    Implementation of streamlining measures in selecting and prioritising complex cases for the cancer multidisciplinary team meeting: a mini review of the recent developments

    In January 2020, NHS England and NHS Improvement, in the United Kingdom, issued a permissive framework for streamlining cancer multidisciplinary (MDT) meetings. Streamlining is defined as a process whereby complex cases are prioritized for full discussion by an MDT in an MDT meeting (MDM), while the management of straightforward cases is expedited using Standards of Care (SoC). SoC are points in the pathway of patient management where there are recognized guidelines and clear clinical consensus on the options for management and should be regionally agreed and uniformly applied by regional Cancer Alliances. While this report marks the first major change in cancer MDT management since the Calman-Hine report in 1995, its implementation, nationally, has been slow with now nearly four years since its publication. It is argued however that streamlining is a necessary step in ensuring the viability of MDT processes, and therefore maintaining patient care in the current socioeconomic context of rising workload and cancer incidence, financial pressures, and workforce shortages. In this mini review, we offer a succinct summary of the recent developments around the implementation of the 2020 streamlining framework, including challenges and barriers to its implementation, and the potential future directions in this field, which we propose should increase utilisation of implementation science. We conclude that ensuring successful implementation of the framework and the SOC requires securing a buy-in from key stakeholders, including MDTs and hospital management teams, with clearly defined (a) management approaches that include triage (e.g. through a mini MDT meeting), (b) assessment of case complexity (something that directly feeds into the SOC), and (c) roles of the MDT lead and the members, while acknowledging that the SOC cannot be universally applied without the consideration of individual variations across teams and hospital Trusts

    Cascading training down into the classroom: The need for parallel planning

    Cascade models of in-service training are widely considered to be a cost effective means of introducing educational change to large numbers of teachers. Data from 511 teachers completing a cascade training programme that introduced current ideas about and procedures for teaching English to young learners, suggests that provision of training alone is no guarantee that cascade training aims will actually be applied in classrooms. The paper considers implications for cascade projects, suggesting that planning needs to be a parallel process if an adequate return on outlay, in the sense of teachers applying skills introduced in training in their classrooms, is to be achieved

    Transverse instabilities of uprising planar jets formed by wave impacts on vertical walls

    When a steep breaking wave hits a vertical sea wall, in shallow water, a rapidly ascending planar jet forms. This jet is ejected with high acceleration due to pressure created by the violent wave impact on the wall, creating a so-called 'flip-through' event. Previous studies have focused on the impulsive pressures on, and within, the wall and on the velocity of the jet. Here, in contrast, we consider the formation and break-up of the jet itself. Experiments show that during flip-through a fluid sheet, bounded by a rim, forms. This sheet has unstable transitional behaviours and organizing jets; undulations in the thickness of the fluid sheet are rapidly amplified and ruptured into an array of vertical ligaments. Lateral undulations of the rim lead to the formation of finger-jets, which subsequently break up to form droplets and spray. We present a linear stability analysis of the rim-sheet systems that highlights the contributions of rim retraction and sheet stretching to the breakup process. The mechanisms for the sequential surface deformations in the rim-sheet system are also described. Multiple, distinct, instability modes are identified during the rim deceleration, sheet stretch attenuation and rim retraction processes. The wavenumbers (and deformation length scales) associated with these instability modes are shown to lead to the characteristic double peak spectrum of surface displacement observed in the experiments. These mechanisms help to explain the columnar structures often seen in photographs of violent wave impacts on harbour walls

Despite having provided the necessary infrastructure and investment, many governments have struggled to realise such aims due to the various forces that challenge implementation and institutionalisation. Using institutional theory as a lens, we explored the forces influencing the implementation and institutionalisation of ESI in the public sector. While our results reinforced previous research in IT implementation and organisational transformation, they showed that the dynamic nature of technology poses unanticipated pressures, and that these can impede the implementation and institutionalisation process

    The Cortical Basal ganglia Functional Scale (CBFS): Development and preliminary validation

    OBJECTIVE: To develop a patient/care-giver reported scale capable of easily and reliably assessing functional disability in 4 repeat tauopathies (4RTs). BACKGROUND: 4R tauopathies including progressive supranuclear palsy, corticobasal degeneration and a subset of frontotemporal dementias manifest a range of overlapping clinical phenotypes. No available rating scale is capable of evaluating the functional impact of these complex disorders. METHODS: A multi-staged modified Delphi process was used to propose, evaluate and rank potential scale items providing content validity ratios. Staged cognitive pretesting involving input from examiners, patients and caregivers was followed by validation testing in patients participating in the 4R Tauopathy Neuroimaging Initiative or the PROgressive Supranuclear Palsy CorTico-Basal Syndrome MSA Longitudinal Study. Clinimetric properties were examined using classical test theory and item response methods, assessing data quality, reliability, construct validity, convergent validity and known-group validity. RESULTS: The resultant Cortical Basal ganglia Functional Scale (CBFS) included questions on Motor Experiences in Daily Living (14 items) and Non-Motor Experiences of Daily Living (17 items). Reliability was acceptable for internal consistency, test-retest stability, item discrimination, item-scaling thresholds and item-fit. Examination of construct validity revealed a parsimonious two-factor solution, and concurrent validity demonstrated significant correlations between the CBFS and other measures of disease severity and functional impairment. The CBFS significantly discriminated between all diagnostic groups and controls (all AUCs>90). The CBFS scores demonstrated sensitivity to change over a 12 month follow-up in patients with probable 4RTs. CONCLUSIONS: The CBFS is a patient/care-giver reported outcome measure with excellent clinimetric properties that captures disability correlated with motor, cognitive and psychiatric impairments

