    Characterization of Faecal Enterococci from Wild Birds in Turkey and Its Importance in Antimicrobial Resistance

    ΔΕΝ ΔΙΑΤΙΘΕΤΑΙ ΠΕΡΙΛΗΨΗThis research aimed to investigate the diversity of faecal enterococci isolated from wild birds, to detecttheir antibiotic resistance patterns and to determine their distribution of genes related to vancomycin resistance. Additionally, to investigate their virulence factors that are important in the development of the disease. One hundred seven cloacal/rectal samples were inoculated onto Enterococcus Agar, and presumptive colonies were identified and confirmed by PCR. Multiplex PCR assays were used to screen vanA, vanB, vanC1 and vanC2/3. The virulence-related genes; ace, gelE, efa and agg were determined by PCR. Among the 103 enterococci, 62 E.faecalis, 23 E.faecium 3 E.gallinarum, 2 E.durans, 1 E.casseliflavus and 12 Enterococcus spp. were identified. Of the 103 enterococci, 26 were found to be resistant against to three or more antibiotics. The highest percentages were detected for chloramphenicol (52%), tetracycline (33%) and erythromycin (30%). Two E.gallinarum isolates were harboring three virulence factors, and one isolate was carrying a single virulence factor. There is no virulence factor in the E.casseliflavus isolate. Also, vanA and vanB genes were not found. Forty-two of 103 enterococci were harboring virulence factors, more frequently in E.faecalis. Forty-two enterococci carried efa A, 31 isolates carried gel E, and ace was found in 18 isolates. Virulence gene agg was not detected. When the results of the study were evaluated in general, multiple drug resistance was described as 25%. Considering the risk of polluting the water resources of wild animals, it is suggested that the continuity of this type of epidemiological study in wildlife animals is necessary. In conclusion, the wild birds may act as substantial reservoirs carrying antimicrobial resistance among enterococci and estimate the potential risk for man, pets and farm animals

    Wear Behaviour of Crankshaft Journals Filled by Submerged Arc Welding

    In this study, wear behaviour of crankshaft journals filled by a submerged arc welding with different welding wires and powders is investigated. Hardness and microstructure variations in the submerged arc welding are examined. In addition, the amount of weight loss in abrasion samples of filling dry and lubrication ambience is measured under 40 N loads. It is observed that the amount of abrasion decreased with the increase of hardness at a certain point. Thin wire structure is provided for better abrasion resistance, and abrasion decreases with lubrication.В работе исследован режим износа цапф коленчатого вала, обработанных дуговой сваркой под флюсом с различными сварочными проволоками и порошками. Изучены вариации твёрдости и микроструктуры при дуговой сварке под флюсом. Кроме того, была измерена величина потери веса в образцах, подвергнутых истиранию в сухих и смазочных средах при нагрузках в 40 Н. Наблюдалось уменьшение величины истирания с увеличением твёрдости до определённого предела. Было обеспечено использование тонкой проволоки для лучшего сопротивления истиранию, а истирание уменьшалось при смазке.В роботі досліджено режим спрацювання цапф колінчастого валу, оброблених дуговим зварюванням під флюсом з різними зварювальними дротами і порошками. Досліджено варіації твердості та мікроструктури при дуговому зварюванні під флюсом. Крім цього, було виміряно величину втрати ваги в зразках, підданих стиранню в сухих і змащувальних середовищах при навантаженнях у 40 Н. Спостерігалося зменшення величини стирання зі збільшенням твердості до певної границі. Було забезпечено використання тонкого дроту для кращого опору стиранню, а стирання зменшувалося при змащуванні

    Eltrombopag for the treatment of immune thrombocytopenia: The aegean region of Turkey experience

    Objective: Immune thrombocytopenia (ITP) is an immune-mediated disease characterized by transient or persistent decrease of the platelet count to less than 100x109/L. Although it is included in a benign disease group, bleeding complications may be mortal. With a better understanding of the pathophysiology of the disease, thrombopoietin receptor agonists, which came into use in recent years, seem to be an effective option in the treatment of resistant cases. This study aimed to retrospectively assess the efficacy, long-term safety, and tolerability of eltrombopag in Turkish patients with chronic ITP in the Aegean region of Turkey. Materials and Methods: Retrospective data of 40 patients with refractory ITP who were treated with eltrombopag in the Aegean region were examined and evaluated. Results: The total rate of response was 87%, and the median duration of response defined as the number of the platelets being over 50x109/L was 19.5 (interquartile range: 5-60) days. In one patient, venous sinus thrombosis was observed with no other additional risk factors due to or related to thrombosis. Another patient with complete response and irregular follow-up for 12 months was lost due to sudden death as the result of probable acute myocardial infarction. Conclusion: Although the responses to eltrombopag were satisfactory, patients need to be monitored closely for overshooting platelet counts as well as thromboembolic events. © 2015 Turkish Society of Hematology.     Impaired toll like receptor-7 and 9 induced immune activation in chronic spinal cord injured patients contributes to immune dysfunction

    Reduced immune activation or immunosuppression is seen in patients withneurological diseases. Urinary and respiratory infections mainly manifested as septicemia and pneumonia are the most frequent complications following spinal cord injuries and they account for the majority of deaths. The underlying reason of these losses is believed to arise due to impaired immune responses to pathogens. Here, we hypothesized that susceptibility to infections of chronic spinal cord injured (SCI) patients might be due to impairment in recognition of pathogen associated molecular patterns and subsequently declining innate and adaptive immune responses that lead to immune dysfunction. We tested our hypothesis on healthy and chronic SCI patients with a level of injury above T-6. Donor PBMCs were isolated and stimulated with different toll like receptor ligands and T-cell inducers aiming to investigate whether chronic SCI patients display differential immune activation to multiple innate and adaptive immune cell stimulants. We demonstrate that SCI patients' B-cell and plasmacytoid dendritic cells retain their functionality in response to TLR7 and TLR9 ligand stimulation as they secreted similar levels of IL6 and IFNα. The immune dysfunction is not probably due to impaired T-cell function, since neither CD4+ T-cell dependent IFNγ producing cell number nor IL10 producing regulatory T-cells resulted different outcomes in response to PMA-Ionomycin and PHA-LPS stimulation, respectively. We showed that TLR7 dependent IFNγ and IP10 levels and TLR9 mediated APC function reduced substantially in SCI patients compared to healthy subjects. More importantly, IP10 producing monocytes were significantly fewer compared to healthy subjects in response to TLR7 and TLR9 stimulation of SCI PBMCs. When taken together this work implicated that these defects could contribute to persistent complications due to increased susceptibility to infections of chronic SCI patients. © 2017 Gucluler et al.
