    Unification: An international aerospace information opportunity

    Science and technology projects are becoming more and more international and interdisciplinary. Other parts of the world, notably Europe, are increasingly powerful players in the aerospace industry. This change has led to the development of various aerospace information initiatives in other countries. With scarce resources in all areas of government and industry, the NASA STI Program is reviewing its current acquisition and exchange practices and policies to factor in the changing requirements and new opportunities within the international community. Current NASA goals and activities are reviewed with a new view toward developing a scenario for establishing an international aerospace database, maintaining compatibility among national aerospace information systems, eliminating duplication of effort, and sharing resources through international cooperation wherever possible

    Genetic analysis of Eclosion hormone action during Drosophila larval ecdysis

    Insect growth is punctuated by molts, during which the animal produces a new exoskeleton. The molt culminates with ecdysis, an ordered sequence of behaviors that causes the old cuticle to be shed. This sequence is activated by Ecdysis Triggering Hormone (ETH), which acts on the CNS to activate neurons that produce neuropeptides implicated in ecdysis, including Eclosion hormone (EH), Crustacean Cardioactive Peptide (CCAP), and bursicon. Despite over 40 years of research on ecdysis, our understanding of the precise roles of these neurohormones remains rudimentary. Of particular interest is EH, whose role beyond the well-accepted action of massively upregulating ETH release has remained elusive. We report on the isolation of an eh null mutant in Drosophila, and use it to investigate the role of EH in larval ecdysis. We found that null mutant animals invariably died at around the time of ecdysis, revealing an essential role in its control. Unexpectedly, however, they failed to express the preparatory behavior of pre-ecdysis while directly expressing the motor program of ecdysis. In addition, although ETH release could not be detected in these animals, the lack of pre-ecdysis could not be rescued by injections of ETH, suggesting that EH is required within the CNS for ETH to trigger the normal ecdysial sequence. Using a genetically-encoded calcium probe we show that EH configures the response of the CNS to ETH. These findings show that EH plays an essential role in the Drosophila CNS in the control of ecdysis, in addition to its known role in the periphery of triggering ETH release.</jats:p

    What does it take to make integrated care work? A ‘cookbook’ for large-scale deployment of coordinated care and telehealth

    The Advancing Care Coordination & Telehealth Deployment (ACT) Programme is the first to explore the organisational and structural processes needed to successfully implement care coordination and telehealth (CC&TH) services on a large scale. A number of insights and conclusions were identified by the ACT programme. These will prove useful and valuable in supporting the large-scale deployment of CC&TH. Targeted at populations of chronic patients and elderly people, these insights and conclusions are a useful benchmark for implementing and exchanging best practices across the EU. Examples are: Perceptions between managers, frontline staff and patients do not always match; Organisational structure does influence the views and experiences of patients: a dedicated contact person is considered both important and helpful; Successful patient adherence happens when staff are engaged; There is a willingness by patients to participate in healthcare programmes; Patients overestimate their level of knowledge and adherence behaviour; The responsibility for adherence must be shared between patients and health care providers; Awareness of the adherence concept is an important factor for adherence promotion; The ability to track the use of resources is a useful feature of a stratification strategy, however, current regional case finding tools are difficult to benchmark and evaluate; Data availability and homogeneity are the biggest challenges when evaluating the performance of the programmes

    Postoperative recovery of accelerometer-based physical activity in older cancer patients

    Introduction: Recovery of physical activity is an important functional outcome measure after cancer surgery. However, objective data on physical activity for older cancer patients is scarce. The aims of this study were to quantify perioperative physical activity levels, assess recovery of physical activity three months after surgery, and characterise patients who achieved recovery. Materials and methods: This observational cohort study analysed physical activity data collected from patients aged >65 who were scheduled for cancer surgery between May 2018 and July 2019. Perioperative daily step count was measured using a Fitbit device. The primary outcome measure was the percentage of patients who returned to (>90% of) their preoperative (baseline) physical activity levels three months after surgery. Results: Fifty patients (mean age 73) were recruited, and available Fitbit data was analysed. Median daily step counts at baseline (n = 40), before hospital discharge (n = 40), and three months postoperative (n = 37) were 5,974 (IQR 4,250-7,922),1,619 (IQR 920-2,839), and 4,674 (IQR 3,047-7,592), respectively. The 15/37 (41%) patients who had reached baseline levels three months after surgery seemed to have more preoperative self-reported physical activity, better anaesthesiologists' physical status classification, and fewer in-hospital complications compared to patients who had not, although the differences were statistically non-significant. Conclusion: Perioperative physical activity was quantified for older cancer patients, and 41% returned to baseline levels within three months. Accelerometer-based physical activity provided a valuable outcome measure for postoperative physical recovery. Future studies using objective physical activity measures are needed to evaluate effects of interventional studies aimed at improving physical activity. (C) 2020 The Authors.     Door de STOWA is eind vorig jaar een literatuurstudie naar de verwijdering van hormoonverstorende stoffen (ook wel endocrine disrupting chemicals of EDC's genoemd) in rioolwaterzuiveringsinstallaties verricht. Hieruit blijkt dat, ondanks een redelijke verwijdering in de rwzi, de concentraties van bepaalde EDC's in het effluent nog steeds kunnen leiden tot biologische effecten. De grootste risico's geven de oestrogene hormonen 17a-ethinyloestradiol ('de pil'), 17beta-oestradiol en oestron en de industriële detergenten nonylfenol en nonylfenolethoxylaten. Het is onduidelijk hoe de huidige rioolwaterzuiveringsinstallaties geoptimaliseerd kunnen worden om hormoonverstorende stoffen te verwijderen.     Pro–B-Type Natriuretic Peptide1–108 Circulates in the General Community Plasma Determinants and Detection of Left Ventricular Dysfunction

    ObjectivesThe purpose of this study was to investigate circulating pro–B-type natriuretic peptide (proBNP1–108) in the general community and evaluate its ability to detect left ventricular (LV) dysfunction.BackgroundThe current concept for cardiac endocrine function is that, in response to cardiac stress, the heart secretes B-type natriuretic peptide (BNP1–32) and amino-terminal pro–B-type natriuretic peptide (NT-proBNP1–76) after intracardiac cleavage of their molecular precursor, proBNP1–108. We hypothesized that proBNP1–108 circulates in normal human subjects and that it is a useful biomarker for LV dysfunction.MethodsOur population-based study included a cohort of 1,939 adults (age ≥45 years) from Olmsted County, Minnesota, with 672 participants defined as healthy. Subjects underwent in-depth clinical characterization, detailed echocardiography, and measurement of proBNP1–108. Independent factors associated with proBNP1–108 and test characteristics for the detection of LV dysfunction were determined.ResultsProBNP1–108 in normal humans was strongly influenced by sex, age, heart rate, and body mass index. The median concentration was 20 ng/l with a mean proBNP1–108 to NT-proBNP1–76 ratio of 0.366, which decreased with heart failure stage. ProBNP1–108 was a sensitive (78.8%) and specific (86.1%) biomarker for detecting LV systolic dysfunction, which was comparable to BNP1–32, but less than NT-proBNP1–76, in several subsets of the population.ConclusionsProBNP1–108 circulates in the majority of healthy humans in the general population and is a sensitive and specific biomarker for the detection of systolic dysfunction. The proBNP1–108 to NT-proBNP1–76 ratio may provide insights into altered proBNP1–108 processing during heart failure progression. Thus, this highly specific assay for proBNP1–108 provides important new insights into the biology of the BNP system
