Osteoporosis frequency and the association of rank, rankl, osteoprotegerin gene polymorphisms with osteoporosis in gout patients

zamanda kanıtlanmıştır. Daha önce osteoporoz hastaları ve romatoid artrit hastalarında
RANK, RANKL, OPG gen varyasyonlarının yeri araştırılmış ve farklı sonuçlar ortaya konmuştur. Yine psöriazis, psöriatik artrit ve ankilozan spondilit gibi romatolojik hastalıklarda da bu genlerin patogenezdeki yeri araştırma konusu olmuştur. Gut
hastalarında RANK, RANKL, OPG gen polimorfizmi ve bu genlerin osteoporoz ile ilişkisi
daha önce çalışılmamıştır, şu anki bilgilerimizle bizim çalışmamız bu alanda yapılmış ilk
çalışma olma özelliği göstermektedir. Bu çalışmaya 69 gut, 50 RA hastası, 51 sağlıklı gönüllü alındı. Femoral ve lomber bölge KMD leri DEXA kullanılarak ölçüldü. T- skorları elde edildikten sonra hasta ve kontroller DSÖ kriterlerine göre normal, osteopenik, osteoporoz olarak gruplandırıldı. 7 SNP real time PCR ile çalışıldı, bunlar RANK geni (2 SNP: rs1805034, rs35211496), OPG geni (2 SNP: rs3102735, rs 2073618) ve RANKL geni (3 SNP: rs9533156, rs1054016, rs2277438). Gruplar arasında lomber ve femoral bölge KMD açısından fark saptanmadı. Gruplar arasında RANK- RANKL- OPG genleri SNP polimorfizmleri açısından fark saptanmadı. Gruplar arası aynı SNP lerin allel sıklığı açısından fark saptanmadı. Rs1805034 genotipi wild olan gut hastalarında femur bölgesinde osteoporoz diğer gut hastalarına göre düşük saptandı. Rs1805034 genotipi heterozigot olan gut hastalarının femoral KMD sonuçları diğer gut hastalarına göre daha yüksek oranda normal saptandı. rs2277438 genotipi wild olan sağlıklı grupta osteoporoz diğer sağlıklı gönüllülere göre düşük saptandı, genotipi mutant olanlarda osteoporoz daha yüksek oranda saptandı. Kontrol grubunda lomber bölge KMD sonucu normal olan hastalarda rs35211496 wild genotip diğer kontrollerden daha yüksek saptandı. Sonuç olarak RANK geni rs1805034 C allelinin gut hastalarında femoral bölge osteoporoz gelişimini engellediği görülmektedir. RANKL geni rs2277438 A alleli sağlıklı polpulasyonda hem femoral bölge hem de lomber bölge osteoporoz gelişimini engellediği görülmektedir. Gut, Ra ve sağlıklı popülasyonda KMD’nin farklı olmaması daha önceki çalışmalara uymamaktadır. Bunun sebebi çalışmaya alınan hasta ve sağlıklı popülasyon sayısının az olması olabilir. Konunun net aydınlatılabilmesi için geniş hasta popülasyonlu çalışmalara ihtiyaç vardır.Gout is a disease characterized by acute arthritis as a result of increased serum uric acid concentration and deposition of monosodium urate crystals into tissues. In recent times the significant role of RANK- RANKL- OPG network in bone remodeling and osteoclastogenesis was proven. In patients with osteoporosis and in rheumatoid arthritis patients, genetic variations in the genes encoding RANK, RANKL and OPG was investigated and conflicting results were obtained. Also the role of these genes in the pathogenesis of psoriasis, psoriatic arthritis and ankylosing spondylitis has been researched. There is not any study that evaluates RANK, RANKL, Osteoprotegerin gene polymorphisms and their association with the susceptibility to osteoporosis in gout patients. To the best of our knowledge, this is the first study to investigate this subject. 69 gout patients, 50 RA patients and 51 healthy controls were recruited for this study. BMD of lumbar spine and femoral sites were measured using DEXA. After acquisition of the BMD T- score, the patients and controls divided into normal, osteopenic and osteoporotic groups according to WHO criteria. We studied 7 SNP in the genes of RANK gene (2 SNP: s1805034, rs35211496), OPG gene (2 SNP: rs3102735, rs 2073618) and RANKL gene (3 SNP: rs9533156, rs1054016, rs2277438) using real time PCR. There was no significant difference for BMD scores between three groups. The 7 SNP genotypes showed no significant difference between three groups. The 7 SNP allel frequencies showed no significant difference between three groups. Gout patients with rs1805034 wild genotype had lower incidence of osteoporosis at the femoral sites. Gout patients with rs1805034 heterozygous genotype had higher incidence of normal BMD scores at the femoral sites than other gout patients. Controls with rs2277238 wild genotype, had lower incidence of osteoporosis and those with mutant genotype had higher incidence of osteoporosis at any sites than other controls. Controls with normal BMD at lumbar spine, frequency of rs35211496 wild genotype was higher
than other controls. As a result it is seen that the C allel of the RANK SNP rs1805034 is protective against osteoporosis at femoral site in gout patients. The A allel of the RANKL SNP rs2277438 is protective against osteoporosis at either femoral sites or lumbar spine in healthy people. BMD in patients with Gout, RA and healthy population unexpectedly is not different from each other which may be related to low number of cases. There is a need for more studies with larger patient populations to explain the subject

Kan şekerinin kısa yada uzun süre yüksek olarak kalması da, hem doğrudan doğruya hem de dolaylı yollarla birçok hastalığa sebep olmakta; bağışıklık sistemini zayıflattığı için özellikle kanser hastalıları riskini arttırmaktadır.İçindekiler

