    Foot Blister Incidence in Trekking and Associated Factors

    Objetivo: Determinar los factores asociados con la presencia de ampollas durante el senderismo. Método: La población de estudio fueron 1155 peregrinos que recibieron asistencia podológica. La información fue obtenida mediante una anamnesis y una exploración podológica. Los factores analizados fueron: edad, sexo, entrenamiento previo, nacionalidad, tipo de calzado, antigüedad del mismo y kilómetros realizados. Resultados: Las ampollas se encontraron en el 71,9 % de los peregrinos y se distribuyeron de manera similar en ambos pies, afectando principalmente el talón (20%) y el quinto dedo (14,6 %). Se encontró asociación con el sexo (p < 0,05). Las deportivas fueron las menos asociadas con ampollas (p = 0,03). No hubo relación significativa con el entrenamiento previo y el uso de calzado nuevo. Conclusiones: La incidencia de ampollas entre los senderistas fue del 80 %, y se asoció con el sexo (mayor en la mujer) y el tipo de calzado.Objective: The study objective was to determine the factors associated with presence of blisters during hiking. Methods: The study population included 1155 pilgrims who received podiatry care. The information sources used to obtain the study variables were history by a previously devised questionnaire and foot examination. The analyzed factors were age, gender, prior training, nationality, footwear type, worn versus new shoes and kilometers hiked. Results: Blisters were found in 71.9% of pilgrims. Blisters were distributed similarly on both feet, mostly on the heel (20%) and the 5th toe (14.6%). Gender was an associated variable (p<0.05).The sport shoes were the least associated with presence of blisters (p=0.03). We found no significant relationship with prior training and use of worn shoes. Conclusions: The incidence of blisters among hikers is almost 80%, and is associated with gender (more in woman) and footwear type

    Learning of paediatric dentistry with the flipped classroom model

    Introduction: The flipped classroom (FC) is a pedagogical model that can be very useful in obtaining a dentistry degree. The main objective of this study was to compare learning between student groups introducing FC in the area of paediatric dentistry at the Complutense University of Madrid in 2019. Materials and Methods: In this study, 76 students enrolled for the first time in the Pediatric Dentistry programme and completed a questionnaire (pre-Q) regarding specific theoretical knowledge about pulpotomies. Subsequently, they were divided into two groups: group A, which had free access before the class to an explanatory video about the indications and techniques of pulpotomy and group B, which viewed the same video only one time in class. After that, all students completed the same questionnaire (post-Q) again and a survey on the degree of satisfaction about the teaching method. Results: The differences in score between the post-Q and pre-Q in group A had a mean (SD) value of 3.5 (2.4) and in group B, a mean (SD) of 2.5 (2.2), with a p value of .07. In questions 3, 5 and 8, learning was greater in the experimental group (p = .007, p = .02 and p = .001, respectively). For 74.4% of the students in group A, accessing the video previous to the class was a very useful tool. Conclusions: These results suggest that implementing FC in paediatric dentistry classes help students to acquire more theoretical knowledge, which is necessary before practical teaching

    Consensus of Spanish Society of Hospital Pharmacy on optimal medication therapy management of atopic dermatitis.

Methods: A two-round Delphi survey with participation from HPs from all over Spain. Three theme-based blocks were set out: 1) AD; 2) Management of patients with severe AD in the Hospital Pharmacy setting; and 3) Unmet needs (pathology, patient, treatment and management). Results: The 42 HPs participating reached a consensus in recognising the impact of severe AD on the patients suffering from it, the need to encourage adherence and the recommendations to use scales that take into account the patient's quality of life and indicators of the patient's experience. It has also been demonstrated that it is worthwhile evaluating the results in real clinical practice in consensus with other specialists from the multidisciplinary team. Finally, it is advisable to use drugs that have demonstrated long-term effectiveness and safety for patients with severe AD, given the disease's chronic nature. Conclusions: This Delphi consensus highlights the impact of severe AD on patients, the importance of a multidisciplinary and holistic approach, in which HP play a major role. It also highlights the importance of increased access to new drugs to improve health outcomes

    Effectiveness of 1% versus 0.2% chlorhexidine gels in reducing alveolar osteitis from mandibular third molar surgery: a randomiz, double-blind clinical trial

