Crystal Structure of Wild-type Penicillin-binding Protein 5 from Escherichia coli: IMPLICATIONS FOR DEACYLATION OF THE ACYL-ENZYME COMPLEX
Penicillin-binding protein 5 (PBP 5) of Escherichia coli functions as a d-alanine carboxypeptidase (CPase), cleaving d-alanine from the C terminus of cell wall peptides. Like all PBPs, PBP 5 forms a covalent acyl-enzyme complex with beta-lactam antibiotics; however, PBP 5 is distinguished by its high rate of deacylation of the acylenzyme complex (t(1/2) approximately 10 min). A Gly105 --> Asp mutation in PBP 5 markedly impairs deacylation with only minor effects on acylation, and abolishes CPase activity. We have determined the three-dimensional structure of a soluble form of wild-type PBP 5 at 1.85-A resolution and have also refined the structure of the G105D mutant form of PBP 5 to 1.9-A resolution. Comparison of the two structures reveals that the major effect of the mutation is to disorder a loop comprising residues 74-90 that sits atop the SXN motif of the active site. Deletion of the 74-90 loop in wild-type PBP 5 markedly diminished the deacylation rate of penicillin G with a minimal impact on acylation, and abolished CPase activity. These effects were very similar to those observed in the G105D mutant, reinforcing the idea that this mutation causes disordering of the 74-90 loop. Mutation of two consecutive serines within this loop, which hydrogen bond to Ser110 and Asn112 in the SXN motif, had marked effects on CPase activity, but not beta-lactam antibiotic binding or hydrolysis. These data suggest a direct role for the SXN motif in deacylation of the acyl-enzyme complex and imply that the functioning of this motif is modulated by the 74-90 loop
Virushepatitis B und D im Jahr 2020
Infektionen mit Hepatitis-B-Viren (HBV) gehĂśren zu den häufigsten Infektionskrankheiten weltweit. Hepatitis D ist ebenfalls weltweit verbreitet, auch wenn Deutschland zu den Niedrigprävalenzländern gehĂśrt. Sowohl die WHO als auch das Bundesministerium fĂźr Gesundheit haben Strategien zur Eindämmung von Hepatitis B verĂśffentlicht - die COVID-19-Pandemie stellt jedoch eine groĂe HerausÂŹforderung beim Erreichen der Eliminierungsziele bis 2030 dar. In Deutschland wurden im Jahr 2020 24% weniger HBV-Infektionen Ăźbermittelt als im Vorjahr. Wie das Epidemiologische Bulletin 29/2021 anlässlich des Welt-Hepatitis-Tages ausfĂźhrt, sind die GrĂźnde fĂźr die ReÂŹduktion Ăźbermittelter Fallzahlen vielfältig. Die COVID-19-Pandemie hat die GesundheitsversorÂŹgung, den Ăśffentlichen Gesundheitsdienst aber auch das Kontaktverhalten der BevĂślkerung drasÂŹtisch verändert. Die Reduktion der Fallzahlen kĂśnnte aber auch durch die Ănderungen im IfSG im Jahr 2017 begrĂźndet sein.Peer Reviewe
Zur Situation bei wichtigen Infektionskrankheiten in Deutschland â Virushepatitis C im Jahr 2020
Die Anzahl Ăźbermittelter HCV-Infektionen im Jahr 2020 ist im Vergleich zu den Vorjahren 2018 und 2019 deutlich abgefallen. Die Meldedaten von 2020 sind nach den grundsätzlichen Ănderungen des Surveillancesystems und der Meldepflicht nur bedingt mit den Vorjahren vergleichbar und der beobachtete deutliche RĂźckgang der Infektionen kann durch mehrere Faktoren beeinflusst worden sein. Die COVID-19-Pandemie hat durch die Belastung des Ăffentlichen Gesundheitsdienstes auch erhebliche Auswirkungen auf die Erfassung, Meldung und Ăbermittlung von anderen meldepflichtigen Infektionskrankheiten. Ob es sich daher um einen tatsächlichen Abfall der Hepatitis-C-Neudiagnosen handelt oder ob dieser artifiziell bedingt ist durch Untererfassung und RĂźckgang der Diagnostik, lässt sich erst im Laufe der nächsten Jahre bewerten. Virushepatitis C im Jahr 2021
FĂźr das Jahr 2021 wurden insgesamt 4.747 Fälle von Hepatitis C Ăźbermittelt, was einer bundesweiten Inzidenz von 5,7 Ăźbermittelten Infektionen/100.000 Einwohner entspricht und die Inzidenz des Vorjahres (5,5) nur minimal Ăźbersteigt. Der beobachtete RĂźckgang der Anzahl Ăźbermittelter HCV-Infektionen seit 2020 ist mĂśglicherweise bedingt durch Unterdiagnose und -erfassung während der COVID-19-Pandemie. Ein Teil des RĂźckgangs kann aber auch einem tatsächlichen RĂźckgang der Inzidenz durch effektive Hepatitis-C-Behandlungen und dadurch seltenerem Auftreten von Transmissionsereignissen entsprechen. Gleichzeitig sinkt jedoch die Zahl der Behandelten kontinuierlich ab, wodurch das Erreichen der Hepatitis-C-Eliminationsziele bis 2030 gefährdet ist. Virushepatitis B und D im Jahr 2021
Hepatitis B ist eine der häufigsten Infektionskrankheiten weltweit und im Fall eines chronischen Verlaufs eine der bedeutendsten Ursachen von Leberzirrhose und Leberzellkarzinom. Auch Hepatitis D kommt weltweit vor. Deutschland hat sich den Eliminierungszielen viraler Hepatitiden der WHO bis 2030 verschrieben, allerdings stellte die COVID-19-Pandemie eine groĂe Herausforderung beim Erreichen dieser Ziele dar. Im Jahr 2021 wurde eine Zunahme von Ăźbermittelten Hepatitis-B-Fällen um 22 % im Vergleich zu 2020 verzeichnet und damit nach einem wohl hauptsächlich pandemiebedingten RĂźckgang das Niveau von 2019 nahezu wieder erreicht. Anlässlich des Welt-Hepatitis-Tages werden im Epidemiologischen Bulletin 30/2022 die Meldedaten zu Hepatitis B und D des Jahres 2021 nach Infektionsschutzgesetz dargestellt sowie aktuelle Studien zur Hepatitis-B-Virus-Prävalenz und Impfabdeckung in der AllgemeinbevĂślkerung und in ausgewählten Subpopulationen zusammengefasst

