    Spectroscopy of the Lens Galaxy of Q0957+561A,B. Implications of a possible central massive dark object

    We present new long-slit William Herschel Telescope spectroscopic observations of the lens galaxy G1 associated with the double-imaged QSO 0957+561A,B. The obtained central stellar velocity dispersion, sigma_l = 310 +/- 20 km/s, is in reasonable agreement with other measurements of this dynamical parameter. Using all updated measurements of the stellar velocity dispersion in the internal region of the galaxy (at angular separations < 1".5) and a simple isotropic model, we discuss the mass of a possible central massive dark object. It is found that the data of Falco et al. (1997) suggest the existence of an extremely massive object of (0.5-2.1) x 10E10/h M_\odot (80% confidence level), whereas the inclusion of very recent data (Tonry & Franx 1998, and this paper) substantially changes the results: the compact central mass must be \le 6 x10E9/h M_\odot at the 90% confidence level. We note that, taking into account all the available dynamical data, a compact nucleus with a mass of 10E9/h M_\odot (best fit) cannot be ruled out.Comment: 20 pages, 10 figures ApJ, in pres

    Comparison of uptake and prices of biosimilars in the US, Germany, and Switzerland

    Importance: Biologics account for a substantial proportion of health care expenditures. Their costs have been projected to reach US $452 billion in global spending by 2022. Given recent expiration of patent protection of biologics, a shift toward greater follow-on competition among biosimilars would be expected that would allow greater uptake and lower drug costs. Objective: To assess uptake and prices of biosimilars in the US compared with 2 European countries (Germany and Switzerland) with national mechanisms for drug price negotiation. Design, Setting, and Participants: In this cohort study, biologics and biosimilars that were approved in the US, Germany, and Switzerland until August 2020 were identified. Prices and sales data were extracted from public and commercial databases for the years 2011 to 2020. Data were analyzed from August 1, 2021, to February 28, 2022. Main Outcomes and Measures: Descriptive statistics were used to show temporal trends in the uptake of biosimilars and relative prices compared with those of reference products (ie, biologic agents) for each country. Descriptive analysis was also performed to compare the uptake of biosimilars between the 3 countries limited to biologics that have biosimilars on the market in all countries. To test if biosimilar awareness in each country increased over the last decade, a linear least squares regression was applied. Results: The study cohort included 15 biosimilars and 6 biologics for the US, 52 biosimilars and 15 biologics for Germany, and 28 biosimilars and 13 biologics for Switzerland. Uptake of biosimilars increased over time in all countries. On average, the biosimilar market share at launch was highest in Germany; however, it increased at the fastest rate in the US. Monthly treatment costs of biosimilars in the US were a median of 1.94 (IQR, 1.78-2.44) and 2.74 (IQR, 1.91-3.46) higher than corresponding costs in Germany and Switzerland, respectively. Conclusions and Relevance: The findings of this cohort study suggest that more biosimilars have been marketed in Germany and Switzerland than in the US. Policies that counter anticompetitive practices in the US could allow biosimilars to enter the market sooner and could also lower health care costs with improved access. Awareness of biosimilars should be promoted to increase uptake of biosimilars globally

    The INT Search for Metal-Poor Stars. Spectroscopic Observations and Classification via Artificial Neural Networks

    With the dual aims of enlarging the list of extremely metal-poor stars identified in the Galaxy, and boosting the numbers of moderately metal-deficient stars in directions that sample the rotational properties of the thick disk, we have used the 2.5m Isaac Newton Telescope and the Intermediate Dispersion Spectrograph to carry out a survey of brighter (primarily northern hemisphere) metal-poor candidates selected from the HK objective-prism/interference-filter survey of Beers and collaborators. Over the course of only three observing runs (15 nights) we have obtained medium-resolution (resolving power ~ 2000) spectra for 1203 objects (V ~ 11-15). Spectral absorption-line indices and radial velocities have been measured for all of the candidates. Metallicities, quantified by [Fe/H], and intrinsic (B-V)o colors have been estimated for 731 stars with effective temperatures cooler than roughly 6500 K, making use of artificial neural networks (ANNs), trained with spectral indices. We show that this method performs as well as a previously explored Ca II K calibration technique, yet it presents some practical advantages. Among the candidates in our sample, we identify 195 stars with [Fe/H] <= -1.0, 67 stars with [Fe/H] <= -2.0, and 12 new stars with [Fe/H] <= -3.0.     New fecal bacterial signature for colorectal cancer screening reduces the fecal immunochemical test false-positive rate in a screening population

