    Evaluation of a combined HIV and geriatrics clinic for older people living with HIV: the Silver clinic in Brighton, UK

    As life expectancy in people living with HIV (PLWH) has increased, the focus of management has shifted to preventing and treating chronic illnesses, but few services exist for the assessment and management of these individuals. Here, we provide an initial description of a geriatric service for people living with HIV and present data from a service evaluation undertaken in the clinic. We conducted an evaluation of the first 52 patients seen in the clinic between 2016 and 2019. We present patient demographic data, assessment outcomes, diagnoses given, and interventions delivered to those seen in the clinic. The average age of attendees was 67. Primary reasons for referral to the clinic included management of complex comorbidities, polypharmacy, and suspected geriatric syndrome (falls, frailty, poor mobility, or cognitive decline). The median (range) number of comorbidities and comedications (non-antiretrovirals) was 7 (2–19) and 9 (1–15), respectively. All attendees had an undetectable viral load. Geriatric syndromes were observed in 26 (50%) patients reviewed in the clinic, with frailty and mental health disease being the most common syndromes. Interventions offered to patients included combination antiretroviral therapy modification, further health investigations, signposting to rehabilitation or social care services, and in-clinic advice. High levels of acceptability among patients and healthcare professionals were reported. The evaluation suggests that specialist geriatric HIV services might play a role in the management of older people with HIV with geriatric syndromes

    Enhancing road verges to aid pollinator conservation: A review

    Road verges provide habitats that have considerable potential as a tool for pollinator conservation, especially given the significant area of land that they collectively cover. Growing societal interest in managing road verges for pollinators suggests an immediate need for evidence-based management guidance. We used a formal, global literature review to assess evidence for the benefits of road verges for pollinators (as habitats and corridors), the potential negative impacts of roads on pollinators (vehicle-pollinator collisions, pollution, barriers to movement) and how to enhance road verges for pollinators through management. We identified, reviewed and synthesised 140 relevant studies. Overall, the literature review demonstrated that: (i) road verges are often hotspots of flowers and pollinators (well established), (ii) traffic and road pollution can cause mortality and other negative impacts on pollinators (well established), but available evidence suggests that the benefits of road verges to pollinators far outweigh the costs (established but incomplete), and (iii) road verges can be enhanced for pollinators through strategic management (well established). Future research should address the lack of holistic and large-scale understanding of the net effects of road verges on pollinators. We provide management recommendations for enhancing both individual road verges for pollinators (e.g. optimised mowing regimes) and entire road networks (e.g. prioritising enhancement of verges with the greatest capacity to benefit pollinators), and highlight three of the most strongly supported recommendations: (i) creating high quality habitats on new and existing road verges, (ii) reducing mowing frequency to 0–2 cuts/year and (iii) reducing impacts of street lighting

    The importance of monitoring the Greater Agulhas Current and its inter-ocean exchanges using large mooring arrays

    The 2013 Intergovernmental Panel on Climate Change report, using CMIP5 and EMIC model outputs suggests that the Atlantic Meridional Overturning Circulation (MOC) is very likely to weaken by 11–34% over the next century, with consequences for global rainfall and temperature patterns. However, these coupled, global climate models cannot resolve important oceanic features such as the Agulhas Current and its leakage around South Africa, which a number of studies have suggested may act to balance MOC weakening in the future. To properly understand oceanic changes and feedbacks on anthropogenic climate change we need to substantially improve global ocean observations, particularly within boundary current regions such as the Agulhas Current, which represent the fastest warming regions across the world’s oceans. The South African science community, in collaboration with governing bodies and international partners, has recently established one of the world’s most comprehensive observational networks of a western boundary current system, measuring the Greater Agulhas Current System and its inter-ocean exchanges south of Africa. This observational network, through its design for long-term monitoring, collaborative coordination of resources and skills sharing, represents a model for the international community. We highlight progress of the new Agulhas System Climate Array, as well as the South African Meridional Overturning Circulation programme, which includes the Crossroads and GoodHope hydrographic transects, and the South Atlantic MOC Basin-wide Array. We also highlight some of the ongoing challenges that the programmes still face

    Detection and treatment initiation for depression and alcohol use disorders: facility-based cross-sectional studies in five low-income and middle-income country districts.

    OBJECTIVES: To estimate the proportion of adult primary care outpatients who are clinically detected and initiate treatment for depression and alcohol use disorder (AUD) in low-income and middle-income country (LMIC) settings. DESIGN: Five cross-sectional studies. SETTING: Adult outpatient services in 36 primary healthcare facilities in Sodo District, Ethiopia (9 facilities); Sehore District, India (3); Chitwan District, Nepal (8); Dr Kenneth Kaunda District, South Africa (3); and Kamuli District, Uganda (13). PARTICIPANTS: Between 760 and 1893 adults were screened in each district. Across five districts, between 4.2% and 20.1% screened positive for depression and between 1.2% and 16.4% screened positive for AUD. 96% of screen-positive participants provided details about their clinical consultations that day. PRIMARY OUTCOMES: Detection of depression, treatment initiation for depression, detection of AUD and treatment initiation for AUD. RESULTS: Among depression screen-positive participants, clinical detection of depression ranged from 0% in India to 11.7% in Nepal. Small proportions of screen-positive participants received treatment (0% in Ethiopia, India and South Africa to 4.2% in Uganda). Among AUD screen-positive participants, clinical detection of AUD ranged from 0% in Ethiopia and India to 7.8% in Nepal. Treatment was 0% in all countries aside Nepal, where it was 2.2%. CONCLUSIONS: The findings of this study suggest large detection and treatment gaps for adult primary care patients, which are likely contributors to the population-level mental health treatment gap in LMIC. Primary care facilities remain unfulfilled intervention points for reducing the population-level burden of disease in LMIC

    New fossil remains of Homo naledi from the Lesedi Chamber, South Africa

    The Rising Star cave system has produced abundant fossil hominin remains within the Dinaledi Chamber, representing a minimum of 15 individuals attributed to Homo naledi. Further exploration led to the discovery of hominin material, now comprising 131 hominin specimens, within a second chamber, the Lesedi Chamber. The Lesedi Chamber is far separated from the Dinaledi Chamber within the Rising Star cave system, and represents a second depositional context for hominin remains. In each of three collection areas within the Lesedi Chamber, diagnostic skeletal material allows a clear attribution to H. naledi. Both adult and immature material is present. The hominin remains represent at least three individuals based upon duplication of elements, but more individuals are likely present based upon the spatial context. The most significant specimen is the near-complete cranium of a large individual, designated LES1, with an endocranial volume of approximately 610 ml and associated postcranial remains. The Lesedi Chamber skeletal sample extends our knowledge of the morphology and variation of H. naledi, and evidence of H. naledi from both recovery localities shows a consistent pattern of differentiation from other hominin species
