Nae too bad: a survey of job satisfaction, staff morale and qualifications in residential child care in Scotland
The report of the National Children’s Bureau study of staff morale, qualifications and retention in England (Mainey, 2003) rightly highlighted a number of crucial issues facing providers of residential child care. Entitled Better Than You Think the key finding was that the rates of morale and job satisfaction were not low despite the adverse environment in which residential care operates. Residential care in the modern world is intended to be mainly a temporary placement for some of the most demanding young people who require out-of-home care. However, as with foster care, significant numbers of young people are spending years in out-of-home care and many residential services have to accommodate a wide variety of needs within a single unit. This general remit is challenging enough but the sector also continues to struggle with the aftermath of a number of high profile public inquiries which have identified instances of abuse of children and young people in residential care (Kent, 1997; Utting, 1997; Waterhouse, 2000)
Hypnosis for acute procedural pain: a critical review
Clinical evidence for the effectiveness of hypnosis in the treatment of acute procedural pain was critically evaluated based on reports from randomized controlled clinical trials (RCTs). Results from the 29 RCTs meeting inclusion criteria suggest that hypnosis decreases pain compared to standard care and attention control groups and that it is at least as effective as comparable adjunct psychological or behavioral therapies. In addition, applying hypnosis in multiple sessions prior to the day of the procedure produced the highest percentage of significant results. Hypnosis was most effective in minor surgical procedures. However, interpretations are limited by considerable risk of bias. Further studies using minimally effective control conditions and systematic control of intervention dose and timing are required to strengthen conclusions

Detection of Gravitational Lensing in the Cosmic Microwave Background
Gravitational lensing of the cosmic microwave background (CMB), a
long-standing prediction of the standard cosmolgical model, is ultimately
expected to be an important source of cosmological information, but first
detection has not been achieved to date. We report a 3.4 sigma detection, by
applying quadratic estimator techniques to all sky maps from the Wilkinson
Microwave Anisotropy Probe (WMAP) satellite, and correlating the result with
radio galaxy counts from the NRAO VLA Sky Survey (NVSS). We present our
methodology including a detailed discussion of potential contaminants. Our
error estimates include systematic uncertainties from density gradients in
NVSS, beam effects in WMAP, Galactic microwave foregrounds, resolved and
unresolved CMB point sources, and the thermal Sunyaev-Zeldovich effect.Comment: 27 pages, 20 figure
CMBPol Mission Concept Study: A Mission to Map our Origins
Quantum mechanical metric fluctuations during an early inflationary phase of
the universe leave a characteristic imprint in the polarization of the cosmic
microwave background (CMB). The amplitude of this signal depends on the energy
scale at which inflation occurred. Detailed observations by a dedicated
satellite mission (CMBPol) therefore provide information about energy scales as
high as GeV, twelve orders of magnitude greater than the highest
energies accessible to particle accelerators, and probe the earliest moments in
the history of the universe. This summary provides an overview of a set of
studies exploring the scientific payoff of CMBPol in diverse areas of modern
cosmology, such as the physics of inflation, gravitational lensing and cosmic

Osteoarthritis: a narrative review of molecular approaches to disease management
Osteoarthritis (OA) is a chronic, progressive degenerative whole joint disease that affects the articular cartilage, subchondral bone, ligaments, capsule, and synovium. While it is still believed to be a mechanically driven disease, the role of underlying co-existing inflammatory processes and mediators in the onset of OA and its progression is now more appreciated. Post-traumatic osteoarthritis (PTOA) is a subtype of OA that occurs secondary to traumatic joint insults and is widely used in pre-clinical models to help understand OA in general. There is an urgent need to develop new treatments as the global burden is considerable and expanding. In this review, we focus on the recent pharmacological advances in the treatment of OA and summarize the most significant promising agents based on their molecular effects. Those are classified here into broad categories: anti-inflammatory, modulation of the activity of matrix metalloproteases, anabolic, and unconventional pleiotropic agents. We provide a comprehensive analysis of the pharmacological advances in each of these areas and highlight future insights and directions in the OA field
A computational study on altered theta-gamma coupling during learning and phase coding
There is considerable interest in the role of coupling between theta and gamma oscillations in the brain in the context of learning and memory. Here we have used a neural network model which is capable of producing coupling of theta phase to gamma amplitude firstly to explore its ability to reproduce reported learning changes and secondly to memory-span and phase coding effects. The spiking neural network incorporates two kinetically different GABAA receptor-mediated currents to generate both theta and gamma rhythms and we have found that by selective alteration of both NMDA receptors and GABAA,slow receptors it can reproduce learning-related changes in the strength of coupling between theta and gamma either with or without coincident changes in theta amplitude. When the model was used to explore the relationship between theta and gamma oscillations, working memory capacity and phase coding it showed that the potential storage capacity of short term memories, in terms of nested gamma-subcycles, coincides with the maximal theta power. Increasing theta power is also related to the precision of theta phase which functions as a potential timing clock for neuronal firing in the cortex or hippocampus
Boundary Conditions on Internal Three-Body Wave Functions
For a three-body system, a quantum wave function with definite
and quantum numbers may be expressed in terms of an internal wave
function which is a function of three internal coordinates. This
article provides necessary and sufficient constraints on to
ensure that the external wave function is analytic. These
constraints effectively amount to boundary conditions on and its
derivatives at the boundary of the internal space. Such conditions find
similarities in the (planar) two-body problem where the wave function (to
lowest order) has the form at the origin. We expect the boundary
conditions to prove useful for constructing singularity free three-body basis

Exercise for reducing fear of falling in older people living in the community: Cochrane systematic review and meta-analysis
Objective: To determine the effect of exercise interventions on fear of falling in community-living people aged ≥65 years.
Design: Systematic review and meta-analysis. Bibliographic databases, trial registers and other sources were searched for randomised or quasi-randomised trials. Data were independently extracted by pairs of reviewers using a standard form.
Results: Thirty trials (2878 participants) reported 36 interventions (Tai Chi and yoga (n=9); balance training (n=19); strength and resistance training (n=8)). The risk of bias was low in few trials. Most studies were from high income countries (Australia=8, USA=7). Intervention periods (26 weeks=7) and exercise frequency (1-3 times/week=32; ≥4 times/week=4) varied between studies. Fear of falling was measured by single-item questions (7) and scales measuring falls efficacy (14), balance confidence (9) and concern or worry about falling (2). Meta-analyses showed a small to moderate effect of exercise interventions on reducing fear of falling immediately post intervention (standardised mean difference (SMD) 0.37, 95% CI 0.18, 0.56; 24 studies; low quality evidence). There was a small, but not statistically significant effect in the longer term (<6 months (SMD 0.17, 95% CI -0.05, 0.38 (four studies) and ≥ 6 months post intervention SMD 0.20, 95% CI -0.01, 0.41 (three studies)).
Conclusions: Exercise interventions probably reduce fear of falling to a small to moderate degree immediately post-intervention in community-living older people. The high risk of bias in most included trials suggests findings should be interpreted with caution. High quality trials are needed to strengthen the evidence base in this area