    Guidelines recommend routine screening for colorectal cancer (CRC) in asymptomatic adults starting at age 50. The most extensively used noninvasive test for CRC screening is the fecal immunochemical test (FIT), which has an overall sensitivity for CRC of approximately 61.0%-91.0%, which drops to 27.0%-67.0% for advanced adenomas. These figures contain a high false-positive rate and a low positive predictive value. This work aimed to develop a new, noninvasive CRC screening tool based on fecal bacterial markers capable of decreasing FIT false-positive rates in a FIT-positive population. We defined a fecal bacterial signature (RAID-CRC Screen) in a proof-of-concept with 172 FIT-positive individuals and validated the obtained results on an external cohort of 327 FIT-positive subjects. All study participants had joined the national CRC screening program. In the clinical validation of RAID-CRC Screen, a sensitivity of 83.9% and a specificity of 16.3% were obtained for the detection of advanced neoplasm lesions (advanced adenomas and/or CRC). FIT 20 μg/g produced 184 false-positive results. Using RAID-CRC Screen, this value was reduced to 154, thus reducing the false-positive rate by 16.3%. The RAID-CRC Screen test could be implemented in CRC screening programs to allow a significant reduction in the number of colonoscopies performed unnecessarily for FIT-positive participants of CRC screening programs

    Dietary glycemic index and glycemic load are positively associated with risk of developing metabolic syndrome in middle-aged and elderly adults

Objectives To evaluate how glycemic index (GI) and glycemic load (GL) are associated with the metabolic syndrome (MetS) and its features in middle-aged and elderly adults at high cardiovascular risk. Design Prospective, longitudinal, population-based cohort. Setting PREvenciõn con DIeta MEDiterránea study. Participants Men and women (N = 6,606) divided into three age groups (<65, 65-74, ≥75). Measurements Energy and nutrient intake was evaluated using a validated 137-item food frequency questionnaire. MetS and its features were defined in accordance with the criteria of the American Heart Association and National Heart, Lung, and Blood Institute. Results A positive association was observed between GI and MetS prevalence in the youngest and middle age groups for participants without diabetes mellitus, but no relationship was found for those with diabetes mellitus. During the median follow-up of 4.8 years, higher GI and GL were related to greater risk of MetS in the middle age group, independent of the presence of diabetes mellitus. Changes in dietary GI were associated with risk of developing the high fasting glucose component of the MetS in the oldest age category, and changes in dietary GL were associated with risk of developing abdominal obesity, hypertriglyceridemia, low high-density lipoprotein cholesterol, and high blood pressure in the youngest age category.     Cognitive clusters in first-episode psychosis

    Impairments in a broad range of cognitive domains have been consistently reported in some individuals with first-episode psychosis (FEP). Cognitive deficits can be observed during the prodromal stage. However, the course of cognitive deficits is still unclear. The aim of this study was to identify cognitive subgroups over time and to compare their sociodemographic, clinical and functional profiles. A total of 114 patients with Schizophrenia Spectrum Disorders were included in the present study. We assessed subjects through psychiatric scales and eight neuropsychological tests at baseline and at two-year follow-up visit. We performed the Partition Around Medoids algorithm with all cognitive variables. Furthermore, we performed a logistic regression to identify the predictors related to the different cognitive clusters at follow-up. Two distinct subgroups were found: the first cluster characterized by cognitive impairment and a second cluster had relatively intact cognition in comparison with norms. Up to 54.7% of patients with cognitive deficits at baseline tended to improve during the first two years of treatment. Patients with intact cognition at follow-up had a higher socioeconomic status, later age of onset, lower negative symptoms and a higher cognitive reserve (CR) at baseline. CR and age of onset were the baseline variables that predicted cognitive impairment. This research allows us to obtain a better understanding of the heterogeneous profile of psychotic disorders. Identifying the characteristics of patients who will present a cognitive impairment could improve early detection and intervention. These results suggest that enhancing CR could contribute to improving the course of the illness

    Mediterranean Diet and atherothrombosis biomarkers: a randomized controlled trial