    Purpose: Alveolar osteitis (AO) is the most common postoperative complication of dental extractions. The purpose of this study was to compare the effectiveness of 1% versus 0.2% chlorhexidine (CHX) gel in reducing postoperative AO after surgical extraction of mandibular third molars, and assess the impact of treatment on the Oral HealthRelated Quality of Life (OHRQoL). Material and Methods: This clinical study was a randomized, double-blind clinical trial. Eighty eight patients underwent surgical extraction of one retained mandibular third molar with the intra-alveolar application of 0.2% CHX gel. Afterwards, they were assigned to one of two groups: 1% CHX gel (n=42) or 0.2% CHX gel (n=46). The patients applied the gel twice a day to the wound for one week. All patients were evaluated for AO. Results: In the 0.2% CHX gel group, 13% of AO incidence was found, while in the 1% CHX gel group, AO incidence was 7%, a difference that was not statistically significant. Variables such as sensation of pain and inflammation at baseline and during one week, as well as OHRQoL of the patients at 24 hours and 7 days post-extraction, gave no statistically significant differences. Conclusions: There are no significant differences in AO after surgical extraction of mandibular third molars, when comparing applying 1% CHX gel twice a day for 7 days with 0.2% CHX gel

    El Aula Invertida es una modalidad de aprendizaje semipresencial en la que el alumno obtiene información en un tiempo y lugar que no requiere la presencia física del profesor, por lo que es necesario el uso de nuevas tecnologías, especialmente del campo audiovisual. En el Grado de Odontología, la materia de Odontopediatría debe capacitar al alumno en la adquisición de las destrezas necesarias para su incorporación a las prácticas clínicas con pacientes en edad infantil. El uso de vídeos educativos nos ha permitido enseñar determinadas maniobras que sería complejo describir de forma teórica y que al ser visualizadas, han facilitado al alumno su entendimiento y posterior ejecución. Hemos evaluado la aplicación educativa diseñada para el desarrollo del aula invertida "Ed-puzzle" que permite al profesor mejorar sus propios vídeos e incorporar preguntas en cualquier punto de su contenido, además de hacer seguimiento de los estudiantes, sus respuestas y visualizaciones. Se ha realizado un estudio comparativo con el uso del "Campus Virtual UCM", respecto al aprendizaje de los alumnos

    Electrically Tunable Reactivity of Substrate-Supported Cobalt Oxide Nanocrystals

    [EN] First-row transition metal oxides are promising materials for catalyzing the oxygen evolution reaction. Surface sensitive techniques provide a unique perspective allowing the study of the structure, adsorption sites, and reactivity of catalysts at the atomic scale, which furnishes rationalization and improves the design of highly efficient catalytic materials. Here, a scanning probe microscopy study complemented by density functional theory on the structural and electronic properties of CoO nanoislands grown on Au(111) is reported. Two distinct phases are observed: The most extended displays a Moiré pattern (α-region), while the less abundant is 1Co:1Au coincidental (β-region). As a result of the surface registry, in the β-region the oxide adlayer is compressed by 9%, increasing the unoccupied local density of states and enhancing the selective water adsorption at low temperature through a cobalt inversion mechanism. Tip-induced voltage pulses irreversibly transform α- into β-regions, thus opening avenues to modify the structure and reactivity of transition metal oxides by external stimuli like electric fields.This work was supported by the European Union under the H2020 FET-PROACT A-LEAF (Artificial-Leaf ) project (Grant Agreement No. 732840).     Aprendizaje colaborativo usando Microsoft Teams en la asignatura de Odontopediatría II

    El aprendizaje colaborativo es una técnica didáctica que promueve desarrollar la colaboración entre individuos con el fin de compartir, conocer y ampliar información que tienen cada uno de ellos sobre un tema. No significa repartirse las tareas sino que cada miembro aporte su información para que así el grupo lo pueda modificar, ampliar y así pasar de un trabajo individual a uno más completo. Este aprendizaje colaborativo se puede hacer de manera presencial o virtual. Para la realización de manera virtual contamos con multitud de plataformas para crear grupos y aprender de forma colaborativa. Nosotros hemos elegido la plataforma Microsoft Teams ya que en la Universidad Complutense de Madrid se está implementado está plataforma para la docencia online. Microsoft Teams (MT) es una plataforma unificada de comunicación y colaboración en la que un grupo puede mantener la comunicación. Dentro de los canales que dispone MT, pueden organizarse reuniones, mantener conversaciones, compartir y almacenar archivos e integrar aplicaciones. En el curso 2021/2022 hemos creado varios Equipo de trabajo en MT con los alumnos matriculados en el asignatura de Odontopeadiatría II. Los profesores hemos subido casos clínicos, preguntas así como temas de debates relacionados con la Odontopediatría, donde se crearon debates entre los alumnos y los profesores de manera on-line. Posteriormente se les pasó a los alumnos encuesta de satisfacción donde valoraron el aprendizaje con este tipo de docencia y esta plataforma. Una vez obtenidas las respuestas, hemos analizados los resultados y elaborado un documento para mejorar el Aprendizaje colaborativo con MT de manera online, intentando establecer mejoras para posteriores años académico