The molecular basis of breast cancer pathological phenotypes
The histopathological evaluation of morphological features in breast tumours provides prognostic information to guide therapy. Adjunct molecular analyses provide further diagnostic, prognostic and predictive information. However, there is limited knowledge of the molecular basis of morphological phenotypes in invasive breast cancer. This study integrated genomic, transcriptomic and protein data to provide a comprehensive molecular profiling of morphological features in breast cancer. Fifteen pathologists assessed 850 invasive breast cancer cases from The Cancer Genome Atlas (TCGA). Morphological features were significantly associated with genomic alteration, DNA methylation subtype, PAM50 and microRNA subtypes, proliferation scores, gene expression and/or RPPA subtype. Marked nuclear pleomorphism, necrosis, inflammation and high mitotic count were associated with Basal-like subtype and have similar molecular basis. Omics-based signatures were constructed to predict morphological features. The association of morphology transcriptome signatures with overall survival in oestrogen receptor (ER)-positive and ER-negative breast cancer was first assessed using the METABRIC dataset; signatures that remained prognostic in the METABRIC multivariate analysis were further evaluated in five additional datasets. The transcriptomic signature of epithelial tubule formation was prognostic in ER-positive breast cancer. No signature was prognostic in ER-negative. This study provided new insights into the molecular basis of breast cancer morphological phenotypes. The integration of morphological with molecular data has potential to refine breast cancer classification, predict response to therapy, enhance our understanding of breast cancer biology and improve clinical management. Pathogenesis, diagnosis and management of pneumorrhachis
Pneumorrhachis (PR), the presence of intraspinal air, is an exceptional but eminent radiographic finding, accompanied by different aetiologies and possible pathways of air entry into the spinal canal. By reviewing the literature and analysing a personal case of traumatic cervical PR after head injury, we present current data regarding the pathoanatomy, clinical and radiological presentation, diagnosis and differential diagnosis and treatment modalities of patients with PR and associated pathologies to highlight this uncommon phenomenon and outline aetiology-based guidelines for the practical management of PR. Air within the spinal canal can be divided into primary and secondary PR, descriptively classified into extra- or intradural PR and aetiologically subsumed into iatrogenic, traumatic and nontraumatic PR. Intraspinal air is usually found isolated not only in the cervical, thoracic and, less frequently, the lumbosacral regions but can also be located in the entire spinal canal. PR is almost exceptional associated with further air distributions in the body. The pathogenesis and aetiologies of PR are multifold and can be a diagnostic challenge. The diagnostic procedure should include spinal CT, the imaging tool of choice. PR has to be differentiated from free intraspinal gas collections and the coexistence of air and gas within the spinal canal has to be considered differential diagnostically. PR usually represents an asymptomatic epiphenomenon but can also be symptomatic by itself as well as by its underlying pathology. The latter, although often severe, might be concealed and has to be examined carefully to enable adequate patient treatment. The management of PR has to be individualized and frequently requires a multidisciplinary regime
Prevalence of chronic HCV infection in EU/EEA countries in 2019 using multiparameter evidence synthesis

Our aim was to estimate the prevalence of chronic HCV infection (cHCV) in the European Union (EU)/European Economic Area (EEA) countries in 2019. Methods: Multi-parameter evidence synthesis (MPES) was used to produce national estimates of cHCV defined as: Ď = ĎrecĎrec + ĎexĎex + ĎnonĎnon; Ďrec, Ďex, and Ďnon represent cHCV prevalence among recent people who inject drugs (PWID), ex-PWID, and non-PWID, respectively, while Ďrec, Ďex, and Ďnon represent the proportions of these groups in the population. Information sources included the European Centre for Disease Prevention and Control (ECDC) national operational contact points (NCPs) and prevalence database, the European Monitoring Centre for Drugs and Drug Addiction databases, and the published literature. Findings: The cHCV prevalence in 29 of 30 EU/EEA countries in 2019 was 0.50% [95% Credible Interval (CrI): 0.46%, 0.55%]. The highest cHCV prevalence was observed in the eastern EU/EEA (0.88%; 95% CrI: 0.81%, 0.94%). At least 35.76% (95% CrI: 33.07%, 38.60%) of the overall cHCV prevalence in EU/EEA countries was associated with injecting drugs. Interpretation: Using MPES and collaborating with ECDC NCPs, we estimated the prevalence of cHCV in the EU/EEA to be low. Some areas experience higher cHCV prevalence while a third of prevalent cHCV infections was attributed to PWID. Further efforts are needed to scale up prevention measures and the diagnosis and treatment of infected individuals, especially in the east of the EU/EEA and among PWID. Funding: ECDC.Peer reviewe