    Scope. To assess whether following a Mediterranean diet (MedDiet) improved atherothrombosis biomarkers in high cardiovascular risk individuals. Methods and results. In 358 random volunteers from the PREvención con DIeta MEDiterránea trial, we assessed the 1-year effects on atherothrombosis markers of an intervention with MedDiet, enriched with virgin olive oil (MedDiet-VOO; n=120) or nuts (MedDiet-Nuts; n=119) versus a low-fat control diet (n=119). We also studied whether large increments in MedDiet adherence (≥3 score points, relative to compliance decreases) and intake changes in key food items were associated with 1-year differences in biomarkers. We observed differences between 1-year changes in the MedDiet-VOO intervention and control diet on the activity of platelet activating factor acetylhydrolase in HDLs (+7.5% [95% confidence interval: 0.17; 14.8]) and HDL-bound α1-antitrypsin levels (- 6.1% [-11.8; -0.29]), and between the MedDiet-Nuts intervention and the control arm on non-esterified fatty acid concentrations (-9.3% [-18.1; -0.53]). Large MedDiet adherence increments were associated with less fibrinogen (-9.5% [-18.3; -0.60]) and non-esterified fatty acid concentrations (-16.7% [-31.7; -1.74]). Increases in nut, fruit, vegetable, and fatty fish consumption, and decreases in processed meat intake were linked to beneficial changes in atherothrombosis biomarkers. Conclusion.     TOI-1135 b: A young hot Saturn-size planet orbiting a solar-type star

    Despite the thousands of planets in orbit around stars known to date, the mechanisms of planetary formation, migration, and atmospheric loss remain unresolved. In this work, we confirm the planetary nature of a young Saturn-size planet transiting a solar-type star every 8.03 d, TOI-1135\,b. The age of the parent star is estimated to be in the interval of 125--1000 Myr based on various activity and age indicators, including its stellar rotation period of 5.13\,±\pm\,0.27 d and the intensity of photospheric lithium. We obtained follow-up photometry and spectroscopy, including precise radial velocity measurements using the CARMENES spectrograph, which together with the TESS data allowed us to fully characterise the parent star and its planet. As expected for its youth, the star is rather active and shows strong photometric and spectroscopic variability correlating with its rotation period. We modelled the stellar variability using Gaussian process regression. We measured the planetary radius at 9.02\,±\pm\,0.23 R_\oplus (0.81\,±\pm\,0.02 RJup_{\mathrm{Jup}}) and determined a 3σ\sigma upper limit of <<\,51.4 M_\oplus (<<\,0.16 \,MJup_{\rm{Jup}}) on the planetary mass by adopting a circular orbit. Our results indicate that TOI-1135\,b is an inflated planet less massive than Saturn or Jupiter but with a similar radius, which could be in the process of losing its atmosphere by photoevaporation. This new young planet occupies a region of the mass-radius diagram where older planets are scarse, and it could be very helpful to understanding the lower frequency of planets with sizes between Neptune and Saturn.Comment: Accepted in A&A. 18 pages, 15 figure

    Predictors of adherence to a Mediterranean-type diet in the PREDIMED trial.

    BACKGROUND: Determinants of dietary changes obtained with a nutritional intervention promoting the Mediterranean diet have been rarely evaluated. AIM: To identify predictors of higher success of an intervention aimed to increase adherence to a Mediterranean diet (MeDiet) in individuals at high cardiovascular risk participating in a trial for primary prevention of cardiovascular disease: the PREDIMED (PREvención con DIeta MEDiterránea) trial. Candidate predictors included demographic and socioeconomic characteristics, cardiovascular risk factors, and baseline dietary habits. METHODS: A total of 1,048 asymptomatic subjects aged 55-80 years allocated to the active intervention groups (subjects in the control group were excluded). Participants' characteristics were assessed at baseline among subjects. Dietary changes were evaluated after 12 months. Main outcome measures were: attained changes in five dietary goals: increases in (1) fruit consumption, (2) vegetable consumption, (3) monounsaturated fatty acid (MUFA)/saturated fatty acid (SFA) ratio, and decreases in (4) sweets and pastries consumption, (5) and meat consumption. Univariate and multivariate logistic regression analyses were used to examine associations between the candidate predictors and likelihood of attaining optimum dietary change (improved adherence to a MeDiet). RESULTS: Among men, positive changes toward better compliance with the MeDiet were more frequent among non-diabetics, and among those with worse dietary habits at baseline (higher consumption of meat, higher SFA intake, lower consumption of fruit and vegetables). Among women, marital status (married) and worse baseline dietary habits (high in meats, low in fruits and vegetables) were the strongest predictors of success in improving adherence to the MeDiet. CONCLUSIONS: Some participant characteristics (marital status and baseline dietary habits) could contribute to predicting the likelihood of achieving dietary goals in interventions aimed to improve adherence to a MeDiet, and may be useful for promoting individualized long-term dietary changes and improving the effectiveness of dietary counseling
