    Tango for a dream: narrative liminality and musical sensuality in Richard Linklater's Waking Life

    Richard Linklater’s Waking Life (2001) combines unapologetically cerebral content with a sensuous audiovisual style; fragmentariness in the narrative with an all-encompassing breadth of ideas. Another aspect of Waking Life’s internal contradictions is the juxtaposition of the film’s liminal narrative space with the most “earthly” of music genres, the tango. I will explore the contradictions of Linklater’s film by viewing it in the context of the filmmaker’s metaphysical concerns, showing how all his formal choices, including the rotoscoped visual style, the “narrative of digressions” (Linklater qtd. in Singer) and the choice and placement of music, resonate with the film’s thematic undercurrents and its inquiry into the mysteries of existence, consciousness and time. I will argue that the tension between the film’s narrative liminality and tango’s erotic corporeality addresses the dualistic nature of human experience and that the film’s references to Jean-Luc Godard’s Prénom Carmen (1983) in the scoring not only evoke the French director’s distrust of the film medium itself but also connect to Waking Life’s wider concerns with the nature of reality and our perception of it

    Not so long ago, film music scholars complained habitually that, despite its established and powerful role in the media economy of film, music was being neglected by film and music studies alike. The multidisciplinary nature of film music studies was often cited as a possible reason for this state of affairs: film scholars felt they lacked sufficient musical education, or simply the terminology, to address the matter, while musicologists sniffed at what seemed to them a demeaning appropriation of music in the service of mere illustration or mood-setting. The division between film theory and film music theory was further exacerbated by the perception cultivated by some film scholars that music represented an addition to, rather than an integral part of, film (Kalinak). Not that the early literature on film music did much to suggest otherwise: largely anecdotal in nature, it documented the use of music in silent and early sound films in ways that seemed to reinforce the notion of music as post-production in practice and an after-thought to theory

    Comparative analysis of linear, non-linear and multiple temporal dimensions in music and film reveals that the understanding and utilisation of time in these two arts reflect not only the aesthetic inclinations of its creators and their subjective experiences of temporality but also their philosophical views and, sometimes, spiritual beliefs. Viewed in the context of contemporary theories about Time, particularly Shallis' interpretation of different temporalities as symbolic of various levels of reality and J. T. Fraser's concept of time as a hierarchical nest of different temporalities or Umwelts, the results of this comparison lead to the conclusion that the time in which music and film unfold belongs to a separate, artificial Umwelt of its own - art-temporality

    Vårt intresse i detta examensarbete har varit att studera barn med behov av särskilt stöd samt hur undervisningen sker i en särskola. Syftet är att upptäcka och belysa vilka redskap som pedagogen använder sig av i undervisningen. Redskapen kommer vi att identifiera samt namnge desamma i kategorier som gäller för den specifika klassen. En kvalitativ metod används i examensarbetet och observation i kombination med ostrukturerade intervjuer ligger till grund för vårt resultat. De ostrukturerade intervjuerna leder till en utvidgning av de insamlade materialen, i jämförelse med om enbart observation hade genomförts. I analysen kommer vi att presentera material från observationen och de ostrukturerade intervjuerna och utifrån dessa gruppera pedagogens agerande i olika kategorier som vi sedan kommer att namnge. Fältstudien resulterade i fyra kategorier och de kategorierna som vi kunde särskilja är upprepning, struktur, tydlighet och koncentration. I anslutning till varje kategori har vi jämfört och kopplat till litteraturen. Begreppet ”en skola för alla” har som avsikt att ge varje elev möjlighet till en likvärdig skolgång. I nuläget har skolan svårt att tillgodogöra alla elever och deras olika förutsättningar på allra bästa sätt, vilket delvis beror på skolan organisation. Särskolan är en skolform som ger en unik möjlighet, att se till varje individ och dennes utvecklingsmöjligheter

    Epidemiology and Prognostic Factors in Acute Lower Limb Ischaemia : A Population Based Study

    Objective: The aim of this study was to evaluate the contemporary population based incidence of acute lower limb ischaemia (ALI) and factors associated with major amputation/death at one year. Methods: In this retrospective observational study, in hospital, operation, radiological, and autopsy registries were scrutinised to capture 161 citizens of Malmö, Sweden, with ALI between 2015 and 2018. Age and sex specific incidence rates were calculated in the population of Malmö between 2015 and 2018, expressed as number of patients per 100 000 person years (PY). Independent risk factors for major amputation/death at one year were identified by multivariable logistic regression analysis and expressed as odds ratios (ORs) with 95% confidence intervals (CIs). Results: One hundred and sixty-one patients with ALI gave an overall incidence of 12.2/100 000 PY (95% CI 10.3 – 14.1), with no sex related differences. Embolism (42.2%) was the most common cause of ALI. Among 52 patients with atrial fibrillation, 38.5% were on anticoagulant medication. Endovascular or open vascular revascularisation was performed in 54.7% of patients. The total cause specific mortality ratio was 2.63 (95% CI 1.66 – 3.61)/1 000 deaths, without no sex related differences. The combined major amputation/mortality rate at one year for the whole cohort was 46.6%. Rutherford ≥ IIb ALI (OR 4.19, 95% CI 1.94 – 9.02; p <.001), age (OR 1.03/year, 95% CI 1.00 – 1.06; p =.036), female sex (OR 2.37, 95% 1.07 – 5.26; p =.034), and anaemia (OR 2.46, 95% CI 1.08 – 5.62; p =.033) were associated with an increased risk of major amputation/death at one year. The major amputation/mortality rate at one year was 100% (n = 14/14) for patients living in a nursing home on admission. Conclusion: The incidence of ALI appears to be unchanged, and major amputation and mortality at one year remain high. It is necessary to include the substantial proportion of patients with ALI that do not undergo revascularisation in epidemiological studies. There is room for improvement in anticoagulation therapy in patients with atrial fibrillation to prevent ALI due to embolism. Research on gender inequalities in patients with ALI is warranted