Is the Probiotic Mixture Effective in the Treatment of TNBS-induced Experimental Colitis?
WOS: 000530895000009Objective: Inflammatory bowel disease (IBD) is an idiopathic disease associated with changes in the immune system and in the intestinal microbiota. The most accepted hypothesis of IBD pathogenesis is thought to be the abnormal immunological response and chronic intestinal inflammation, which is caused by the complex interactions between genetic, environmental factors and the host immune system. Microbial flora is important in the maturation of the immune system. Dysbiosis is defined as changes in intestinal microbiota composition and function. Clinical and experimental studies support that dysbiosis plays a significant role in the etiopathogenesis of IBD. Probiotics are useful live microorganisms that provide the intestinal balance in the host. In this study, we aimed to evaluate the anti-inflammatory and anti-oxidant activities of Enterococcus faecium, Lactobacillus acidophilus, Lactobacillus rhamnosus, Bifidobacterium bifidum and Bifidobacterium longum bacteria in the experimental colitis model. Methods: Twenty-four female Wistar-Albino rats and 30 mg 0.5 mL trinitrobenzenesulfonic acid (TNBS) dissolved in 50% ethanol which induced colitis by intrarectal installation. Rats were divided into four groups; healthy control (sham: group A), TNBS colitis (group B), (TNBS + methylprednisolone: group C) and probiotic (TNBS + P: group D). The rats were sacrificed on the 8th day. Macroscopic and microscopic scores, tissue myeloperoxidase (MPO), malondialdehyde (MDA) and superoxide dismutase (SOD) levels were measured. Results: Macroscopic and microscopic scores levels in group A were significantly lower than in group B, C and D. Macroscopic and microscopic scores levels in group C were significantly lower than in group B. Macroscopic scores were statistically similar between group C and D. There was a statistically significant difference between the groups in terms of median MDA levels and median SOD levels (p<0.001). There was no statistically significant difference between the groups in terms of median MPO levels (p=0.114). Median MPO levels were 0.27 (0.15-0.30) in group A, 0.44 (0.22-0.61) in group B, 0.28 (0.25-0.50) in group C, and 0.30 (0.25-0.37) in group D (p=0.114). Median MDA levels were 1.1 (1.0-2.8) in group A, 4.3 (3.1-5.5) in group B, 3.8 (3.2-4.2) in group C, and 3.9 (3.1-4.2) in group D (p<0.001). Median SOD levels were 160.7 (150.1-161.7) in group A, 141.6 (137.9-147.3) in group B, 157.6 (155.2-167.7) in group C, and 164.7 (160.3-168.3) in group D (p<0.001). MDA levels were statistically significantly different between each group. These levels were significantly higher in group B, C and D than in group A; statistically similar in group C and D; and statistically higher in group B than in group C and D (p<0.001 & p=0.047). SOD levels were statistically significantly different between each group. They were significantly lower in group B, C and D than in group B; statistically significantly different in group A, C and D; and statistically higher in group D than in group A and C. Conclusion: Our study showed that probiotics regulate the balance between anti-oxidant and oxidant systems. Therefore, probiotics can be used as a supportive treatment in inflammatory bowel diseases if promoted by clinical trials
Leptin and leptin receptor polymorphisms are related to body mass index in a Turkish population

in plasma as an important determinant of leptin sensitivity. In this study, our goal was to investigate the association between leptin levels
and leptin receptor polymorphisms in a Turkish population.
Materials and methods: The sample pool of this study consisted of 202 subjects. G2548A variant in the promoter region of the leptin
gene and Q223R polymorphism of the leptin receptor gene were evaluated by using PCR-RFLP. Leptin levels were determined by ELISA.
Results: Leptin levels were significantly higher in subjects with the A allele than in subjects without the A allele. Leptin receptor levels
were lower in subjects with the AA genotype than in those with the AG genotype. There was a higher prevalence of the leptin-2548 AA
genotype among subjects with a BMI ≥ 25 kg/m2 than in those with a BMI < 25 kg/m2.
Conclusion: The leptin-2548A allele might be a predisposing factor for obesity

Immediate and Follow-Up Results of Repeat Percutaneous Mitral Balloon Commissurotomy for Restenosis After a Succesful First Procedure
Background: The widespread use of percutaneous mitral commissurotomy (PMC) has led to an increase in restenosis cases. The data regarding follow-up results of repeat PMC are quite limited. The aim of this retrospective analysis is to evaluate the immediate and midterm results of the second PMC, in patients with symptomatic mitral restenosis after a succesful first procedure. Methods: Twenty patients (95% female, mean age 37 +/- 4 years) who have undergone a second PMC, 6.3 +/- 2.5 years after a first successful intervention built the study group. All were in sinus rhythm, with a mean Wilkins score of 8.5 +/- 1.2. Results: The valve area increased from 1.2 +/- 0.2 to 1.9 +/- 0.2 cm2 and mean gradient decreased from 10.5 +/- 3.4 to 6.1 +/- 1.1 mmHg. There were no complications except for a transient embolic event without sequela (5%) and two cases (10%) of severe mitral regurgitation. The immediate success rate was 90%. The mean follow-up was 70 +/- 29 months (36-156 months). The 5-year restenosis and intervention (repeat PMC or valve replacement) rates were 9.1 +/- 5.2% and 3.6 +/- 3.3%, respectively. The intervention free 5-year survival in good functional capacity (New York Heart Association [NYHA] I-II) was 95.1 +/- 5.5% and restenosis and intervention free 5-year survival with good functional capacity was 89.7 +/- 6.8%. Conclusions: Although from a limited number of selected patients, these findings indicate that repeat PMC is a safe and effective method, with follow-up results similar to a first intervention and should be considered as the first therapeutic option in suitable patients.
