    Using the theoretical domains framework to inform strategies to support dietitians undertaking body composition assessments in routine clinical care

    BACKGROUND: Malnutrition, sarcopenia and cachexia are clinical wasting syndromes characterised by muscle loss. Systematic monitoring by body composition assessment (BCA) is recommended for the diagnosis, treatment and monitoring of the syndrome(s). This study investigated practices, competency, and attitudes of Australian dietitians regarding BCA, to inform a local implementation process. METHODS: Applying the Action cycle in the Knowledge to Action framework, surveys were distributed to the 26 dietitians of an 800-bed tertiary hospital. The survey assessed barriers and enablers to performing routine BCA in clinical care. Results were categorised using the Theoretical Domains Framework (TDF) and suitable interventions mapped using the Behaviour Change Wheel. RESULTS: Twenty-two dietitians (84.6%) completed the survey. Barriers to BCA were identified in all TDF domains, particularly in Knowledge, Skills, Social/professional role and identity, Beliefs about capabilities, and Environmental context and resources. Enablers existed in domains of: Skills; Beliefs about consequences; Goals; Environmental context and resources; Social influences; Intentions; Optimism; Reinforcement. CONCLUSIONS: This study showed that hospital dietitians experience individual, team, and organisational barriers to adopt BCAs in clinical practice. We were able to formulate targeted implementation strategies to overcome these barriers to assist BCA adoption into routine practice.     The Role of Burn Centers in the Treatment of Necrotizing Soft-Tissue Infections:A Nationwide Dutch Study

    Patients with extensive and complex wounds due to Necrotizing Soft-Tissue Infections (NSTI) may be referred to a burn center. This study describes the characteristics, outcomes, as well as diagnostic challenges of these patients. Patients admitted to three hospitals with a burn center for the treatment of NSTI in a 5-year period were included. Eighty patients (median age 54 years, 60% male) were identified, of whom 30 (38%) were referred by other centers, usually after survival of the initial septic phase. Those referred from other centers, compared to those primarily admitted to the study hospitals, were more likely to have group A streptococcal involvement (62% vs 35%, p = .02), larger wounds (median 7% vs 2% total body surface area, p &lt; .001), and a longer length of stay (median 49 vs 22 days, p &lt; .001). Despite a high incidence of septic shock (50%), the mortality rate was low (12%) for those primarily admitted. Approximately half (53%) of the patients were initially misdiagnosed upon presentation, which was associated with delay to first surgery (16 hours vs 4 hours, p &lt; .001). Those initially misdiagnosed had more (severe) comorbidities, and less frequently reported pain or blue livid discoloration of the skin. This study underlines the burn centers' function as referral centers for extensively affected patients with NSTI. Besides the unique wound and reconstructive expertise, the low mortality rate indicates these centers provide adequate acute care as well. A major remaining challenge remains recognition of the disease upon presentation. Future studies in which factors associated with misdiagnosis are explored are needed.</p

    In vitro assessment of the combined effect of eicosapentaenoic acid, green tea extract and curcumin C3 on protein loss in C2C12 myotubes

    EPA has been clinically shown to reduce muscle wasting during cancer cachexia. This study investigates whether curcumin or green tea extract (GTE) enhances the ability of low doses of eicosapentaenoic acid (EPA) to reduce loss of muscle protein in an in vitro model. A low dose of EPA with minimal anti-cachectic activity was chosen to evaluate any potential synergistic effect with curcumin or GTE. Depression of protein synthesis and increase in degradation was determined in C2C12 myotubes in response to tumour necrosis factor-α (TNF-α) and proteolysis-inducing factor (PIF). EPA (50 μM) or curcumin (10 μg ml−1) alone had little effect on protein degradation caused by PIF but the combination produced complete inhibition, as did the combination with GTE (10 μg ml−1). In response to TNF-α (25 ng ml−1)-induced protein degradation, EPA had a small, but not significant effect on protein degradation; however, when curcumin and GTE were combined with EPA, the effect was enhanced. EPA completely attenuated the depression of protein synthesis caused by TNF-α, but not that caused by PIF. The combination of EPA with curcumin produced a significant increase in protein synthesis to both agents. GTE alone or in combination with EPA had no effect on the depression of protein synthesis by TNF-α, but did significantly increase protein synthesis in PIF-treated cells. Both TNF-α and PIF significantly reduced myotube diameter from 17 to 13 μm for TNF-α (23.5%) and 15 μm (11.8%) for PIF However the triple combination of EPA, curcumin and GTE returned diameters to values not significantly different from the control. These results suggest that either curcumin or GTE or the combination could enhance the anti-catabolic effect of EPA on lean body mass

