Prominence and Mora in Mongolian
Different opinions about Mongolian prominence structure can be found in the literature. The most puzzling problem is the nature of lexical stress in this language, and neither its placement nor its phonetic nature have been given any final description. We have earlier performed an acoustic investigation of vowel durations, quality and fO to find if any of these three parameters functions as signalling one particular syllable as the most prominent one. Basing ourself on this investigation we reject the existence of lexical stress in Mongolian and find prominence as functioning only at the phrasal level. In the present article two problems are investigated. Firstly, phrasal accentuation is analysed as signalled by tonal means, and we show that the timing of the tonal gestures is best described within a mora analysis of Mongolian. Secondly, we argue for an analysis where nasals in the syllable coda have a moraic function
Vehicle cabin air quality – influence of air recirculation on energy use, particles and CO2
In this study simulations were performed to investigate the influence of different vehicle climate ventilation strategies, mainly the air recirculation (REC) degree, on the cabin air quality and climate system power. The focus of air quality is on the cabin particle concentrations including PM2.5 (particles of aerodynamic diameter less than 2.5 μm), UFP (ultrafine particles of aerodynamic diameter less than 100 nm), and cabin CO2 concentration. Three outside climates (cold, intermediate, and warm) and three outside particle concentrations are studied. The studied vehicle originally shows possibilities to meet WHO PM2.5 guideline of 15 μg/m3 with a new filter. The aged filter have reduced performance, especially when outside concentration is high. Increased REC shows advantages in all the three climates in reducing particles and climate power for the studied vehicle. Application of 70% REC (70% of ventilation air is recirculated air) on average lowers PM2.5 by 55% and 39% for a new and aged filter respectively. 70% REC with a new filter reduces cabin PM2.5 below guideline of 15 μg/m3 in all conditions. The reduction of UFP counts results are generally similar to that of PM2.5. Increased REC also lessens the average climate system power by up to 27 % on average. When REC is increased, the cabin CO2 concentration arise accordingly, and the magnitude is relevant to the passengers. In all studied conditions with 1 passenger, 70% REC does not increase CO2 above the common guideline of 1000 ppm. 70% REC is not recommended with more than 1 passengers in cold and intermediate climate, and 2 passengers in warm climate. Besides, to avoid the potential windscreen fog risk in cold climate, REC should be avoided when passengers are more than 3.Except for constant REC values, a sample study investigates a dynamic control of the REC. It shows the possibility of continuously optimizing REC to reduce the climate power and particles, while maintaining the CO2 concentration below 1000 ppm. In warm climate with 1 passenger boarded, the average optimized REC is 90%, which in comparison with base case lead to 44% PM2.5 reduction and 12% climate power reduction
Oestradiol levels and superoxide dismutase activity in age-related cataract: a case-control study.
Background: It has been suggested that the higher prevalence of cataract in women is caused by a withdrawal effect of oestrogen at menopause. In vitro studies have demonstrated protection of serum oestradiol (E2) against oxidative stress through upregulation of antioxidant enzymes, including superoxide dismutase (SOD). The purpose of the present study was to investigate E2 levels and SOD erythrocyte activity in patients with age-related cataract.
Methods: The studied subjects consisted of 103 patients with age-related cataract and 22 controls. Cataracts were classified as nuclear, cortical, or posterior subcapsular. Blood samples were collected and data on smoking, hormonal use, diabetes and age at menarche/menopause was obtained for all individuals. Serum oestradiol analyses were performed with radioimmunoassay (RIA) and SOD activity was measured in erythrocyte lysates.
Results: A negative correlation between age and E2 concentration was seen in a linear regression analysis. No correlation was seen between SOD activity and age or gender and no correlation between E2 levels and SOD activity was found using multiple linear regression. The mean level of E2 for all male subjects was 50.1 +/- 16.3 pmol/L, significantly higher compared to 13.8 +/- 11.8 pmol/L for postmenopausal women.