    Electromagnetic transitions of the helium atom in superstrong magnetic fields

    We investigate the electromagnetic transition probabilities for the helium atom embedded in a superstrong magnetic field taking into account the finite nuclear mass. We address the regime \gamma=100-10000 a.u. studying several excited states for each symmetry, i.e. for the magnetic quantum numbers 0,-1,-2,-3, positive and negative z parity and singlet and triplet symmetry. The oscillator strengths as a function of the magnetic field, and in particular the influence of the finite nuclear mass on the oscillator strengths are shown and analyzed.Comment: 10 pages, 8 figure

    Studying Cat (Felis catus) Diabetes: Beware of the Acromegalic Imposter

    Naturally occurring diabetes mellitus (DM) is common in domestic cats (Felis catus). It has been proposed as a model for human Type 2 DM given many shared features. Small case studies demonstrate feline DM also occurs as a result of insulin resistance due to a somatotrophinoma. The current study estimates the prevalence of hypersomatotropism or acromegaly in the largest cohort of diabetic cats to date, evaluates clinical presentation and ease of recognition. Diabetic cats were screened for hypersomatotropism using serum total insulin-like growth factor-1 (IGF-1; radioimmunoassay), followed by further evaluation of a subset of cases with suggestive IGF-1 (>1000 ng/ml) through pituitary imaging and/ or histopathology. Clinicians indicated pre-test suspicion for hypersomatotropism. In total 1221 diabetic cats were screened; 319 (26.1%) demonstrated a serum IGF-1>1000 ng/ml (95% confidence interval: 23.6-28.6%). Of these cats a subset of 63 (20%) underwent pituitary imaging and 56/63 (89%) had a pituitary tumour on computed tomography; an additional three on magnetic resonance imaging and one on necropsy. These data suggest a positive predictive value of serum IGF-1 for hypersomatotropism of 95% (95% confidence interval: 90-100%), thus suggesting the overall hypersomatotropism prevalence among UK diabetic cats to be 24.8% (95% confidence interval: 21.2-28.6%). Only 24% of clinicians indicated a strong pre-test suspicion; most hypersomatotropism cats did not display typical phenotypical acromegaly signs. The current data suggest hypersomatotropism screening should be considered when studying diabetic cats and opportunities exist for comparative acromegaly research, especially in light of the many detected communalities with the human disease

    Dialogues:Anthropology and theology

    The past five years have witnessed an increased interest in a dialogue between anthropology and theology, evidenced in part by a suite of edited volumes (e.g. Lauterbach & VĂ€hĂ€kangas 2020; Lemons 2018; Tomlinson & Mathews 2018). Analyses informed by this interdisciplinary nexus have demonstrated the utility of theological concepts for anthropological inquiry (e.g. Robbins 2020; Tomlinson 2020; Williams Green 2021). The following series of dialogues between anthropologists and theologians builds on this growing body of work, expanding it at two main points. First, while the above conversations are all focused on Christian theology, mainly as a means of engaging Christian practice, our dialogues move beyond this religion. The following conversations engage the intersection of anthropology and Islamic, Buddhist, Hindu, as well as Christian, theology.1 Second, many of these dialogues foreground particular experiences of scholars in both anthropology and theology who identify in some way with the religious traditions they study. Some of these dialogues took place between scholars who had an established relationship; others involved partners who had not previously met, but who agreed to correspond in view of a shared interest in this interdisciplinary dialogue. As a starting point, participants were given a series of questions to orient their exchanges, such as, ‘How does faith relate to knowledge in both disciplines?’ Conversations mostly took place over email and were later edited with the help of one of the journal editors, Adam Reed, and one of the members of our Editorial Board, Naomi Haynes.Publisher PDFPeer reviewe

    Isotopic signature in isolated south-western populations of European brown bear (Ursus arctos)

    Stable isotope analysis of animal tissue samples is increasingly used to study the trophic ecology of target species. The isotopic signatures respond to the type of diet, but also to the environmental conditions of their habitat. In the case of omnivorous, seasonal or opportunistic feeding species, the interpretation of isotopic values is more complex, as it is largely determined by food selection, either due to individual choice or because of availability. We analysed C and N isotopes in brown bear (Ursus arctos) hair from four isolated populations of south-western Europe (Cantabrian, Pyrenees, Central Apennines and Alpine) accounting for the geographical and climatic differences among the four areas. We found inter-population differences in isotopic signatures that cannot be attributed to climatic differences alone, indicating that at least some bears from relatively higher altitude populations experiencing higher precipitation (Pyrenees) show a greater consumption of animal foods than those from lower altitudes (Cantabrian and Apennines). The quantification of isotopic niche space using Layman's metrics identified significant similarities between the Cantabrian and Central Apennine samples that markedly differ from the Pyrenean and Alpine. Our study provides a baseline to allow further comparisons in isotopic niche spaces in a broad ranged omnivorous mammal, whose European distribution requires further conservation attention especially for southern isolated populations
