    Investigating Middle Stone Age foraging behaviour in the Karoo, South Africa

    The Middle Stone Age (MSA) in Africa ~500- 50 kyr is recognised as a key time-period associated with important developments in hominin evolution, including the appearance of earliest genetic markers for Homo sapiens. Despite advances, our knowledge of the behaviour of hominins during this period is limited, especially for the early MSA (EMSA) pre-160ka. This study presents new data on animal bones recovered at the Bundu Farm site, in the upper Karoo region of the Northern Cape, South Africa, dated to circa ~300ka and found in association with EMSA type lithic facies, burning and hearth-like features. Previous analysis of the Bundu fauna compared the site to a G/wi hunter-gatherer 'biltong' processing locale, implying primary access to animal carcasses and socially complex hunting behaviour, circa 400-300 ka. An interpretation at odds with other interpretations of the EMSA data that suggest limited hunting and social complexity, and which would therefore have significant implications for MSA archaeology. To test the biltong hypothesis my study presents new data on the fracture characteristics of non-fresh animal bone broken by hammerstone and new environmental data for the site from an analysis of ostrich eggshell isotopes. Experimental and environmental data are used to provide a new interpretation of the Bundu fauna and my conclusion is that the data while not supporting the biltong model, does indicate evidence of delayed communal food consumption, use of fire and the transformation of foodstuffs into meals presaging and echoing social and ecological adaptations seen in the later MSA and LSA. The data also highlights a greater role for carnivores in the accumulation of the faunal assemblage and expedient hominin foraging similar to the preceding ESA and brings attention to the ecological relationships between hominins and carnivores in a Pleistocene Karoo environment that was markedly different from that of today. The study therefore rejects the biltong hypothesis for Bundu Farm as both inconsistent with likely EMSA social structures and ecology and instead proposes the site as evidence for novel behaviour indicative of a transition from ESA to MSA lifeways.     Ostrich eggshell isotope data from Bundu Farm, South Africa and new evidence on middle stone age environments in the Upper Karoo

The final version is available from the South African Archaeological Society via the link in this recordPans and diatomaceous deposits along dry river beds in the Karoo of central South Africa attest to the presence of wet climatic phases in the region during the Pleistocene. Data for these wet climatic phases is largely restricted to the Late Pleistocene and Holocene, with little evidence for earlier periods. This study presents the results of new stable isotope analysis (ÎŽ18O and ÎŽ13C) and amino acid racemisation analysis of ostrich eggshell samples from Middle Stone Age (MSA) site of Bundu Farm, associated with a Florisian faunal assemblage >200ka. Data from the site is compared to previously published data from Wonderwerk Cave and Equus Cave to provide the first direct evidence of Mid-Pleistocene climates and environments in the upper Karoo and their relationship with Middle Stone Age occupation of the region, 150 ka -300 ka, indicating MSA hominins occupation in the region during cooler and wetter phases associated with an expansion of productive grassland.Quaternary Research Fun

    Methods Used in Economic Evaluations of Chronic Kidney Disease Testing — A Systematic Review

    Background: The prevalence of chronic kidney disease (CKD) is high in general populations around the world. Targeted testing and screening for CKD are often conducted to help identify individuals that may benefit from treatment to ameliorate or prevent their disease progression. Aims: This systematic review examines the methods used in economic evaluations of testing and screening in CKD, with a particular focus on whether test accuracy has been considered, and how analysis has incorporated issues that may be important to the patient, such as the impact of testing on quality of life and the costs they incur. Methods: Articles that described model-based economic evaluations of patient testing interventions focused on CKD were identified through the searching of electronic databases and the hand searching of the bibliographies of the included studies. Results: The initial electronic searches identified 2,671 papers of which 21 were included in the final review. Eighteen studies focused on proteinuria, three evaluated glomerular filtration rate testing and one included both tests. The full impact of inaccurate test results was frequently not considered in economic evaluations in this setting as a societal perspective was rarely adopted. The impact of false positive tests on patients in terms of the costs incurred in re-attending for repeat testing, and the anxiety associated with a positive test was almost always overlooked. In one study where the impact of a false positive test on patient quality of life was examined in sensitivity analysis, it had a significant impact on the conclusions drawn from the model. Conclusion: Future economic evaluations of kidney function testing should examine testing and monitoring pathways from the perspective of patients, to ensure that issues that are important to patients, such as the possibility of inaccurate test results, are properly considered in the analysis

