    Secondary Prevention of Cancer in the Older Individual

    The incidence and the mortality of cancer increase with age. This article explores the possibility of decreasing cancerrelated mortality in the aged with secondary prevention of cancer deaths that entails early diagnosis of cancer through the screening of asymptomatic older individuals. We establish that screening of asymptomatic individuals should be based on physiologic rather than chronologic age that may be estimated from a comprehensive geriatric assessment and possibly with the utilization of biologic markers of aging. It is reasonable to offer some form of screening for lung and colorectal cancer to individuals with a life expectancy of at least five years and screening for breast and prostate cancer to women and men respectively with a life expectancy of at least ten years. The ideal number of screening sessions and the ideal interval between screening sessions is unestablished. The aging of the population, the diversity of the older population, the development of new and more sensitive screening interventions, the discovery of new biologic markers of cancer and age represent the main challenges in studying the value of cancer screening in the aged. Probably the most reliable information may be obtained from rapid-learning databases in which information related to each person's physiologic age is included. Worldwide, the incidence and prevalence of cancer increase with age [1]. In the meantime the risk of cancer-related mortality increases with age at diagnosis [2-4]. As the world population is aging, cancer in the older person is an ever more common problem, and the reduction of cancer deaths in older individuals represents the most urgent goal of cancer control. In this article we explore secondary cancer prevention as representing a strategy to reduce the risk of mortality in the aged.This hypothesis is based on four considerations: i. Several studies showed that the practice of screening asymptomatic individuals for cancer becomes less common with the aging of the population [5]. ii. The average life expectancy of the Western population is rapidly increasing1. Consequently the benefits of early detection of cancer that emerges several years after diagnosis may be present even for those undergoing screening at an advanced age. iii. New forms of cancer treatment, including minimally invasive surgery [6], stereotactic radio surgery [7], and targeted systemic therapy [8], are associated with decreased risk of complications. They may be safely utilized in individuals with limited tolerance of stress who might be hurt by more aggressive treatments. It is now possible to personalize cancer screening according to individual life expectancy and tolerance of stress, as the estimate of a person's physiologic age is becoming more precise [1,9]. After reviewing the principles and the effectiveness of cancer screening and early detection, this article will explore the benefits and risks of secondary cancer prevention in older individuals. In particular we will review the current evidence and the limitations of previous clinical trials. At the end we will propose a conceptual framework to guide the screening of older individuals for cancer, and we will propose a research agenda

    The World Health Organization defines smoking as a chronic addictive disease widespread worldwide. It represents one of the greatest threats to public health, affecting children, youth and adults. It is estimated, that there are 1.300 million smokers in the world and about 80% of these live in countries where there is a high rate of morbidity and mortality associated with tobacco mainly in adult consumers. In particular, the consumption of tobacco among young university students is evident and worrisome. Different authors in the world have raised factors for tobacco use among adolescents and young adults. Such studies argue that the habit of smoking usually begins during adolescence and consolidates in the university stage. Smoking in parents, relatives, and friends is a factor, perhaps the most remarkable, which is associated with the acquisition of smoking as a habit.La Organización Mundial de la Salud define al tabaquismo como una enfermedad adictiva crónica muy extendida a nivel mundial. Representa una de las mayores amenazas para la salud púbica, afectando a niños, jóvenes y adultos. Se calcula que existen 1.300 millones de fumadores en el mundo y alrededor del 80% de estos viven en países en los que existe una alta tasa de morbilidad y mortalidad asociada al tabaco principalmente en consumidores adultos. De manera particular, el consumo de tabaco ente jóvenes universitarios es evidente y preocupante.     Introduction: The consumption of tobacco is very harmful to people’s health. According to WHO, smoking is the cause of death of 8 million people a year. Objetive: The purpose of the research was to determine the factors associated with the consumption of tobacco in university students in Metropolitan Lima. Methods: The sample consisted of 447 students from Lima universities, the survey was applied: Fagerstrom test. Subsequently the data was analyzed using tables of: distribution, frequency, contingency.  Results: It was evidenced through the X2 statistical test that there is a significant association between the variables sex and smoking habit, OR = 1.93, IC 95%[1.29-6,519]. It was also observed that having smoking parents is significantly associated with the habit of smoking by the children, with a value p =0,000 of X2 and the OR 3.31, IC 95%[2,145 - 5,116]. Conclusion: The variables sex and parental background have a significant association with tobacco consumption, emphasizing that our family environment deserves to be safe, healthy and free of tobacco smokeIntroducción: El consumo de tabaco es muy perjudicial para la salud de las personas, según la OMS el tabaquismo es la causa de muerte de 8 millones de personas al año. Objetivo: El propósito de la investigación fue determinar los factores asociados al consumo de tabaco en estudiantes universitarios de Lima Metropolitana. Métodos: La muestra estuvo conformada por 447 estudiantes de universidades de Lima, se aplicó la encuesta: Test de Fagerstrom. Los datos fueron analizados utilizando tablas de distribución, frecuencia, y contingencia. Resultados: Se evidenció a través de la prueba estadística X2 que existe asociación significativa entre las variables sexo y el hábito de fumar, OR = 1.93, IC 95% [1.29 – 6,519].  También se pudo observar que tener padres fumadores está asociado significativamente al hábito de fumar por parte de los hijos, con un valor de p=0,000 del X2 y el OR 3,31, IC 95% [2,145 – 5,116].  Conclusión: Las variables sexo masculino y antecedentes parentales tienen asociación significativa con el consumo de tabaco enfatizando que nuestro entorno familiar merece ser seguro, saludable y libre del humo de tabaco

