Binomial level densities
It is shown that nuclear level densities in a finite space are described by a
continuous binomial function, determined by the first three moments of the
Hamiltonian, and the dimensionality of the underlying vector space.
Experimental values for Mn, Fe, and Ni are very well
reproduced by the binomial form, which turns out to be almost perfectly
approximated by Bethe's formula with backshift. A proof is given that binomial
densities reproduce the low moments of Hamiltonians of any rank: A strong form
of the famous central limit result of Mon and French. Conditions under which

The D0 Run IIb Luminosity Measurement
An assessment of the recorded integrated luminosity is presented for data
collected with the D0 detector at the Fermilab Tevatron Collider from June 2006
to September 2011 (Run IIb). In addition, a measurement of the effective cross
section for inelastic interactions, also referred to as the luminosity
constant, is reported. This measurement incorporates new features that lead to
a substantial improvement in the precision of the result. A luminosity constant
of \sigma_{LM} = 48.3\pm1.9\pm0.6 mb is obtained, where the first uncertainty
is due to the accuracy of the inelastic cross section used by both CDF and D0,
and the second uncertainty is due to D0 sources. The recorded luminosity for
the highest E_T jet trigger is L_rec = 9.2 \pm 0.4 fb^{-1}, with a relative
uncertainty of 4.3%.Comment: 20 pages, 23 figure
Immediate post-operative effects of tracheotomy on respiratory function during mechanical ventilation
INTRODUCTION: Tracheotomy is widely performed in the intensive care unit after long-term oral intubation. The present study investigates the immediate influence of tracheotomy on respiratory mechanics and blood gases during mechanical ventilation. METHODS: Tracheotomy was performed in 32 orally intubated patients for 10.5 ± 4.66 days (all results are means ± standard deviations). Airway pressure, flow and arterial blood gases were recorded immediately before tracheotomy and half an hour afterwards. Respiratory system elastance (E(rs)), resistance (R(rs)) and end-expiratory pressure (EEP) were evaluated by multiple linear regression. Respiratory system reactance (X(rs)), impedance (Z(rs)) and phase angle (φ(rs)) were calculated from E(rs )and R(rs). Comparisons of the mechanical parameters, blood gases and pH were performed with the aid of the Wilcoxon signed-rank test (P = 0.05). RESULTS: E(rs )increased (7 ± 11.3%, P = 0.001), whereas R(rs )(-16 ± 18.4%, P = 0.0003), X(rs )(-6 ± 11.6%, P = 0.006) and φ (rs )(-14.3 ± 16.8%, P = <0.001) decreased immediately after tracheotomy. EEP, Z(rs), blood gases and pH did not change significantly. CONCLUSION: Lower R(rs )but also higher E(rs )were noted immediately after tracheotomy. The net effect is a non-significant change in the overall R(rs )(impedance) and the effectiveness of respiratory function. The extra dose of anaesthetics (beyond that used for sedation at the beginning of the procedure) or a higher FiO(2 )(fraction of inspired oxygen) during tracheotomy or aspiration could be related to the immediate elastance increase
Two Interacting Electrons in a Quasiperiodic Chain
We study numerically the effect of on-site Hubbard interaction U between two
electrons in the quasiperiodic Harper's equation. In the periodic chain limit
by mapping the problem to that of one electron in two dimensions with a
diagonal line of impurities of strength U we demonstrate a band of resonance
two particle pairing states starting from E=U. In the ballistic (metallic)
regime we show explicitly interaction-assisted extended pairing states and
multifractal pairing states in the diffusive (critical) regime. We also obtain
localized pairing states in the gaps and the created subband due to U, whose
number increases when going to the localized regime, which are responsible for
reducing the velocity and the diffusion coefficient in the qualitatively
similar to the non-interacting case ballistic and diffusive dynamics. In the
localized regime we find propagation enhancement for small U and stronger