    Pregnancy control in patients with systemic lupus erythematosus/antiphospholipid syndrome. Part 2: Pregnancy follow-up

    Objective: In order to agree on the fundamental aspects related to the management of pregnancy in patients with systemic lupus erythematosus (SLE) and antiphospholipid syndrome (APS), the Spanish Societies of Gynaecology and Obstetrics, Internal Medicine and Rheumatology set up a working group for the preparation of three consensus documents. Methods: Each of the Scientific Societies involved proposed five representatives based on their experience in the field of pregnancy control in patients with autoimmune diseases. The recommendations were developed following the Delphi methodology. Results: This second document contains the recommendations regarding the management of pregnancy in women with SLE and APS, including complications such as lupus activity, congenital heart block, thrombotic and obstetric manifestations of APS and placental vascular disease. Conclusions: These multidisciplinary recommendations are considered decision-making tools for clinicians involved in the care of patients with SLE/APS during pregnancy.Objetivo: Las sociedades españolas de ginecología y obstetricia, de medicina interna y de reumatología han constituido un grupo de trabajo paritario para la elaboración de 3 documentos de consenso sobre el control del embarazo en mujeres con lupus eritematoso sistémico (LES) y síndrome antifosfolípido (SAF).Métodos: Cada una de las sociedades científicas implicadas propuso 5 representantes en base a su experiencia en el área del control del embarazo en pacientes con enfermedades autoinmunes. Las recomendaciones se elaboraron siguiendo la metodología Delphi. Resultados: En este segundo documento se incluyen las recomendaciones que abordan el manejo del embarazo normal y sus complicaciones en mujeres con LES/SAF. Se presentan las recomendaciones relacionadas con el seguimiento del embarazo, la actividad lúpica, el bloqueo cardíaco congénito, las manifestaciones trombóticas y obstétricas del SAF y los defectos de placentación. Conclusiones: Estas recomendaciones multidisciplinares se consideran herramientas en la toma de decisiones para los clínicos involucrados en la asistencia a pacientes con LES/SAF durante el embarazo

    Prognostic models for mortality after cardiac surgery in patients with infective endocarditis: a systematic review and aggregation of prediction models.

    Background There are several prognostic models to estimate the risk of mortality after surgery for active infective endocarditis (IE). However, these models incorporate different predictors and their performance is uncertain. Objective We systematically reviewed and critically appraised all available prediction models of postoperative mortality in patients undergoing surgery for IE, and aggregated them into a meta-model. Data sources We searched Medline and EMBASE databases from inception to June 2020. Study eligibility criteria We included studies that developed or updated a prognostic model of postoperative mortality in patient with IE. Methods We assessed the risk of bias of the models using PROBAST (Prediction model Risk Of Bias ASsessment Tool) and we aggregated them into an aggregate meta-model based on stacked regressions and optimized it for a nationwide registry of IE patients. The meta-model performance was assessed using bootstrap validation methods and adjusted for optimism. Results We identified 11 prognostic models for postoperative mortality. Eight models had a high risk of bias. The meta-model included weighted predictors from the remaining three models (EndoSCORE, specific ES-I and specific ES-II), which were not rated as high risk of bias and provided full model equations. Additionally, two variables (age and infectious agent) that had been modelled differently across studies, were estimated based on the nationwide registry. The performance of the meta-model was better than the original three models, with the corresponding performance measures: C-statistics 0.79 (95% CI 0.76–0.82), calibration slope 0.98 (95% CI 0.86–1.13) and calibration-in-the-large –0.05 (95% CI –0.20 to 0.11). Conclusions The meta-model outperformed published models and showed a robust predictive capacity for predicting the individualized risk of postoperative mortality in patients with IE.