    Death with functioning kidney transplant: an obituarial analysis

    Death with a functioning kidney graft (DWFG) is now a major cause of graft loss after renal transplantation, occurring in up to 40% of cases. Its occurrence provides insight into the medical care of subjects with a functioning kidney transplant. In this study, we used the time to DWFG as an endpoint, to test whether improved medical care has contributed to better kidney transplant outcomes. We used single-center data from the Milwaukee Regional Medical Center and Froedtert Hospital, on kidney-only transplants from 1969 through 2005. A total of 3,157 kidney transplants were done at our center during this time. There were 714 deaths with functioning kidney. We also recorded the major causes of DWFG over the time period from 1969 through 2005 divided into 3 epochs. The data were analyzed as a serial collection of yearly obituaries. The time to DWFG has increased to 10 years despite a 20-year increase in the mean age of transplant recipients over the same time period. Better pre-transplant evaluation, improved treatments for hypertension and hyperlipidemia, improved management of acute myocardial infarction, superior immunosuppressive protocols and better prophylaxis and treatment of infectious diseases have all likely contributed to this trend

    Macrocytosis may be associated with mortality in chronic hemodialysis patients: a prospective study

    <p>Abstract</p> <p>Background</p> <p>Macrocytosis occurs in chronic hemodialysis (CHD) patients; however, its significance is unknown. The purpose of this study was to establish the prevalence and distribution of macrocytosis, to identify its clinical associations and to determine if macrocytosis is associated with mortality in stable, chronic hemodialysis patients.</p> <p>Methods</p> <p>We conducted a single-centre prospective cohort study of 150 stable, adult CHD patients followed for nine months. Macrocytosis was defined as a mean corpuscular volume (MCV) > 97 fl. We analyzed MCV as a continuous variable, in tertiles and using a cutoff point of 102 fl.</p> <p>Results</p> <p>The mean MCV was 99.1 ± 6.4 fl, (range 66-120 fl). MCV was normally distributed. 92 (61%) of patients had an MCV > 97 fl and 45 (30%) > 102 fl. Patients were not B12 or folate deficient in those with available data and three patients with an MCV > 102 fl had hypothyroidism. In a logistic regression analysis, an MCV > 102 fl was associated with a higher Charlson-Age Comorbidity Index (CACI) and higher ratios of darbepoetin alfa to hemoglobin (Hb), [(weekly darbepoetin alfa dose in micrograms per kg body weight / Hb in g/L)*1000]. There were 23 deaths at nine months in this study. Unadjusted MCV > 102 fl was associated with mortality (HR 3.24, 95% CI 1.42-7.39, P = 0.005). Adjusting for the CACI, an MCV > 102 fl was still associated with mortality (HR 2.47, 95% CI 1.07-5.71, P = 0.035).</p> <p>Conclusions</p> <p>Macrocytosis may be associated with mortality in stable, chronic hemodialysis patients. Future studies will need to be conducted to confirm this finding.</p

    Effective peer-to-peer support for young people with end-stage renal disease: a mixed methods evaluation of Camp COOL

