    Macrodystrophia lipomatosa is a rare congenital nonhereditarymesenchymal hamartomatous malformation resulting in localized gigantism of parts of extremities that manifests clinically as macrodactyly or megalodactyly. Radiological and Pathological hallmark is the disproportionate fibroadipose tissue proliferation in subcutaneous tissue, nerve sheaths, and periosteum that leads to soft tissue and bony enlargement. As imaging is the key to early diagnosis, findings on various modalities like plain radiographs, CT scan and MRI are highlighted here

    Ultrasound review of seven cases of breast tuberculosis

    Tuberculosis of the breast is a rare entity. Etiologically it can be divided into Primary Breast Tuberculosis and Secondary Breast Tuberculosis. On ultrasound, three different appearances of breast tuberculosis have been described, depending upon the imaging morphology visualized on sonomammography. The most common type is nodular type, followed by diffuse type, and sclerosing type. Clinical relevance lies in their potential to mimic breast malignancy. Hence an awareness of the different patterns is essential. This article reviews the ultrasound findings in seven cases of clinico-pathologically and microbiologically proven cases of breast tuberculosis that presented in the imaging department in the last 24 months. Secondary tuberculosis was more common than the primary form and the most common pattern on ultrasound was the nodular pattern. Pathological as well as Microbiological studies were in agreement in three cases only Microbiological studies were positive in two cases; only the Pathology report was positive in one case and in one case local site Microbiological as well as Pathological reports were negative although it was a case of abdominal tuberculosis.Keywords: Breast Tuberculosis; Primary; Secondary; Ultrasound; Acid Fast Bacilli; Pathology; Microbiology; Imagin

    DRENCH: A Semi-Distributed Resource Management Framework for NFV based Service Function Chaining

    As networks grow in scale and complexity, the use of Network Function Virtualization (NFV) and the ability to dynamically instantiate network function instances (NFls) allow us to scale out the network's capabilities in response to demand. At the same time, an increasing number of computing resources, deployed closer to users, as well as network equipment are now capable of performing general-purpose computation for NFV. However, NFV management in the presence of Service Function Chaining (SFC) for arbitrary topologies is a challenging task. In this work we argue for the necessity of an algorithmic resource managementframework that captures the involved tradeoffs of NFls minimum workload, load balancing, and flow path stretch. We introduce DRENCH as a low complexity NFV and flow steering management framework. In DRENCH an NFV market is considered where a centralised SDN controller acts as market orchestrator of NFV nodes. Through competition, NFV nodes make flow steering and NFl instantiation/consolidation decisions. DRENCH design enables third party NFV nodes participation while it can coexist with other NFV management solutions. DRENCH orchestrator parameterisation strikes the right balance between path stretch and NFl load balancing, resulting in significantly lower Flow Completion Times, up to 1Ox less, in some cases

    Determining factors of thermoelectric properties of semiconductor nanowires

    It is widely accepted that low dimensionality of semiconductor heterostructures and nanostructures can significantly improve their thermoelectric efficiency. However, what is less well understood is the precise role of electronic and lattice transport coefficients in the improvement. We differentiate and analyze the electronic and lattice contributions to the enhancement by using a nearly parameter-free theory of the thermoelectric properties of semiconductor nanowires. By combining molecular dynamics, density functional theory, and Boltzmann transport theory methods, we provide a complete picture for the competing factors of thermoelectric figure of merit. As an example, we study the thermoelectric properties of ZnO and Si nanowires. We find that the figure of merit can be increased as much as 30 times in 8-Ã…-diameter ZnO nanowires and 20 times in 12-Ã…-diameter Si nanowires, compared with the bulk. Decoupling of thermoelectric contributions reveals that the reduction of lattice thermal conductivity is the predominant factor in the improvement of thermoelectric properties in nanowires. While the lattice contribution to the efficiency enhancement consistently becomes larger with decreasing size of nanowires, the electronic contribution is relatively small in ZnO and disadvantageous in Si

    Altered Insulin Receptor Signalling and β-Cell Cycle Dynamics in Type 2 Diabetes Mellitus

