    Dignity and psychosocial related variables in elderly advanced cancer patients

    Abstract Introduction: Most cancers occur in older individuals, who are more vulnerable due to functional impairment, multiple comorbidities, cognitive impairment, and lack of socio‑familial support. These can undermine patients' sense of dignity. This study seeks to compare dignity scores in older patients with advanced cancer on sociodemographic and clinical variables and analyze the predictive value of anxiety, depression, functional limitations, and social support on dignity scores. Methods: A prospective, multicenter, observational study conducted with participation of 15 hospitals in Spain from February 2020 to October 2021. Patients with newly‑diagnosed, advanced cancer completed the dignity (PPDS), anxiety and depression (BSI), Social Support (Duke-UNC‑11), and functional limitations (EORTC‑C30) scales. Lineal regression analyses explored the effects of anxiety, depression, functional status, and social support on dignity, adjusting for sociodemographic and clinical variables. Results: A total of 180 subjects participated in this study. The results of the correlation analysis revealed that dignity correlated negatively with anxiety, depression, and sex, and positively with social support, functional status, and longer estimated survival. Thus, women, and more anxious and depressed individuals scored lower on the dignity scale, whereas patients with more social support, fewer functional limitations, and longer estimated survival scored higher. Conclusion: In conclusion, being female, having a lower educational level, lower estimated survival, depression, anxiety, less social support, and limited functionality are correlated with less dignity in the elderly with advanced cancer. It is a priority to manage both physical and psychological symptoms in patients with unresectable advanced cancer to mitigate psychological distress and increase their sense of dignit

    Expert range maps of global mammal distributions harmonised to three taxonomic authorities

    AimComprehensive, global information on species' occurrences is an essential biodiversity variable and central to a range of applications in ecology, evolution, biogeography and conservation. Expert range maps often represent a species' only available distributional information and play an increasing role in conservation assessments and macroecology. We provide global range maps for the native ranges of all extant mammal species harmonised to the taxonomy of the Mammal Diversity Database (MDD) mobilised from two sources, the Handbook of the Mammals of the World (HMW) and the Illustrated Checklist of the Mammals of the World (CMW).LocationGlobal.TaxonAll extant mammal species.MethodsRange maps were digitally interpreted, georeferenced, error-checked and subsequently taxonomically aligned between the HMW (6253 species), the CMW (6431 species) and the MDD taxonomies (6362 species).ResultsRange maps can be evaluated and visualised in an online map browser at Map of Life (mol.org) and accessed for individual or batch download for non-commercial use.Main conclusionExpert maps of species' global distributions are limited in their spatial detail and temporal specificity, but form a useful basis for broad-scale characterizations and model-based integration with other data. We provide georeferenced range maps for the native ranges of all extant mammal species as shapefiles, with species-level metadata and source information packaged together in geodatabase format. Across the three taxonomic sources our maps entail, there are 1784 taxonomic name differences compared to the maps currently available on the IUCN Red List website. The expert maps provided here are harmonised to the MDD taxonomic authority and linked to a community of online tools that will enable transparent future updates and version control

    El sistema de yacimientos de mamíferos miocenos del Cerro de los Batallones, Cuenca de Madrid: estado actual y perspectivas

    77 págs, 24 láms.The Cerro de los Batallones (Los Batallones Butte) is located in the central-northern area of the Madrid Basin, central Spain. Nine vertebrates localities containing a large variety of mammals of Upper Vallesian Age (Late Miocene) have been found associated with the sediments forming the butte. From bottom to top, these sediments consist of magnesian lutite beds (Unit I), paleosols formed of sepiolite and opal (Unit II), and siliclastic, marlstone and carbonate beds (Unit III). The set of ERT profiles developed in Los Batallones Butte have demonstrate that electrical imaging techniques are an estimable tool for the characterization and prospecting of fossil sites developed in fine-grained siliciclastic sequences. These localities contain an exceptionally rich, varied and well-preserved vertebrate fauna together with invertebrate and plant fossils. Carnivore species are strikingly well represented at Batallones 1 and 3, and large herbivore species, such as mastodons, rhinoceros and giraffes, at Batallones 2, 4, 5 and 10. The taphonomical studies, together with the morphological features shown by the sedimentary fills of the mammal localities, permit an overall interpretation of these deposits as vertebrate traps. The study of these localities should offer a significant contribution to our understanding of the formation pattern of trap-like paleontological sites - which so far have been typically reported in karstic-type systems -, as well as an important source of paleobiological information about numerous vertebrate groups.Nuestro sincero agradecimiento a la Comunidad de Madrid (Dirección General de Patrimonio Histórico Artístico) por su apoyo a lo largo de todos estos años.     The use of cost per life year gained as a measurement of cost-effectiveness in Spain: a systematic review of recent publications

    The objective of this study was to evaluate the methodological characteristics of cost-effectiveness evaluations carried out in Spain, since 1990, which include LYG as an outcome to measure the incremental cost-effectiveness ratio. METHODS: A systematic review of published studies was conducted describing their characteristics and methodological quality. We analyse the cost per LYG results in relation with a commonly accepted Spanish cost-effectiveness threshold and the possible relation with the cost per quality adjusted life year (QALY) gained when they both were calculated for the same economic evaluation. RESULTS: A total of 62 economic evaluations fulfilled the selection criteria, 24 of them including the cost per QALY gained result as well. The methodological quality of the studies was good (55%) or very good (26%). A total of 124 cost per LYG results were obtained with a mean ratio of 49,529 and a median of 11,490 (standard deviation of 183,080). Since 2003, a commonly accepted Spanish threshold has been referenced by 66% of studies. A significant correlation was found between the cost per LYG and cost per QALY gained results (0.89 Spearman-Rho, 0.91 Pearson). CONCLUSIONS: There is an increasing interest for economic health care evaluations in Spain, and the quality of the studies is also improving. Although a commonly accepted threshold exists, further information is needed for decision-making as well as to identify the relationship between the costs per LYG and per QALY gained