    Inadequacies of Physical Examination in Patients with Acute Lower Limb Ischemia Are Associated with Dreadful Consequences

    Background: Acute lower limb ischemia (ALI) is limb and life-threatening. The aim of this study was to explore the association between adherence to guidelines on clinical diagnosis of ALI and outcome at 1 year. The hypothesis was that that better examination was associated with favorable outcome in ALI patients. Methods: Retrospective cohort study between 2015 and 2018. In-hospital, operation, radiological and autopsy registries captured 161 citizens of Malmö with ALI. The initial bedside evaluation was performed by an emergency physician. Scoring was based on evaluation of the 6 “Ps” and 1 point was given for pain, pallor, pulselessness, perishing cold, paresthesia, paralysis or ankle-brachial pressure index (ABI). The performance was scored (range 0–7), and a score ≥5 was defined as a satisfactory vascular leg status. A multivariate logistic regression was performed to adjust for confounders and expressed in Odds Ratios (OR) with 95% confidence intervals (CI). Results: A satisfactory first clinical examination was performed in 55.3% of the patients. Measurement of ABI (OR 0.25, 95% CI 0.11–0.55), performing complete pulse status (OR 0.41, 95% CI 0.20–0.85), evaluating paralysis (OR 0.43, 95% CI 0.20–0.89), and a bedside score ≥5 points (OR 0.48, 95% CI 0.23–0.97) were independently associated with reduced risk of major amputation/mortality at 1-year follow up. Conclusions: Quality of initial bedside evaluation in patients with ALI was unsatisfactory to a large extent and better clinical examinations were associated with favorable outcome at 1 year. Skills in clinical diagnostics in ALI needs to be much improved

    Extravascular incidental findings in computed tomography angiography are associated with lower amputation-free survival in patients with acute lower limb ischaemia

    BACKGROUND: Emergency computed tomography angiography (CTA) is the most important imaging modality to visualize arterial occlusions in patients with acute lower limb ischaemia (ALI). Extravascular incidental findings (EVIFs) have received less attention.PURPOSE: The aims of this study were to evaluate the incidence of EVIFs of immediate clinical relevance in patients with ALI undergoing CTA and evaluate the association between EVIFs and emergency revascularization and amputation-free survival at 1 year.METHODS: Retrospective cohort study. Emergency CTA in patients with ALI between 2015 and 2018 were independently scrutinized by two senior radiologists. EVIFs were classified into immediate (category I), potential (category II) or no clinical relevance (category III). Multi-variable binary logistic regression analysis was expressed in Odds ratios (OR) with 95% confidence intervals (CI).RESULTS: The intra-class correlation (ICC) coefficient for EVIF category I between the raters was 0.94 (95% CI 0.92-0.96). Among 118 patients with ALI, 78 patients underwent emergency revascularization. Forty-six EVIFs (34 patients) were category I, of which 63% were found in the chest, including pleural effusion (n = 12), pneumonia (n = 8) and cardiac thrombus (n = 4). Ascites (n = 4) and cancer disease (n = 4) were other category I findings. Category I EVIFs were associated with reduced rate of emergency revascularization (OR 0.26, 95% CI 0.10-0.66) and increased rate of combined major amputation/mortality at 1 year (OR 2.9, 95% CI 1.1-8.2) in adjusted analysis.CONCLUSION: It is important to evaluate EVIFs in emergency CTA in patients with ALI since these findings are both common and associated with reduced emergency revascularization and amputation-free survival at 1 year

    Healthy diet and fiber intake are associated with decreased risk of incident symptomatic peripheral artery disease – A prospective cohort study

    Peripheral artery disease (PAD) is caused by atherosclerosis and associated with an increased risk of leg amputation, cardiovascular disease, and death. A healthy diet has been shown to reduce the risk of cardiovascular events, but relationships between diet, fiber intake, and incidence of PAD are virtually unknown. The aim was to investigate the long-term impact of diet on the development of PAD among 26,010 middle-aged individuals in the prospective Malmö Diet and Cancer study (MDCS). Data on dietary intake were collected through a 7-day food diary combined with a food questionnaire and a 1-hour interview. Adherence to a recommended intake of six dietary components – saturated fat, polyunsaturated fat, fish and shellfish, fiber, fruit and vegetables, and sucrose – was scored (sum 0–6 points) to assess a diet quality index, adjusting for potential confounders. Cox regression analysis was used to estimate associations between diet variables and PAD incidence expressed in hazard ratios (HR) with 95% CI. During a median follow-up of 21.7 years, 1122 participants developed PAD. Diet score was associated with a reduced risk of PAD in multivariable analysis (p = 0.03). When mutually adjusting for all dietary variables, only adherence to recommended levels of fiber intake was associated with a reduced risk of incident PAD (HR 0.84; 95% CI 0.72–0.99). In this prospective, population-based study including 26,010 participants with over 20 years of follow-up, a healthy diet, especially a high intake of fiber, was associated with a reduced risk of PAD. Primary prevention programs directed against PAD should therefore include a fiber recommendation