    Insulin resistance, reduced β-cell mass, and hyperglucagonemia are consistent features in type 2 diabetes mellitus (T2DM). We used pancreas and islets from humans with T2DM to examine the regulation of insulin signaling and cell-cycle control of islet cells. We observed reduced β-cell mass and increased α-cell mass in the Type 2 diabetic pancreas. Confocal microscopy, real-time PCR and western blotting analyses revealed increased expression of PCNA and down-regulation of p27-Kip1 and altered expression of insulin receptors, insulin receptor substrate-2 and phosphorylated BAD. To investigate the mechanisms underlying these findings, we examined a mouse model of insulin resistance in β-cells – which also exhibits reduced β-cell mass, the β-cell-specific insulin receptor knockout (βIRKO). Freshly isolated islets and β-cell lines derived from βIRKO mice exhibited poor cell-cycle progression, nuclear restriction of FoxO1 and reduced expression of cell-cycle proteins favoring growth arrest. Re-expression of insulin receptors in βIRKO β-cells reversed the defects and promoted cell cycle progression and proliferation implying a role for insulin-signaling in β-cell growth. These data provide evidence that human β- and α-cells can enter the cell-cycle, but proliferation of β-cells in T2DM fails due to G1-to-S phase arrest secondary to defective insulin signaling. Activation of insulin signaling, FoxO1 and proteins in β-cell-cycle progression are attractive therapeutic targets to enhance β-cell regeneration in the treatment of T2DM

    Accuracy and differential bias in copy number measurement of CCL3L1 in association studies with three auto-immune disorders

    Contains fulltext : 97604.pdf (publisher's version ) (Open Access)BACKGROUND: Copy number variation (CNV) contributes to the variation observed between individuals and can influence human disease progression, but the accurate measurement of individual copy numbers is technically challenging. In the work presented here we describe a modification to a previously described paralogue ratio test (PRT) method for genotyping the CCL3L1/CCL4L1 copy variable region, which we use to ascertain CCL3L1/CCL4L1 copy number in 1581 European samples. As the products of CCL3L1 and CCL4L1 potentially play a role in autoimmunity we performed case control association studies with Crohn's disease, rheumatoid arthritis and psoriasis clinical cohorts. RESULTS: We evaluate the PRT methodology used, paying particular attention to accuracy and precision, and highlight the problems of differential bias in copy number measurements. Our PRT methods for measuring copy number were of sufficient precision to detect very slight but systematic differential bias between results from case and control DNA samples in one study. We find no evidence for an association between CCL3L1 copy number and Crohn's disease, rheumatoid arthritis or psoriasis. CONCLUSIONS: Differential bias of this small magnitude, but applied systematically across large numbers of samples, would create a serious risk of false positive associations in copy number, if measured using methods of lower precision, or methods relying on single uncorroborated measurements.     Antidepressants during and after Menopausal Transition: A Systematic Review and Meta-Analysis

    To assess the therapeutic benefits of antidepressants in depressive women during and after menopausal transition, PubMed, Cochrane Library, EMBASE and Science Direct were systematically searched from inception to February 1, 2020 for randomized controlled trials examining antidepressants compared to placebo. Primary outcome was change in depressive symptom severity, while secondary outcomes were rates of response/remission rates and dropout/discontinuation due to adverse events. Seven trials involving 1,676 participants (mean age = 52.6 years) showed significant improvement in depressive symptoms (k = 7, Hedges’ g = 0.44, 95% confidence interval (CI) = 0.32 to 0.57, p < 0.001) relative to that in controls. Furthermore, response (k = 3, odds ratio (OR) = 2.53, 95% CI = 1.24 to 5.15, p = 0.01) and remission (k = 3, OR = 1.84, 95% CI = 1.32 to 2.57, p < 0.001) rates were significantly higher in antidepressant-treated groups compared to those with controls. Although dropout rates did not differ between antidepressant and control groups (k = 6, OR = 0.93, 95% CI = 0.70 to 1.26, p = 0.68), the rate of discontinuation due to adverse events was significantly higher in antidepressant-treated groups (k = 6, OR = 0.55, 95% CI = 0.35 to 0.86, p = 0.01). Subgroup analysis indicated that antidepressants were also efficacious for depressive symptoms in those without diagnosis of MDD. The results demonstrated that antidepressants were efficacious for women with depressive syndromes during and after menopausal transition but associated with a higher risk of discontinuation due to adverse events