    Bistability in the Tunnelling Current through a Ring of NN Coupled Quantum Dots

    We study bistability in the electron transport through a ring of N coupled quantum dots with two orbitals in each dot. One orbital is localized (called b orbital) and coupling of the b orbitals in any two dots is negligible; the other is delocalized in the plane of the ring (called d orbital), due to coupling of the d orbitals in the neighboring dots, as described by a tight-binding model. The d orbitals thereby form a band with finite width. The b and d orbitals are connected to the source and drain electrodes with a voltage bias V, allowing the electron tunnelling. Tunnelling current is calculated by using a nonequilibrium Green function method recently developed to treat nanostructures with multiple energy levels. We find a bistable effect in the tunnelling current as a function of bias V, when the size N>50; this effect scales with the size N and becomes sizable at N~100. The temperature effect on bistability is also discussed. In comparison, mean-field treatment tends to overestimate the bistable effect.Comment: Published in JPSJ; minor typos correcte

    In silico ischaemia-induced reentry at the Purkinjeventricle interface

    This computational modelling work illustrates the influence of hyperkalaemia and electrical uncoupling induced by defined ischaemia on action potential (AP) propagation and the incidence of reentry at the Purkinjeventricle interface in mammalian hearts. Unidimensional and bidimensional models of the Purkinjeventricle subsystem, including ischaemic conditions (defined as phase 1B) in the ventricle and an ischaemic border zone, were developed by altering several important electrophysiological parameters of the LuoRudy AP model of the ventricular myocyte. Purkinje electrical activity was modelled using the equations of DiFrancesco and Noble. Our study suggests that an extracellular potassium concentration [K](o) 14 mM and a slight decrease in intercellular coupling induced by ischaemia in ventricle can cause conduction block from Purkinje to ventricle. Under these conditions, propagation from ventricle to Purkinje is possible. Thus, unidirectional block (UDB) and reentry can result. When conditions of UDB are met, retrograde propagation with a long delay (320 ms) may re-excite Purkinje cells, and give rise to a reentrant pathway. This induced reentry may be the origin of arrhythmias observed in phase 1B ischaemia. In a defined setting of ischaemia (phase 1B), a small amount of uncoupling between ventricular cells, as well as between Purkinje and ventricular tissue, may induce UDBs and reentry.     The Essential Interactions in Oxides and Spectral Weight Transfer in Doped Manganites

    We calculate the value of the Fr\"ohlich electron-phonon interaction in manganites, cuprates, and some other charge-transfer insulators and show that this interaction is much stronger than any relevant magnetic interaction. A polaron shift due to the Fr\"ohlich interaction, which is about 1 eV, suggests that carriers in those systems are small (bi)polarons at all temperatures and doping levels, in agreement with the oxygen isotope effect and other data. An opposite conclusion, recently suggested in the literature, is shown to be incorrect. The frequency and temperature dependence of the optical conductivity of ferromagnetic manganites is explained within the framework of the bipolaron theory.Comment: 6 pages, REVTeX 3.1 with 3 eps-figures.     Kerr black holes and time profiles of gamma-ray bursts