    __Abstract__ __Background__ The Camp COOL programme aims to help young Dutch people with end-stage renal disease (ESRD) develop self-management skills. Fellow patients already treated in adult care (hereafter referred to as ‘buddies’) organise the day-to-day program, run the camp, counsel the attendees, and also participate in the activities. The attendees are young people who still have to transfer to adult care. This study aimed to explore the effects of this specific form of peer-to-peer support on the self-management of young people (16–25 years) with ESRD who participated in Camp COOL (CC) (hereafter referred to as ‘participants’). __Methods__ A mixed methods research design was employed. Semi-structured interviews (n = 19) with initiators/staff, participants, and healthcare professionals were conducted. These were combined with retrospective and pre-post surveys among participants (n = 62), and observations during two camp weeks. __Results__ Self-reported effects of participants were: increased self-confidence, more disease-related knowledge, feeling capable of being more responsible and open towards others, and daring to stand up for yourself. According to participants, being a buddy or having one positively affected them. Self-efficacy of attendees and independence of buddies increased, while attendees’ sense of social inclusion decreased (measured as domains of health-related quality of life). The buddy role was a pro-active combination of being supervisor, advisor, and leader. __Conclusions__ Camp COOL allowed young people to support each other in adjusting to everyday life with ESRD. Participating in the camp positively influenced self-management in this group. Peerto- peer support through buddies was much appreciated. Support from young adults was not only beneficial for adolescent attendees, but also for young adult buddies. Paediatric nephrologists are encouraged to refer patients to CC and to facilitate such initiatives. Together with nephrologists in adult care, they could take on a role in selecting buddies

    Incidence of cardiovascular events after kidney transplantation and cardiovascular risk scores: study protocol

    <p>Abstract</p> <p>Background</p> <p>Cardiovascular disease (CVD) is the major cause of death after renal transplantation. Not only conventional CVD risk factors, but also transplant-specific risk factors can influence the development of CVD in kidney transplant recipients.</p> <p>The main objective of this study will be to determine the incidence of post-transplant CVD after renal transplantation and related factors. A secondary objective will be to examine the ability of standard cardiovascular risk scores (Framingham, Regicor, SCORE, and DORICA) to predict post-transplantation cardiovascular events in renal transplant recipients, and to develop a new score for predicting the risk of CVD after kidney transplantation.</p> <p>Methods/Design</p> <p>Observational prospective cohort study of all kidney transplant recipients in the A Coruña Hospital (Spain) in the period 1981-2008 (2059 transplants corresponding to 1794 patients).</p> <p>The variables included will be: donor and recipient characteristics, chronic kidney disease-related risk factors, pre-transplant and post-transplant cardiovascular risk factors, routine biochemistry, and immunosuppressive, antihypertensive and lipid-lowering treatment. The events studied in the follow-up will be: patient and graft survival, acute rejection episodes and cardiovascular events (myocardial infarction, invasive coronary artery therapy, cerebral vascular events, new-onset angina, congestive heart failure, rhythm disturbances and peripheral vascular disease).</p> <p>Four cardiovascular risk scores were calculated at the time of transplantation: the Framingham score, the European Systematic Coronary Risk Evaluation (SCORE) equation, and the REGICOR (Registre Gironí del COR (Gerona Heart Registry)), and DORICA (Dyslipidemia, Obesity, and Cardiovascular Risk) functions.</p> <p>The cumulative incidence of cardiovascular events will be analyzed by competing risk survival methods. The clinical relevance of different variables will be calculated using the ARR (Absolute Risk Reduction), RRR (Relative Risk Reduction) and NNT (Number Needed to Treat).</p> <p>The ability of different cardiovascular risk scores to predict cardiovascular events will be analyzed by using the c index and the area under ROC curves. Based on the competing risks analysis, a nomogram to predict the probability of cardiovascular events after kidney transplantation will be developed.</p> <p>Discussion</p> <p>This study will make it possible to determine the post-transplant incidence of cardiovascular events in a large cohort of renal transplant recipients in Spain, to confirm the relationship between traditional and transplant-specific cardiovascular risk factors and CVD, and to develop a score to predict the risk of CVD in these patients.</p
