    Optical Coherence Tomography Angiography Features of Iris Racemose Hemangioma in 4 Cases.

    Importance: Optical coherence tomography angiography (OCTA) allows visualization of iris racemose hemangioma course and its relation to the normal iris microvasculature. Objective: To describe OCTA features of iris racemose hemangioma. Design, Setting, and Participants: Descriptive, noncomparative case series at a tertiary referral center (Ocular Oncology Service of Wills Eye Hospital). Patients diagnosed with unilateral iris racemose hemangioma were included in the study. Main Outcomes and Measures: Features of iris racemose hemangioma on OCTA. Results: Four eyes of 4 patients with unilateral iris racemose hemangioma were included in the study. Mean patient age was 50 years, all patients were white, and Snellen visual acuity was 20/20 in each case. All eyes had sectoral iris racemose hemangioma without associated iris or ciliary body solid tumor on clinical examination and ultrasound biomicroscopy. By anterior segment OCT, the racemose hemangioma was partially visualized in all cases. By OCTA, the hemangioma was clearly visualized as a uniform large-caliber vascular tortuous loop with intense flow characteristics superimposed over small-caliber radial iris vessels against a background of low-signal iris stroma. The vascular course on OCTA resembled a light bulb filament (filament sign), arising from the peripheral iris (base of light bulb) and forming a tortuous loop on reaching its peak (midfilament) near the pupil (n = 3) or midzonal iris (n = 1), before returning to the peripheral iris (base of light bulb). Intravenous fluorescein angiography performed in 1 eye depicted the iris hemangioma; however, small-caliber radial iris vessels were more distinct on OCTA than intravenous fluorescein angiography. Conclusions and Relevance: Optical coherence tomography angiography is a noninvasive vascular imaging modality that clearly depicts the looping course of iris racemose hemangioma. Optical coherence tomography angiography depicted fine details of radial iris vessels, not distinct on intravenous fluorescein angiography

    Among recent enterprise systems (ES) research, not much attention and focus have been given to the use and management of ES. Thus, we will examine the daily operations of ES from the social integration (SI) perspective. The main purpose is to understand the influence of SI mechanisms in ES application. We adopt an interpretive case study approach, using 40 interviews with employees from a company with 10 years of experience in the management and application of ES. Based on our findings, we have identified three social integration mechanisms- relational reformation, structural reformation and cognitive construction that had influenced the transformation of individual knowledge and expertise to achieve efficient and effective use of ES

    Results of a World Health Organization scoping of sexual dysfunction-related guidelines: what exists and what is needed

    BACKGROUND: Sexual dysfunction has long been overlooked in the broader context of sexual and reproductive health services. As a result, and in the absence of international sexual dysfunction treatment guidelines, recommendations have instead been developed by a variety of professional associations, worldwide. AIM: We conducted a mapping of existing recommendations addressing a wide array of conditions related to sexual function/dysfunction. METHODS: We contacted 25 professional associations from around the world and held an online open call for guidelines. OUTCOMES: Eligible submissions had to provide recommendations on treatment related to male or female sexual dysfunction. RESULTS: Twelve guidelines from 11 associations were included. Of the 195 recommendations extracted, 61% were related to men, 53% were related specifically to treatment, and 48% did some form of evidence assessment. CONCLUSION: Recommendations from this exercise are provided for WHO, international and national research institutions, ministries of health, and professional associations.     Regulating pharmacists as contraception providers: a qualitative study from Coastal Kenya on injectable contraception provision to youth

    Introduction Young people worldwide are often reticent to access family planning services from public health facilities: instead, they choose to get contraception from private, retail pharmacies. In Kenya, certain contraceptives are available in pharmacies: these include injectables, which can be dispensed but not administered, according national guidelines. However, Kenya struggles with enforcement of its pharmacy regulations and addressing illegal activity. Therefore, in this qualitative study, we assessed private pharmacies as an existing source of injectable contraception for young Kenyans (age 18–24), and investigated the perceived quality of service provision. Methods This study used: focus group discussions (6) with young community members; in-depth interviews (18) with youth who had purchased contraception from pharmacies; key informant interviews with pharmacy personnel and pharmacy stakeholders (25); and a mystery shopper (visiting 45 pharmacies). Results The study found that for injectable contraception, private pharmacies had expanded to service provision, and pharmacy personnel’s roles had transcended formal or informal training previously received–young people could both purchase and be injected in many pharmacies. Pharmacies were perceived to lack consistent quality or strong regulation, resulting in young clients, pharmacy personnel, and regulators being concerned about illegal activity. Participants’ suggestions to improve pharmacy service quality and regulation compliance focused on empowering consumers to demand quality service; strengthening regulatory mechanisms; expanding training opportunities to personnel in private pharmacies; and establishing a quality-based ‘brand’ for pharmacies. Discussion Kenya’s recent commitments to universal health coverage and interest in revising pharmacy policy provide an opportunity to improve pharmacy quality. Multi-pronged initiatives with both public and private partners are needed to improve pharmacy practice, update and enforce regulations, and educate the public. Additionally, the advent of self-administrable injectables present a new possible role for pharmacies, and could offer young clients a clean, discreet place to self-inject, with pharmacy personnel serving as educators and dispensers

    L-Lysine Imprinted Nanoparticles for Antibody Biorecognition

    The aim of this study was to prepare L-lysine-imprinted poly(HEMA-MAAsp) nanoparticles which can be used for the adsorption of IgG from aqueous solutions. L-lysine was complexed with MAAsp and Llysine- imprinted poly(HEMA-MAAsp) nanoparticles were synthesized by miniemulsion polymerization. Also, non-imprinted nanoparticles were synthesized without L-lysine for control purpose. L-lysine-imprinted poly(HEMA-MAAsp) nanoparticles were characterized by means of elemental analysis, Fourier transform infrared spectroscopy (FTIR) and transmission electron microscopy (TEM).     Agility in responding to disruptive digital innovation: Case study of an SME

    Disruptive digital innovation (DDI) often creates hypercompetitive market environment that forces firms to be agile to survive and remain competitive. Whereas most studies have focused on larger firms' effort to be agile, few have looked at how small‐ and medium‐sized enterprises (SMEs) respond to DDI. The study attempts to answer the research question of how SMEs achieve agility to respond to DDI. Drawing on a case study of an innovative SME, our study develops a framework on agility based on the processes of mitigating organizational rigidity, developing innovative capabilities, and balancing the tension of organizational ambidexterity. Specifically, our findings show that for SMEs, mitigating organizational rigidity is enabled by the mechanism of achieving boundary openness while developing innovative capability is enabled by the mechanism of achieving organizational adaptability. At the same time, given the inherent challenges of resource constraints, SMEs also need to balance the tension of organizational ambidexterity