    The cumulative light curves of gamma-ray bursts (GRBs) smooth the spiky nature of the running light curve. The cumulative count increases in an approximately linear way with time t for most bursts. In 19 out of 398 GRBs with T90 > 2s, the cumulative light curve was found to increase with time as \~t^2 implying a linear increase in the running light curve. The non-linear sections last for a substantial fraction of the GRB duration, have a large proportion of the cumulative count and many resolved pulses that usually end with the highest pulse in the burst. The reverse behaviour was found in 11 GRBs where the running light curve decreased with time and some bursts are good mirror images of the increases. These GRBs are among the spectrally hardest bursts observed by BATSE. The most likely interpretation is that these effects are signatures of black holes that are either being spun up or down in the accretion process. In the spin up case, the increasing Kerr parameter of the black hole allows additional rotational and accretion energy to become available for extraction. The process is reversed if the black hole is spun down by magnetic field torques. The luminosity changes in GRBs are consistent with the predictions of the BZ process and neutrino annihilation and thus provide the link to spinning black holes.     Specificity of the E. coli LysR-Type Transcriptional Regulators

    Families of paralogous oligomeric proteins are common in biology. How the specificity of assembly evolves is a fundamental question of biology. The LysR-Type Transcriptional Regulators (LTTR) form perhaps the largest family of transcriptional regulators in bacteria. Because genomes often encode many LTTR family members, it is assumed that many distinct homooligomers are formed simultaneously in the same cell without interfering with each other's activities, suggesting specificity in the interactions. However, this assumption has not been systematically tested.A negative-dominant assay with λcI repressor fusions was used to evaluate the assembly of the LTTRs in E. coli K-12. Thioredoxin (Trx)-LTTR fusions were used to challenge the homooligomeric interactions of λcI-LTTR fusions. Eight cI-LTTR fusions were challenged with twenty-eight Trx fusions. LTTRs could be divided into three classes based on their interactions with other LTTRs.Multimerization of LTTRs in E. coli K-12 is mostly specific. However, under the conditions of the assay, many LTTRs interact with more than one noncognate partner. The physiological significance and physical basis for these interactions are not known

    Quantification of toxins in a Cry1Ac + CpTI cotton cultivar and its potential effects on the honey bee Apis mellifera L.

    Transgenic Cry1Ac + CpTI cotton (CCRI41) is increasingly planted throughout China. However, negative effects of this cultivar on the honey bee Apis mellifera L., the most important pollinator for cultivated ecosystem, remained poorly investigated. The objective of our study was to evaluate the potential side effects of transgenic Cry1Ac + CpTI pollen from cotton on young adult honey bees A. mellifera L. Two points emphasized the significance of our study: (1) A higher expression level of insecticidal protein Cry1Ac in pollen tissues was detected (when compared with previous reports). In particular, Cry1Ac protein was detected at 300 ± 4.52 ng g−1 [part per billion (ppb)] in pollen collected in July, (2) Effects on chronic mortality and feeding behaviour in honey bees were evaluated using a no-choice dietary feeding protocol with treated pollen, which guarantee the highest exposure level to bees potentially occurring in natural conditions (worst case scenario). Tests were also conducted using imidacloprid-treated pollen at a concentration of 48 ppb as positive control for sublethal effect on feeding behaviour. Our results suggested that Cry1Ac + CpTI pollen carried no lethal risk for honey bees. However, during a 7-day oral exposure to the various treatments (transgenic, imidacloprid-treated and control), honey bee feeding behaviour was disturbed and bees consumed significantly less CCRI41 cotton pollen than in the control group in which bees were exposed to conventional cotton pollen. It may indicate an antifeedant effect of CCRI41 pollen on honey bees and thus bees may be at risk because of large areas are planted with transgenic Bt cotton in China. This is the first report suggesting a potential sublethal effect of CCRI41 cotton pollen on honey bees. The implications of the results are discussed in terms of risk assessment for bees as well as for directions of future work involving risk assessment of CCRI41 cotton

    Demographic responses of Daphnia magna fed transgenic Bt-maize

    The food/feed quality of a variety of genetically modified (GM) maize expressing Cry1Ab Bt-toxin was tested over the life-cycle of Daphnia magna, an arthropod commonly used as model organism in ecotoxicological studies. Demographic responses were compared between animals fed GM or unmodified (UM) near isogenic maize, with and without the addition of predator smell. Age-specific data on survival and birth rates were integrated and analysed using life tables and Leslie matrices. Survival, fecundity and population growth rate (PGR) data generally disfavoured transgenic Bt-maize as feed for D. magna compared to animals fed the unmodified (UM) near isogenic line of maize. Decomposition of age-specific effects revealed that the most important contributions to a reduced PGR in the GM-fed group came from both fecundity and survival differences early in life. We conclude that juvenile and young adult stages are the most sensitive experimental units and should be prioritized in future research. These stages are often omitted in toxicological/ecotoxicological studies and in feeding trials