Conclusion: The present study does not support a role for E2-induced effects on SOD in cataract formation. The findings of higher E2 levels in men than in postmenopausal women may suggest that decreased oestrogen at menopause is partially responsible for the gender-related difference in cataract prevalence. However, the latter can only be verified through prospective randomized trials using hormonal replacement therapy
Ectopic fat is associated with cardiac remodeling—A comprehensive assessment of regional fat depots in type 2 diabetes using multi-parametric MRI
BackgroundDifferent regional depots of fat have distinct metabolic properties and may relate differently to adverse cardiac remodeling. We sought to quantify regional depots of body fat and to investigate their relationship to cardiac structure and function in Type 2 Diabetes (T2D) and controls.MethodsFrom the SCAPIS cohort in Linköping, Sweden, we recruited 92 subjects (35% female, mean age 59.5 ± 4.6 years): 46 with T2D and 46 matched controls. In addition to the core SCAPIS data collection, participants underwent a comprehensive magnetic resonance imaging examination at 1.5 T for assessment of left ventricular (LV) structure and function (end-diastolic volume, mass, concentricity, ejection fraction), as well as regional body composition (liver proton density fat fraction, visceral adipose tissue, abdominal subcutaneous adipose tissue, thigh muscle fat infiltration, fat tissue-free thigh muscle volume and epicardial adipose tissue).ResultsCompared to the control group, the T2D group had increased: visceral adipose tissue volume index (P < 0.001), liver fat percentage (P < 0.001), thigh muscle fat infiltration percentage (P = 0.02), LV concentricity (P < 0.001) and LV E/e'-ratio (P < 0.001). In a multiple linear regression analysis, a negative association between liver fat percentage and LV mass (St Beta −0.23, P < 0.05) as well as LV end-diastolic volume (St Beta −0.27, P < 0.05) was found. Epicardial adipose tissue volume and abdominal subcutaneous adipose tissue volume index were the only parameters of fat associated with LV diastolic dysfunction (E/e'-ratio) (St Beta 0.24, P < 0.05; St Beta 0.34, P < 0.01, respectively). In a multivariate logistic regression analysis, only visceral adipose tissue volume index was significantly associated with T2D, with an odds ratio for T2D of 3.01 (95% CI 1.28–7.05, P < 0.05) per L/m2 increase in visceral adipose tissue volume.ConclusionsEctopic fat is predominantly associated with cardiac remodeling, independently of type 2 diabetes. Intriguingly, liver fat appears to be related to LV structure independently of VAT, while epicardial fat is linked to impaired LV diastolic function. Visceral fat is associated with T2D independently of liver fat and abdominal subcutaneous adipose tissue
Effects of 6 weeks of treatment with dapagliflozin, a sodium- glucose co-transporter-2 inhibitor, on myocardial function and metabolism in patients with type 2 diabetes: A randomized, placebo-controlled, exploratory study
Aim: To explore the early effects of dapagliflozin on myocardial function and metabolism in patients with type 2 diabetes without heart failure.Materials and methods: Patients with type 2 diabetes on metformin treatment were randomized to double-blind, 6-week placebo or dapagliflozin 10 mg daily treatment. Investigations included cardiac function and structure with myocardial resonance imaging; cardiac oxygen consumption, perfusion and efficiency with [11 C]-acetate positron emission tomography (PET); and cardiac and hepatic fatty acid uptake with [18 F]-6-thia-heptadecanoic acid PET, analysed by ANCOVA as least square means with 95% confidence intervals.Results: Evaluable patients (placebo: n = 24, dapagliflozin: n = 25; 53% males) had a mean age of 64.4 years, a body mass index of 30.2 kg/m2 and an HbA1c of 6.7%. Body weight and HbA1c were significantly decreased by dapagliflozin versus placebo. Dapagliflozin had no effect on myocardial efficiency, but external left ventricular (LV) work (-0.095 [-0.145, -0.043] J/g/min) and LV oxygen consumption were significantly reduced (-0.30 [-0.49, -0.12] J/g/min) by dapagliflozin, although the changes were not statistically significant versus changes in the placebo group. Change in left atrial maximal volume