the existing knowledge on urban waste heat recovery implementation. Heat recovery from a data center, hospital and from water were demonstrated. Additionally, the project generated knowledge of stakeholders, risk profile, bankability and business models. The recovery of urban waste heat is characterized by high potential, high competitiveness compared to other heating alternatives, high avoidance of GHG emissions, payback within three years and low utilization. These characteristics reveal that barriers for increased utilization exist. The barriers are not technical. Instead, the absence of a waste heat EU level policy adds risk. Other showstoppers are low knowledge on the urban waste heat opportunity and new stakeholder relationships being needed for successful recovery. By combining key results and lessons learned from the project this article outlines the frontier of urban waste heat recovery research and practice in 2022