Microbleed Prevalence and Burden in Anticoagulant-Associated Intracerebral Bleed
Prior studies suggest an association between Vitamin K antagonists (VKA) and cerebral microbleeds (CMBs); less is known about nonvitamin K oral anticoagulants (NOACs). In this observational study we describe CMB profiles in a multicenter cohort of 89 anticoagulation-related intracerebral hemorrhage (ICH) patients. CMB prevalence was 51% (52% in VKA-ICH, 48% in NOAC-ICH). NOAC-ICH patients had lower median CMB count [2(IQR:1–3) vs. 7(4–11); P \u3c 0.001]; ≥5 CMBs were less prevalent in NOAC-ICH (4% vs. 31%, P = 0.006). This inverse association between NOAC exposure and high CMB count persisted in multivariable logistic regression models adjusting for potential confounders (OR 0.10, 95%CI: 0.01–0.83; P = 0.034)
Multi-modality curative treatment of salivary gland cancer liver metastases with drug-eluting bead chemoembolization, radiofrequency ablation, and surgical resection: a case report
<p>Abstract</p> <p>Introduction</p> <p>Liver metastases are rare in salivary gland tumors and have been reported only once to be the first manifestation of the disease. They are usually treated with surgical resection of the primary tumor and systemic chemotherapy. Drug-eluting bead chemoembolization has an evolving role in the treatment of hepatocellular carcinoma, as well as in the treatment of metastatic disease of the liver. Nevertheless, it has never been used in a patient with salivary gland liver metastases.</p> <p>Case presentation</p> <p>We report a case of a 51-year-old Caucasian Greek woman who presented to our hospital with liver metastases as the first manifestation of an adenoid cystic carcinoma of the left submandibular gland. The liver lesions were deemed inoperable because of their size and multi-focality and proved resistant to systemic chemotherapy. She was curatively treated with a combination of doxorubicin eluting bead (DC Beads) chemoembolization, intra-operative and percutaneous radiofrequency ablation, and radiofrequency-assisted surgical resection. The patient remained disease-free one year after the surgical resection.</p> <p>Conclusion</p> <p>In conclusion, this complex case is an example of inoperable liver metastatic disease from the salivary glands that was refractory to systemic chemotherapy but was curatively treated with a combination of locoregional therapies and surgery. A multi-disciplinary approach and the adoption of modern radiological techniques produced good results after conventional therapies failed and there were no other available treatment modalities.</p
Spectral Statistics in Chiral-Orthogonal Disordered Systems
We describe the singularities in the averaged density of states and the
corresponding statistics of the energy levels in two- (2D) and
three-dimensional (3D) chiral symmetric and time-reversal invariant disordered
systems, realized in bipartite lattices with real off-diagonal disorder. For
off-diagonal disorder of zero mean we obtain a singular density of states in 2D
which becomes much less pronounced in 3D, while the level-statistics can be
described by semi-Poisson distribution with mostly critical fractal states in
2D and Wigner surmise with mostly delocalized states in 3D. For logarithmic
off-diagonal disorder of large strength we find indistinguishable behavior from
ordinary disorder with strong localization in any dimension but in addition
one-dimensional Dyson-like asymptotic spectral singularities. The
off-diagonal disorder is also shown to enhance the propagation of two
interacting particles similarly to systems with diagonal disorder. Although
disordered models with chiral symmetry differ from non-chiral ones due to the
presence of spectral singularities, both share the same qualitative
localization properties except at the chiral symmetry point E=0 which is

Early Anticoagulation in Patients with Acute Ischemic Stroke Due to Atrial Fibrillation: A Systematic Review and Meta-Analysis.
Introduction: There is uncertainty regarding the optimal timing for initiation of oral anticoagulation in patients with acute ischemic stroke (AIS) due to atrial fibrillation (AF). Methods: We performed a systematic review and meta-analysis of randomized-controlled clinical trials (RCTs) and prospective observational studies to assess the efficacy and safety of early anticoagulation in AF-related AIS (within 1 week versus 2 weeks). A second comparison was performed assessing the efficacy and safety of direct oral anticoagulants (DOACs) versus vitamin-K antagonists (VKAs) in the two early time windows. The outcomes of interest were IS recurrence, all-cause mortality, symptomatic intracerebral haemorrhage (sICH) and any ICH. Results: Eight eligible studies (6 observational, 2 RCTs) were identified, including 5616 patients with AF-related AIS who received early anticoagulation. Patients that received anticoagulants within the first week after index stroke had similar rate of recurrent IS, sICH and all-cause mortality compared to patients that received anticoagulation within two weeks (test for subgroup differences p = 0.1677; p = 0.8941; and p = 0.7786, respectively). When DOACs were compared to VKAs, there was a significant decline of IS recurrence in DOAC-treated patients compared to VKAs (RR: 0.65; 95%CI: 0.52-0.82), which was evident in both time windows of treatment initiation. DOACs were also associated with lower likelihood of sICH and all-cause mortality. Conclusions: Early initiation of anticoagulation within the first week may have a similar efficacy and safety profile compared to later anticoagulation (within two weeks), while DOACs seem more effective in terms of IS recurrence and survival compared to VKAs
Measurement of Leptonic Asymmetries and Top Quark Polarization in ttbar Production
We present measurements of lepton (l) angular distributions in ttbar -> W+ b
W- b -> l+ nu b l- nubar bbar decays produced in ppbar collisions at a
center-of-mass energy of sqrt(s)=1.96TeV, where l is an electron or muon. Using
data corresponding to an integrated luminosity of 5.4fb^-1, collected with the
D0 detector at the Fermilab Collider, we find that the angular distributions of
l- relative to anti-protons and l+ relative to protons are in agreement with
each other. Combining the two distributions and correcting for detector
acceptance we obtain the forward-backward asymmetry A^l_FB = (5.8 +- 5.1(stat)
+- 1.3(syst))%, compared to the standard model prediction of A^l_FB (predicted)
= (4.7 +- 0.1)%. This result is further combined with the measurement based on
the analysis of the l+jets final state to obtain A^l_FB = (11.8 +- 3.2)%.