with dapagliflozin versus placebo was -3.19 (-6.32, -0.07) mL/m2 (p = .056). Peak global radial strain decreased with dapagliflozin versus placebo (-3.92% [-7.57%, -0.28%]; p = .035), while peak global longitudinal and circumferential strains were unchanged. Hepatic fatty acid uptake was increased by dapagliflozin versus placebo (0.024 [0.004, 0.044] μmol/g/min; p = .018), while cardiac uptake was unchanged.Conclusions: This exploratory study indicates reduced heart work but limited effects on myocardial function, efficiency and cardiac fatty acid uptake, while hepatic fatty acid uptake increased, after 6 weeks of treatment with dapagliflozin.</p
Design and baseline characteristics of the finerenone in reducing cardiovascular mortality and morbidity in diabetic kidney disease trial
Background: Among people with diabetes, those with kidney disease have exceptionally high rates of cardiovascular (CV) morbidity and mortality and progression of their underlying kidney disease. Finerenone is a novel, nonsteroidal, selective mineralocorticoid receptor antagonist that has shown to reduce albuminuria in type 2 diabetes (T2D) patients with chronic kidney disease (CKD) while revealing only a low risk of hyperkalemia. However, the effect of finerenone on CV and renal outcomes has not yet been investigated in long-term trials.
Patients and Methods: The Finerenone in Reducing CV Mortality and Morbidity in Diabetic Kidney Disease (FIGARO-DKD) trial aims to assess the efficacy and safety of finerenone compared to placebo at reducing clinically important CV and renal outcomes in T2D patients with CKD. FIGARO-DKD is a randomized, double-blind, placebo-controlled, parallel-group, event-driven trial running in 47 countries with an expected duration of approximately 6 years. FIGARO-DKD randomized 7,437 patients with an estimated glomerular filtration rate >= 25 mL/min/1.73 m(2) and albuminuria (urinary albumin-to-creatinine ratio >= 30 to <= 5,000 mg/g). The study has at least 90% power to detect a 20% reduction in the risk of the primary outcome (overall two-sided significance level alpha = 0.05), the composite of time to first occurrence of CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure.
Conclusions: FIGARO-DKD will determine whether an optimally treated cohort of T2D patients with CKD at high risk of CV and renal events will experience cardiorenal benefits with the addition of finerenone to their treatment regimen.

May fermented Baltic Sea herring help in conditions of gut disorders, such as gastric catarrh and heartburn?
Background: It has been suggested that disruption of the gut microbiota can be significant with respect to pathological intestinal conditions, such as irritable bowel syndrome (IBS), gastric catarrh (GC), and heartburn (HB). Through history, an essential part of the colonization of the human gut took place by ingestion of food preserved by fermentation. The natural replenishment of microbes via food and beverage is today low because food is "sterilized" through boiling, broiling, and pasteurization. Modulating the gut rnicrobiota with fermented food products may hence be considered as a strategy to treat such conditions. Fermented Baltic Sea herring (FBSH) is an example of a Lactobacillus-fermented food product, which was tested in the present study. Methods: A 30-day open study was performed in 42 volunteers with IBS, GC, or HB. Volunteers were recruited by advertisements in daily newspapers. The volunteers were provided with gelatin capsules for the study, each containing approximately 100 mg freeze dried FBSH. They were also provided with forms that contained columns and rows for every test day where the volunteers were ask to fill in number of capsules taken, and to report possible improvements according to a 0-10 scale, where 10 stands for full recovery. Results: The most reported common disorder symptom was IBS and 7 of 14 of these volunteers reported recovery, with a mean recovery of 4.4. All of the 9 volunteers reported recovery from GC, with a mean recovery of 8.4. Five of 6 volunteers reported recovery from HB, with a mean recovery of 6.8. Conclusion: Although the present study is a small open study, the overall results are exciting and merits further studies in volunteers, ideally in a double-blind placebo-controlled manner