    Testosterone levels are negatively associated with childlessness in males, but positively related to offspring count in fathers

    Variation in testosterone (T) is thought to affect the allocation of effort between reproductive and parenting strategies. Here, using a large sample of elderly American men (n = 754) and women (n = 669) we examined the relationship between T and self-reported parenthood, as well as the relationship between T and number of reported children. Results supported previous findings from the literature, showing that fathers had lower T levels than men who report no children. Furthermore, we found that among fathers T levels were positively associated with the number of children a man reports close to the end of his lifespan. Results were maintained when controlling for a number of relevant factors such as time of T sampling, participant age, educational attainment, BMI, marital status and reported number of sex partners. In contrast, T was not associated with either motherhood or the number of children women had, suggesting that, at least in this sample, T does not influence the allocation of effort between reproductive and parenting strategies among women. Voor de instellingen is het een belangrijke uitdaging om aansluiting te vinden bij de regio. Hoe verander je van een onderwijsinstelling in een regionaal kenniscentrum? Hoe wordt je een volwaardige partner in een kenniswerkplaats? Een tweede belangrijke uitdaging ligt in de koppeling van het leren in de regio aan de lerende regio. In het programma Regionale Transitie is deze taak bij de kenniswerkplaatsen gelegd.     Balancing risks of injury and disturbance to marine mammals when pile driving at offshore windfarms

    1. Offshore windfarms require construction procedures that minimize impacts on protected marine mammals. Uncertainty over the efficacy of existing guidelines for mitigating near-field injury when pile-driving recently resulted in the development of alternative measures, which integrated the routine deployment of acoustic deterrent devices (ADD) into engineering installation procedures without prior monitoring by marine mammal observers. 2. We conducted research around the installation of jacket foundations at the UK's first deep-water offshore windfarm to address data gaps identified by regulators when consenting this new approach. Specifically, we aimed to (a) measure the relationship between noise levels and hammer energy to inform assessments of near-field injury zones and (b) assess the efficacy of ADDs to disperse harbour porpoises from these zones. 3. Distance from piling vessel had the biggest influence on received noise levels but, unexpectedly, received levels at any given distance were highest at low hammer energies. Modelling highlighted that this was because noise from pin pile installations was dominated by the strong negative relationship with pile penetration depth with only a weak positive relationship with hammer energy. 4. Acoustic detections of porpoises along a gradient of ADD exposure decreased in the 3-h following a 15-min ADD playback, with a 50% probability of response within 21.7 km. The minimum time to the first porpoise detection after playbacks was > 2 h for sites within 1 km of the playback. 5. Our data suggest that the current regulatory focus on maximum hammer energies needs review, and future assessments of noise exposure should also consider foundation type. Despite higher piling noise levels than predicted, responses to ADD playback suggest mitigation was sufficiently conservative. Conversely, strong responses of porpoises to ADDs resulted in far-field disturbance beyond that required to mitigate injury. We recommend that risks to marine mammals can be further minimized by (1) optimizing ADD source signals and/or deployment schedules to minimize broad-scale disturbance; (2) minimizing initial hammer energies when received noise levels were highest; (3) extending the initial phase of soft start with minimum hammer energies and low blow rates
